1.Application of intraaortic balloon counterpulsation during primary percutaneous coronary intervention in patients with cardiogenic shock complicating with acute myocardial infarction

Objective To investigate the safety,efficacy and effects of emergent percutaneous coronary intervention (PCI) in patients from Second Affiliated Hospital,Medical College,Zhejiang University with cardiogenic shock (CS) complicating acute myocardial infarction (AMI).Method Twenty-seven patients with CS complicating with AMI were treated by PCI with intraaortic balloon counterpulsation (IABP) support.The change of hemodynamics before and after IABP and PCI,the characteristics of PCI,the mortality during hospitalization, the major adverse cardiac events (MACE) and left ventricular ejection fraction at 30-day follow-up were observed.Results The hemodynamics were significantly improved after IABP.No patients died during PCI.Two patients died after PCI and the total mortality was 7.4% in hospital.During the period of 30-day follow-up, one patient died of heart failure.The left ventricular ejection fraction greatly improved at 30 days after PCI. Conclusions The data suggested that the use of IABP during PCI in patients with CS complicating AMI was safe, decreased mortality and improved prognosis.
2.Determination of luteolin-7-O-glycoside in the herb of Dracocephalum rupestra by HPLC.

OBJECTIVETo develop a method for determination of luteolin-7-O-glycoside in the roots, stems, leafs, flowers and aerial parts of Dracocephalum rupestra sampled in different seasons.
METHODThe samples were analyzed on an phenomenex C18 column, with mobile phase of methanol-acetonitrile-0.4% phosphoric acid (30:8:62) at flow rate 1.0 mL x min(-1) and detection at wavelength of 350 nm.
RESULTThe content of luteolin-7-O-glycoside in different parts of D. rupestra was different maximum in leaves, while minimum in stems. Luteolin-7-O-glycoside in D. rupestra sampled before blossoming was the highest.
CONCLUSIONThe method simple, accurate and suitable for the quality evaluation of this plant medicine.

4.The functional magnetic resonance imaging (fMRI) study of auditory sensory gating in the siblings of schizophrenic

Objective To investigate auditory sensory gating potential in the siblings of schizophrenic, to discover the neuroimaging features of sensory gating associated with schizophrenia as intermediate phenotypes. Methods Twelve siblings of schizophrenia patients in the first episodic, and 12 healthy controls matched in gender, age, education under?went MRI scan when performing a auditory sensory gating fMRI task (repeated clicks and single click auditory stimuli). The measure of sensory gating was the contrast of repeated clicks minus single click. The analysis of result was imple?mented in SPM2. Results Compared with healthy controls, schizophrenia patients showed significant overactivation in the right inter cingulate gyrus (x=4,y=8,z=32) and left anterior cingulate gyrus (x=-8,y=4,z=28) during the fMRI senso?ry gating task. Conclusion Sensory gating is overactivated in the cingulate gyrus in siblings of schizophrenic. This pat?tern may be a potential endophenotype of schizophrenia.
5.Protective effect of baicalin against rotenone induced injury on PC12 cells.

OBJECTIVETo explore the protective effect of baicalin against rotenone-induced injury on PC12 cells, and the po-tential mechanism of action action was also explored.
METHODPC12 cells were injured by rotenone and were treated with different concentrations (0.1, 1, 10 μmol x L(-1)) of baicalin at the same time. Cell viability was analyzed by MTT, and morphology was observed by phase-contrast microscopy. The cell apoptosis was detected by flow cytometry by Annexin V-FITC/PI staining. The intracellular ROS level was determined by fluorescence microscope with DCF-DA staining. The expression of Bcl-2, Bax and Caspase-3 was analyzed by Western blot.
RESULTThe viability of PC12 cells exposure to rotenone for 24 hour was gradually decreased with dose escalating and 1.5 μmol x L was adopted to do the following experiment. Baicalin increased cell viability, improved cell morphology and decreased intracellular ROS level. Moreover, FACS indicated baicalin attenuated the apoptosis induced by rotenone significantly. Western blot showed that Bcl-2, Bax and Caspase-3 expression in rotenone-induced PC12 cells was reversed by baicalin.
CONCLUSIONThis study has demonstrated that baicalin protects PC12 cells against rotenone-induced apoptosis, at least in part, by scavenging excessive ROS and inhibiting the mitochondrion-dependent apoptotic pathway.

6.Investigation of fluoride level in drinking water and state of endemic fluorosis in Yan'an city

Objective To investigate the distribution of water-borne fluoride and the current situation of endemic fluorosis in Yan'an city in 2006, and to evaluate the effect of water defluoridation project by improving driking water quality. Method In 2006 in Yan'an city, 5 samples from water source were collected in each selected village that was chosen according to 5 directions of East, West, South, North, and Central. Meanwhile, 1 sample from water source, 1 sample from water processing factory and 2 tap water samples were collected from each water defluoridation project. Water fluoride was determined by spectrophotometric method, teeth and skeletal fluorosis examination were performed by Dean method and "national criteria of endemic skeletal fluorosis diagnosis of China" in children aged 8-12 year and adults, respectively if water fluoride level > 1.00 mg/L Results Of 726 water samples from 293 villages tested, samples from 25 villages had higher fluoride( > 1.00 mg/L), and these villages covered a population of 11 610 people and most of these people were in Wuqi and Yanchuan counties. Water fluoride ranged from 0.10 mg/L to 3.50 mg/L, with median being 0.59 mg/L. Of 100 water samples from 25 water defluoridation projects, only 1 sample exceeded the national criteria in Yanchuan, and Wuqi counties, respectively,with fluoride level being 1.85 mg/L and 1.60 mg/L, respectively, and population exposed was 3083 and 708, respectively, with water fluoride ranged 0.30 - 2.00 mg/L In the examination of 1281 children aged 8 - 12, we detected 238 cases of dental fluorosis, and the detection rate reached 18.58%; 13 900 adults were checked, and 375 cases were confirmed of skeletal fluorosis, a detection rate reached 2.70%. Conclusions Yan'an has a wide range of water with high fluoride and severe fluorosis people. The water defluoridation projects need to be further improved. The task of prevention of endemic fluorosis is still arduous, and we should speed up the implementation of comprehensive water defluoridation measures.
7.Evaluation of the value of shear wave elastography in differential diagnosis of benign and malignant breast lesions by logistic regression

Objective To obtain the elasticity value of solid breast lesions with supersonic shear wave elastrography (SWE) and apply the binary Logistic regression in order to evaluate the value of SWE in differential diagnosis of benign and malignant breast lesions. Methods SWE quantitative elastography was preformed in 91 breast lesions of 91 patients in Zhenghai Longsai Hospital to obtain the maximum and mean elasticity value (Emax, Emean). And receiver operating characteristic (ROC) curves were used to assess the diagnostic performance. A Logistic regression for the gray scale ultrasound and the elastic modulus was conducted with multiple variables including Emax, Emean, border, echo, form, calcification. Results Pathological examination showed 73 benign lesions and 18 malignant lesions. Emax and Emean of malignant lesions were obviously higher than those of benign lesions [(99.73±41.15) kPa vs (38.59±14.28) kPa, (61.45±24.88) kPa vs (23.46±11.44) kPa, t=-15.05,-14.12, both P=0.000]. The area under the ROC curve of Emax and Emean were 0.932 and 0.915. Taking 63.70 kPa as the threshold of Emax, the sensitivity was 77.8%and the speciifcity was 97.3%. Then taking 44.22 kPa as the threshold of Emean, the sensitivity was 83.3%and the speciifcity was 94.5%. The results of Logistic regression analysis showed:the 3 most effective variables were Emax, border of the lesions and Emean. Conclusions The multivariate analysis model of binary Logistic regression can select the valuable indexes of differential diagnosis of benign and malignant breast lesions. SWE plays an important role in differentiating benign and malignant lesions and it is valuable in clinical practice.
8.Application of nursing inspection log on nursing quality control

Objective To study the role of nursing inspection log in the management of nursing quality control.Methods Totals of 10 wards of internal medicine were randomized into the intervention group (5 wards) and the control group (5 wards).From January 2012,nursing inspections log was used in the intervention group,combined with the rounds of the wards to inspect the daily work.The existed problem of last week was reviewed every week.Specified nurse was responsible for the quality control and recorded when the head nurse was off the shift.The head nurse checked the results of quality control regularly.The incidence of adverse events and quality control results were compared between groups.Results In the intervention group,the times of adverse events of completion of core system,nursing quality of hospital admission and discharge,nursing quality of severe patients,quality of nursing document,quality of disinfection and isolation and quality of wards' management were 26,28,25,15,10 and 14,which were less than 52,48,40,30,23 and 28 in the control group.The differences were significant (x2 =11.93,7.45,4.87,6.32,6.17,5.86,respectively; P < 0.05).The control group discharged 6 259 patients,and the intervention group discharged 6 347 patients.The total number of adverse events was 32 patients in the intervention group (0.50%) and 73 patients in the control group (1.17%).The difference was significant (x2 =16.73,P < 0.05).Conclusions The inspection log can improve nursing quality control and decrease adverse events.
9.Adenosine reduces intracellular free calcium concentration in guinea pig ventricular myocytes.

AIMTo observe the effects of adenosine on intracellular calcium concentration ([Ca2+]i) level in guinea pig ventricular myocytes and to define the possible mechanisms involved.
METHODSThe effects of adenosine on [Ca2+]i were investigated in guinea pig ventricular myocytes. [Ca2+]i was detected by laser confocal microscopy and represented by relative fluorescent intensity ((FI-FI0)/FI0, %, FIo: control, FI: administration of drugs).
RESULTS(1) Adenosine (10, 50, 100 micromol/L) reduced [Ca2+]i of ventricular myocytes in both normal Tyrode's solution and Ca(2+) -free Tyrode's solution in a concentration-dependent manner. (2) Tyrode's solution containing 30 mmol/L KCl (high K+ Tyrode's solution) induced [Ca2+]i elevation in ventricular myocytes, while adenosine (10, 50, 100 micromol/L) markedly inhibited the increase in [Ca2+]i induced by KCl. (3) Pretreatment with DPCPX (1 micromol/L) significantly reduced the effects of adenosine (100 micromol/L) in high K+ Tyrode's solution. The effects of adenosine (100 micromol/L) on [Ca2+]i in high K+ Tyrode's solution were also partially attenuated by pretreatment with L-NAME (1 mmol/L). (4) Adenosine (100 micromol/L) markedly inhibited the low concentration of ryanodine-induced [Ca2+]i increase in Ca(2+) -free Tyrode's solution. (5) When the propagating waves of elevated [Ca2+]i (Ca2+ waves) were produced by increasing extracellular Ca2+ concentration from 1 mmol/L to 10 mmol/L, adenosine (100 micromol/L) could block the propagating waves of elevated [Ca2+]i, reduce the frequency and duration of propagating waves, and reduce [Ca2+]i as well.
CONCLUSIONAdenosine may reduce the [Ca2+]i in isolated guinea pig ventricular myocytes via inhibiting Ca2+ influx and alleviating Ca2+ release from sarcoplasmic reticulum(SR). The reduction of Ca2+ influx might be due to the inhibition of voltage-dependent Ca2+ channel via adenosine A1 receptor, and NO might be involved in this process.

10.Chemical constituents of Swertia delavayi and their anti-hepatitis B virus activity.

Fifteen known compounds were isolated from Swertia delavayi by silica gel, Sephadex LH-20 and Rp-18 column chromatographies. Based on extensive spectroscopic analysis (MS, 1H, 13C-NMR), their structures were identified aserythrocentaurin (1), erythrocentaurindimethylacetal (2), sweroside (3), swertiamarin (4), gentiopicroside (5), swertiakoside A (6), 2'-O-acetylswertiamarin (7), 4'-O-[(Z) -coumaroyl] swertiamarin (8), 1,5,8-trihydroxy-3-methoxyxanthone (9), 8-O-β-D-glucopyranosyl-1-hydroxy-2,3, 5-trimethoxyxanthone (10), 8-O-[β-D-xyl- opyranosyl-(1 --> 6)-β-D-glucopyranosyl]-7,8-dihydroxy-3-methoxyxanthone (11), isovitexin (12), β-sitosterol (13), daucosterol (14), and oleanolic acid (15). Among them, ten ones (14, 7-11, 13) were obtained from S. delavayi for the first time. The isolates were evaluated for their anti-HBV activities in HepG 2. 2. 15 cell line in vitro. The results showed that compound 1, 2, 6, 7, 9 and 12 exhibited significant inhibitory activity on HBV DNA replication with IC50 values from 0.05 to 1.46 mmol x L(-1).
