Chinese Journal of Rehabilitation Theory and Practice 2006;12(7):574-575
ObjectiveTo observe the effect of early rehabilitation on motor function and prognosis of the patients with spinal cord injury (SCI).MethodsAccording to started date of rehabilitation, 60 SCI patients were divided into the early rehabilitation group (within two weeks after operation) and control group (three months after operation) with 30 cases in each group. The motor function and activities of daily living (ADL) of all patients were evaluated before and 3 months after treatment, and the incidence rates of complications of two groups were compared.ResultsAfter 3 months treatment, the incidence rates of complications often occurred after SCI such as pulmonary diseases, urinary infection, osteoporosis and adductor, extensor spasticity in the lower limbs etc. were lower than those in the control group. There was a significant difference between two groups. The motor function and ADL of the patients in the early rehabilitation group were better than those of patients in the control group ( P<0.05~0.01).ConclusionEarly rehabilitation after SCI can significantly decrease the incidence rate of complications and improve patient's motor function and ADL.

Objective To explore the relationship between ULI43 sequence variability and clini-cal disease. Methods UL143 from samples obtained from suspected congenitally human cytomegalovirus (HCMV) infected symptomatic infants were PCR amplified and sequenced. Results There were not too much sequence variability of UL143 compared with Toledo. But no one was completely identical to Toledo, and all UL143 ORFs were shorter than Toledo for frame-shift. Conclusion HCMV-UL143 existed in moat of low passage isolates and sequences were variable. No obvious linkage was observed between UL143 poly-morphisms and outcome of suspected congenital HCMV infection.

Objective To study the expression of thymic stromal lymphopoietin(TSLP) and the activation of DCs in OVA-induced murine asthma model, and investigate the effects and underlying mecha-nisms of TSLP on lung inflammation. Methods Thirty BALB/c mice were randomly divided into control group, OVA group and TSLP neutralizing antibody treated group. The asthma model was evaluated by airway responsiveness and histological analysis of lung tissues ; The levels of TSLP mRNA in lungs were determined by quantitative real-time PCR; The expression of TSLP in lungs were determined by immunohistochemistry and Western blot; The expression of CD40, CD80, CD86 in BALF was detected by FACS. Results Both the histological analysis of lung tissues and the airway responsiveness were all consistent with the characteris-tic of murine asthma model. The expression of TSLP and TSLP mRNA in the OVA group was significantly in-creased compared with blank group. The expression of CD40, CD80, CD86 in BALF from OVA group was increased significantly compared with the control group. Furthermore, treating mice with TSLP neutralizing antibody reduced the expression of CD40, CD80, CD86 on dendritic cells, and IL-4, IL-5, IL-13 in the OVA group. Conclusion Our study indicate that TSLP was highly expressed in the bronchial epithelia of murine asthma model, via upregulation of CD40, CD80, CD86, induce DCs to active CD4~+ T cells and pro-duce type 2 responses, so that aggravating the lung inflammation of asthma. Blocking TSLP is capable of in-hibiting the production of Th2 cytokines, thus presents a promising strategy for the treatment of asthma.

Objective To identify the peptide that have strong ability binding to HCMV-UL149 encoded protein, and to analyze the characteristics of the amino acid sequence of UL149-binding peptides. Methods Expressed UL149 proteins of three genotypes were used to screen the binding peptide in the random peptide display library, then the encoding sequence of binding peptides in the selected clones were sequenced. The amino acid sequences of the binding peptides were analyzed for their homology, and were com pared with those of the known protein in protein banks. Results The homologous amino acid sequence W/A/F/V-D/E-D/E-G-W/F/I/L were found within the binding peptides selected by proteins of all the three UL149 genotypes proteins, and no difference between three groups was found. The alignment with amino acid sequences of the known proteins in protein banks showed that the binding peptides of UL149 putative protein have homologous amino acid sequences with immunoglobulin heavy chain variable region(IgHV),the serine/threonine protein kinases, compliment factor H,zinc finger protein,MHC Ⅰ molecule, eukaryotic translation initiation factor, nuclear factor and so on.Conclusion The UL149 encoding proteins have binding ability to proteins mentioned above, and might interfere with the immunity responds to HCMV infection through multiple mechanisms.

ObjectiveTo evaluate the clinical efficacy of cutaneous branches of reverse second and third dorsal metacarpal artery fasciocutaneous flaps for repair of distal- and middle-segment finger soft tissue defects. MethodsA total of 14 patients with distal- and middle-segment finger soft tissue defects complicated by exposure of the phalanx or tendon were repaired by using cutaneous branches of second and third dorsal metacarpal artery fasciocutaneous flaps ranging between 2.0 cm × 4.5 cm and 3.0 cm × 7.0 cm.ResuitsAll of the skin flaps survived after surgery.Follow-up data during a 6-40 month period showed that the flaps exhibited a satisfactory appearance. They were not fat or clumsy, with a 2-point discrimination of 59 mm,and there was good recovery of finger function. The donor site was able to be directly sutured without dermoplasty.Pigmented linear surgical streaks appeared in the donor site. Conclusion The cutaneous branches of Second and third dorsal metacarpal artery fasciocutaneous flaps provide a good approach for the repair of distal- and middle-segment finger soft tissue defects and functional reconstruction because of convenient dissection, little trauma, sufficient use of the dorsal metacarpal artery.

Objective To investigate the influence of motor impersistence on functional recovery and prog- nosis of patients with left hemiplegia. Methods A total of 64 in-patients with left side hemiplegia who were able to understand the oral instructions were assigned into a motor impersistence group (n = 31 ) and a control group (n = 34) , respectively, according to the assessment with Scale of Motor Impersistence developed by Joynt. Both groups were comparable in terms of the basic clinical characteristic and motor function of patients except motor impersis- tence. The conventional rehabilitation training programs were adopted for all patients in this study. Brunnstrom func- tional category, Ueda Satoshi hemiplegic finger function assessment scale, upper limb function assessment scale, Berg balance scale, Functional ambulatory category, Modified Barthel Index were employed to evaluate the subjects before and after 1 and 2 months of treatment, with regard to the motor function of their upper limbs, walking ability, activities of daily living performance and balance ability. Results After 1 month of treatment, the patients in the M1 group were significantly improved with regard to the motor function of upper limbs, hand function, standing ba- lance and activity of daily living performance( P0.05 ) , while those in the control group were improved with all the parameters except the lower limb function. After 2 months of treatment, patients in both groups were all significantly improved ( P

Objective To report the clinical effect of free transplanting for soft tissue defects pedieled with high site cutaneous branches of the transverse branch of lateral cirumflex femoral artery. Methods Cu- taneous branches of the descending branch of lateral circumflex femoral artery were found small or abscent in 7 patients. The anterolateral femoral skin flap was pedicled with high site cutaneous branches of the transverse branch to repair the soft tissue defects of the arm, hand, leg and foot, rather than with the descending ones. The size of the flap ranged from 15 cm?6 cm to 28 cm?13 cm,with part muscle valve, iliotibal tract and lat- eral femoral cutaneous nerve. The fractures were performed with internal or external fixation. Results All of the anterolateral femoral skin flap survived well postoperatively in the 7 cases and had good appearance and sensation at one stage. The function of the repaired extremities recovered well. Conclusion The anterolat- eral femoral skin flap pedicled with high site cutaneous branches of the transuvrse branch of lateral circumflex femoral artery has many advantages of good blood supply and large size. The flap was secluding, and can be taken with some muscle and lateral femoral cutaneous nerve. When cutaneous branches of the descending branch of lateral circumflex femoral artery is small or abscent, the anterolateral femoral skin flap with high site cutaneous branches of the transverse branch of lateral circumflex femoral artery is an optimal alternative.
