1.Analysis on the relative risk factor of bleeding again of patients with obscure gastrointestinal bleeding in negative result of capsule endoscopy

Objective:To study the relative risk factor of bleeding again of patients with obscure gastrointestinal bleeding (OGIB) of negative result in capsule endoscopy(CE). Methods: 92 patients with OGIB whose extermination results of CE were negative were enrolled in the retrospective analysis, and they were divided into observation group(46 cases, with bleeding again) and control group(46 cases, without bleeding again). The data of casehistory of these patients were compared, respectively, and then these data were analyzed by using Logistic regression analysis.Results: The ratio of 50 years old or older than 50 years old and the number of abnormal blood coagulation of observation group were significantly higher than that of control group(x2=5.386,x2=14.331,P<0.05), respectively. And the number of taking Aspirin of observation group was significantly higher than that of control group, while the number of received special treatment of observation group was lower than that of control group(x2=7.180, x2=23.545,P<0.01). As the results of Logistic regression analysis, 50 years old or older than 50 years old, abnormal blood coagulation, taking Aspirin and non-receiving special treatment were the independent risk factors, respectively, for patients with negative results of CE.Conclusion: In the examination of CE, although the results of patients with OGIB were negative, they may blood again, and all of these factors including 50 years old or older than 50 years old, abnormal blood coagulation, taking Aspirin and non-receiving special treatment are the risk factors which can affect blooding again. These factors should be paid more attention.
2.Mining analysis on composition and medication of menstruation prescriptions in Fu Qingzhu's Obstetrics and Gynecology.

In this paper, menstruation prescriptions were selected from "Fu Qingzhu's Obstetrics and Gynecology" and analyzed by using GRI algorithm, correlation analysis, hierarchical clustering method through SPSS, Clementine and traditional Chinese medicine (TCM) inheritance auxiliary systems, in order to screen out 15 menopathy prescriptions, which involve 45 traditional Chinese medicine herbs. In the study, blood-tonifying and qi-tonifying herbs were found to be frequent in the prescriptions. The most frequent single herb was white paeony root, accounting for 9.6% in the total number of prescriptions; The most frequent herb pairs were white paeony root-radix rehmanniae preparata and paeony root-angelica sinensis. Among Fu Shan's menopathy prescriptions, 61 herbal pairs showed a correlation coefficient exceeding 0.05, which evolved into 16 pairs of core combinations. The analysis showed that menopathy prescriptions in volume 1 of "Fu Qingzhu's Obstetrics and Gynecology" focused on tonic traditional Chinese medicines involving liver, spleen and kidney and were adjusted according to changes in qi, blood, cold, hot and wet, which could provide a specific reference for further studies on Fu Shan's academic thoughts and traditional Chinese medicine clinical treatment of menopathy.

3.Changes of Red Blood Cell and Erythropoietin in Rat Models with Liver Depression and Spleen Deficiency Syndrome

Objective To test red cells of bone marrow and hematopoietic cells of rats with liver depression and spleen deficiency syndrome in different time period;To discuss the change rules in the blood system. Methods Rat models with liver depression and spleen deficiency syndrome were made by composite method of anger by restraint stress, clipping tails, over fatigue, and abnormal diet. The hemogram, red cells of bone marrow, marrow nucleated red blood cells, stem cell factor, and erythropoietin of male rats in normal group and model group were detected at 2nd, 6th and 9th weeks. Results Compared with normal group, hematokrit and metarubricyte in model group of 2 weeks was higher in the model group (P<0.05);erythropoietin in model group of 6 weeks decreased (P<0.01);red cell counts, red protein level, hemoglobin, hematokrit, and erythropoietin in model group of 9 weeks were lower (P<0.05);red cell counts, hemoglobin, hematokrit, prorubricyte, metarubricyte, and erythropoietin in model group of 6 and 9 weeks were significantly lower than those in model group of 2 weeks (P<0.05, P<0.01). The number of marrow nucleated red blood cells in model group of 2 weeks decreased (P<0.05). Stem cell factor showed no significant difference. Conclusion The rats with liver depression and spleen deficiency syndrome can increase the number of red blood cells in a short time when they suffer from stress stimulation, so that they can fight the damage factors. But long-term stress stimulation will lead the rats to anemia, which may be associated with the reduction of erythropoietin.
4.Expressions of PCNA, c-erbB-2, p53, ER and PR in tissues adjacent to breast cancer

Objective To detect the expressions of proliferating cell nuclear antigen (PCNA), C-erbB-2, p53, estrogen receptor (ER) and progesterone receptor (PR) in order to explore the proper margin for breast conservative surgery on Chinese women. Methods We collected 40 resection specimens from breast cancer patients who had received radical surgery. Then we divided each specimen into primary tumor group and hyperplasia and gene expression characteristics of PCNA, c-erbB-2, p53, ER and PR were measured by pathological and immunohistochemical assay in the five groups. Results With the further distance from the primary tumor, the proportions of high-risk disease and positive gene expressions of PCNA, c-erbB-2 and p53 in paracarcinoma tissues gradually decreased (P<0.05). Higher risk and more positive expressions were related to paracarcinoma was no correlation of PCNA, ER and PR expressions adjacent to breast cancer with tissue differentiation and lymph node metastasis (P>0.05). a safe and appropriater region for breast conservative surgery.
5.Effect of processing on antitussive, antiasthmatic and diuretic activities of Morrus alba L.

AIM: To investigate the antitussive and antiasthmatic activities and diuretic activity of Morrus alba L. before and after being processed. METHODS: The antitussive and antiasthmatic activities in vivo (hypersensitivity) and in vitro (functional experiments) of Morrus alba L. before and after being processed were investigated. The diuretic activity of Morrus alba L. before and after being processed also were studied on rat and rabbit models. RESULTS: The processed Morrus alba L. could inhibite histamine-induced airway hypersensitivity. It's diuretic activity was lower but its antitussive activity was better. CONCLUSION: These results indicate that there are some differentes between Morrus alba L. before and after being processed in antitussive, antiasthmatic and diuretic activities. The diuretic activity of crude Morrus alba L. is superior to the processed Morrus alba L. But its antitussive and antiasthmatic activities are inferior to the processed Morrus alba L.
6.Acute renal failure after cardiac surgery in intuit in adult patients : evaluation of the ARF-specific scoring systems

Objective The aim of this study was to evaluate of adilty of two acute renal failure-specific scoring systenms (the classification by Bellome et al and the AKIN criteria) for predicting hospital mortality after cardiac surgery in adult patients. Methods Between October 1 st 2006 to Decemjber 31 st 2006, 509 adult patients who ungerwent coronary artery bypass grafting (CABG) and/ or valve operation were enrolled in this study. The medical data collection included gender, age, types of operation, perioperative he- modynamic parameters, urine output, biochemical parameters and outcome. Renal function was assessed daily according to the classi- ficatinn by Bellomo and the AKIN criteria, respectively. As references, Acure Physiology and Chronic Health Evaluation(APACHE) Ⅱ and Sepsis-related Organ Failure Assessment (SOFA) score were also calculated. Resuits Three hundred and forty-one patients were male (67.0%), and 168 were female (33.0%), mean age was (56.2±12.0) years old. Tnree hundred and nine patieats un- derwent CABG, 182 underwent valve operation and 18 underwent CABG plus valve operation, Mean duration of ventilation support was (20.4±17.7) houra, and the ICU stay was (1.4±1.0) days. Postoperative hospital stay was (13.8±9.1) days. According to the classification by Bellomo., the highest in-hospital mortality was 52.9% in ARFS group. Mahiplicatinn of in-hospital morality rate was abserved (X2 for trend, P<0.01) in 0.4% (non-ARF), 1.2% (stage 1), 12.0% (stal~ 2) and 32.4% (stage 3) of pa- tients based on the AKIN criteria. By applying the area under the receiver operating characteristic ourve, the classification by Bellomo and the AKIN criteria had good discriminative power. Furthering, multivariate logistic regression analysis verified that the Odds Ratio of the AKIN criteria was 5.478 (P =0.028, 95% Confidence Interval 1.027- 24.856), after adjusting for gender and age. Con- clusion Analytical data confinned good discriminative power of both the AKIN criteria and the classification by Bellomo for predicting hospital mortality of adult postoperative patient with ARF.
7.Expression of genes related to Sonic Hedgehog signaling in human hepatocellular carcinomas

Objective:To investigate the expression status of Sonic Hedgehog signaling genes and molecules in human hepatocellular carcinomas(HCC),and to explore the relationship between these genes and clinical prognosis.Methods:HCC tissue and adjacent normal tissue from 29 HCC patients were assayed for the expression of hedgehog signaling genes by reverse transcription-polymerase chain reaction chain reaction(RT-PCR) techniques and for the expression of hedgehog signaling molecules by immunohistochemistry.The expressions of Shh,Ptch,Smoh,Gli-1 mRNA were assayed as well as Shh,Ptch proteins in 29 cases of HCC and in 29 liver tissues adjacent to the tumor.Results:Expression of Shh mRNA was detectable in about 51% of HCCs examined.Consistent with this,hedgehog target genes Ptch,Smoh and Gli-1 mRNA were expressed in over 68%,48% and 62% of the tumors,respectively,and the expressions of Shh and Ptch proteins in HCC tumor tissues correlated with those of Shh and Ptch mRNA in tumor tissues(P=0.041 and P=0.035).This suggested that the hedgehog pathway was frequently activated in HCCs.The simultaneous expression of Gli-1 in HCC and liver tissues adjacent to the tumor had significantly relationship with poor prognosis.Conclusion:Hedgehog signaling activation is an important event for development of human HCCs.It also suggests that markers for hedgehog signaling activation may be useful for the determination of prognosis.

9.Perioperative Rehabilitation Nursing of Thoracic Breathing Reconstruction for Cervical Spinal Cord Injury

Objective To summarize experience of nursing for thoracic breathing reconstruction in patients after cervical spinal cord in-jury (CSCI). Methods 6 cases accepting thoracic breathing reconstruction in our hospital from March 2013 to June 2014 were reviewed from 1 week before operation to 4 weeks after operation. Results All the patients, aged (41.7 ± 16.2) years, received tracheotomy (6.0 ± 5.8) days after injury because of weakened cough, retention of respiratory secretions and dyspnea;the ventilator was used for (45.3±25.9) days, and they received thoracic breathing reconstruction surgery (84±26.7) days after injury. Their vital capacity was (1085±92) ml before opera-tion and was (1680±283) ml 2 weeks after operation with 55%increment (P<0.01). Only 1 patient underwent a delayed wound healing. No pulmonary infection, urinary tract infections and pressure sore occurred until 4 weeks after operation. Conclusion Enhancement of airway care and prevention of pressure sore are the keys to reduce postoperative complications of thoracic breathing reconstruction.
10.Value of introvoxel incoherent motion model in assessment of differentiation and blood supply of cervical cancer

Objective To investigate the value of intravoxel incoherent motion (IVIM) model of diffusion weighted MRI in assessing grades and enhancement patten of uterine cervical cancer. Methods Thirty one patients with pathologically proven cervical cancer, who underwent MRI scan preoperatively, were analyzed retrospectively and were divided into 3 groups according to their pathological grading of cacer, including 6 with G1 cancer, 17 with G2 and 8 with G3. The diameter of each lesion was≥1 cm. 10 b values (0, 30, 50, 100, 150, 200, 400, 800, 1 000, 1 500 s/mm2) were used in DWI, and DCE-MRI was performed with a time resolution of 9.8 s. Parameters of DWI (ADC, D, f, D*) and semiquantitative parameters of DCE-MRI (Slop, Maxslop, CER, Washout, AUC90) were measured. One-way ANOVA analysis of variance and Pearson correlation were used to analyze normally distributed continuous data. Kruskal-Wallis H test and Spearman correlation were used to analyze abnormally distributed continuous data. Tumor volume and all of the MRI parameters were compared as well as correlated with pathological grading.The perfusion parameters derived from IVIM were correlated with those derived from dynamic enhanced MR imaging. The sensitivity and specificity of f value to to diagnose G3 cervical cancer and the best cutoff were calculated from areas under the ROC curves.Results Tumor volume of G1,G2 and G3 cancers were(33.8±31.1),(19.6±16.9)and(31.2±29.1)cm3(F=1.147,P=0.332), respectively.ADC values of the three groups were(1.03 ± 0.11)× 10-3,(1.00 ± 0.10)× 10-3 and(0.90 ± 0.05)× 10-3mm2/s,respectively(F=4.619,P=0.018).D values of the three groups were (0.80 ± 0.11) × 10-3, (0.77 ± 0.06) × 10-3and (0.69 ± 0.06) × 10-3mm2/s ,respectively(F=5.272, P=0.011).f values of the three groups were 0.20±0.02, 0.22±0.03 and 0.24± 0.03, respectively (F=3.524, P=0.043).All of the others were of no significant difference (P>0.05).Both ADC and D correlated negatively with tumor grading (r=-0.464 and-0.493, P=0.009 and 0.005, respectively). f value correlated positively with tumor grading (r=0.436, P=0.014).Areas of ADC, D and f value under ROC curves to diagnose G3 cancers were 0.179, 0.147 and 0.690, respectively. While the cut-off value of f was 0.22, the diagnostic performance for G3 cancer was with a sensitivity of 75.0% (6/8) and a specificity of 60.9% (14/23). The value of f had weak positive correlations with Slop, Maxslop, CER and AUC90 of semiquantitative analysis of DCE-MRI (r=0.319, 0.337, 0.293 and 0.344, respectively, P<0.01). Conclusion IVIM model of multi-b value DWI may provide information in the assessment of differentiation and en hancement pattern of cervical cancer.
