1.Vitamin D3 for the treatment of breast cancer:a study of Balb/c nude mouse model

Objective To study the effect of vitamin D3 on breast cancer xenografted on a nude mouse model. MethodBalb/c nude mouse was injected subcutaneously with 10 6 human breast cancer cell line MCF-7.Two thousand units Vitamin D3 was administered in mice i?m every the other day. Tamoxifen (5 mg/kg) was fed per mouse daily. Mice were divided into four groups with ten mice in each group: Vitamin D3 group, Vitamin D3 and tamoxifen group, tamoxifen group and control group. After 4 weeks, mice were killed to measure volume of tumor and calcium and phosphorus levels in serum. Tumor cells were analyzed with flow cytometery to detect apoptosis and cell cycle. Results Calcium levels in serum of mice treated with vitamin D3 were higher than that of other two groups, but phosphorus levels in serum were lower . Apoptosis rate was higher than that of control group and tumor cells were arrested in G0/G1 phase, especially those treated with vitamin D3 and tamoxifen. ConclusionsVitamin D3 could significantly induce apoptosis of breast cancer cells and arrest tumor cells in G0/G1 phase. It is synergic with tamoxifen and this may have clinical significance in the treatment of breast cancer.

In the process of internship education management, through the establishment of the secondary medical colleges' interconnection management working mecha-nism, we can effectively solve the outstanding problems in the current internship edu-cation management, to achieve mutual trust between the secondary clinical medical college and the training hospital, and en-hance the effectiveness of the management of interns.
4.Posterior reversible encephalopathy syndrome in children: a report of 2 cases and literature review

Objective To improve recognition of posterior reversible encephalopathy syndrome (PRES) in children. Methods Two children with PRES admitted to children's hospital were included. Clinical data were retrospectively studied and related literatures were reviewed. Results The primary diseases of the two patients are systemic lupus erythematosus and nephritic syndrome, respectively. They all developed an acute onset of headache, visual changes, consciousness disturbance, hypertension and seizures. Cranial MRI showed bilateral parietal, temporal and occipital cortical or subcortical lesions with hypointensity on Tl-weighted imaging and hyperintensity on T2-weighted imaging. Clinical symptoms resolved soon and radiographic recovery occurred within 14 to 21 days with prompt anti-hypertension treatment and supportive care. Among the mechanisms which might contribute to the development of PRES, acute elevated blood pressure seems to be the most important factors in these two cases. Prompt anti-hypertension treatment usually can reverse the PRES lesion. Conclusions It is important to improve cognition of PRES in children. Delayed treatment can cause permanent neurological impairment. Doctors should be alert to this syndrome. Early diagnosis and prompt treatment are very important. (J Clin Pediatr,2010,28(2):168-170)
5.Setting Upper and Lower Limit for Drugs in Hospital Pharmacy Stock Drug Management and the Influencing Factors

OBJECTIVE:To control scientifically the limits of stock drugs in Hospital.METHODS:The method of setting the upper and lower limit of stored drugs through Hospital Information System(HIS)and the influencing factors were discussed by taking the outpatient pharmacy of our hospital as an example.RESULTS&CONCLUSION:Working out scientific and reasonable upper and lower limit for stock drugs is helpful for the improvement of the management quality of hospital pharmacy.
6.Quantization and Enforcement of Medicine Provision Management Aim in Our Hospital

OBJECTIVE:To improve hospital's drug control level.METHODS:Quantized medicine provision management aims like stock limit,conformity rate of object-account of single category,medicine loss,mistake rate of recipe preparation,etc.were made and enforced.RESULTS:Through the above aims,medicine stock has been reduced effectively;the conformity rate of object-account has been increased;the medicine loss and incidences of mistakes in recipe preparation have been decreased.CONCLUSION:Working out feasible medicine provision management aims is an effective way to strengthen drug control.
7.CT and MRI manifestations of Kimura disease in head and neck

Objective To explore the CT and MRI manifestations of Kimura disease in head and neck.Methods The CT and MR findings of 20 cases with Kimura disease confirmed by pathology were collected.The lesion location,boundary,size,number,density of CT and signal intensity of MRI were analyzed.Results In 20 cases,multiple lesions were seen in 19 cases.There were 11 cases involving parotid gland,6 cases involving the submandibular area,2 cases in cheek,1 case in palate,2 cases involving the upper arm,1 case involving axillary fossa,1 case involving cervical lymph node alone,and 8 cases with subcutaneous nodules in the front or back of the ear,16 cases accompanied by lymphadenopathy in the neck (8 cases with lymphadenopathy in the parotid gland).The largest diameter of lesion was (3.09± 1.08)cm.The margin was blurred in 19 cases and well defined in 1 case.The isodensity lesions were found in 16 cases and slightly lower density lesions were in 4 cases in non-contrast enhanced CT scan.Calcification was not found in all cases.Homogeneous enhancement was found in 16 cases and inhomogeneous enhancement was found in 1 case.The enhancement degree varies from mild to strong enhancement.Homogenous signal and strong enhancement was found in 2 cases with MR scan.Conclusion The typical manifestations of Kimura disease are involvement of parotid gland and subcutaneous fat,accompanied by lymphadenopathy in the neck,homogeneous density on CT or MRI,which may indicate Kimura disease.
8.Clinical features and prognosis of gastric neuroendocrine neoplasms

Objective To investigate the clinicopathological features,treatment method and prognostic factors of the gastric neuroendocrine neoplasms (gNENs).Methods The clinical data of 80 patients with gNENs who were admitted to the Zhongshan Hospital of Fudan University from January 2002 to December 2011 were retrospectively analyzed.All the patients received gastroscopic examination.Patients with well-differentiated and diameter ≤ 2 cm gNENs received endoscopic mucosal resection or endoscopic submucosal dissection.Patients with poordifferentiated and diameter > 2cm tumors received surgical resection of gNENs.Patients were followed up via phone call,mail or out-patient examination till October 31,2013.The Kaplan-Meier method was used for drawing the survival curve and calculating the survival rate.The univariate analysis was done by Log-rank test and the multivariate analysis was done by COX proportional hazards model.Results Fifty-eight patients had pain and discomfort in the epigastric region.Thirty-two gNENs were located at the cardia,40 at the body and 8 at the pylorus.Fortyfour gNENs were ulcerative type,25 were polypoid type,11 were protruded type.The mean diameter of the gNENs was 2.6 cm (range,0.4-7.5 cm).Twenty-seven gNENs were in grade 1,10 in grade 2 and 43 in grade 3.Forty-five gNENs were localized,34 gNENs had lymph node involvement (2 gNENs had distal metastasis),and 1 gNENs had distal metastasis.Thirty-one patients received endoscopic resection,and did not receive adjuvant therapy.Forty-nine patients (6 with gastric neuroendocrine tumor and 43 with gastric neuroendocrine carcinoma)received radical resection,including proximal subtotal gastrectomy in 16 patients,distal subtotal gastrectomy in 15 patients,total gastrectomy in 15 patients and distal subtotal gastrectomy + resection of the liver metastasis in 3 patients.Thirty-four patients received adjuvant chemotherapy and 15 did not receive adjuvant chemotherapy.Seventy-nine patients were followed up for a median time of 42.0 months (range,2.0-113.0 months).The mean time of survival was 75.6 months(range,2.8-100.8 months),and the 1-,3-,5-year overall survival rates were 91.3%,75.8% and 66.5%,respectively.The 5-year survival rates of patients with gNENs in grade 1,grade 2 and grade 3 were 100.0%,100.0% and 38.0%,respectively.The results of univariate analysis showed that the gender,treatment methods,adjuvant chemotherapy,types of tumor,tumor diameter,classification and staging of the tumor were correlated with the prognosis of the patients (x2=9.550,17.488,25.038,14.994,6.897,25.234,22.066,P < 0.05).The results of multivariate analysis showed that the gender was the independent risk factor influencing the prognosis of the patients (RR =11.280,95% confidence interval:5.353-19.121,P < 0.05).Conclusions The clinical presentations of gNENs are often nonspecific.The main presentation of gNENs is pain in the epigastric region of the abdomen,and most of the gNENs are located at the cardia or body of the stomach.The staging and grading of the gNENs are varied,and the prognosis is related with the gender of the patients.Endoscopic or surigcal resection is the main treatment method for gNENs.Female patients have a better prognosis than male patients.
9.Effect of the disassembled recipes of dahuang zhechong pill on proliferation and apoptosis of vascular smooth muscle cells in atherosclerotic rabbits.

OBJECTIVETo study the effects of the disassembled recipes of Dahuang Zhechong Pill (DRDZP) on proliferation and apoptosis of vascular smooth muscle cell (VSMC) of thoracic aorta in atherosclerotic rabbits.
METHODSThe atherosclerotic rabbit model was induced by high-cholesterol diet and immune injury of endothelium. Expression of proliferating cell nuclear antigen (PCNA) in VSMC was detected by SP immunohistochemical technique, and VSMC apoptosis was observed with TUNEL technique.
RESULTSCompared with the model control, PCNA expression in VSMC decreased, while apoptotic cells increased significantly in all the groups treated by DR-DZP (P < 0.01).
CONCLUSIONAll the DR-DZP could inhibit VSMC proliferation and promote its apoptosis to modulate the balance between cell proliferation and apoptosis, so as to exert antiatherosclerotic action, among which the disassembled-recipe I is the main composition to contribute to antiatherosclerotic action of Dahuang Zhechong Pill.

10.Imaging diagnosis of solid psedopapillary tumor of pancreas

Objective To retrospectively analyze and conclude the characteristics of imaging appearances of solid psedopapillary tumor of pancreas.Methods Among 7 cases with pathologically proved solid psedopapillary tumor of pancreas,6 cases underwent CT examinations of upper abdomen preoperatively,and the rest One had MRI examination.The mean age of these 7 cases(all female)was 30.7 years(range,14—44 years).Results The tumors were usually quite large in the largest diameter ranged from 2.8 to 15.9cm(mean largest diameter,7.9 cm);Tumors were all well demarcated,and 5 of them were of capsule on CT or MR imaging.All tumors were well-encapsulated on pathologic specimens, except for the capsule of 1 tumor was partially invaded;In 6 cases underwent CT examination,scattered, punctate and linear calcification were noted in the capsule of 2 tumors and the rim of another one;Except for 1 tumor was almost solid,the other 6 tumors contained both solid and cystic components;Scattered sheets of high attenuation shown in the cystic or solid parts on CT imaging in several cases and the high signal intensity on T_1-weighted MR imaging signified the possibility of bleeding in tumors,which then was testified by pathologic evaluation.Conclusion The solid psedopapillary tumor of pancreas has comparatively characteristic clinical and imaging features.
