Studying classic cases is one of the applicable methods for student's self-learning,especially at clinical rotation period.Seven-year program pediatric students (2004 grade) from pediatric medical collage,Capital Medical University of China shared classic cases from New England Journal of Medicine (NEJM),reported clinical cases they took part in and summarized their writing of case report.Through these measures, students felt the significance of studying classic cases and noticed that case study played an important role in training of clinical reasoning, multidisciplinary approach,as well as self-learning ability for medical students.

Objective To determine the expression of cancer stem cell markers, CD133, CD44 and OCT-4, in small cell lung cancer cell line NCI-H82 and confirm the specific markers. Methods NCI-H82 cells were cultured, and the expressions of markers (CD133, CD44 and OCT-4) were detected by immunofluorescence staining. The immunohistochemistry was performed to detect the expressions of CD133, CD44 and OCT-4 in 79 tissues with small cell lung cancer. Results In NCI-H82 cells, the florescence was positive in CD133 and CD44 and negative in OCT-4. In the tissues, the expression of CD133 and CD 44 was related with tumor size, lymph node metastasis and clinical stage (P< 0.05), while OCT-4 was negative expression. Conclusion CD133 and CD44 might be the cancer stem cell markers of small cell lung cancer, which maybe have clinical significance on diagnosis and treatment.

Objective: To observe the clinical efficacy of electroacupuncture (EA) plus Tanbo-plucking the trigger points for scapulohumeral periarthritis (SP). Methods:A total of 80 patients with SP were randomized into an observation group and an EA group by the random number table, with 40 cases in each group. The EA group was treated with EA therapy, and the observation group was treated with EA therapy plus Tanbo-plucking the trigger points. After treatment, the visual analog scale (VAS) and Melle scores of the two groups were compared to evaluate the improvement of shoulder pain and functional activity, and meanwhile the clinical efficacy was observed. Results: After treatment, the total effective rate of the observation group was 95.0% and the cure and markedly effective rate was 72.5%. The total effective rate of the EA group was 87.5% and the cure and markedly effective rate was 42.5%. There was no significant difference in the total effective rate between the two groups (P>0.05). The cure and markedly effective rate of the observation group was higher than that of the EA group, and the difference between the two groups was statistically significant (P<0.05). After treatment, the intra-group differences in VAS and Melle scores of both groups were statistically significant (bothP<0.001). The inter-group differences in the changes of the VAS and Melle scores after treatment were statistically significant (bothP<0.001). Conclusion: EA plus Tanbo-plucking the trigger points has a better curative effect than EA therapy alone in the treatment of SP.

Objective To study the effect of glucose regulated protein 75(Grp75) on the alteration of Bax and NF-κB induced by glucose deprivation through the stably transfected PC12 cells with Grp75. Methods The cells of Grp75-overexpressing group and control group incubated in glucose-free DMEM medium for indicated time (6, 12, 24 and 48hours). The expression level of Grp75, Bax and the activity of NF-κB were determined by Western blotting, and the expression level of Bax was determined by semi-quantitative RT-PCR and Western blotting. Immunocytochemistry was performed using a conformation specific anti-Bax (6A7) antibody to detect the activation of Bax. Results The activation of Bax and the decline of NF-κB activity played important roles in the apoptosis of PC12 cells induced by glucose deprivation. Grp75 inhibited the apoptosis induced by glucose deprivation through inhibition of the activation of Bax and the decline of NF-κB activity. There was no change in Bax expression level under glucose deprivation in two groups. Conclusion The activation of Bax and the decline of NF-κB activity were associated with apoptosis of PC12 cells induced by glucose deprivation, and Grp75 provided protection to PC12 cells through inhibition of activation of Bax and maintaining activation of NF-κB.

Objective To elucidate the biocompatibility differences of 4 dialyzers with different membranes in maintenance hemodialysis (MHD)patients. Methods A total of 60 MHD patients were enrolled in the prospective, randomized, control, cohort study. In baseline, synthetic polysulfone(PS)membrane dialyzer was used in all the patients for at least 3 months. Then the patients were randomly divided into three groups:ployethersulfone(PES)membrane group, cellulose triacetate (CTA)membrane group, and synthetic polymethylmethacrylate(PMMA)membrane group.Study duration was 6 months. No dialyzer was reused. The biocompatibility markers were detected repeatedly at different time points and compared with each other in different dialyzer groups. Results The blood levels of high sensitive C reactive protein,interleukin-1β and interleukin-13 were not significantly different among different groups on every time point. However, the blood complements levels and WBC count were significantly different among four kinds of dialyzer. When the dialyzers changed from PS to PMMA membrane,C3a levels and WBC count changed significantly(P<0.05).Moreover,the change of C5a level was significantly different between PES group and PMMA group on month 3(P<0.05). Conclusion There are some differences of biocompatibiliy among different dialyzer membranes.

Military Medical Sciences 2016;(1):7-11
Objective To compare the standards for recruitment of cadet pilots between PLA Air Force and the US Air Force,so as to offer reference for revising PLAAF medical standards for recruiting cadet pilots. Methods Literature from medical bibliographic databases and foreign military official websites was retrieved, four published reports and network information were cited and PLAAF medical standards were compared with those in China. Results The American medical standards include Department of Defense Instruction, Air Force Medical Examination and Standards, Air Force Medical Standards Directory, and Air Force Waiver Guide, which are independent and aimed directly at different examinations so that medical requirements have been gradually broadened.Compared to our standards, the American medical selection is more dependent on clinic consultations, and more detailed in specifications of diseases, more focused on body functions than on body shape, and the psychological screening is more subtle. Conclusion The standards adopted by US Air Force are more evidence-based.We are to rectify our medical standards accordingly, which should be efficacious, safe and effective for improving the overall quality of our military pilots.

10.The OPG/RANKL/RANK system and bone resorptive disease.

Chinese Journal of Biotechnology 2003;19(6):655-660
The OPG/RANKL/RANK system plays an important role in osteoclastogenesis and represents a great progress in bone biology. RANKL, which expresses on the surface of osteoblast/stromal cells and activated T cells, binds to RANK on the osteoclastic precursors or mature osteoclasts, and promotes osteoclastogenesis and bone resorption. While osteoprotegerin (OPG), which is expressed by osteoblasts/stromal cells, strongly inhibits bone resorption by binding to its ligand RANKL and thereby blocks the interaction between BANKL and RANK. A number of cytokines and hormones exert their effects on bone metabolism by regulating the OPG/RANKL ratio in the bone marrow microenvironment. RANK is also expressed on mammary epithelial cells and RANKL expression in these cells is induced by pregnancy hormones, RANKL and RANK are essential for the formation of the lactating mammary gland and the transmission of maternal calcium to neonates in mammalian species. Modulation of these systems provides a unique opportunity to develop novel therapeutics to inhibit bone loss in osteoporosis, rheumatoid arthritis, and bone metastasis of cancer. Further research should be focused on the cooperation of OPG/RANKL/RANK system with other signal pathways and the interactions among bone remodeling, immune system and endocrinology system. Currently, the development of OPG analogues or compounds which may stimulate OPG expression is becoming an attractive industry which may be profitable to both patients and manufacturers.
