1.Bacterial infections following liver retransplantation

Objective To evaluate the epidemiology, prophylaxis and treatment of bacterial infections after orthotopic liver retransplantation.Methods A retrospective study on bacterial infections in 22 patients undergoing othotopic liver retransplantation was carried out.Results Bacterial infections were confirmed in 8 of 22 preoperation and in 20 of 22 (90.9 %) postoperation. The median time of bacterial infections was 7 days after liver retransplantation. Fifty-one pathogenic organisms including 25 types were separated. The most commonly seen infected sites were biliary tract (50.0 %, 11/22), abdomen (45.4 %, 10/22), respiratory tract (36.4 %, 8/22), blood (31.7 %, 7/22), incision ((9.1) %, 2/22), bowel (4.5 %, 1/22), veins with venipuncture (4.5 %, 1/22). Seven cases of sepsis died.Conclusion The infectious rate after liver retransplantation is high and the types of bacterial disseminating loosely on organs are varied. The mortality will be increased if the patients develop sepsis. Reasonable use of immunosuppressants, selection of antibiotics and the supporting therapy are the keys to prevent and treat infections following liver retransplantation.
3.Clinical Observation on Acupuncture plus Oral Taking Chinese Medicine for Insomnia

Objective To observe the therapeutic effect of acupuncture plus oral taking Chinese medicine compared with that of oral taking Chinese medicine only for the treatment of insomnia.Methods Totally 64 patients were randomized into treatment group(33 cases)and control group(31 cases).The treatment group was treated by acupuncture with filiform needle[Baihui(GV 20),Sishen- cong(EX-HN1),Shenting(GV 24),Neiguan(PC 6),Shenmai(BL 62)and Zhaohai(KI 6)being the main points] plus oral taking Chinese medicine.The control group was treated by oral taking Chinese medicine only.All 64 cases were examined by SPIEGEL scale. 4.Analysis on the relationship between the human EV71 associated HFMD children's clinical manifestation and the genetic polymorphism and serum concentration of mannan binding lectin

Objective To explore the relationship between the genetic polymorphism and serum concentration of mannan binding lectin (MBL)and the clinical manifestation of the hand-foot-mouth disease (HFMD) children infection by human enterovirus 71 (HEV71).Methods One hundred and thirty-eight children diagnosed as HFMD infected by HEV71 (including 80 mild cases and 58 severe cases) and 40 healthy,symptom-free children were investigated.The concentrations of serum MBL were measured in 40 healthy controls,80 mild HFMD cases and 56 severe HFMD cases at both acute and convalescent phases by a sandwich enzyme immunoassay with a human MBL ELISA kit.And the genomic DNA of all cases were extracted from blood according to standard phenol-chloroform procedure.Six SNPs in the MBL gene(-550G/C,-221G/C and +4C/T of the promoter,CGT52TGT,GGC54GAC,and GGA57GAA of the exon 1) were analyzed by a sequencing-based typing method.Results The MBL serum level of the severe HFMD circulatory respiratory failure group in acute phase was significantly increased compared with severe HFMD encephalitis group,the mild cases and the control,but in the convalescence phase it significantly decreased compared with them.The frequencis of type B/B mutation (+230 of the exon 1),type P/P mutation (+4C/T of the promoter),and type H/H mutation (-550G/C of the promoter) were a significant difference among mild group,severe group and the control(P=0.006,0.043,0.028,respectively).The frequencies of LYPB/LYPB genotype and HYPA/HYPA genotype were a significant difference among mild group,severe groupand the control (P=0.028,0.014,respectively).Conclusion Low MBL protein level as a result genetic polymorphism seems to be correlative with clinical manifestation of HFMD disease.The MBL gene mutation and low MBI.protein level may be used as one of the evaluation method of HFMD severeity.
5.Effect of acupuncture combined with training of the rehabilitation apparatus on upper limb motor function of patients with ischemic stroke

ObjectiveTo explore the effect of acupuncture combined with training of the modified rehabilitation apparatus on upper limb motor function of patients with ischemic stroke.Methods86 patients with ischemic stroke were divided randomly into the treatment group ( treated by acupuncture combined with training of the rehabilitation apparatus) and the control group (treated by simple acupuncture). The upper limb motor function of two groups was evaluated by Fugl-Meyer Assessment (FMA) before and after treatment.ResultsAfter 28 days treatment, scores of FMA of two groups were all raised significantly, but it was significantly greater for the treatment group than the control group (P<0.01).ConclusionIt is shown that acupuncture combined with training of the rehabilitation apparatus can improve the upper limb motor function and activity of daily living of patients with ischemic stroke.
6.MRI-based follow-up study of spinal tuberculosis in children.

OBJECTIVETo investigate the value of MRI follow-up in pediatric patients with spinal tuberculosis.
METHODSFrom February 2004 to April 2013, MRI imaging data in 21 children with spinal tuberculosis by pathologically and clinically proven were retrospectively analysed. There were 11 males and 10 males, with an average age of 9.4 years old ranging from 2 to 14 years old. Eighteen patients were received conservative treatment, 3 patients were treated by surgical operation. The course of disease ranged from 2 months to 3 years. All cases underwent two or more MRI examinations for follow-up. MRI findings of the first and second examination were comparatively analyzed,including signal changes of vertebral body and intervertebral, paraspinal abscess and the change of kyphosis angle.
RESULTSIn the 21 cases with tuberculosis, 85 vertebral bodies were affected in total, including 10.6%(9/85), 49.4%(42/85) and 40%(34/85) vertebral bodies separately in the cervical,thoracic, lumbosacral spine. Follow-up MRI showed vertebral body shape and signal had no obvious change during or after treatment, 15 cases with vertebral bodies changed flat or cuneate, 10 cases with disc involvement,and intervertebral space became narrowing at different level. Among 13 patients with kyphosis angle greater than 0°, 7 were located on thoracic, 2, 3, 1 were on thoracolumbar, lumbar and cervical spine, respectively. Among 18 cases with conservative treatment,kyphosis angle were 0° on the first and the follow up MRI of 8 patients. One case of mild (<10°) kyphosis,follow-up MRI during therapy showed the angle slightly increased; 5 cases with moderate (10° to 30°), follow-up MRI during therapy showed no obvious change; 4 cases with severe (>30°), follow-up MRI during and after therapy showed kyphosis increased in varying degrees.
CONCLUSIONFollow-up MRI can make a big difference in the spinal tuberculosis of children; it can clearly show the change of the vertebral body and intervertebral space, paraspinal abscess and the kyphosis angle after the treatment,which can provide reference for clinical treatment and estimating prognosis.

7.Protection of Transcutaneous Acupoint Electrical Stimulation for Brain Injury Undergoing Intervention: a Clinical Observation.

OBJECTIVETo observe the effect of transcutaneous acupoint electrical stimulation (TAES) combined dexmedetomidine on hemodynamic of intracranial aneurysmal subarachnoid hemorrhage patients undergoing intervention, and their protection for brain Injury.
METHODSTotally 108 intracranial aneurysmal subarachnoid hemorrhage patients undergoing intervention were randomly assigned to the electroacupuncture (EA) group and the control group according to random digit table, 54 in each group. All patients were anesthetized with dexmedetomidine. Patients in the EA group were needled at bilateral Neiguan (PC6), Lieque (LU7), and Yunmen (LU2). Parameter setting was as follows: The dilatational wave at 1. 5 Hz, strength 2 - 4 mA, 30 min. Systolic blood pressure (SBP), diastolic blood pressure (DBP), mean arterial pressure (MAP), and heart rate (HR) were compared between the two groups immediately after entry into the room (T0), after administration (T1), intubating (T2), resuscitation (T3), extubation (T4), and leaving the operating room (T5). Levels of S100β protein (S100β) and neuron specific enolase (NSE) were compared between the two groups at T0, immediately after surgery (T6), 6 h after operation (T7), 12 h after operation (T8), and 24 h after operation (T9).
RESULTSCompared with the same group at T0, SBP, DBP, MAP, and HR were significantly reduced in the two groups at T1-T5(P <0. 05), serum levels of S100β and NSE in the two groups were significantly increased at T6-T9 (P<0. 05). Compared with the control group at T1 - T5, SBP, DBP, MAP, and HR decreased in the EA group (P <0. 05). Compared with the control group at T6-T9, serum levels of S100β and NSE decreased in the EA group (P <0. 05).
CONCLUSIONTAES combined dexmedetomidine could effectively maintain stable hemodynamics of intracranial aneurysmal subarachnoid hemorrhage patients undergoing intervention, and regulate their serum levels of S100β and NSE.

8.Chemical constituents of Physalis pubescens.

Chemical constituents of 95% ethanol extract of the dried persistent calyx of Physalis pubescens were investigated. By chromatography on a silica gel column and reverse-phase preparative HPLC, 10 compounds were isolated from the dichloromethane fraction. Based on the MS and 1D/2D NMR data, these compounds were identified as 5-O-(E-feruloyl) blumenol (1), isovanillin (2), (E) -ethyl 3-(4-hydroxyphenyl) acrylate (3), 4-hydroxybenzaldehyde(4), 4-methylphenol (5), (E) -methyl cinnamate (6), 7,3',4' trimethoxyquercetin (7), 5,3', 5'-trihydroxy-3,7,4'-trimethoxyflavone(8), danielone (9), and 5,5'-diisobutoxy-2,2'-bifuran (10).

10.Professor ZHANG Ji's clinical experience.

Professor ZHANG Ji has unique experience on therapeutic methods for rheumatoid arthritis, ankylosing spondylitis and obstinate facial palsy. (1) In acupuncture and moxibustion, he adopts the Governor Vessel and etiological analysis and differentiation, local acupuncture three step acupoint selection for treatment of rheumatoid arthritis; and supplementing the liver and kidney and strengthening the Governor Vessel and tonifying yang for ankylosing spondylitis; and dispelling wind and removing dampness, and dredging channels and activating the collaterals for obstinate facial palsy. (2) In Chinese drugs, on the basis of 50 year's clinical practice, he summarizes recipes Guanjie No. I and No. II. Modified Guanjie No. I is mainly used for treatment of rheumatoid arthritis, and modified Guanjie No. II mainly for ankylosing spondylitis, and Qianzheng Powder combined with drugs for clearing away heat and toxic substances are used for treatment of obstinate facial palsy.
