Objective To explore the clinical application of sural neurovascular flap with anastomosing small saphenous vein to be transferred distally for repairing soft tissue defect of the leg.Methods Seven patients with soft tissue defects in the distal one third of anterior tibia,the foot dorsum,the heel or the ankle were treated with the distally based sural neurovascular flap with anastomosing small saphenous vein.The size of the flap ranged 8 cm?6 cm ~ 18 cm?10 cm.Results The flaps survived in all the 7 patients,with excellent color and texture.Their appearance and function were satisfactory during 6~40 months of follow-up. Conclusions Use of distally based sural neurovascular flap with anastomosing small saphenous vein is reliable.The performance is simple.This flap has a rich blood supply without the sacrifice of major arteries.Venous blood reflux was unobstructed.It can be used for the reconstruction of soft tissue defects in the distal one third of anterior tibia,the foot dorsum,the heel or the ankle.

Objective To investigate the role of mitochondrial KATP channel in the mechanism of the protective effect of ischemic preconditioning (IP) against ischemia-reperfusion (I/R) injury.Methods Forty-eight Wistar rats of both sexs weighing 250-350 g were used in this study. Forty rats were randomly divided into 5 groups ( n = 8 each): group A I/R; group B IP+ I/R; group C diazoxide (DZ mito-KATP channel activator) + I/R; group D 5-HD (mito-KATP channel blocker) + IP + I/R and group E 5-HD + DZ + I/R. Another 8 animals were used for electron microscopic examination of normal mitochondria as control. The animals were anesthetized with intraperitoneal pentobarbital 30 mg?kg-1. The hearts were immediately excised and passively perfused in a Langendorff apparatus with K-H solution at 5.8 kPa perfusion pressure and 36.5-37.5℃ via aortic cannulation. A fluid-filled latex balloon was via left atrium in left ventricle for the measurement of left ventricular function. I/R was induced after 30 min stabilization by clamping aortic cannula for 40 min followed by 30 min reperfusion. In group B and D the isolated hearts underwent 2 episodes of 5 min ischemia followed by 5 min reperfusion before I/R. In group C and E DZ 50 ?mol?L-1 was infused for 10 min and in group D and E 5-HD 100 ?mol?L-1 was infused for 10 min before I/R. HR, LVSP, LVEDP and coronary flow (CF) were measured at the end of stabilization (T0 , baseline), immediately before I/R (T1 ) and at 10, 20 and 30 min of reperfusion (T2.3.4.), and left ventricular developed pressure (LVDP= LVSP- LVEDP) was calculated. Myocardial tissue was obtained at the end of 30 min reperfusion for electron microscopic examination of mitochondria. Mitochondrial ultrastructure was assessed by Flameng scoring system (0 = normal, 4 = severely damaged) .Results Ischemic and DZ preconditioning significantly increased LVDP and decreased LVEDP and Flameng score. 5-HD pretreatment partly antagonized the protective effect of IP and completely antagonized that of DZ against I/R injury. Conclusion Ischemic preconditioning protects the heart against I/R injury mainly by activating mitochondrial KATP channel.

Objective To study the molecular identification method of Candida species and lay the foundation of molecular diagnosis for deep candidiasis. Methods PCR was used to amplify the internal transcribed spacer region of ribosomal DNA from 11 type strains of 9 pathogenic Candida spp. and 39 clinical Candida isolates, and the products were digested with three types of restriction endonucleases (MspⅠ,DdeⅠand HaeⅢ). Results Five patterns of products with different molecular weights were obtained after amplification from 9 Candida spp., and the products were digested with MspⅠ, DdeⅠand HaeⅢ, producing 8, 5 and 4 unique band patterns, respectively. Conclusion PCR-RFLP is reliable, specific and useful for the identification of common pathogenic Candida species.

Objective　To assess the CT manifestations and diagnostic value in the pancreatic tuberculosis(PTB)with review of the literatures. Methods　All cases of PTB proved by surgery or biopsy were examined with plain and enhanced CT scans. Results　The CT findings in one case with multiple-nodular type of PTB were diffuse enlargement of the pancreas with multiple, nodular, and low-density lesions; The nodular lesions had peripheral enhancement. 7 cases of local type of PTB encroached on pancreatic head. 4 cases showed local soft tissue masses with multiple flecked calcifications in 2 cases and mild enhancement in one case; Cystic masses was found in 2 cases, with mural calcification in 1 case and multiloculated cystic mass in 1 case, respectively; Massive pancreatic head calcification was demonstrated in one case. In these 8 cases of PTB, the lesion extended out of pancreas in 4 cases, including abdominal tuberculous lymph nodes, tuberculous peritonitis, and hepatosplenic tuberculosis. Conclusion　CT findings of PTB were various but had some characteristics. Pancreatic masses with multiple flecked calcification or mild enhancement could suggest the diagnosis. Abdominal tuberculosis accompanied with the pancreatic lesion, especially tuberculous lymph nodes, was highly suggestive of the diagnosis of PTB.

ObjectiveTo observe effects of modulated medium frequency low frequency current hydroelectric bath therapy on sequela of ankle fracture.Methods32 sequela patients of ankle fracture were treated with ZM-C type intellectual faculties intermediate frequency therapeutics instrument. Modulated medium frequency square all wave,the frequency of carry wave was 4 KHz,and the frequency of modulated wave was 100 Hz. The water temperature was 40℃. 30 minutes was a time,12 times was a course. Objective To observe the clinical efficacy of triple needling at myofascial trigger points plus warm needling in treating periarthritis of shoulder. Method Eighty patients with periarthritis of shoulder were randomized into a treatment group of 44 cases and a control group of 40 cases. The treatment group was intervened by triple needling at myofascial trigger points plus warm needling, while the control group was by warm needling alone. The two groups were treated once a day, 10 times as a treatment course. The Pain Rating Index (PRI), Visual Analogue Scale (VAS), Present Pain Intensity (PPI), and motion of shoulder joint were observed before the treatment and after 2 treatment courses, and the clinical efficacies were compared between the two groups.Result In the treatment group, the PRI, VAS, and PPI scores respectively after 1 and 2 treatment courses were significantly different from those before the treatment (P<0.05,P<0.01). In the control group, the scores after 2 treatment courses were significantly different from those before the treatment (P<0.05). After 2 treatment courses, the scores in the treatment group were significantly different from those in the control group (P<0.01). In both groups, the motions of shoulder joint respectively after 1 and 2 treatment courses were significantly changed compared with those before the treatment (P<0.01). The motions of shoulder joint in the treatment group were significantly different from those in the control group respectively after 1 and 2 treatment courses (P<0.05,P<0.01). The recovery plus markedly-effective rate and total effective rate were respectively 70.5% and 95.5% in the treatment group, versus 52.5% and 90.0% in the control group, and the between-group differences were statistically significant (P<0.01,P<0.05).Conclusion Triple needling at myofascial trigger points plus warm needling is an effective approach in treating periarthritis of shoulder.

OBJECTIVE To study the distributive properties and susceptibility of yeasts to six antifungal agents. METHODS To analyze the distributive properties of 264 clinical Candida spp isolates and study the susceptibility to amphotericin B,nystatin,fluconazole,ketoconazole,miconazole and clotrimazole.The susceptibility of yeasts was tested according to the National Committee for Clinical Laboratory Standards guideline(NCCLS M27-A2). RESULTS Strains of Candida albicans were the most frequent organism isolated accounted for 62.5% of all the isolates.C.tropicalis,C.glabrata,and C.parapsilosis accounted for 20.8%,12.5%,and 1.9%,the others accounted for only 2.3%.The main infected organs were lungs,urinary tract,and digestive tract;the susceptibility tests showed strains of Candida spp to nystatin,amphotericin B,and fluconazole were more active than to the other antifungal agents.The resistance to triazole antifugal agents could be shown. CONCLUSIONS We should strengthen the diagnosis of Candida spp and strengthen the surveillance on susceptibility of clinical isolates Candida spp so as to help the doctors choose the antifungal agents reasonably.
