1.Effect of hyperthermia-induced MMP-9 on expression of claudin-1 in in vitro blood brain barrier model

Objective To study the effect of hyperthermia-induced MMP-9 on expression of claudin-1 in in vitro BBB model.Methods BMEC and astrocytes were cultured.An in vitro BBB model was established with its permeability assayed by LY test.The BBB model was randomly divided into 37℃ group,39℃ group,37℃+MMP-9 group and 39℃+MMP-9 group.The permeability of BBB model was recorded at 30,60 and 90 min respectively.Expression of claudin-1 in BMEC was detected by Western blot with immunocytochemical staining.Results The permeability of BBB model was higher in 39℃ group than in 37℃ group (P<0.05) and was significantly higher in 39℃ +MMP-9 group than in 37℃ group (P<0.05).The number of claudin-1 positive BMEC was smaller in 39℃ +MMP-9 group than in 37℃ group at the same time points (P<0.05) and in 39℃ +MMP-9 group than in 39℃ group at 90 min (75.0±12.8/HP vs 97.0±6.1/HP,P<0.05).The expression level of claudin-1 was lower in 39℃ group than in 37℃ group (P<0.05) and was significantly higher in 39℃ +MMP-9 group than in 37℃ group (P<0.05),which further increased with the increasing time.Conclusion Hyperthermia can downregulate the expression of claudin-1 in in vitro BBB model and upregulate the permeability of in vitro BBB model.Adding MMP-9 can further aggrevate the damage of in vitro BBB model,indicating that hyperthermia can aggrevate the damage of in vitro BBB model through MMP-9.
2.Matrix metalloproteinase-9 and ischemic stroke

The pathophysiological mechanism of ischemic stroke is very complex.Many intracellular and extracellular factors play the important roles in it.Studies have shown that the expression of matrix metalloproteinase-9 (MMP-9) has the changes of quantity,time and space after stroke.These changes are closely associated with the occurrence and development of ischemic stroke.The experimental studies focusing on MMP-9 also have made corresponding effects.The in-depth study of the mechanism of action of MMP-9 is expected to provide a new approach for the diagnosis and treatment of ischemic stroke.
3.Comparison of effect of standard Gamma nail and proximal femoral nail anti-rotation for geriatric intertrochanteric fractures

[Objective]To investigate and compare the effects of standard gamma nail (SGN) and proximal femoral nail anti-rotation (PFNA) in the treatment of osteoporotic intertrochanteric fractures in the elderly.[Method]From May 2002 to September 2005,55 patients with intertrochanteric fractures were treated with SGN.Sixty-eight patients were treated with PFNA from November 2005 to June 2008.The effects of the two implants about perioperative complications,bone healing and the function of the operated hip joint were compared.[Result]No statistically significant difference was found between the two groups in blood loss,blood transfusion,infection,deep venous thrombosis and hospital stay.One patient developed femoral head avascular necrosis postoperatively in Gamma nail group.The incidence of postoperative hip pain in Gamma nail group was higher than that in the PFNA group,but there was no significant difference in the Harris score.[Conclusion]Gamma nail and PFNA nail are reliable methods to treat geriatric intertrochanteric fractures,with advantages of minimal iatrogenic injury,stable fixation and few complications.
4.Research progress of Traditional Chinese Medicine on treating primary bronchial lung cancer

The clinical effect of Traditional Chinese Medicine (TCM) in the treatment of primary bronchial lung cancer is good. To a certain extent, TCM can control the progress of the disease and improve quality of life by the treatment of syndrome differentiation, disease-modified prescription, experience prescription, Chinese patent medicine, TCM injection, acupuncture and acupoint application, which have been combined with chemotherapy. The mechanism of TCM in the treatment of primary bronchial lung cancer mainly includes inhibition of lung cancer cell proliferation, induction of lung cancer cell apoptosis, improvement of body immunity and overcoming chemotherapy resistance.
5.The Correlation Analysis between Short Term Variation and Basal Heart Rate and Short Time Monitoring Analysis

Objective: Fetal monitoring is extremely important in perinatal care. Analysis of cardiotocogragh(CTG) can suppose if the fetus is lack of oxygen, distressed, acidosis, etc. in order to take measures timely. Methods: We have designed Meticare analysis software package to analyze 867 pieces of CTG data, and researched the relationship between short term variation (STV) and basal heart rate(BHR) by statistics, and short time monitoring analysis based on STV. Results: The amplitude of fetal heart rate curve variation(long term variation) tended to decrease when BHR increasing, with the negative correlation and regression equation STV = 9.866-0.02×BHR. Positive correlation exists between 5 and 10 mimute-STV, with the regression equation STV_(10min)=2.861-0.596×STV_(5min).Conclusion: STV calculated by 5 minutes CTG data can forecast the one by 10 minutes data as a reference demanded by Dawes & Redman criterion. If fetal doppler contains STV analysis function, out-patient fetal heart auscultation will be more significant. After 30 weeks pregnancy, with the STV decreasing, fetal distress, asphyxia, low birth weight, and even mortality increasing. BHR increasing should cause attention that STV might decrease, when monitoring with no analysis functional devices or fetal dopplars.
6.Clinical effect of Zero-profile anterior cervical interbody fusion on single-segment cervical spondylosis in the older adults

Objective To investigate the clinical effect of Zero-profile anterior cervical interbody fusion on single-segment cervical spondylosis in the elderly, and to compare with the effect of traditional anterior cervical decompression and fusion (ACDF) with titanium plate.Methods Clinical data of elderly patients who underwent anterior cervical discectomy and fusion operation (one-or two-level) in our department from June 2009 to March 2014 were retrospectively analyzed.The 49 patients who took anterior cervical interbody fusion with the Zero-profile cage were considered as the Zero-P group, and the other 60 patients who received ACDF with titanium plate as control (ACDFP group).The operation time, blood loss, Neck Disability Index (NDI) score before and after operation, Cobb angle of the cervical spine and Cobb angle of the operated segment before and after operation, the fusion rate 1 year after operation, the dysphagia rate after operation according to Bazaz index were recorded and assessed.Results The operation time and blood loss had no statistical difference between the two group (P>0.05 for both).The NDI scores were declined in the two group after operation as compared with before treatment (P<0.05 for both), while there were no statistical difference in NDI score between the two groups (P>0.05).The Cobb angle of cervical spine had little improvement 1 day after operation as compared with before operation, and improved significantly 3 month after operation in the two group.The Cobb angle of operated segment (Cobb S) was reduced 3 months in the two group after operation as compared with before treatment, and the Cobb S was declined much more in Zero-P group than in control group (P<0.01).There was no statistical difference in the number of patients with successful fusion between the two group (46 cases vs.56 cases, x2 =0.08, P> 0.05).Conclusions The clinical effect of Zero-profile anterior cervical interbody fusion is equivalent as the traditional ACDFP.Zero-profile anterior cervical interbody fusion has less exposure and blood loss, which is more suitable for the elderly patients with cervical degenerative disease.
7.Postoperative bile leakage induced by pancreaticobiliary maljunction complicated with aberrant bile ducts upon gallbladder bed

Laparoscopic cholecystectomy is the gold standard for the treatment of gallstones and chronic cholecystitis.A patient with gallstone was admitted to the 88th Hospital of PLA in August 2012 and received laparoscopic cholecystectomy.She had bile leakage after operation,and had to accept reoperation.Pancreaticobiliary maljunction and aberrant duct upon gallbladder bed were found postoperatively by T-tube cholangiography and pancreatic amylase test in the bile,which led to a complicated treatment and a tortuous recovery.This article analyzed the treatment process for this disease and explore the proper diagnosis and treatment.
8.Application of 3D Gait Analysis in Rehabilitation of Amputees

The 3D gait analysis system (Vicon) was used for the gait analysis of health persons and amputees who wear lower limb prosthesis. The result indicated that gait analysis can supply changes of kinematics and biomechanics; which can be used to analyze the difference of joint movement between the health persons and amputees. The 3D gait analysis is feasible for amputees in rehabilitation.
9.Influence of pH values on different zirconia systems

BACKGROUND: There are many studies on the effect of different experimental parameters on the tetragonal phase of zirconia, but it is unclear whether pH values have effects on the tetragonal phase in hydrothermal reactionOBJECTIVE: To prepare two zirconia systems via hydrothermal method and to regulate the tetragonal phase purity and control the grain size of zirconia (ZrO2) crystal by pH values.METHODS: Analytical reagents Zr(NO3)4?5H2O and Y(NO3)3?6H2O were adopted as reactors, and nano-sized neat ZrO2 and yttria-stabilized tetragonal ZrO2 polycrystal (Y-TZP) powders were synthesized using the hydrothermal method. X-ray diffraction was adopted to characterize the influence of pH values and pH regulators (NaOH solution and ammonium hydroxide/NaOH solution) on phase purity and grain size of tetragonal zirconia.RESULTS AND CONCLUSION: Pure zirconia samples with a grain size lower than 20 nm were obtained through the hydrothermal method, and samples with higher tetragonal phase content compared to monoclinic phase could be derived when the pH value was higher than 10. The optimal pH values for neat zirconia and Y-TZP were 13 and 12,respectively. Furthermore, the ammonium hydroxide/NaOH solution was more favorable to enhance the content of tetragonal zircona. Therefore, the control of pH values and rational choice of pH regulators can improve the tetragonal phase purity and grain size of zirconia.
10.A meta-analysis on surgical treatments for chronic pancreatitis: duodenum-preserving pancreatic head resection versus pylorus-preserving pancreaticoduodenectomy

Objective To compare the safety and effectiveness of duodenum-preserving pancreatic head resection (DPPHR) with pylorus-preserving pancreaticoduodenectomy (PPPD) in the treatment of chronic pancreatitis with a pancreatic head mass.Methods Medline,Biosis,Cochrane Library,Science Citation Index Database,CBM Database,Wan Fang and CNKI were searched systematically.The bias risk of the included trials was assessed according to the assessing tools as suggested by the Cochrane Handbook.Review Manage 5.2 was used to perform the statistical analysis.Results 7 RCTs with 226 patients were included in the meta-analysis which showed that there were no significant differences between PPPD and DPPHR in overall postoperative morbidity,postoperative hospital stay,complete pain relief,pancreatic fistula,exocrine insufficiency,symptom score at 5 to 7-year follow-up,and quality of life score at 14 to 15-year follow-up (P > 0.05).While DPPHR had significant superiorities in operation time,blood replacement,delayed gastric emptying,occupational rehabilitation after the operations,weight gain,quality of life score at 1 to 2-year follow-up,symptom score at 5 to 7-year follow-up,and physical functioning score at 14 to 15-year follow-up.Conclusions DPPHR is more favourable than PPPD in reducing the use of blood replacement,shortening operation time,delayed gastric emptying,occupational rehabilitation after the operations,weight gain,physical functioning,and in improving quality of life of patients.
