1.Significance of serum total cholesterol to high density lipoprotein cholesterol ratio as a risk marker for coronary heart disease

Objective To analyze blood lipid levels in patients with coronary heart disease ,and to explore clinical significance of serum total cholesterol to high density lipoprotein cholesterol ratio as a risk marker for coronary heart disease .Methods Serum lev‐els of total cholesterol ,triglycerides ,high density lipoprotein cholesterol ,and low density lipoprotein cholesterol were measured for 295 patients with coronary heart disease .Ratio of total cholesterol to high density lipoprotein cholesterol was calculated based on the lipid tests .Results The percentages of abnormal total cholesterol ,triglycerides ,high density lipoprotein cholesterol ,and low density lipoprotein cholesterol were 32 .20% 、34 .24% ,39 .32% ,and 37 .63% ,respectively ,for patients with coronary heart disease according to normal ranges issued by Guidelines on Prevention and Treatment of Dyslipidemia in Adults in Chinese Population .The abnormal percentage of total cholesterol to high density lipoprotein cholesterol ratio was 57 .29% .The percentage of raising total cholesterol to high density lipoprotein cholesterol ratio was significantly higher than abnormal ratios of total cholesterol ,triglycer‐ides ,high density lipoprotein cholesterol ,and low density lipoprotein cholesterol for patients with coronary heart disease (χ2 =37 .540 ,31 .576 ,19 .066 ,22 .866 ;P<0 .01) .Conclusion Comparing to any blood lipid test ,total cholesterol to high density lipopro‐tein cholesterol ratio is a marker that helps us estimate the risk of developing coronary heart disease .Ratio of total cholesterol to high density lipoprotein cholesterol may be as a test estimating the risk of developing coronary heart disease .
2.Fibromyalgia syndrome treated with acupuncture at the acupoints of the affected meridians and heavy moxibustion at painful points: a randomized controlled trial.

OBJECTIVETo observe the clinical efficacy on fibromyalgia syndrome treated with acupuncture at the acupoints selected from the affected meridians based on the location differentiation and the heavy moxibustion at painful points by the randomized controlled trial with medication.
METHODSThirty-two patients of fibromyalgia were randomized into an acupuncture-moxihustion group and a medication group, 16 cases in each one. In the acupuncture-moxibustion group, acupuncture at the acupoints selected from the affected meridians based on the location differentiation and the heavy moxibustion at painful points were applied, once every two days, three times a week, for 4 weeks totally. In the medication group, tramadol sustained release tablets and amitriptyline were prescribed for oral administration, once a day, for 4 weeks. The follow-up visit was conducted for 4 weeks in the two groups. Before and after treatment and in follow-up visit, the revised version of the fibromyalgia impact questionnaire (FIQR) and visual analogue scale (VAS) were adopted for assessment and comparison of the efficacy between the two groups.
RESULTSAfter treatment and in follow-up visit, FIQR score and VAS score were improved apparently as compared with those before treatment in the two groups (all P<0. 05). At the end of treatment, the differences were not significant statistically between the two groups (all P>0. 05). But the differences were significant in follow-up visit (all P<0. 05). The scores in the acupuncture-moxibustion group were improved persistently, but those in the medication group were rebound.
CONCLUSIONThe acupuncture at the acupoints selected from the affected meridians based on the location differentiation and the heavy moxibustion at painful points are safe and effective in the treatment of fibromyalgia syndrome and present the better persistent effect as compared with the combined medication of tramadol sustained release tablets and amitriptyline.

4.The influence of mass screening for prostate cancer on the diagnostic status of the clinical prostate cancer

Objective To investigate the influence of t he mass screening on the diagnostic status of the conventional ordinary prostati c cancer (CaP). Methods Sixty seven cases of prostate ca ncer detected by mass screening (January 1996~December 2001) and 358 cases diag n osed at the outpatient of various hospitals in Changchun (January 1986~December 2001) were compared in terms of age distribution,clinical stages,level of serum prostate special antigen(PSA),pathological grade and treatment modalities,with a SPSS analysis system. Results The annual average incide nce of CaP during 1999 to 2001 increased 4.72 times when compared to the 1986 to 1989’s.In the screening group,the cases with clinical stage A and B accounted for 58.2%. In the clinical group, it was only 27.9%,and mostly detected incident ally.Furthermore,in the screening group the percentage of the cases beyond 20 ng /ml PSA and of poorly differentiated carcinoma were much lower compared to the c linical group.These differences were statistically significant.The cases of radi cal prostatectomy in the screening group increased 15.3% compared with that of t he clinical group. Conclusions Only the mass screening c an really reveal the actual prevalence of prostate cancer.It can markedly raise the number of prostate cancer,especially the cancer in the early stage.The mass screening is inevitable to detect the curable early stage prostate cancer.
5.Application of DSA in interventional treatment of acute ischemic cerebrovascular disease

Objective To study the utilization of DSA for interventional examination and therapy in acute ischemic cerebrovascular disease. Methods The summarization of properly utilizing the DSA equipment and events happening in the process were analyzed after application on 550 cases with acute ischemic cerebrovascular attacks. Results Application of perfusion DSA is useful to obtain rapid diagnosis and evaluation of interventional therapeutic efficacy for acute ischemic cerebrovascular disease. The practical projection angle can display the features and extent of vasculopathy clearly, and offer the best operative position for promotion of rotation DSA and the diagnostic imaging and therapeutic efficacy, three-dimensional reconstruction are usful to the success of interventional management. Imaging measuring technique of DSA would provide precise data for doctors to choose the accurate intervention materials. Conclusion Reasonable utilization of DSA equipment plays an important role in interventional therapy of acute ischemic cerebrovascular disease, and also is the gold standard for displaying images for rapid diagnosis.

Objective: To observe the clinical efficacy of electroacupuncture (EA) plus Tanbo-plucking the trigger points for scapulohumeral periarthritis (SP). Methods:A total of 80 patients with SP were randomized into an observation group and an EA group by the random number table, with 40 cases in each group. The EA group was treated with EA therapy, and the observation group was treated with EA therapy plus Tanbo-plucking the trigger points. After treatment, the visual analog scale (VAS) and Melle scores of the two groups were compared to evaluate the improvement of shoulder pain and functional activity, and meanwhile the clinical efficacy was observed. Results: After treatment, the total effective rate of the observation group was 95.0% and the cure and markedly effective rate was 72.5%. The total effective rate of the EA group was 87.5% and the cure and markedly effective rate was 42.5%. There was no significant difference in the total effective rate between the two groups (P>0.05). The cure and markedly effective rate of the observation group was higher than that of the EA group, and the difference between the two groups was statistically significant (P<0.05). After treatment, the intra-group differences in VAS and Melle scores of both groups were statistically significant (bothP<0.001). The inter-group differences in the changes of the VAS and Melle scores after treatment were statistically significant (bothP<0.001). Conclusion: EA plus Tanbo-plucking the trigger points has a better curative effect than EA therapy alone in the treatment of SP.
9.Photodynamic therapy of basal cell carcinoma and factors affecting the response to treatment

Objective To investigate efficacy and safety of photodynamic therapy on basal cell carcinomas.Methods Twenty three cases of basal cell carcinomas were given with photosensitizer 5-amino acid (ALA) ketone spreading on skin lesions.Reddish illumination was taken at 4h later with energy density at 100 J/cm2,40 min/time,and weekly for a total of 4 weeks; and 2 year followup in the study was evaluated recurrence retrospectively.Results Among twenty three patients with basal cell carcinomas,15 were cured(65.22%),5 partially (21.74%) remitted,and 3 ineffective (13.04%),with the efficiency 86.96%.None of the resolved lesions recurred during the 6-month follow-up period,and 3 cases were recurred within 1 year (20%).Repeated ALA-photodynamic therapy (PDT) treatment,recurred lesions were completely resolved.There were no additional recurrences after 2 year.ALA-PDT was well tolerated,and adverse effects were limited to mild local pain and erythema.Only three patients were complained of erythema and edema.Complete response rate was higher in younger patients (t =-2.94,P <0.01) and those with smaller lesions (t =-5.92,P < 0.01).Superficial type had also significant higher response rate (x2 =27.17,P < 0.01).Conclusions The application of photodynamic therapy in treatment of basal cell carcinoma is effective with less painfulness,non-invasiveness,and without side effect.It demonstrated a good efficacy and satisfactory cosmetic outcomes.
10.Study of intracardiac flow patterns of left ventricle in patients with premature ventricular complexes from the right ventricular outflow tract

Objective To evaluate the patterns of intracardiac flow of left ventricle (LV) in patients with premature ventricular complexes (PVCs) from the right ventricular outflow tract (RVOT) by analyzing the vortex during isovolumetric contraction phase and the distribution rules of flow-time curves in each layer of LV.Methods Twenty-seven patients with frequent isolated PVCs from RVOT were involved and 25 healthy subjects as control.The color Doppler image of LV at apical four-chamber view was acquired.Vector flow mapping (VFM) was performed to assess the parameters of vortex during isovolumic contraction phase, including diameter (transverse and vertical diameter), velocity (maximal positive and negative velocity) and the number of vortex rings.Positive flow during systole and negative flow during diastole of LV in each layer were measured by flow-time curve.All values of patients with PVCs were recorded during sinus beats (PVC-S) and PVC (PVC-V) respectively.Results Significant differences were demonstrated in all parameters of vortex between the PVC-V and control subjects.And the flow-time curves disarrayed in PVC-V.The velocity of vortex in PVC-S was lower than that in control subjects.And the distribution pattern of flow-time curves in LV of PVC-S differed from that of control subjects.Conclusions Alternation of intracardiac fluid pattern in LV was demonstrated in patients with PVCs from RVOT during both sinus beats and PVC.VFM can be used to analyze the intracardiac flow field in normal and pathological electrical activation.
