1.X-ray Features of Heart and Lung in Chronic Renal Failure

Objective To improve the knowledge of X-ray findings of heart and lung in chronic renal failure ( CRF ) . Methods X-ray findings of heart and lung in 120 cases with CRF were analysed retrospectively.Results The X-ray appearances of lung and heart in CRF included augment and blur of hilum, increased, tortuous and disorder lung markings, accretion of heart, and the abnormal shadows in lung were irregular, inhomogeneus and changing fast in morphology. Conclusion Radiography can supply valuable messages and is ofconsiderable value in the diagnosis of CRF.
2.Effects of excessive weight gain during pregnancy on offspring health: a review

Excessive gestational weight gain has already become a global clinical and public health problem that seriously affects maternal health. Excessive gestational weight gain not only increases the cesarean section rate and induces adverse pregnant outcomes, but also affects offspring development and health. This article reviews the effects of excessive weight gain during pregnancy on offspring health and its underlying mechanisms. Excessive gestational weight gain may increase the risk of obesity, cardiovascular diseases, infectious diseases of the respiratory tract, diabetes, polycystic ovary syndrome, mental or psychological illness among offspring, and the pathophysiological mechanisms include inflammatory response, intestinal flora dysbiosis and epigenetics theory. However, further studies are required to validate these hypotheses and to evaluate the effect of excessive weight gain at different gestational stages on offspring health, so as to provide insights into reasonable management of weight gain during pregnancy and improvements of offspring health.
3.Morphological characteristics of Barrett's esophagus under confocal laser endomicroscopy

Objective To explore the morphological characteristics of Barrett's esophagus under confocal laser endomicroscopy (CLE).Methods A total of 112 patients with suspected Barrett's esophagus (BE) under endoscopy were enrolled in this study.The lower esophageal columnar mucosa was examined with CLE to real-time forecast the histological type and extent of intestinal metaplasia of BE,followed by mucosal biopsies and histopathological examination in inspected area.The relationship between the features under common endoscopy or CLE and histopathological diagnosis was analyzed.Results A total of 94 patients were diagnosed as BE.Island type is the most common type under common endoscopy (54.3%),and the incidence of short-segment BE was higher than that of longsegment BE (86.2 % vs 13.8 %).Specialized intestinal metaplasia,cardiac and fundic epithelium can be distinguished under CLE,and the accuracy was 94.7%,89.4% and 91.5% respectively.The histopathological examination in targeted biopsy specimens indicated that the proportion of intestinal metaplasia was higher in long-segment BE (9/13) than that of short-segment BE (33.3 %,x2 =4.684,P=0.039),and the proportion of intestinal metaplasia was higher in circumferential(11/17) and tongue-like BE (65.4%) than that of island type (15.7%,x2 =15.217 and 19.399 respectively,P<0.01).There were no significant difference in the extent of intestinal metaplasia among island type,circumferential type and tongue-like type in short-segment and long-segment BE.Conclusions The histological type of BE can be accurately distinguished with CLE.The most common type of BE is island type.The incidence of intestinal metaplasia is higher in circumferential and tongue-like type.The incidence of intestinal metaplasia is higher in long-segment BE than that of short-segment BE.
4.Changes of S-opsin expression in guinea pigs with flickering light-induced and form-deprived myopia

Objective To observe the changes of S-opsin expression in guinea pigs with flickering light-induced and form-deprived myopia,and to investigate the causes.Methods Thirty-six two-week-old healthy guinea pigs were randomly assigned to three groups:Flickering light-induced myopia group(FLM group,n=13),form-deprived myopia group(FDM group,n=12) and control group(n=11).For the FLM group,the cages were equipped with astroboscope(0.5 Hz),and LEDs were used as the light source.The right eyes of the guinea pigs in FDM group wore translucent goggles which did not interfere with the normal activity of their eyelids.No special treatment was given to the guinea pigs in the normal groups.All measurements were performed prior to and then after 6 weeks of treatment.The first measurement day was recorded as 0 week.Biological parameters,such as the refraction,axial length(AL) and corneal radius of curvature(CRC),were measured and fundus photography is performed before and after 6 weeks of the treatment.The expression of S-opsin was observed and analyzed by immunofluorescence technique and image analysis system.Results Before the treatment,no significant difference was found in three biometric measurements including refraction,AL and CRC between the groups at 0 week(P>0.05).After the treatment for 6 weeks,significant differences were found in changes of both the biometric measurements between the FLM and control groups,and between the FDM and control groups(P<0.05).However,no significant differences were found in the changes of CRC among the FLM,FDM and control groups(P=0.358),indicating that myopia models were established successfully.No significant differences were found in the changes of values of refraction,AL and CRC between the FLM and FDM groups(P>0.05).Expression of S-opsin differed in the FLM and FDM groups.For the mean gray values of green channel,compared with the control group respectively,significant differences were found in both the FLM and FDM group(P<0.001).The mean gray value of green channel of the FLM group was higher,however the mean gray value of green channel of the FDM group was lower.Conclusions Both guinea pig models of flickering light-induced and form-deprived myopia can be established successfully.S-opsin is increased in the flickering light-induced myopia model and decreased in the form-deprived myopia model,indicating that the mechanisms of formation of these two experimental myopia models may be different.
5.Confocal laser endomicroscopy in pathological classifications of gastric cancer in vivo

Objective To establish diagnostic standards of confocal laser endomiroscopy (CLE) in pathological classifications of gastric cancer and evaluate the diagnostic value of CLE in vivo. Methods The confocal images of 36 patients, who were diagnosed as gastric cancer with confirmation of pathology, were collected and analyzed to establish diagnostic standards of CLE for pathological classifications of gastric cancer. The sensitivity, specificity and accuracy of CLE were determined with reference to histopathologic findiugs retrospectively. Results According to changes of glandular architecture and microvessels under CLE, gastric cancers were divided into differentiated and undifferentiated type. The sensitivity, specificity and accuracy of CLE in diagnosing differentiated carcinoma were 85.7%, 92. 3% and 90. 5%, respectively, which were 85.7%, 92. 3% and 90. 5%, respectively, in undifferentiated carcinoma. Conclusion CLE is a new tool to make pathological classifications of gastric cancer during routine endoscopic examination procedure, which correlates well with histopathologic fingdings.
6.Confocal laser endomicroscopy for differential diagnosis of gastric ulcer

Objective To estimate the value of confocal laser endomicroscopy (CLE)for differential diagnosis of gastric ulcer. Methods Patients diagnosed as gastric ulcer by conventional endoscopic mode of CLE were enrolled. The eonfocal endoscopic mode of CLE was used to observe the borderline and/or the surface of the ulcers. Biopsy specimen was taken for histopathological examination and the result of CLE was compared with histopathological finding. Results A total of 12 769 CLE images were obtained from 150 sites of 43 patients. The sensitivity and specificity of CLE for phlegmonosis process were 83.54% and 85.92%, respectively, with positive predictive value of 86.84% and negative predictive value of 82.43%. Whereas the sensitivity and specificity of CLE for intraepithelial neoplasia were 53.57% and 88.52%, respectively, with positive predictive value of 51.72% and negative predictive value of 89.26%. As for diagnosis of gastric cancer, the sensitivity and specificity of CLE were 88.37% and 93.46%, respectively, with positive predictive value of 84.44 % and negative predictive value of 95.24%. The kappa values for agreement of histopathological findings and CLE for diagnosis of phlegmonosis process, intraepithelial neoplasia and gastric cancer were 0.69,0.42 and 0.81, respectively. Conclusions CLE can provide a real-time histopathological diagnosis of the gastrointestinal tract with high sensitivity, specificity, positive and negative predictive values. Targeted biopsy can be done under CLE and the detective rates of precancerous lesions and tumors will be increased.
8.Two new sesquiterpenoids from basidiomycete Tyromyces chioneus.

Two new sesquiterpenoids, named as tyromols A and B (1 and 2), were isolated from cultures of basidiomycete Tyromyces chioneus, along with two previously reported 15-hydroxy-6 α, 12-epoxy-7β, 10αH, 11βH-spiroax-4-ene (3) and agripilol C (4). Compounds 1-4 were separated and purified by silica gel, RP-18, Sephadex LH-20 column chromatography. Their structures were elucidated on the basis of extensive spectroscopic analysis including IR, MS, 1D and 2D NMR experiments.

9.Application of web based problem-based learning in the teaching of bilingual curriculum

Objective To investigate the effect of applying web based problem-based learning in the teaching of bilingual curriculum.Methods 2009 grade nursing undergraduate bilingual class (n =57) was taken as control group and traditional teaching method was used.2010 grade nursing undergraduate bilingual class (n =60) was taken as experimental group and bilingual course of introduction to nursing was used to do web based (Blackboard platform) PBL teaching.Teaching process of the grouping-question posing-data gathering-discussion-mid-term feedback-presentation-summary and evaluation was implemented.Result comparison method,network based PBL learning evaluation sheet and content analysis were employed to complete teaching effect evaluation.Data were analyzed by t test and P < 0.05 signifies statistically significant differences.Results Test scores of experimental group were significantly higher than that in control group (P < 0.05).Mean scores of positive items in web based PBL teaching scale were ≥3.84.Electronic learning file content analysis showed that excellent rates of 7 indicators (student participation,thinking ability,inquiry,etc) were ≥66.7%.Conclusion Web based PBL teaching model is good for improving teaching effectiveness in the teaching of bilingual curriculum.
10.Risk Factors and Precaution for Senile Diabetics Complicated with Urinary Tract Infections

OBJECTIVE To study risk factors in senile diabetics complicated with urinary tract infections and find out measures to cut down the incidence rate with urinary tract infection.METHODS The age,sex,hospital day,course of diabetes,blood sugar level and catheterization in 181 hospital patients of senile diabetics complicated with urinary tract infections from 2004 to 2007 were analyzed retrospectively.RESULTS The incidence of senile diabetics complicated with urinary tract infections was closely related with age,sex,hospitalization days,course of diabetes,blood sugar level and catheterization.CONCLUSIONS Control measure for risk factors in senile diabetics can actively cut down the incidence rate of urinary tract infection.
