1.The significance of intensifying the test for anaerobic bacteria in clinical laboratory
Chinese Journal of Laboratory Medicine 2001;0(01):-
Anaerobic bacteria are responsible for many cases of infection, but anaerobic bacteria are not detected in most of clinical labs in China yet.It is very important to intensify the test for anaerobic bacteria in clinical laboratory.Attention to detection for anaerobic bacteria should be paid.

Objective To analyze the risk of smoking on early diabetic foot patients. Methods One hundred and four early diabetic foot patients were involved in the study.According to cigarette smoking, patients were divided into the smoking group(n=45) and the no-smoking group(n=59).FBG,PBG2h,HbA_1c,TC,TG,HDL,LDL, BUN,Cr,CRP,ABI(ankle-branchial index)were examined.T-test and chi-square test were used to assess the risk of smoking on early diabetic foot patients. Results Compared with the no-smoking group, the smoking group were as- sociated with HDL and CRP.Chi-square-test demonstrated a statistically significant difference between the groups with smoking and no smoking on peripheral vascular disease in early diabetic foot patients. Conclusion Smoking re- mains significantly an independent risk of early diabetic foot patients.

BACKGROUND: Early withdrawal of invasive mechanical ventilation (IMV) followed by noninvasive MV (NIMV) is a new strategy for changing modes of treatment in patients with acute exacerbations of chronic obstructive pulmonary disease (AECOPD) with acute respiratory failure (ARF). Using pulmonary infection control window (PIC window) as the switch point for transferring from invasive to noninvasive MV, the time for early extubation can be more accurately judged, and therapy efficacy can be improved. This study aimed to prospectively investigate the clinical effectiveness of fiberoptic bronchscopy (FOB) in patients with AECOPD during sequential weaning of invasive-noninvasive MV. METHODS: Since July 2006 to January 2011, 106 AECOPD patients with ARF were treated with comprehensive medication and IMV after hospitalization. Patients were randomly divided into two groups according to whether fiberoptic bronchoscope is used (group A, n=54) or not (group B, n=52) during sequential weaning from invasive to noninvasive MV. In group A, for sputum suction and bronchoalveolar lavage (BAL), a fiberoptic bronchoscope was put into the airway from the outside of an endotracheal tube, which was accompanied with uninterrupted use of a ventilator. After achieving PIC window, patients of both groups changed to NIMV mode, and weaned from ventilation. The fol owing listed indices were used to compare between the groups after treatment: 1) the occurrence time of PIC, the duration of MV, the length of ICU stay, the success rate of weaning from MV for the first time, the rate of reventilation and the occurrence rate of ventilator-associated pneumonia (VAP); 2) the convenience and safety of FOB manipulation. The results were compared using Student's t test and the Chi-square test. RESULTS: The occurrence time of PIC was (5.01±1.49) d, (5.87±1.87) d in groups A and B, respectively (P<0.05); the duration of MV was (6.98±1.84) d, (8.69±2.41) d in groups A and B, respectively (P<0.01); the length of ICU stay was (9.25±1.84) d, (11.10±2.63) d in groups A and B, respectively (P<0.01); the success rate of weaning for the first time was 96.30%, 76.92% in groups A and B, respectively (P<0.01); the rate of reventilation was 5.56%, 19.23% in groups A and B, respectively (P<0.05); and the occurrence rate of VAP was 3.70%, 23.07% in groups A and B, respectively (P<0.01). Moreover, it was easy and safe to manipulate FOB, and no side effect was observed. CONCLUSIONS: The application of FOB in patients with AECOPD during sequential weaning of invasive-noninvasive MV is effective in ICU. It can decrease the duration of MV and the length of ICU stay, increase the success rate from weaning MV for the first time, reduce the rate of reventilation and the occurrence rate of VAP. In addition, such a method is convenient and safe in patients of this kind.

With more and more attention and investment on acupuncture scientific researches, considerable outcomes and achievements has been acquired, but the shortcoming of low transformation rate of acupuncture research achievements is gradually exposed. Nowadays there is no related report on this problem, so based on achievement translational research in other areas and practical situation of acupuncture, the existing problems and solutions are analyzed. As a result, the existing problems include (1) the research content is mainly basic research and clinical research but less acupuncture device research, leading to limited transformation efficiency; (2) the evaluation system and transformation pattern are still needed to be perfect. The solutions are (1) to properly evaluate the research achievements of acupuncture, (2) to advocate the concept and method of translational medicine, (3) to reform the policy and system, and (4) to establish valid platforms covering research, outcomes and transformation.

Objective To observe the effect of Weichangshu Capsule (WCSC) in inhibiting experimental gastric ulcer and the damage of gastric mucosa in SD rats. Methods The experimental gastric ulcer rat models were established by ligation of pylorus, water immersing method and subcutaneous injection of indometacin, and rat models with acute damage of gastric mucosa were induced by hydrochloric acid- ethanol (HAE).Then the effect of WCSC in inhibiting experimental gastric ulcer and the damage of gastric mucosa in SD rats were observed. Results WCSC had obviously inhibitory effects on stress gastric ulcer, indometacin- induced gastric ulcer and HAE- induced acute damage of gastric mucosa. WCSC also inhibited the secretion of gastric juice and increased pepsase activity. Conclusion WCSC has counteraction on experimental gastric ulcer.

Objective To evaluate the protecting effects of matrine on chemotherapy related hepatic lesion and its possible mechanism. Methods The positive rate and severity of hepatic lesion were compared between pa- tients being treated with or without matrine during chemotherapy processes.Furthermore, the difference of liver pro- tecting effect of this Chinese medicine between hepatitis B virus(HBV)infection chemotherapy patients and disinfec- tion patients were also analyzed. Results Both the rate of hepatic lesion and level of ALT in matrine treated group were much lower than those in untreated group in chemotherapy patients. The rate of hepatic lesion and level of ALT in HBV infection patients were higher than those in HBV disinfection patients in untreated group, while the signifi- cant difference of these two parameters between HBV infection patients and disinfection patients were disappeared in matrine treated group. Conclusion Matrine has hepatic protecting effect in chemotherapy related liver lesion.

ObjectiveTo explore the effectiveness and practicability of training technique in hemiplegia.Methods313 stroke patients with hemiplegia were divided randomly into the control group and training group, and treated with integrative training technique for hemiplegia impairment during 3 months. Locomotor recovery and the performance of activities of daily living (ADL) of patients in two groups were assessed before and after one, two and three months training respectively, using Fugl-Meyer Assessment (FMA), Functional Independence Measure (FIM) and the scale of impairment in community rehabilitation.ResultsAfter treatment, FMA, FIM and the scale of impairment in community rehabilitation of the patients in the training group improved significantly ( P<0.001), the effectiveness was better than that of the control group ( P<0.05~0.001).The scales of FMA and FIM were proportional to the scale of impairment in community rehabilitation.ConclusionThe training technique for hemiplegia has ideal rehabilitation effectiveness.
