1. Optimization of extraction and purification technology for Huangqi Baihe Granules based on G1-entropy method and orthogonal design

Objective To determine the optimum extraction and purification technology of Huangqi Baihe Granules (HBG) by using orthogonal design and combination empowerment based on G1-entropy method, so as to provide a reference for the industrial production. Methods With the ethanol extraction amount and paste-forming rate of calycosin7-O-β-D-glucopyranoside, hesperidin, crude polysaccharide as evaluation index, and with the extraction times, extraction time and the water adding amount as investigate factors, then the combination empowerment method based on G1-entropy method and orthogonal design were used to optimize the extraction technology of HBG. The same combined methods were used to optimize the purification technology of HBG with retention rate and removal rate of impurity of ethanol extract of calycosin7-O-β-D-glucopyranoside, hesperidin and crude polysaccharide, as evaluation index, and with the membrane pore diameter, operating pressure and filtration temperature of multi-channel tubular ceramic ultrafiltration membrane as investigate factors. Results The test results showed that the optimum extraction technology was as follows: Extracted 2 times with 20 times the amount of water and each for 75 min. The optimum ultrafiltration technology was as follows: Multi-channel tubular inorganic ceramic membrane at 50 nm, operating pressure at 0.10 MPa, filtration temperature at 45 ℃. Under such condition, there was no significant difference between verification groups of three batches. Conclusion The optimized extraction and purification process is stable and feasible by verification, which can provide experimental basis for industrial production of HBG.
2.Development of gasless laparoscopic devices and clinical application of gasless laparoscopic surgery

Objective To research the clinical safety and feasibility of self-made devices in gasless laparoscopic surgery. Methods A total of 5 generations of gasless laparoscopic devices (8 models) were designed and manufactured by ourselves from April 1993 to September 2004. After animal experiments, 15 types of different gasless laparoscopic operations were carried out in 87 patients. Results Out of the 87 cases, conversions to open surgery were required in 7 cases (conversion rate, 8.0%). The other 80 cases of gasless laparoscopic operations were successfully performed, involving cholecystectomy, colorectal resection, partial cecectomy, subtotal gastrectomy, inguinal herniorrhaphy, abdominal wall contraction, radical resection for carcinoma of renal pelvis, haital herniorrhaphy with Toupet’s fundoplication and total hysterectomy. Conclusions Our self-made gasless laparoscopic devices may be applied safely and effectively in clinical cases. Gasless laparoscopic surgery has been reflecting the complementary advantages of modern laparoscopic surgery and traditional open surgery. It is of supplementary significance in patients contraindicated to pneumoperitoneum and of distinctive importance for laparoscopic gastrointestinal surgery requiring abdominal fistulization or prolonged incision, as well as hand-assisted hepatobiliary or pancreatic laparoscopic surgery.
3.The treatment of hepatic cavernous hemangioma by selective hepatic arterial embolization

Objective To evaluate the effectiveness and method of intraarterial embolization in treatment of hepatic cavernous hemangioma. Methods 17 patients,8males and 9 females, were treated by super selective hepatic artery embolization with iodized oil, gelfoam pieces, and embolization coils. Results All patients had been embolized successfully. The rate of success was 100%. 11 cases were followed up by CT/USG for 1~12 months. The size of tumor was decreased more than 50% in 8 cases, more than 30% in 2 cases, and the effect was uncertain in 1 case. Symptoms of 11 patients were disappeared and relief was obvious in 5 cases.Total effective rate was 94% with no significant complications. Conclusions HAE is a safe and effective method. It may be considered as the first method of choice in treatment of hepatic cavernous hemangioma.
4.Establishment of a rat model of alcoholic liver fibrosis induced by complex factors

OBJECTIVE: To establish a model of alcoholic liver fibrosis (ALF) in rats induced by complex factors. METHODS: Forty-seven healthy male rats were divided into three groups: normal control group (n=12), minor CCl4 group (n=12) and complex factors group (n=27). The rats in the complex factors group were fed a complex diet including alcohol, corn oil and pyrazole, and administered with intraperitoneal injection of minor CCl4 to induce ALF. During induction process, the histopathological changes of liver tissue and the values of liver-to-body weight ratio were both observed regularly. The serum levels of alanine aminotransferase (ALT), aspartate aminotransferase (AST) and gamma-glutamyltransferase (gamma-GT) in these three groups were all examined at the 12th week of the induction process. RESULTS: At the 12th week of the induction process, the model of ALF induced by complex factors was successfully established in rats, and the histopathological presentations showed alcoholic fatty liver, hepatitis and liver fibrosis in a sequence along with the induction process. The value of liver-to-body weight ratio and the serum levels of ALT, AST and gamma-GT of rats in the complex factors group were all significantly different from those in the other two groups. CONCLUSION: It is a steady and effective way to induce ALF in rats with complex diet and minor CCI4 injection.

Objective According to Kaschin-Beck Disea se monitory standardization that had been adjusted by our country,we monitored the state of Kaschin-Beck Disease in Gansu province.Methods So as to understand change of illness,we took methods of epidemiological investigation,clinical examination and X-ray diagnosis.Results It is not detected in the clinical that patient suffered from more than I of KBD among 7~12 years old in Qingyang monitory netw ork.X-ray detectable rate is 3%,but 12 cases patients were showed in Zhangjiach uan.X-ray detectable rate is 22.22%.Conclusions Illness was showed steady state and was con trolled in Qingyang region,but illness recurred clearly in Zhangjiachuan region.
7.Effects of XinShu Parenteral Solution on the function of platelet,the changes of fibrinolysis system after intima denudation of rabbits

ObjectiveTo observe the effects of XinShu parenteral solution (XSPS) on the activation of platelet, the activity of fibrinolysis system after intima denudation of rabbits.Methods20 male Japanese white rabbits (2.5±0.5)kg were randomly divided into the control group and XSPS group .The celiac arterial endothelium of all rabbits were denuded with balloon. Before the operation and 3d, 7d, after balloon denudation, vein blood samples were obtained from each group rabbits for measurement of α granule membrane protein of platelets(GMP-140), tissue-type plasminogen activator(t-PA) and plasminogen activator inhibitor-type Ⅰ(PAI-1).ResultsPlasma GMP-140 and PAI-1 activity obviously elevated after balloon injury, and there was a little elevation in plasma t-PA activity in control group. Activity of plasma GMP-140 in XSPS group remained bottom level after balloon injury, and there was a significant increase in plasma t-PA activity and a marked reduce in PAI-1 activity in XSPS group. There was a notable difference between group B and group C (P<0.05). ConclusionXSPS obviously inhibits platelet activation, and improves fibrinolysis activity after balloon injury.
8.Effect of SIRT6/NF-κB signal axis in delaying hematopoietic stem/progenitor cell senescence with ginsenoside Rg1.

OBJECTIVETo investigate the effect of SIRT6/NF-κB signal axis in delaying hematopoietic stem/progenitor cell senescence with ginsenoside Rg1, in order to provide theatrical and experimental basis for looking for methods for delaying HSC senescence.
METHODSca-1 + HSC/HPC was isolated by magnetic cell sorting (MACS) and divided into five groups: the normal control group, the aging group, the positive control group, the Rg1 anti-senescence group, and the Rg1-treated group. Senescence-associated β-galactosidase (SA-β-Gal) staining, cell cycle analysis and hemopoietic progenitor cell mix (CFU-Mix) were adopted to determine the effect Rg1 in delaying or treating Sca-1 + HSC/HPC senescence biology. The mRNA and protein of senescence regulation molecules SIRT6 and NF-KB were examined by realtime fluorescence quantitative PCR (FQ-PCR) and western blotting.
RESULTCompared with the senescence group, the Rg1 anti-senescence group and the Rg1-treated group showed lower percentage in SA-β-Gal-stained positive cells, decreased cell proportion in G1 phase, increased number of CFU-Mix, up-regulated in SIRT6 mRNA and protein expression, down-regulation in NF-KB mRNA and protein expression. The Rg1 anti-senescence group showed more evident changes in indexes than the Rg1-treated group.
CONCLUSIONRg, may inhibit Sca-1 + HSC/HPC senescence induced by t-BHP by regulating SIRT6/NF-KB signal path.

9.The relationship between lumbar disc degeneration of lumbar spinal stenosis and the dural sac cross-sec-tional area by MRI measurement

Objective To explore the relationship between lumbar disc degeneration (LDD) of lumbar spinal stenosis(LSS)and the dural sac cross-sectional area(DSCA)by MRI measurement. Methods 91 patients with central degenerative LSS were randomly selected and 91 age-and sex-matched people without LSS were select-ed as a control group. LDD was classified into five grades by MRI detection according to the method proposed by Pfirrmann and DSCA were measured. Results LDD was not associated with age in LSS. The proportion of severe degenerated disc in lower lumbar levels were higher than that of L2/3 in the two groups;DSCA in severe degenerat-ed disc group was significantly smaller than that in light degenerated group only in L2/3 and L3/4 in LSS. There were no statistical differences in every lumbar level in the control group. Conclusions LDD in L4/5 and L5/S1 of LSS is more severe than that of the normal people. DSCA and LDD are positively correlated in L2/3 and L3/4,but not in L4/5 and L5/S1 for LSS.
