1. Clinical survey of cancer-related fatigue in patients with advanced non-small cell lung cancer

Objective: To examine the incidence of cancer-related fatigue in patients with advanced nonsmallcell lung cancer (NSCLC) and explore the relationship between chemotherapy and cancer-relatedfatigue. Methods: One hundred and twenty patients with advanced NSCLC were enrolled between January2009 and December 2009. All patients received four cycles of platinum-based doublet chemotherapy, andthen they were followed-up for one year. The fatigue score was evaluated before chemotherapy, every 2cycles during the period of chemotherapy and every three months during the follow-up by using fatiguenumerical rating scale. Results: In 120 patients with advanced NSCLC, 29 (24.2%) patients had mild fatigue,64 (53.3%) patients had moderate fatigue, and 27 (22.5%) patients had severe fatigue. The average cancer-relatedfatigue score was increased progressively during chemotherapy and reached the highest value of 5.31after 4 cycles of chemotherapy, and then it tended to decline and touch the lowest value of 3.95 after six-monthfollow-up. The Eastern Cooperative Oncology Group performance status (ECOG PS) score is the onlyone factor related to cancer-related fatigue (P<0.01), and the age, gender, pathology and clinical stagewere not associated with cancer-related fatigue. The fatigue score was decreased in patients with stabledisease (4.26±2.27 vs 3.64±1.75, P<0.01) or partial response (4.27±2.28 vs 2.33plusmn;1.50, P<0.01) after three-month follow-up as compared with that before chemotherapy, whereas the fatigue score was increased in patients with progressive disease (4.48±2.09 vs 5.00±1.75, P<0.01). Conclusion: The cancer-related fatigue occurred in advanced NSCLC with a very high incidence, and the chemotherapy can strengthen the intensity of cancer-related fatigue, which is related to ECOG PS score and response to chemotherapy. 2. Polymorphism of adinoponectin gene in pedigree of type 2 diabetes mellitus

Objective: To investigate the correlation between the single nucleotide polymorphism (SNP) of adiponectin (APM1) gene and type 2 diabetes mellitus. Methods: Three common binding sites (-11377C>G, +45T>G, and +276G> T) of SNP on the APM1 gene were genotyped by polymerase chain reaction-restriction fragment length polymorphism (PCR-RFLP) assay in 75 pedigrees (337 individuals) of type 2 diabetes mellitus. Genehunt software was used to analyze the transmission-disequilibrium (TDT) and to calculate SNP combining haplotypes. Meanwhile, the physiological and biochemical parameters were also determined in the pedigrees of type 2 diabetes mellitus. Results: We found no preferential transmission in the tested binding sites or any haplotypes of SNP in the APM1 gene in the filial generation of type 2 diabetes mellitus. In type 2 diabetes mellitus group, GG genotype had higher body mass index (BMD (P=O.032) and waist circumference (WC) (P= 0.030) compared to CC genotype in the patients with SNP-11377 binding site; patients with G allele had lower levels of high-density lipoprotein cholesterol (HDL-C) (P = 0.006) and higher levels of fasting plasma insulin (FINS) (P = 0.011) as compared to CC genotype. However, FINS levels (P=O.021) in subjects with CC genotype were significantly lower than those with G allele in healthy control group. For patients with SNP+45 binding site, those with GG genotypes had higher BMI (P= 0.036) and lower levels of FINS (P = 0.014) than those with TT genotypes in both groups. For patients with SNP + 276 binding site, those with GG genotypes had higher BMI(P = 0.043) than those with T allele in the control group. Conclusion: The SNP of the APM1 gene is not associated with the pedigrees of type 2 diabetes mellitus, but APMl gene has influence on the function of insulin B cells and the development of obesity.
3.Air Particle Concentration and Meteorological Factors

Many researches have shown that atmosphere pollution and meteorological factors are related closely. The relations between the particle concentration in atmosphere and meteorological factors including wind speed,wind direction,air pressure,temperature,humidity,precipitation and atmosphere stability vary in different researches. Taking into consideration the complexity of meteorological factors and the components and concentration index of particles,it is more reasonable to do multi-factor analysis. But the results of multi-factor analysis also show obvious diversity and complexity due to different research objects,methods and measuring indexes. Therefore,different models for the prediction of particle concentration are needed for the factors such as area,time and meteorological conditions always change.
4.The effect of patient participation in humanistic nursing on quality of life and psychology of patients with gastric cancer

Objective To explore the effect of participatory humanistic nursing on the quality of life and psychology of patients with gastric cancer. Methods From May 2015 to May 2016, 68 patients with gastric cancer were enrolled in the study. All patients were divided into the experimental group and the control group with 34 patients in each group according to the principle of double-blind randomization. Patients in the control group were given routine clinical nursing, and patients in the experimental group were given the participatory humanistic nursing care. The quality of life and psychological state were evaluated by European Organization for Research and Treatment of Cancer Quality of Life Questionnaire and Symptom Checklist 90 edited by Derogatis and compared before nursing and after 6 months of nursing. Results There was no significant difference in the quality of life before nursing between two groups (P >0.05). After nursing, the scores of physical function, role function, emotional function, cognitive function, social function of the quality of life were 57.85±4.64, 68.43±5.23, 61.43±4.97, 58.47± 6.12, 53.57±5.54 in the experimental group and 43.56±4.56, 54.67±4.67, 56.34±4.74, 51.34±4.13, 50.46± 4.27 in the control group,and there were significant differences (t=2.593-12.808,P<0.01 or 0.05). There was no significant difference in the psychological state before nursing between two groups (P>0.05). After nursing, the scores of interpersonal sensitivity, depression, anxiety, hostility, terror, paranoia, psychosis of the psychological state were 1.65 ± 0.17, 1.40 ± 0.15, 1.45 ± 0.14, 1.46 ± 0.12, 1.52 ± 0.16, 1.54 ± 0.10, 1.43 ± 0.11 in the experimental group and 1.75±0.21, 1.72±0.24, 1.63±0.21, 1.75±0.23, 1.71±0.24, 1.65±0.10, 1.56 ± 0.12 in the control group, and there were significant differences (t=2.158-6.593, P<0.05 or 0.01). Conclusions Participating humanistic care can improve the quality of life and improve the psychological state, and the clinical effect is significant, it is worthy of being popularized and applied.

6.Analysis of tobacco use and associated factors among migrants in construction sites of Western China

Objective:To investigate the behavior of tobacco use among migrants in construction sites and explore the associated factors .Methods: A total of 652 migrants in 10 construction sites were selected in Xi’ an and Tongchuan .Chi-square and logistic regression were used to analyze the influencing factors of tobacco use.Results:The average age of the migrants in the construction site was (38.23 ± 10.61), and males occupied 82.7%(535/647) of the total.The current smoking rate of the migrants was 55.8%(364/652), with 64.3%(344/535) in males and 14.3%(16/112) in females.82.5%(329/399) smokers wanted to quit smoke , however only 52.7%(210/399) had tried quitting smoking in action, and 8.8%(35/399) quitted smoking successfully .Multivariable regression indicated that the migrants who were at lower age , and sick within 2 weeks, had perceived not difficult to stop smoking and who disagree with the benefits of smoking were more likely to try to quit smoking .Conclusion:Migrants in construction sites show their characteristics of old age , low level of health literacy , male-domination , high smoking rate and high intention of quitting smoking .Tobacco control projects should be implemented in construction sites to promote the translation of smoking quitting intention into action .

8.Insulin induces PKC-dependent proliferation of mesenteric vascular smooth muscle cells from hypertensive patients

Background and objectives Proliferation of human vascular smooth muscle cells (VSMCs) induced by hyperinsulinemia is a very common clinical pathology. Extensive research has focused on PKC (Protein kinase C)-MAPK (mitogen-activated protein kinase)intracellular signal transduction and the phenotypic modulation accompanied by reorganization of intracellular F-actins in VSMCs.Methods DNA synthesis, signaling of ERK1/2 MAPKs, and changes in α-smooth muscle (SM) actin and F-actin were studied in hypertensive and normotensive human arterial VSMCs exposed to insulin and PMA with and without the PKC inhibitor, GF109203X.Results Differences among cell types in MAPK signaling, α-SM actin, and F-actin isoforms in VSMCs harvested from the arteries of patients with essential hypertension (EH) and normotension (NT) were identified in response to insulin treatment. Proliferation and activation of MAPK were more pronounced in EH VSMCs than in NEH VSMCs. Insulin exposure decreased expression of α-SM actin and was accompanied by rearrangement of intracellular F-actins in VSMCs, especially in the EH group. These effects were reversed by treatment with the PKC inhibitor. Conclusions Human mesenteric VSMCs of EH and NT patients differed in proliferation, MAPK signaling, and degree of changes in α-SM actin and F-actin isoforms immediately following insulin exposure in vitro.
9.The correlation of cerebral perfusion characteristics and cognitive function on patients with unipolar and bipolar depression

Objective To analyze the correlation of cerebral perfusion characteristics and cognitive function on patients with unipolar and bipolar depression. Methods The r-CBF imaging was detected by SPECT forall subjects. Evaluated cognitive function of subjects using WAIS-CR and WMA, and analysed data using SPSS.Results Data showed that some regions in the brain, e.g. temporal lobe, frontal lobe, hippocampus, and basal ganglia, had significant alterations of r-CBF in depressed patients as compared with healthy controls (P < 0. 05 ).Compared with UPD, BPD showed increased perfusion in basal ganglia ( 0,22.73%; 4.54%, 36. 36% ) (P <0.05 ). Furthermore there was no significant different incognitive deficits between UPD and BPD(P > 0. 05 ) , and correlations between cognitive impairment and hypoperfusion of rCBF (P < 0. 05 ). Conclusion There be exist difference in r-CBF between UPD and BPD. The perfusion of r-CBF in BPD is significantly increased than that in UPD,and it suggest that hyperperfusion in basal ganglia may be the biological basis when bipolar depression turning to manic.
10.Application of classic cases in teaching for seven-year program pediatric majors

Studying classic cases is one of the applicable methods for student's self-learning,especially at clinical rotation period.Seven-year program pediatric students (2004 grade) from pediatric medical collage,Capital Medical University of China shared classic cases from New England Journal of Medicine (NEJM),reported clinical cases they took part in and summarized their writing of case report.Through these measures,students felt the significance of studying classic cases and noticed that case study played an important role in training of clinical reasoning,multidisciplinary approach,as well as self-learning ability for medical students.
