Objective To investigate the value of BISAP scoring system in predicting the severity of acute pancreatitis.Methods Medline,EMBASE,Science Direct,Springer link,CBM,Cnki,Wan fang and VIP database were retrieved by computer between January 2000 and March 2013,and articles of BISAP score system in predicting the acute pancreatitis were collected according to strict inclusion criteria.Quality assessment was made by QUADAS scale.Meta-Disc1.4 software was used to analyze the heterogeneity of included articles and perform quantitative synthesis,as well as calculate the pooled sensitivity and specificity,positive likelihood ratio,negative likelihood ratio,and draw the ROC curve,and the results were presented with 95％ CI.Results A total of 11 articles were included,and there were 7 Chinese articles and 4 English articles.According to QUADAS scale,there were 4 articles of A grade,5 articles of B grade,2 articles of C grade.There were 6 studies using 2 as BISAP cut-off value,and 9 articles using 3 as cut-off value,while 4 studies using 2 cut-off values.Pooled analysis showed diagnostic odds ratio of 8.03 (95 ％ CI 5.66-11.38) when cut-off value was 2,and diagnostic odds ratio of 7.49 (95％ CI 5.35-10.49) when cut-off value was 3.There were moderate heterogeneity in both groups (I2 =63.3 ％,P =0.018; I2 =56.1％,P =0.019).When BISAP cut-off value was 2,the pooled analysis of sensitivity,specificity,positive likelihood ratio,negative likelihood ratio and area under curve (AUC) were 59％ (95％ CI56％-63％),82％ (95％ CI 80％-83％),3.50 (95％ CI2.96-4.14),0.45 (95％ CI0.36-0.56) and0.82; and when BISAP cut-off value was 3,the corresponding values were 44％ (95％ CI41％-47％),90％ (95％ CI 89％-91％),4.59 (95％ CI 3.31-6.37),0.64 (95％ CI 0.61-0.68) and 0.64.The former had a high sensitivity,low specificity and large AUC,while the latter had a low sensitivity,high specificity and small AUC.Conclusions The best cut-off value of BISAP in predicting SAP is 2.When cut-off value is 2,the misdiagnosis rate is low,and the false positive rate is in the acceptable range,which is suitable for clinical application.

Objective To statistically analyze the 128 cases of thumb abductor defects and evaluate the commonly used operative methods. Methods Statistical analysis and evaluation were done in regard to the muscle strength, injury cause, injured parts, injured nerves and tendon transfer. Results We found by following up after the operation: FDS, PL, ECU were the most commonly used operative methods and their average excellent and good rate was 87.2%. Abductor digiti quinti and flexor pollicis brevis were less used but their effects were good. Conclusion When the median nerve of thenar is injured, the abductor of the thumb will be damaged. We should restore its function by tendon transfer as long as the muscle strength remains and the passive motion of the carpometacarpal joint is good.

Four cases of pituitary acromegalic cardiomyopathy were reported and relative literature was reviewed in this article.Four patients were all males aged 45-63 y.The patients were diagnosed as pituitary adenoma,then NYHA grade Ⅲ-Ⅳ heart failure developed.One patient had both acromegaly and coronary heart disease,and all patients had arrhythmia attacks.The growth hormone levels were not under control in 3 patients,and for the remaining patient with normalized growth hormone the end-stage heart failure also developed.The onset of pituitary acromegalic cardiomyopathy is hidden; when the disease progresses to advanced heart failure,the prognosis would be poor in most cases.So early diagnosis and timely treatment is of great importance in acromegalic cardiomyopathy.

Purpose　The aim is to develop a new method for the determination of the activity of hepatocyte growth factor (HGF). Met hods　 Rat hepatocytes were prep ared by collagenase perfusion and incubated at 5% CO2 incubator for 1.5 h. The n HGF was added into the culture medium and incubated at 5% CO2 incubator for 1.5 h. The cell viability was determined by MTT method.Results　～1 142 μg/ml improved the number of cell viability significantly （AHGF/Acontrol） ＞2.Conclusion　Th is new method was stable, time-saving and of good reappearance. It was adapted to the determination of HGF activity.

Objective To investigate the impact of umbilical cord around the neck(UCAN) on infants developmental quotient. Methods Intelligence of 60 infants of UCAN under 3 years old were tested by Gesell. Results DQ of UCAN infants was significantly less than normal infants,mainly mild to moderate retardation. DQ between the male and female infants had no significant difference, parents' cognitive level had significant difference with real DQ of infants. Conclusions UCAN has certain influence on mental development of infants,we should strengthen health education,pay close attention to UCAN infants,give regular intelligence surveillance, find early growth retardation,give rehabilitation training to reduce the incidence of intellectual disabilities.

Objective To evaluate the protective effect of Caspase-1 inhibitor on acute lung injury(ALI) induced by experimental severe acute pancreatitis(SAP).Methods Forty-two SD rats were randomly assigned into 3 groups: healthy controls group(HC,n=6),SAP+normal saline group(SAP-S,n=18) and SAP+ICE inhibitor group(SAP-ICE-I,n=18).SAP was reproduced by retrograde injection of 5% sodium taurocholate into the pancreaticobiliary duct in SD rats.In animals in HC group same surgical procedure and duct cannulation were performed but without introduction of sodium taurocholate.In SAP-S group,rats received the first intraperitoneal injection of isotonic saline 2h after induction of acute pancreatitis,and the injection of saline was repeated at the 12th hour after induction.In SAP-ICE-I group,rats were given ICE inhibitor intraperitoneally 2h after induction of pancreatitis.Surviving rats were sacrificed at designated time points,and blood and all organs were obtained for subsequent analysis.Serum IL-1? was measured by ELISA.Intrapulmonary expressions of Caspase-1,IL-1? and IL-18 mRNA were detected by semi-quantitative RT-PCR.The ratios of wet/dry weight and histopathological changes in lung were also evaluated.Results The serum IL-1? levels in SAP-S group were 276.77?44.92pg/ml at 6h,308.99?34.95pg/ml at 12h and 311.60?46.51pg/ml at 18h after induction of SAP,respectively,which were increased significantly compared with that in HC group(P0.05).The wet/dry weight ratio of lung in SAP-S group was increased significantly compared with that in HC group(P

Objective To evaluate peri-operative clinical effects of transurethral electrovaporization of the prostate(TUVP) combined with transurethral resection of the prostate(TURP) versus open prostatectomy for benign prostatic hyperplasia(BPH).Methods A total of 156 cases of BPH were surgically treated in this hospital from November 1997 to December 2004,including 53 cases of TUVP combined with TURP(Combination Group) and 103 cases of open suprapublic prostatectomy(Open Group). Results There were no significant differences in the operation time between the two groups(90?70 min vs 108?68 min;t=(-1.550),P=0.123).The Combination Group was superior to the Open Group in the blood transfusion rate(8% vs 38%;?~2=16.109,P=0.000),the time to clear urine(2?1 d vs 4?1 d;t=-11.831,P=0.000),the postoperative catheterization time(10?8 d vs 19?10 d;t=-5.680,P=0.000),the time to get out of bed(6?4 d vs 20?10 d;t=-9.785,P=0.000),and the postoperative hospital stay(20?14 d vs 34?24 d;t=-3.914,P=0.000).The incidence of complications was higher in the Combination Group than in the Open Group(15.1% vs 2.9%;?~2=6.173,P=0.013).Conclusions TUVP in combination with TURP for benign prostate hyperplasia has advantages of little invasion and rapid recovery.

Background Diabetes mellitus (DM) can provoke the apoptosis of retinal cells and downregulate the expression of calcitonin gene related peptide (CGRP) in the retina.Capsaicin promotes the release of CGRP and elicits protective effects on human organs.However,whether CGRP protects retinal cells in diabetic retinopathy (DR) is still unclear.Objective The study was designed to examine the effect of capsaicin on the apoptosis of retinal cells in diabetic rats and its relationship with CGRP.Methods Forty clean healthy adult male Sprague-Dawey rats were randomly divided into the diabetes group,capsaicin pretreated group,streptozocin (STZ)control group,capsaicin control group and plain control group,with 8 rats per group.The diabetic model was established by the intraperitoneal injection of 60 mg/kg in all rats except those of the plain control group.0.4 mL of a 1％ capsaicin injected at 20 mg/kg was subcutaneously injected for 3 consecutive days prior to model establishment in the capsaicin pretreated group,after which 1.2 mL of STZ was intraperitoneally injected on the fourth day.Rats from the STZ control group were administered intraperitoneally 1.2 mL of 0.1 mol/L,pH 4.5,citrate buffer.The capsaicin control group received subcutaneous injections of 0.4 mL of 1％ capsaicin at 20 mg/kg for 3 consecutive days,after which 1.2 mL of 0.1 mol/L,pH 4.5,citrate buffer was administered intraperitoneally.The rats were sacrificed at the tenth week after model establishment and retinal specimens were prepared for the apoptosis assay by TUNEL staining and the quantitative analysis of caspase-3 activity.Expression of CGRP in the retina and serum was detected using ELISA.The use of experimental animals followed the Regulations for the Administration of Affairs Concerning Experimental Animals by State Science and Technology Commission.Results Retinal cell apoptosis was mainly localized to the retinal ganglion cell (RGC) layer.The apoptosis rate of RGCs was (43.4±5.0)％ in the DR model group and (30.0±5.1)％ in the capsaicin pretreated group,showing a significant difference (t =5.930,P＜0.01).Compared with the DR model group and capsaicin pretreated group,the apoptosis rates of the DR control group (12.4±9.9) ％ and the capsaicin control group (17.6-±6.1) ％ were significantly lower (t =8.800,t =4.925,P＜0.01).The apoptosis rate of the plain control group was (16.2±6.9)％,exhibiting significant differences in comparison with the DR control group and capsaicin control group (t =-0.989,t =0.951,P＞0.05).The specific activity of caspase-3 was (2.19±0.86) in the DR model group and (1.96±0.56) in the capsaicin pretreated group,presenting a significant difference (t =-0.515,P＜0.05).Those of the DR control group and capsaicin control group were (1.47±0.14) and (0.74±0.27),respectively,with considerable decline in comparison with the DR model group and capsaicin pretreated group (t=2.142,t=2.797,P＜0.05).The retinal and serum CGRP levels were (424.4±44.2)and (148.8±39.1) ng/L,respectively,displaying significantly lower levels than (543.2±74.4) and (237.5±78.7) ng/L (t =3.070,2.359,P＜0.05) from the capsaicin pretreated group.Conclusions Apoptosis of retinal ganglion cells occurs in the STZ-induced diabetic rats.Pretreatment of capsaicin reduces retinal cell apoptosis,which may be associated with an increase of CGRP in the retina.
