2.Interaction of butylphthalide with rat and human liver CYP450 isoenzymes.

The work aims to study the drug metabolizing enzymes involved in the metabolism of butylphthalide and evaluate the induction and inhibition activities of butylphthalide on CYP450 isoenzymes by using in vitro (liver microsome incubation system of rats and human) and in vivo (CYP induced model of rats) method. Butylphthalide was incubated with selective inhibitors of CYP450, and its metabolic rate was determined to identify the metabolizing isoenzymes of NBP in rat (normal and induced rats) and human liver microsomes. The in vitro inhibition effect of butylphthalide on 6 main liver microsomal CYP450 isoenzymes was evaluated by using probe drugs; the induction and inhibition activities in vivo of butylphthalide on CYP450 isoenzymes were evaluated by NBP ig dosing (160 mg x kg(-1)) and iv dosing (20 mg x kg(-1)) in rats. After adding the specific inhibitors of CYP2C11, 2E1 and 3A 1/2 for rat, CYP2C19, 2E1 and 3A4/5 for human, the metabolism of NBP in rat and human liver microsomes were reduced 38.8%, 86.2%, 78.4% and 51.0%, 92.0%, 58.9% of control, respectively. The metabolic rates of NBP in CYP2E1 and 3A 1/2 induced rat liver microsomes were increased 25.5% and 68.9%. High concentration of NBP (≥ 200 μmol x L(-1), in vitro) could inhibit the activities of CYP1A2, 2C6, 2C11 and 2D2 in rats, and high concentration of NBP ( ≥ 15 μmol x L(-1), in vitro) could inhibit the activity of CYP2C19 in human. All the results indicated that NBP should be mainly metabolized by CYP2E1, 2C11 and 3A 1/2 in rats and CYP2E1, 2C19 and 3A4/5 in human. High concentration of NBP could inhibit human CYP2C19 in vitro. No significant induction/inhibition effects of NBP were observed on rat liver CYP450 isoforms after ig 160 mg x kg(-1) NBP or iv 20 mg x kg(-1) NBP.

4.Diagnosis related groups technology as used in hospital management

The diagnosis related groups( DRGs) method is applied in hospital management at both hospital and clinical department level. This technology enabled us to evaluate hospital medical services in general, objectively evaluate discipline development, and formulate individualized discipline guidelines, supporting scientific distribution of hospital resources. At the department level, the technology supported clinical departments in their discipline construction and quality management. The application of DRGs can improve fine management of hospitals.
5.Atractylodes lancea and Its Geo-Varieties

The geo-herbalism of Cangzhu, Atractylodes lancea (Thunb.) DC. , was studied for itsstrict quality control. Samples of typical and representative populations of geo-herbal and non-geo herbalorigin were collected, examined and cultivated through field work. Volatile oils from samples of varioushabitats were determined by GC/MS, and their respective bacteriostatic minimum inhibitory concentration(MIC)determined. Results of the study showed that both the size of capitulum and cleft on the leaves var-ied greatly even in species of the same population. The main components in their volatile oil also variedgreatly with Maocangzhu (MCZ) contains atractylone (29.54%) and atractylodin (17.82%); that of Nan-cangzhu (NCZ) contains hinesol (61.86%) and β-eudesmol (19.79%), and Baicangzhu (BCZ) cantains βeudesmol or mixture of β-eudesmol and atractylone (over 30%). Volatile oils of speciaes from Luotina,Taibai, and Huanglong showed potent bacteriostatic activity against Streptococcus hemolyticus (SH), com-parable to penicillin or streptomycin. In conclusion, A. lancea is a polymorphic complex greatly influencedby geographic environment and a reasonable proposal is to name them by geo-herbal names such as: MCZas A. lancea var. maoshanensis Hu et Feng; NCZ as A. lancea subsp. luotianensis Hu et Feng; and BCZ asA. lancea var. chinensis (Bunge) Kitam other than independent species as A. chinensis Bunge. It was in-correct to say that MCZ has no atractylone in its volatile oil and therefore differs from A. macrocephalaKoidz. (Baizhu). Inhibitory activity of the volatile oils against SH and Pseudomonas aeruginosa (PA) arealso quite different due to the habitats and imply that the rhizome from Luotian may be used to cure infec-tion diseases caused by SH and PA besides their traditional uses.
8.Comparison of clinical efficacy between vaginal tightening surgeries of buried lead needle suture and vaginal posterior wall mucosa excision for treatment of vaginal relaxation

Objective To compare the clinical effects of needle buried suture and the posterior wall of vagina mucosa excision vaginal tightening surgery for the treatment of mild to moderate vaginal relaxation.Methods A total of 71 patients with mild to moderate vaginal relaxation were randomly divided into observation group (39 cases) and control group (32 cases).The patients in the observation group were given buried lead needle suture,and the control group were treated with posterior wall of vagina mucosa resection.Analysis of two groups was conducted in patients with operation time,intraoperative bleeding,vaginal secretion color,postoperative wound healing and complications;sexual satisfaction was evaluated after 6,12 and 24 months in two groups of patients.Results There was no significant difference between two groups in operation time and vaginal secretion color (P> 0.05).Intraoperative blood loss was much more in the observation group than those in the control group,with statistically significant differences between the two groups [(6.23±2.43) ml vs (15.79 ± 7.31) ml,P<0.05)].The follow-up for a number of months showed that sexual satisfaction had significant difference between the two groups (P<0.05).None of the patients had rectal or urethral injury,no fistula,incision infection or other complications occurred,and the incision healed in stage Ⅰ.Conclusions For patients with mild to moderate vaginal relaxation,buried lead needle suture vaginal tightening surgery does not damage the vaginal mucosa,no scar,and fewer complications,which is a more simple and effective method to improve the quality of life in patients with vaginal relaxation.

10.Effect of dietary fiber on carbohydrate metabolism in healthy volunteers with 13C breath test

Objective To investigate the effect of dietary fiber on carbohydrate metabolism of healthy volunteers after the intake of corn starch meal. Methods Totally 12 healthy volunteers aged (25. 8 ± 5. 3) years were enrolled in this study, and they were equally randomly divided into two groups (group A and group B). This was an open, randomized, cross-over, and two-period study, and each period lasted for one day. In period 1, the subjects in group A received fiber-free corn starch and group B received high-fiber corn starch (containing 16 g dietary fiber). In period 2, the two groups are crossed. There was a one-week wash-out time between the two study days. On the study day, breath samples of fasting and 0. 5, 1.0, 1.5, 2.0, 2.5, 3.0, 3.5, 4.0, 5.0, 6.0,7. 0, 8. 0 hours post meal were collected to measure13 CO2. Results The delta over baseline at 0. 5, 1.5, 2. 0,2. 5, 3. 0, 4. 0 hour after test meal in fiber free group and in high fiber group were 0. 79, 2. 03, 2. 57, 2. 86,3. 02, 3. 18 and 0. 16, 1. 33, 1.77, 2. 10, 2. 34, 2.42, respectively (the P value was 0. 014, 0. 014, 0. 011,0.018, 0. 036, and 0.020, respectively). Peak concentration of delta over baseline of fiber free group and high fiber group was 3.18 and 2. 56 respectively, there was no significant difference between two groups (P > 0. 05) ,peak time of the group at 4. 0 hour and 3. 5 hour respectively, showed significant difference (P = 0. 032). The cumulative percentage dose recovered 0. 5-6. 0 hours after test meal in fiber-free group and in high-fiber group were 0.41, 1.46, 3.15, 5.50, 8.28, 11.30, 14.42, 17. 62, 23. 65, 28. 78 and 0. 09, 0. 55, 1.61, 3.22,5.23,7.53, 10.09, 12.68, 17.60, 22.27 respectively (the P value was 0.014, 0.018, 0.018, 0.014, 0.013,0.014, 0.018, 0.020, 0.025, and 0.044, respectively). However, there was no significant difference 6.0 hours after meal (P > 0. 05 ). Conclusion The dietary fiber used in this study can delay the absorption of carbohydrate 6. 0 hours within intake without influencing its total absorption amount.
