1.Evaluation of clinical effects of AO clavicula hook plate in treatment of complete acromioclavicular dislocation

Objective To evaluate the clinical effects of AO clavicula hook plate in treatment of acromioclavicular dislocation. Methods According to the specific anatomic, physiological and biomechanical conditions of the patients, 43 cases of complete acromioclavicular dislocation were treated either by AO clavicula hook plate (23 cases),or by Kirschner pin and steer wire tension band (20 cases). Results The results were evaluated by radiographic representations and joint function recovery according to Karlsson's criteria. The excellent results of the 2 methods were 95.6% and 65% respectively. There was significant difference. Conclusions The AO clavicula hook plate can meet the clinic requirements and be used with satisfactory results in the treatment of acromioclavicular dislocation.

3.Analysis of prognostic factors in patients with non-small cell lung cancer treated by surgery and chemotherapy

Background and Purpose:Lung cancer is the most malignant tumour in the world.Its incidence is growing and NSCLC is predominent(80%) in lung cancer.Most patients with lung cancer were diagnosed in late stages.The tumour could be shrunk by neoadjuvant chemotherapy when the case with stage Ⅲ NSCLC was considered not possible for radical operated neoadjuvant chemotherapy may lead to the following,operation could be improved,micrometastasis could be annihilated and survival could be extended.Objective of this paper was to analyse the prognostic factors for survival in patients treated by surgery and chemotherapy for NSCLC.Methods:98 cases of neoadjuvant chemotherapy combined with surgery for NSCLC,stageⅠ~Ⅲ,were collected retrospectively in our hospital from 1995 to 1997.35 cases were stageⅠ.21 cases were stage Ⅱ.42 cases were stage Ⅲ.83 cases had 1 cycle of chemotherapy pre-operatively.15 cases had 2 cycles chemotherapy pre-operatively.Regimes of chemotherapy were MVP,MOP and MAP et al.Response rate(RR) of chemotherapy was:45 cases had partial response(PR) and 53 cases were stable disease(SD).Operative mode was lobectomy and pneumectomy with lymph nodes dissection.Pathologic type was squamous,adeno,adeno-squamous and others.All the patients were treated by chemotherapy for two or three cycles after surgery except for the patients in stageⅠin 1996~1997.After being followed-up for more than 5 years,data were examined using life table,KaplanMeier method,Log Rank statistic and Cox-mantel test.The possible factors affecting survival were tested with univariate and multivariate analysis.Results:The median followed-up time of 98 cases for NSCLC was 41.2 months.36 cases were alive.62 cases were dead.The 1-,3-,5-year survival rate of 98 cases for NSCLC was 88.78%、49.63% and 18.46%.The 5-year survival rates of stageⅠ、ⅡandⅢ were 33.23%、20.26% and 5.52% respectively(P=0.0002).The 5-year survival rates of N_(0)、N_(1)、N_(2) were 35.49%、19.08% and 4.90% respectively(P=0.0004).In the 98 cases of NSCLC,better prognosis was correlated with earlier stage.The prognosis was better if the period from last chemotherapy before operation to operation was no more than 1 month. The prognosis of lobectomy,lung hila activity,thorax lymph nodes negativity and squamous cancer was better.The prognosis was poorer if the tumor had invaded big vessels,viscera,chest wall,pericardium and quantity bleeding during≥400ml.The prognosis was better if the tumor was fibrotic.The prognosis of 2 cycles of chemotherapy pre-operatively might be better than 1 cycle.The prognosis of tumor necrosis was poorer and the prognosis of chemotherapy post-operatively was better.Conclusions:The main prognostic factors affecting survival in patients treated by surgery and chemotherapy for NSCLC was stage,the period from last chemotherapy before operation to operation,operation mode,lung hila activity,thorax lymph nodes,site of tumor invasion,bleeding quantity,pathologic type,tumor fibrosis and necrotis,cycles chemotherapy pro-operation and chemotherapy post-operation.
4.Clinical application of large craniotomy decompression in treatment of severe craniocerebral injury combined with massive cerebral infarction

Objective To retrospectively review the efficacy of large craniotomy with subtemporal decompression in the treatment of severe craniocerebral injury combined with cerebral infarction.Methods Forty-eight patients suffering from severe craniocerebral injury combined with cerebral infarction were randomized into the treatment group (managed by large craniotomy decompression) and the control group (managed by clearance of hematoma by routine craniotomy ).A retrospective analysis was carried out to compare the cerebral infarction size between the two groups before operation and at day 1 and 1 week after operation and the outcomes of the two corresponding groups three months post-operatively.Results The infarction size of the treatment group was insignificant in comparison with that of the control group pre-operatively and at day 1 post-operatively,but was significantly smaller than that of the control group one week post-operatively (P < 0.05 ).The mortality and moderate disability/good rate of the treatment and control groups presented significant differences three months post-operatively (21%,42% vs 33%,25% respectively,P < 0.05 ).Conclusion Large craniotomy decompression is effective in treating severe craniocerebral injury combined with cerebral infarction and plays an important role in reducing disability and mortality.
5.Expressions of NMDA receptor subunits NR1 and NR2B in the hypothalamus of severe burn rats

ObjectiveTo observe the early changes of N-methyl-D-aspartic acid (NMDA) receptor subunit NR1 and NR2B expressions in the hypothalamus so as to discuss the underlying mechanism of the excitability alternation of hypothalamus-pituitary-adrenal (HPA) axis following severe burn in rats.MethodsA total of 40 Sprague-Dawley rats were randomized into the control group and the experimental groups ( including 6,24 and 48 hours after burn) and a 30% total body surface area full-thickness burn was induced by immersing the shaved dorsum into boiling water for 40 s.Then,the expression changes of the NMDA receptor subunits NR1 and NR2B in the hypothalamus tissue were examined by using immunohistochemistry and Western blot.Results There was an obvious enhancement of the excitability of HPA axis and obvious changes of serum concentration of cortisol and hemodynamics.The serum concentrations of cortisol of the experimental groups reached the peak at 6 hour after burn and were significantly higher than the control group even 48 hours later (P < 0.05 ).Systolic pressure (SP),diastolic pressure (DP) and left ventricular systolic pressure (LVSP) of the experimental groups were all decreased at 6 hours after burn,of which SP and DP were still lower than the control group at 48 hours after burn (P <0.05).Based on the Western blot assay,the NR1 level was increased and reached the peak (1.12 ±0.27) in the experimental groups and 0.45 -±0.15 in the control group at 24 hours after burn (P < 0.05).In the meantime,the NR2B level was decreased significantly and reached the lowest for 0.65 ±0.21 at 6 hours after burn,with statistical difference in comparison with 1.38 ±0.51 in the control group (P <0.05.The NR2B level recovered to pre-burn level for 1.25 ±0.30 at 48 hours in the experimental groups,with no statistical difference compared with 1.38 ± 0.51 in the control group (P > 0.05 ).The immunohistochemistry results on hypothalamus paraventricular nucleus (PVN) was consistent with the Western blot results.ConclusionThe persistent plasticity changes of hypothalamus subunits NR1 and NR2B may be on of important mechanisms for early excitability alteration of HPA axis after severe burn.
6.Serum osteocalcin in patients with spinal cord injury

Objective To explore the correlation between the time course of disease and bone metabolism changes in patients with spinal cord injury. Methods Data on serum osteocalcin(SO)measured by radioimmunoassay(RIA)from a database on 167 patients with spinal cord injury(SCI)were analyzed retrospectively.The patients were divided into four groups according to the time post-injury:group A(<3 months post-injury,n=44),group B(3-6 months post-injury,n=42),group C(7-12 months post-injury,n=37)and group D(>12 months post-injury,n=44). Results The mean SO value in all 4 groups of patients was significantly higher than normal.The mean SO value of group B was significantly higher than that of group A,but not significantly different from that of group C.The mean SO value of group D was significantly lower than that of group C,but not significantly different from that of group A.Conclusion Serum osteocalcin is elevated in the early stages of SCI,and its subsequent de-cline is closely correlated with the time post-injury.
7.An experimental study of neuronal activation through body-weight-supposed treadmill training after spinal cord injury using laser confocal microscopy

Objective To investigate the effects of locomotor training on improving locomotor function after spinal cord injury(SCI)and the mechanism of spinal cord plasticity.Methods A model of complete thoracic cord transection was established using 84 adult female rats divided into sham,SCI and treadmill training(BWSTT) groups.The hind limb locomotor function of all the rats was evaluated.The fluorescence intensities due to (EphA4),vesicular glutamate transporter 2(VGluT2)and EphA4/VGluT2 double-positive neurons in the ventromedial area of the anterior horn of the lumbar COrd were detected using immunofluorescence double labeling and laser confocal microscopy.Results The rats in the BWSTT group showed better functional recovery in their hind limbs than those in the SCI group.BWSTT was correlated with markedly increased EphA4.VGIuT2 and EphA4/VGluT2 intensities in the ventromedial area.Conclusion BWSTT improves hind limb locomotor function in rats with thoracic cord transections by elevating the expression of EphA4/VGluT2,promoting neuronal plasticity in the lumbar anterior horn.
8.Therapeutic effect of recombinant human brain natriuretic peptide on aged patients with acute decompensated heart failure

Objective: To explore therapeutic effect of recombinant human brain natriuretic peptide (rhBNP) in aged patients with acute decompensated heart failure (ADHF).Methods: A total of 98 aged ADHF patients treated in our hospital were selected and randomly divided into routine treatment group (n=50) and rhBNP group (n=48, received intravenous injection of rhBNP based on routine treatment group).After 72h treatment, cardiac function indexes, total effective rate and incidence of adverse drug reactions (ADR) were measured and compared between two groups.Results: Compared with before treatment, after 72h treatment, there were significant rise in left ventricular ejection fraction (LVEF) and 24h urine volume in both groups(P=0.001 all);compared with routine treatment group after treatment, there were significant rise in LVEF [(45.9±7.8)% vs.(57.4±7.9)%] and 24h urine volume [(1637.5±103.2)ml vs.(1836.4±118.4)ml], P=0.001 all.On 7d after treatment, total effective rate of rhBNP group was significantly higher than that of routine treatment group (93.75% vs.82.00%, P=0.033).There was no significant difference in incidence rate of ADR between two groups, P=0.898.Conclusion: Recombinant human brain natriuretic peptide can significantly improve heart function, and the therapeutic effect is significant in aged patients with acute decompensated heart failure.
9.Combination rules of Chinese herbal prescriptions for treating unstable angina based on complex network.

OBJECTIVETo explore combination rules of Chinese herbal prescriptions from effective cases for treatment of unstable angina (UA).
METHODSPrescription data from 156 UA patients effectively treated at Cardiovascular Diseases Centre of Xiyuan Hospital were analyzed using complex network method.
RESULTSAccording to multi-scale analysis of backbone network and pointwise mutual information analysis, core prescriptions from the 156 UA patients were presented as follows: Rhizoma Ligustici wallichii, Radix Paeoniae rubra, Radix Codonopsis, Rhizoma Pinelliae, poria, and Angelica sinensis. Meanwhile, core couplet medicines for these patients covered Rhizoma Ligustici wallichii and Radix paeoniaerubra, Angelica sinensis and Rhizoma Ligustici wallichii, Radix Codonopsis and Rhizoma Ligustici wallichii, Rhizoma Ligustici wallichii and Rhizoma Pinelliae, Rhizoma Atractylodis Macrocephalae and poriacocos, Bulbus Alli Macrostemi and Rhizoma Pinelliae. Among different primary symptoms, there was slightly difference in core prescriptions.
CONCLUSIONThe core prescriptions for the treatment of UA include blood-activating drug, phlem-resolving drugs. As an exploration of combination rules of Chinese herbal prescriptions in treating UA based on complex network, it can be used as a reference for further researches.

10.Role of Epidermal Growth Factor in Invasion and Metastasis of Pancreatic Cancer Cells

Objective Overexpressions of epidermal growth factor(EGF)and EGF receptor have been associ- ated with progression and invasive phenotype of pancreatic cancer.However,the underlying molecular mechanism by which EGF worked in pancreatic cancer cells has not been completely understood.In this study,effect of EGF on the invasion and metastasis of pancreatic cancer cells and its regulatory mechanism were investigated.Methods The effects of EGF on the proliferation,adhesion and invasion of pancreatic cancer cells were detected by WST-1 prolif- eration assay,adhesion assay and invasive assay,respectively.The activity and expression of MMP-2 and MMP-9 were examined by zymography,Western blot and RT-PCR,respectively.The activity of NF-?B was examined by EMSA.Results EGF could significantly promote the invasiveness of pancreatic cancer cells but did not affect cell proliferation or adhesion.The expressions of NF-?B and MMP-9 were significantly increased by EGF,but EGF did not affect the activity and expression of MMP-2.Furthermore,EGF stimulated the NF-?B binding activity.Pre- treatment with NF-?B inhibitors,pyrrolidine dithiocarbamate(PDTC),could significantly inhibit the activity of NF-?B induced by EGF.Meanwhile,the EGF-induced expression and activity of MMP-9,as well as cell invasiveness were also inhibited by NF-?B inhibitor.Conclusion EGF could increase the expression and promote the invasiveness of MMP-9 via the activation of NF-?B in pancreatic cancer cells,which implies that NF-?B inhibitant,such as PDTC,may diminish the invasiveness of pancreatic cancer cells.
