1.Diagnosis and treatment of catecholaminergic polymorphic ventricular tachycardia

Catecholaminergic polymorphic ventricular tachycardia is a rare hereditary ion channel disease, which mainly affects children and young adults.Clinically, the pathogenic genes of CPVT in adolescents are mainly RYR2, CASQ2, TRDN, CALM1 and TRD.Currently, beta blockers remain the first-line treatment for CPVT, and moderate exercise training is considered as an effective potential synergism for CPVT patients.Implantable cardioverter defibrillators are commonly used in the treatment of CPVT, but inappropriate shocks and device-related complications need to be carefully considered before implantation.Flecainide has been shown to be effective therapeutic agent, especially for the treatment of resistant CPVT cases with beta blockers.Left cardiac sympathetic denervation has also shown to be highly effective in preventing CPVT symtoms and cardiac events, and a combination of beta blockers can precede implantable cardioverter defibrillator implantation.
2.Screening genes trans-regulated by HBeAg-binding protein 1 with suppression subtractive hybridization technique

Objective To investigate the biological functions of HBeAg-binding protein 1(HBEBP1), suppression subtractive hybridization(SSH) technique was used to screen genes regulated by HBEBP1. Methods HBEBP1(GenBank number:AF529372) was screened and identified by yeast two-hybrid system 3 and co-immunoprecipitation technique. The HBEBP1 coding DNA fragment was amplified by reverse transcription polymerase chain reaction (RT-PCR) technique from HepG2 cell genome. The expressive vector of pcDNA3.1(-)-HBEBP1 was constructed by routine molecular biological methods. The HepG2 cells were transfected with pcDNA3.1(-) and pcDNA3.1(-)-HBEBP1, respectively by using FuGENE6 transfection reagent, then the mRNA was isolated. SSH method was employed to analyze the differentially expressed DNA sequences between the two groups. After restriction enzyme Rsa I digestion, small sized cDNAs were obtained. Then tester cDNAs were divided into two groups and ligated to the specific adaptor 1 and adaptor 2, respectively. After tester cDNAs were hybridized with driver cDNAs twice and underwent polymerase chain reaction (PCR) twice, they were then subcloned into pGEM-Teasy plasmid vectors to set up the subtractive library. Amplification of the library was carried out with E. coli strain DH5?. The cDNAs were sequenced and analyzed in GenBank with Blast search after PCR. Results The subtractive library of genes up-regulated by HBEBP1 was constructed successfully. The amplified library contained 85 positive clones. Colony PCR showed that these clones contained 200-1 000bp inserts. Sequence analysis was performed in 26 clones at random, and the full length sequences were obtained with bioinformatics method. Altogether 15 coding sequences were obtained. Conclusions The obtained sequences may be target genes up-regulated by HBEBP1, among which some genes coding proteins were involved in cell cycle regulation, metabolism, immunity and cell apoptosis. This finding brought some new clues for the study of the biological functions of HBEBP1 and HBeAg.
3.The effect of cognitive intervention on language function and quality of life in elderly patients with post-stroke aphasia

Objective To investigate the effect of cognitive intervention on language function and quality of life (QOL) in elderly patients with post-stroke aphasia.Methods Fifty-five elderly patients with post-stroke aphasia were randomly divided into the control group (n =27) and the experimental group (n=28).The control group received comprehensive treatment including speech-language therapy (SLT),while the experimental group was subjected to additional cognitive intervention.All patients were enrolled in an eight-week rehabilitation program.The mini-mental state examination (MMSE) was used to assess cognitive function,a device for diagnosis and treatment of language disorders,ZM 2.1,was used to evaluate language function,and the 36-item short-form health survey (SF-36) (Chinese version) was used to assess quality of life (QOL),before and after treatment.Results After treatment,MMSE scores,language function scores in all categories and QOL scores in all dimensions improved in both groups (P<0.05 for both).Furthermore,after treatment,there were significant differences between the two groups in MMSE scores of orientation,recall,attention and calculation,and in scores of overall language ability and the individual categories (P<0.05 for all).Scores of the experimental group vs.the control group for the individual categories of language function were as follows:simple instructions (76.6 ± 14.1 vs.67.4± 19.3),complex instructions (66.1±12.8 vs.58.2±14.9),yes or no (72.5±12.1 vs.63.0±14.1),naming (55.0 19.5 vs.43.3±22.2),simple comprehension (67.5±21.4 vs.55.620.6),complex comprehension (44.8±17.0 vs.35.0±18.9),listening and reading words (65.4±16.7 vs.53.7±19.3),calculation (39.3±25.8 vs.25.9±19.2),memory (36.4±18.7 vs.26.3±17.8),matching (75.9±18.6 vs.65.3±17.1),simple general knowledge (68.3±18.2 vs.58.0±19.5),complex general knowledge (58.7±17.4 vs.50.0±13.3),orientation (70.7±19.6 vs.60.5±17.2) and comparison (59.9± 14.6 vs.50.2±17.5) (P<0.05 for all).There were significant differences between the two groups in emotional function (66.7±18.2 vs.53.1±21.2),general health (67.2±12.6 vs.60.7±9.8),mental health (71.0±5.6 vs.63.1±4.2),social functioning (64.7±9.0 vs.59.3±10.2) and vitality (55.4±14.8 vs.46.9±15.6) (P<0.05 for all).Conclusions Cognitive intervention combined with SLT can contribute to the recovery of language function and the improvement of QOL in elderly patients with post-stroke aphasia.

5.Association between air pollutants and pediatric outpatient volumes in Baoshan District

Objective:
To explore the relationship between air pollutants and pediatric outpatient volumes in Baoshan District, Shanghai Municipality.
Methods:
Data of meteorological factors, air pollutants and pediatric outpatient volumes in four general hospitals were collected in Baoshan District from 2015 to 2019, and a generalized additive model was used to fit the Poisson-like distribution. The exposure-response relationship between fine particulate matter (PM2.5), sulfur dioxide (SO2), nitrogen dioxide (NO2), ozone (O3) and total pediatric outpatient volumes and pediatric respiratory outpatients.
Results:
The median of the average daily temperature and relative humidity were 18.7 (interquartile range, 14.4) ℃ and 74.5% (interquartile range, 18.0%) in Baoshan District from 2015 to 2019, respectively. The median of the average daily concentrations of PM2.5, SO2, NO2 and O3 were 35.0 (interquartile range, 35.0), 11.0 (interquartile range, 7.0), 45.0 (interquartile range, 31.8) and 84.5 (interquartile range, 50.0) μg/m3, respectively. The median of the average daily total and respiratory pediatric outpatient volumes were 680 (interquartile range, 246) and 392 (interquartile range, 253). Spearman rank correlation analysis showed that temperature, relative humidity, PM2.5, SO2, NO2 and O3 were associated with total and respiratory pediatric outpatient volume (all P<0.05). Under the single pollutant model, the excess risk of total and respiratory pediatric outpatient volume due to PM2.5 (ER=0.318, 0.257), SO2 (ER=1.610, 2.546), and NO2 (ER=0.808, 0.839) reached the maximum effect on the same day, and the effect of O3 (ER=0.102, 0.222) reached its maximum at the first day of lag. Under the multi-pollutant model, after O3, SO2, NO2 and PM2.5 were introduced, a exposure-response relationship between air pollutants and total pediatric outpatient volumes was the largest on the sixth day after the lag (ER=0.419). There was no exposure-response relationship between air pollutants and respiratory pediatric outpatient volumes.
Conclusion
PM2.5, SO2, NO2 and O3 are associated with total and respiratory pediatric outpatient volumes, and the lag effects due to different air pollutants are different.
6.Identification of the related substances in docetaxel injection by LC-MS/MS.

The related substances in docetaxel injection were identified by LC-MS/MS. Ethyl acetate was used to extract the injection to remove the pharmaceutical excipients. HPLC separation was carried out on a Hedera ODS-2 column (150 mm x 2.1 mm, 5 microm) with a mobile phase consisting of acetonitrile - 0.1% acetate acid aqueous solution (40: 60). Electrospray ionization source was set in the positive mode for the LC-ESI-MS/MS, and the ion monitoring modes were full scan and product ion scan. According to the mass spectra of the related substances, the fragment profiles were explained, and the chemical structures were elucidated. Docetaxel and its main related substances were well separated. Nine related substances in docetaxel injection were detected by LC-MS/MS. Their chemical structures were proposed, and four of them were identified in the docetaxel injection for the first time. The established LC-MS/MS method is effective in the separation and identification of the related substances in docetaxel injection. The test results are useful for its quality control.
7.Analysis of monitoring results of Kaschin-Beck disease in Yan'an city Shaanxi province from 2006 to 2008

Objective To find out the situation of Kashin-Beck disease and it's dynamic changes in Yan'an city, and provide basis for decision-making on prevention and control of the disease. Methods In accordance with the national monitoring program on Kaschin-Beck disease, 21 villages were selected from 6counties in Yan'an, clinical and X-ray inspection of 7 to 13 year-old children were made according to historical illness information from 2006 to 2008. Clinical monitoring on Kashin-Beck disease was made in 2008 for people over 16 years old from 5 villages in 5 counties. Results One thousand and one hundred eighty children were found positive with the illness from 2006 to 2008, with 7 cases of grade Ⅰ , the detection rate was 0.59%(7/1180).One thousand and one hundred sixty-two people were taken X-ray photo on right hand, with no positive case found.In 2008, 1444 adults were taken clinical examination, the detection of grade Ⅰ and over were 160 cases, the detection rate was 11.08%(160/1444), mainly in the age of 36 years[93.75%(150/160)]. Conclusion KashinBeck disease in Yan'an city is in stable condition, but surveillance and preventing measures are stilled needed.
8.Clinical and Pathological Analysis of 91 Children with Lupus Nephritis

Objective To analyze the clinical and pathological characteristics of children with lupus nephritis(LN).Methods Ninety-one children with LN were diagnosed from 1993 to 2005,according to the clinical literature and renal pathology and the data were retrospectively summarized.Results Within the cohort of 91 children,there were 69 females and 22 males(female to male ratio 3.1).Most of the sick children were at the school age ranging from 6.0 to 15.5 years old.Nephrotic syndrome(44.0%) was the most common clinical manifestation.Fifty-nine renal biopsies were performed.Class Ⅳ LN(59.3%) was the most frequent pathological findings."A full-house pattern" on immunofluorescence was found in 72.0% of biopsies.The clinical and pathological manifestations of some children were atypical.There were 3 patients characterized by predominant deposits of immunoglobulin M(IgM),1 patient with predominant deposits of IgA,and 2 children with pauci-immune LN.Three children with class Ⅱ LN in our study presented with nephrotic syndrome.LN was initially controlled by aggressive treatment in 93.7% of thse patients.Relapses of nephritis covered 27.1% of them,mostly caused by the intermittent treatment.Conclusions The clinical and pathological manifestations of LN were variable.Some atypical LN was considered to be associated with the distinct pathogenesis.Most of LN could be controlled by aggressive treatment.Long and regular treatment is necessary to improve the prognosis of LN.

It is a trend that innovate the traditional bilingual education model and select a new teaching model.Code-switching is a lingual phenomenon when a passages or articles are expressed with two or more language.To guarantee effect of bilingual education and improve education quality,penetration bilingual education was applied during microbiology teaching.Professional English vocabulary,words,passages or articles were introduced to students timely and by measure by the way of language code-switching.The results showed that bilingual teaching mode with language code-switching inspire study emotion and self-confidence of English expression from students.
10.Effects of nitric oxide on peritoneal lymphatic stomata and lymph drainage via NO-cGMP-Ca2+ pathway.

To study the cell signal transduction mechanism of nitric oxide (NO) on the peritoneal lymphatic stomata and lymph drainage in the rat, cGMP content were measured by a commercially available radioimmunoassay kit, and the [Ca(2+)](i) were observed by a confocal laser scanning microscope in the cultured peritoneal mesothelial cell. Animal experiment was practiced to study the effect of NO-cGMP-Ca(2+) pathway on the lymphatic stomata and lymph absorption. The results showed that: (1) Sper/NO increased cGMP of the rat peritoneal mesothelial cell (RPMC) in a dose-dependent manner (P<0.01) compared to the control group. This effect was blocked by 1H-[1,2,4] oxadiazolo [4,3-a] quinoxalin-1-one (ODQ) (P<0.05), a specific inhibitor of soluble guanylyl cyclase (sGC). The level of [Ca(2+)](i) in single RPMC decreased by adding Sper/NO (P<0.05). Pretreatment with ODQ for 10 min blocked the Sper/NO-induced decrease in [Ca(2+)](i). L-typed calcium channel blocker nifedipine induced an immediate and marked decrease in [Ca(2+)](i) (P<0.05).. After [Ca(2+)](i) reached a balance again, adding Sper/NO could not change [Ca(2+)](i) (P>0.05). (2) Sper/NO increased the area of the stomata (P<0.01) and the quantity of the tracer in a dose-dependent manner (P<0.05) compared to the control group. Pretreatment with ODQ significantly inhibited Sper/NO-induced change of lymphatic stomata and lymph drainage (P<0.01). Nifedipine increased the opening area of the lymphatic stomata (P< 0.01) and the concentration of absorbed trypan blue of the diaphragm (P<0.05). Sper/NO could not make a further change in the samples pretreated by nifedipine (P> 0.05). The results indicate that NO can decrease [Ca(2+)](i) in the RPMC through the NO-cGMP pathway. This procession is related with the L- type voltage-gated Ca(2+) channel. NO enlarges the opening area of the lymphatic stomata and enhances the lymph drainage of tracer by NO-cGMP-[Ca(2+)](i) pathway.
