2.Effect of paclitaxel on carcino-embryonic antigen carcino-embryonic antigen, carbohydrate antigen 19-9 and carbohydrate antigen 72-4 levels in patients with advanced gastric cancer

Objective To analysis the effect of paclitaxel on CEA, CA19-9 and CA72-4 levels in patients with advanced gastric cancer.Methods46 patients who were diagnosed with advanced gastric cancer in Pinghu hospital of traditional Chinese medicine were collected.All patients were randomly divided into experimental group and control group,23 cases in each group.The control group was treated with cis-platinum complexes and capecitabine, and the experimental group was treated with paclitaxel and capecitabine, after the treatment, the serum levels of CEA, CA19-9, CA72-4, vascular endothelial growth factor, endostatin and clinical curative effect were detected in all patients.ResultsAfter treatment, compared with control group,①the serum level of CEA was lower in the experimental group (P<0.05);②the serum level of CA19-9 and CA72-4 were lower in the experimental group (P<0.05);③the serum level of VEGF was lower in the experimental group (P<0.05);④the serum level of Endostatin was higher in the experimental group (P<0.05);⑤The clinical efficiency of the experimental group was higher(P<0.05).ConclusionsThe paclitaxel can significantly reduce the serumCEA, CA19-9, CA72-4 and VEGF levels, elevated serum Endostatin level in patients with advanced gastric cancer, improve the treatment effect, and have a guiding significance for clinica.
3.Recent progress in molecular genetics and therapy for X-linked juvenile retinoschisis

Juvenile retinoschisis (RS or XLRS,MIM#312700)is a rare X-linked inherited disorder,mainly affects bilateral retina,and is characterized by cartwheel-like changes of the macular region of the retina and schisis or splitting within the inner retinal layers,leading to visual deterioration.The electroretinogram is beneficial in the diagnosis of juvenile retinoschisis.The a-wave can be of normal or nearly normal amplitude in this disorder,whereas the amplitude of the b-wave is appreciably reduced,giving a decrease in the proportion of b/a.The responsible gene,XLRSl,maps to Xp22 and was identified by positional cloning.This paper makes a brief review about the latest XLRS research of pathogenesis,animal experiments,clinical therapy,and 25 references are cited.
5. Progress in molecular targeted therapy for thyroid carcinoma

The treatment for thyroid cancer is mainly based on surgery. However, for the patients with metastatic, invasive and advanced thyroid cancer, the therapeutic efficacy of surgical operation is limited. Meanwhile, the radioactive iodine therapy and chemotherapy for high-grade thyroid cancer exert no substantial effects. The development of molecular targeted therapy brings new hopes for these patients. This article reviews the molecular pathogenesis of thyroid cancer, the mechanism of action of molecular targeted drugs and the therapeutic effects and adverse reactions of these targeted drugs. 6. Role of ultrasound-guided radiofrequency ablation for hepatic pedicle lesioning combined with temporary occlusion of the hepatic artery in precise liver resection

Objective To evaluate the safety and feasibility of radiofrequency ablation (RFA) for hepatic pedicle lesioning in combination with hepatic artery clamping for precise liver resection by establishing a model of precise liver resection in animals. Methods Six healthy Guangxi Bama miniature pigs were selected for this study, and S3, S5 and S6 used as target hepatic segments. The hepatic pedicles were lesioned by radioablation under ultrasound guidance. The hepatic artery was clamped temporarily at the time of radioablation. Three segments (S3, S5 and S6) were resected on each pig. The main outcome measures included the local and general tolerance, the change of blood flow parameters of each segmental arterial and portal branches, and the microscopic appearance of the segmental arterial, portal and biliary tract branches. ResultsThere was no any complication or death following RFA application, and the liver function was restored to normal 14 days after the operation in all animals. Demarcation areas were clear in all target hepatic segments. The blood flow velocity of the portal branches was 0cm/s in all the target hepatic segments after RFA. The arterial blood flow velocity was 15.1± 15.4cm/s, which was significantly lower compared with that before RFA (P>0.000). There was no difference in the flow rate of the unaffected segmental vessels following RFA. The mean ablated length of segmental portal, arterial and biliary tract branches was 1.7± 0.5cm, 1.4± 0.4cm, 1.6± 0.4cm, respectively. ConclusionsThe technique is expedient, safe and effective. Simultaneous temporary occlusion of the hepatic artery adds further benefit. The probe needs to be at least 1.5cm away from non targeted structures to avoid unintended thermal injury.
7.Progress in the personalized medicine using pharmacometabonomics.

Pharmacometabonomics, as an emerging branch of system biology, has been increasingly used in personalized medicine and showed broad prospects. By means of metabonomics, the complicated and detailed metabolic profile of the patient is described, thus providing more detailed description of the disease phenotype. With this understanding, response of different individuals to the drugs are predicted or evaluated through inherent genetic information of the individual combined with the environmental factors. As a result, appropriate drugs and dosage are chosen, which greatly promotes the realization of the individualized therapy goals. This article describes the emerging field of pharmacometabonomics, and the research results of personalized medicine based on the pharmacometabonomics in recent years are reviewed in detail.

8.Comparison of the Effects of Shengxuening and Iron Sucrose on Anemia,Oxidative Stress and Micro-in-flammation State in Hemodialysis Patients

OBJECTIVE:To compare therapeutic efficacies of Shengxuening tablet and Iron sucrose injection for anemia in maintenance hemodialysis(MHD)patients,and their effects on oxidative stress and micro-inflammation state. METHODS:80 pa-tients who had received MHD over half year were assigned into shengxuening group and iron sucrose group with 40 cases in each group. Both groups received MHD 3 times a week,4 h/time,blood flow of 200~300 ml/min;erythropoietin(EPO)was used at a dose of 10 000 IU/time hypodermically in each group. Shengxuening group was additionally given Shengxuening tablet orally,0.5 g/time,tid. Iron sucrose group was given Iron sucrose injection 100 mg 2 h intravenously via dialyser after the beginning of hemo-dialysis,twice a week,received iron treatment every one week until complete the total dose of 1 000 mg,to maintain ferritin be-tween 100~300 μg/L. The levels of Hb,Hct,SF,TSAT,hs-CRP,IL-6,TNF-α,MDA,SOD and GSH-Px were tested in 2 groups before and 12 weeks after the treatment. RESULTS:After 12 weeks of treatment,the levels of Hb,Hct,SF and TSAT in 2 groups were significantly increased,with statistical significance (P<0.01);but there was no statistical significance between 2 groups (P>0.05). Compared with before treatment,the levels of hs-CRP,IL-6,TNF-α and MDA increased significantly in iron sucrose group,while SOD and GSH-Px levels declined significantly,with statistical significance (P<0.01). Above indicators of shengxuening group had no significant change,compared with before treatment;there was statistical significance between 2 groups after treatment(P<0.01). No obvious ADR was found in 2 groups. CONCLUSIONS:Therapeutic efficacy of shengxuening is simi-lar to that of iron sucrose in improving the anemia and iron deficiency status of MHD patients,but doesn't aggravate oxidative stress and micro-inflammation state. It is an safe and effective oral iron supplement drugs.
9.Preparation and Characterization of Andrographolide Phospholipid Compound

OBJECTIVE:To prepare andrographolide phospholipid compound,optimize its formulation technology and charac-terizeit. METHODS:Andrographolide phospholipid compound was prepared with solvent method. Using compound rate as index, the effects of reaction solvent,reaction time,reaction temperature,drug concentration,the ratio of reactants and other single fac-tors on compound rate were investigated. The ratio of reactants,reaction temperature and drug concentration were optimized by or-thogonal test and validated. The dissolubilities of andrographolide,andrographolide phospholipid physical mixture and androgra-pholide phospholipid compound in water,soybean oil and Lauroglycol FCC were compared and characterized with UV and DSC. RESULTS:The optimal preparation technology was tetrahydrofuran as reaction solvent,reaction time of 4 h,reaction temperature of 50 ℃,drug concentration of 10 mg/ml,ratio of andrographolide to phospholipid 1:3. The compound rate of andrographolide phospholipid compound was 83.2%(n=3). Compared with raw material and physical mixture,phospholipid compound significant-ly improved the dissolubility of andrographolide in water,soybean oil and Lauroglycol FCC. UV and DSC showed that compound had been prepared. CONCLUSIONS:Andrographolide phospholipid compound is prepared successfully.
