Stomach carcinoma is more common disease in korea than western contries. The reported ultrasonographicfindings of gastric carcinoma were thickening of gastric wall and “pseudokidney” sign. The auther analizedultrasonographic findings of 101 cases with gastric carcinoma who were performed ultrasonography and gastroscopyat Kyung Hee Universtiy Hospital from Oct. 1982 to Oct. 1985. The results were as followings; 1. Types of gastriccarcinoma were consisted with infiltrative type 68 cases, infiltrative type with ulceration 16 cases, polypoidtype with ulceration 1 cases, infiltrative and polypoid type 4 cases, linities plastica type 3 cases, lcerativetype 1 case and polypoid type 1 case. 2. Extent of the lesions were in body and antrum 45 cases, entire stomach 18cases, antrum 18 cases, body 12 cases, body and fundus 6 cases. 3. Ultrasonography was useful in demonstrating theextent of the tumor and the presence of materials elsewhere in abdoment.

3.CT findings of tuberculous lymphadenitis in the abdominal cavity

Authors analyzed CT findings of 8 patients with tuberculous lymphadenitis and one case of tuberculous abscessdiagnosed surgically or clinically. The results were as follows; 1. Soft tissue density masses were noted in 8patients in paraaortic, mesenteric, peripancreatic, celiac, porta hepatis, and esophagogastric junction areas inorder of frequency, and these correspond to lylmph node groups of the same name. On contrast enhanced CT, rimenhancement with multilocular low dinsties indicating caseous necrosis were noted in 3 patients, ill defined lowdensities were seen in 3 patients, and no definite changes were noted in 2 patients. 2. Two or more lymph nodegroups were involved in 6 patients, and one lymph node group was involved in two patients. 3. A huge cystic masswith relatively irregular rim enhancement and small anount of solid component occupied nearly entire upper abdomenin 1 patient and this was confiremd as tuberculous abscess in peritoneum. 4. In 2 cases, bowel wall thickening wassuggested in cecum, ascending colon, and terminal ileum.

4.Ultrasonographic findings of appendiceal mucocele

The authors analyse the ultrasonographic findings of 10 cases of appendiceal mucocele retrospectively. In atotal of 10 cases, 6 cases showed only cystic masses with posterior reinforcement. The remaining 4 cases showedinternal echoes in the cystic masses. Differential diagnosis of mucocele from periappendiceal abscess is notpossible in ultrasound. The possibility of mucocele shold be considered if cystic mass in right lower quadrantabdomen is observed in ultrasound.

5.SUTI (Single Umbilical Tangential Incision) Laparoscopic TEP Inguinal Hernia Repair.

PURPOSE: Single port laparoscopic inguinal hernia repair has been performed to further reduce port related morbidities and to improve the cosmetic outcome. However, the cosmetic result of single port laparoscopic totally extraperitoneal inguinal hernia repair (TEP) has not been superior to that of transabdominal preperitoneal hernia repair (TAPP). The aim of this study is to introduce and assess the Single Umbilical Tangential Incision (SUTI) for Lap-TEP repair. METHODS: Forty one hernia repairs of 34 patients were performed using SUTI-TEP method. SUTI was made all along the inferolateral skin of the umbilical pit and a vertical incision of less than one centimeter was made in the supra- or infraumbilical skin if needed; the total length of the incision was 2~2.5 cm. SILS(TM) port and conventional instruments were used for the procedure. RESULTS: All procedures were completed without conversion to the conventional TEP procedure. Mean operation time was 73 minutes for unilateral primary hernia, 119 minutes for bilateral primary hernia, 88.5 minutes for unilateral recurrent hernia, and 120 minutes for bilateral recurrent hernia. There were two cases of wound seroma and four cases of urinary retention postoperatively. Other significant complications were not noted. There was no recurrence of hernia during the follow-up period of one to 24 months. At routine follow-up visit at three months postoperative, the scar was barely visible and overall level of patient satisfaction was very high. CONCLUSION: The SUTI-TEP inguinal hernia repair is safe and shows superior cosmetic results. Further studies are required in order to assess the long-term outcome.

6.Analysis of Landing Error Scoring System during Drop Vertical Jump on Anterior Cruciate Ligament Injury Risk Factors in Female Ballet Dancers and Female Soccer Players.

The incidence of anterior cruciate ligament (ACL) injuries among female ballet dancers is much lower than among female soccer players, there is currently little research examining the landing error scoring system (LESS) of female ballet dancers and female soccer players. The purpose of this study was to investigate the LESS during drop vertical jump on ACL injury risk factors in female ballet dancers and female soccer players. Thirteen professional female ballet dancers and elite female soccer players participated in the study. Independent sample t-test was used to compare the LESS score between the groups with Bonferroni collection. The elite female soccer player group showed significantly higher stance width-wide, foot position-toe out, symmetric initial foot contact, overall impression and total LESS score than those of the professional ballet dancers.

7.Intravitreal Tissue Plasminogen Activator with C3F8 Injection in Branch Retinal Vein Occlusion.

PURPOSE: To evaluate the efficacy of intravitreal tissue plasminogen activator (tPA) with C3F8 injection for branch retinal vein occlusion (BRVO) involving fovea. METHODS: Seven patients (7 eyes) presenting with subfoveal hemorrhage caused by BRVO were treated with an intravitreal tPA and C3F8 injection. We assessed the visual acuity (VA) and foveal thickness measured with optical coherence tomography. RESULTS: The mean duration of symptoms before surgery was 5.42+/-1.90 weeks. The mean logMAR VA improved from 1.14+/-0.19 at baseline to 0.87+/-0.34 at one week and 0.30+/-0.32 at six months. The mean foveal thickness decreased from 564.421+/-186.88 micrometer at baseline to 483.14+/-275.06 micrometer at one week and 353.28+/-152.99 micrometer at six months. There was no adverse effect related to the treatment. CONCLUSIONS: Intravitreal tPA with C3F8 injection may be an effective treatment for resolving macular edema with subfoveal hemorrhage and improving the VA in recently developed BRVO.

8.Effect of substance P on production of interleukin-12 and interferon-gamma from peripheral blood mononuclear cells of chronic severe atopic dermatitis patients.

BACKGROUND: In atopic patients, allergen-specific T cells express the Th2 phenotype. It is known that interleukin-12 (IL-12) is a major cytokine for the induction of Th1 responses and that a reduced release of IL-12 in atopic dermatitis patient is closely related to reduced production of interferon-gamma(INF-gamma). Recently, it has been reported that substance P (SP) positive nerve fibers express increased IL-12 and that elevated levels of this neuropeptide were seen in AD patients. OBJECTIVE: This study was designed to examine the effects of SP and its fragment on the production of IL-12 and INF-gamma from peripheral blood mononuclear cells (PBMCs) of chronic severe atopic dermatitis (AD) patients, and moreover to establish neuro-immunomodulatory mechanisms in chronic severe AD patients. MATERIALS AND METHOD: The PBMCs from fifteen chronic severe AD patients and non-atopic individuals were stimulated with Staphylococcus aureus Cowan I strain. After stimulating the PBMCs, the effects of SP and SP fragments (SP1-4 and SP4-11) on the production of IL-12 and INF-gamma were analysed by enzyme-linked immunosorbent assay. RESULTS: The levels of IL-12 and INF-gamma were significantly decreased in un-stimulated AD patients than in non-atopic controls. In chronic severe AD patients, SP and SP4-11 significantly increased the production of both IL-12 and INF-gamma compared to non-atopic controls. The enhancing effect of SP and SP4-11 was most significant at the concentrations of 10-8M and 10-6M. When the spantide, an SP antagonist, was added to SP and SP4-11 stimulated PBMCs, the productions of IL-12 and INF-gamma were downregulated in chronic severe AD patients but not in non-atopic controls. In contrast, SP1-4 fragment did not show any specific cytokine production. CONCLUSION: These data suggests that SP and SP4-11, a carboxyl terminal of SP, are involved in the immunomodulation of chronic severe AD patient by regulating IL-12 and INF-gamma production, while SP1-4, an amino terminal of SP, has no effect on the cytokine production in AD patients.

9.CT Findings of Pulmonary Hamartoma: Analysis of 16 Histopathologically-proven Cases.

PURPOSE: To evaluate the CT findings of histopathologically proven pulmonary hamartoma MATERIALS AND METHODS: CT findings of 16 patients with pulmonary hamartoma, histopatologically proven between 1990 and 1996, wereretrospectively reviewed. Diagnosis was based on thoracotomy9n=11) or fine-needle aspiration biopsy(n=5). Weanalyzed the location, margin discreteness, shape, and size of the mass, and the presence of calcification or fatdensity, as seen on CT scan. RESULTS: All 16 tumors showed a discrete and smooth margin and there was nopredilection for any specific site. They were lobulated(11/16, 69%), round(4/16, 25%) or oval(1/16, 6%) and wereless than 1cm(2/16), 1~ < 2cm(4/16), 2~ < 3cm(5/16), 3~ < 4cm(3/16) or 4~5cm(2/16) in size. Nine of 16 cases(56%)showed calcification;this was either popcorn-type(n=3), stippled(n=3), eccentric(n=2), or diffuse(n=1). Five of 16cases(31%) showed fat attenuation. CONCLUSION: On CT, pulmonary hamartomas showed a discrete margin, werelobulated (rather than round or oval) and varied in size. calcification was more common than fat density. These CTfindings may be useful for the differential diagnosis of pulmonary hamartoma.
