1.Curative Effect Observation in the Treatment of Distal Humeral Shaft Fracture with Double Steel Plate Internal Fixation

ObjectiveTo investigate the curative effect of the treatment for the distal humeral shaft fracture with double steel plate internal fixation. Methods A total of 60 individuals who were humeral shaft fractures were included from March 2009 to April 2014 in the second people's hospital of Yunnan province. 28 of them were treated using double plate fixation(group A)and 32 of them were treated using single-plate treatment (group B). Blood loss, hospital stay, postoperative drainage, healing time and Mayo elbow performance score were compared between the two groups. Results(1)Blood loss, hospital stay and postoperative drainage showed no differences between two groups. However, the healing time between these two groups showed difference(P < 0.05).(2)MEPS score results showed a significantly difference in the excellent rate of patients between groups A and B(96.4% and 75%, respectively,P < 0.05). Conclusion The double plate fixation and single plate fixation for treatment of humeral shaft fracture showed no differences in operation time,blood loss,postoperative drainage and hospital stay. However,the double plate fixation showed a shorter healing time and better efficacy. Thus,the double plate fixation of humeral shaft fracture can contribute to obtain the rigid internal fixation,early functional exercise and better clinical efficacy.
2.Effects of neoadjuvant chemotherapy on the hepatic function and histology of liver in patients with hepatic metastasis of colorectal cancer

Objective To investigate the effects of neoadjuvant chemotherapy on the hepatic function and histology of liver in patients with hepatic metastasis of colorectal cancer.Methods The clinical data of 65 patients with hepatic metastasis of colorectal cancer who received hepatectomy at the First People's Hospital of Foshan from January 2010 to September 2012 were retrospectively analyzed.Thirty-nine patients were in the experimental group,and they received partial hepatectomy 1 month after receiving a 6-month continous FOLFOX7 chemotherapy (5-fluorouracil + calcium leucovorin + oxaloplatin) ; 39 patients were in the control group,and they received partial hepatectomy without adjuvant chemotherapy.The perioperative condition,pre-and postoperative hepatic function,postoperative complications and the histopathological changes of the hepatic tissues of the 2 groups were compared by t test or chi-square test,data repeated measured were analyzed using the repeated measure analysis of variance.Results The operation time of the experimental group and the control group were (195 ± 37)minutes and (190 ±41) minutes,respectively,with no significant difference (t =0.1,P>0.05).The operative blood loss of the experimental group was (410 ± 75)ml,which was significantly greater than (348 ± 44) ml of the control group (t =6.3,P < 0.05).The levels of aspartate aminotransferase (AST) and alanine aminotransferase (ALT) at postoperative day 3 were (328 ± 121)U/L and (330 ± 120)U/L in the experimental group,which were significantly higher than (160 ±22) U/L and (168 ±26) U/L of the control group (t =13.4,12.8,P < 0.05).There were no significant differences in the levels of prothrombin time,AST,ALT,total bilirubin and albumin between the experimental group and the control group (t =1.0,0.0,1.4,1.3,0.4,P > 0.05).The levels of AST and ALT at postoperative day 7 were (243 ± 132) U/L and (253 ± 147)U/L in the experimental group,which were significantly higher than (90 ± 17)U/L and (99 ± 16)U/L of the control group (t=12.5,12.0,P<0.05).The incidence of congestion and edema of the liver was 71.8% (28/39) in the experimental group,which was significantly higher than 0 (0/26) of the control group (x2 =90.0,P < 0.05).The incidence of hepatic sinusoid expansion with liver cellular altrophy and necrosis was 48.7% (19/39),which was significantly higher than 15.4% (4/26) of the control group (x2=89.2,P < 0.05).Conclusion There is no effect of long-time FOLFOX7 chemotherapy on the hepatic function of the patients with hepatic metastasis of colorectal cancer who received hepatectomy 1 month later,but the postoperative hepatic function is affected.The hepatic parenchyma is changed after adjuvant chemotherapy,which might have adverse effect on hepatectomy.
3.Evaluation of fluid balance and dry weight with BIA in patients with maintenance hemodialysis

Objective To evaluate the fluid balance and dry weight with whole body bioelectrical impedance analysis (BIA) in patients with maintenance hemodialysis ( MHD). Methods Twenty patients in stable condition with MHD for more than one year were selected ( MHD group). Extra-cellular water ( ECW) and intra-cellular water (ICW) were measured by BIA before hemodialysis, after hemodialysis and at intervals of hemodialysis (24 h and 48 h after hemodialysis), and related parameters were calculated. Whether patients with MHD achieved dry weight at 0 h was analysed, and the effects of average daily urine volume on ECW and ICW were explored. Another 20 healthy volunteers were served as controls. Results Compared with control group, ECW retrieved by ideal body weight was significantly higher before hemodialysis in MHD group (P<0.05). In MHD group, ECW was significantly lower after hemodialysis than that before hemodialysis (P < 0.05). ECW/total body water (TBW) was significantly lower and ICW/TBW was significantly higher after hemodialysis than those before hemodialysis and at intervals of hemodialysis ( P < 0.05). ICW and ECW at each time point were significantly related to body weight. ICW and ECW retrieved by ideal body weight at 0 h and 24 h of those who achieved dry weight were significantly higher than those who did not achieve dry weight ( P < 0.05). ECW and ICW before hemodialysis and 24 h after hemodialysis were significantly lower in those with urine volume <400 mL/d than those with urine volume ≥400 mL/d (P <0.05). Conclusion BIA analysis indicates that changes of fluid balance before hemodialysis, after hemodialysis and at intervals of hemodialysis in patients with MHD are characterized by ECW, and ECW/TBW is a more sensitive parameter. ICW and ECW retrieved by ideal body weight may be sensitive parameters to determine the dry weight in patients with MHD.
4.Breeding of Bacillus tkuringiensis (Bt) with High Insecticidal Activity to Spodoptera exigua

By the breeding model of physics mutagenests (60Co,UV light) and re-strong in the body of insects, We obtained a Bt strain Bt99985 with high insecticidal activity. The shake flask and 40t tank fermentation tests showed that, this strain showed special activity against Spodoptera exjgua, Comparing with original Bt strain: 93005, CK strain: HD-1-580 and GC-91, the value of potency increased respectively by 429%, 655% and 114% in shake flask test. Comparing with GC-91, the value of potency increased by 180% in 40t tank fermentation test.
5.Application value of contrast-enhanced ultrasound combined with time-intensity curve to identify benign and malignant orbital neoplasms

Objective To explore the value of ultrasound-enhanced ultrasound (CEUS) in the differential diagnosis of orbital neoplasms by observing the perfusion characteristics of orbital benign and malignant neoplasms for the application of ultrasound contrast microbubbles.Methods Preoperative ultrasound imaging of 47 patients with orbital neoplasms(benign group 35 cases,malignant group 12 cases)were observed for the tumor characteristics of CEUS.The parameters of tumor imaging obtained time intensity curve (TIC) were obtained by quantitative analysis of SonoLiver analysis software,which were contrast agent arrive time (AT),rise time (RT),time to peak (TTP),mean transit time (mTT),area under the curve (AUC),maximum intensity(IMAX) and perfusion index (PI).Results Different characteristics of perfusion were shown between benign and malignant groups.Time parameters of TIC:rising slope,semidescending slope,IMAX,PI,mTT,semi-washed out time and RT were significantly different between the two groups (P < 0.05) while there was no significant difference of parameters as AT,TTP,AUC.Conclusions There is certain clinical value of the CEUS in the differential diagnosis of ocular benign and malignant neoplasms.
6.The experimental model of orthotopic left allograft lung transplantation in rats without using cuff

Objective To establish a stable model of rat orthotopic left lung transplantation using direct suture of vessels and bronchi. Methods Ten Sprague Dawley (SD) rats weighted 250 to 350 g were used as lung donors and recipients respectively. Airway and pulmonary vessels were reconstructed microsurgically using continuous running suture technique. Survival time were recorded and donor lungs were checked by autopsy. Results All 10 rats received left lung transplantation were weaned from ventilator successfully. Both of cold ischemia time and warm ischemia time were about 40 minutes. The total procedure took about 130 minutes. Autopsy was used to check the patency of anastomotic sites. No thrombosis or air leak was found. Conclusions Direct microsurgical surture can be used to establish an experimental model of orthotopic left allograft lung transplantation in rats. This method is proved to be stable, reliable and similar to clinical practices.
7.Amplification and functional identification of immature dendritic cells from rat bone marrow in vitro

Objective To establish a method for amplification of immature dendritic cells(DC) from murine bone marrow in vitro and investigate correlations between maturation degree of DC and varying dosages of granulocyte-macrophage-colony stimulating factor(GM-CSF).Methods Dendritic cells from murine bone marrow were cultured with different dosages of rm GM-CSF.The suspension cells were examined with scanning electronic microscope,and the non-sensitized T lymphocyte proliferation was observed by mixed lymphocyte reaction.Results DC cultured in lower dosage of rmGM-CSF(GMlow DC) exhibited typical characteristics of DC,and had immature characteristics in cell phenotype and cell functions with high expression of CD11c and low expression of CD80,CD86 and MHC II on the surface of the cells.The ability of GmlowDC to stimulate the proliferation of non-sensitized T lymphocyte in vitro was weaker than that of GmhighDC.Conclusions The methods of immature DCs culturing establised by allthors was feasible.The dosage of rm GM-CSF has a direct relationship with the maturation degree of DC.

9.MDCT findings and characteristics of metastatic pancreatic tumors

Objective To evaluate MDCT findings and characteristics of metastatic pancreatic tumors. Methods A total of 23 cases with metastatic pancreatic tumors were collected and the CT findings including the size, number, location, density patterns, enhancement patterns and secondary signs (dilatation of the pancreatic duct and/or bile duct, involvement of peripancreatic vessels, atrophy of distal pancreas) were analyzed retrospectively. Results A total of 35 metastatic pancreatic tumors were identified in the 23 patients. Tumors were multiple in 7 patients, the other 16 patients had single metastatic pancreatic tumor. The mean maximum diameter of the tumors was 3. 3 cm; the majority was located in the pancreatic body/tail. Thirty tumors demonstrated no enhancement in the arterial phase, and in the portal phase 23 tumors were hypodensive compared with normal pancreatic tissue, 4 tumors were isodense and 3 tumors showed a peripheral rim of enhancement. Five tumors showed markedly enhancement in the arterial and portal phase. Only 1 patients had dilated biliary and pancreatic duct, and 2 patients had dilated pancreatic duct. Only 1 case demonstrated involvement of splenic vein. 13 of the 23 patients had extra-pancreatic metastasis. Conclusions Pancreatic metastasis showed certain imaging characteristics. The accuracy of CT diagnosis could be improved by understanding of these CT findings and clinical features.
10.Analysis of clinical characters in old patients undergoing coronary artery bypass grafting

Objective To analyze the clinical characters in old patients undergoing coronary artery bypass grafting (CABG). Methods The clinical data of 426 patients undergoing CABG from January 2000 to April 2009 in our hospital were retrospectively analyzed. One hundred eighteen patients were 70-82 years old (older group), 308 patients were 24-69 years old (younger group). The perioperative risk factors, surgical complication and outcomes between the two groups were compared.Results The older group had higher incidences of post-operative complications than younger group.Pre-operative risk factors included the female, chronic obstructive pulmonary disease, peripheral vessel disease, New york heart association(NYHA) class Ⅳ, unstable angina requiring intravenous nitrates until arrival in the anaesthetic room, left ventricular ejection fraction <30%. The older group showed higher incidences of postoperative severe complication, operative mortality, and more grafts and longer time in intensive care unit (ICU), and had lower incidents of valve disease and less use of left internal mammary artery [16 patients (13. 6%) vs. 152 patients (49.4%), all P<0. 05].Conclusions The many CABG risk factors in China are different from those in the western countries.Although the higher incidents of postoperative severe complication and higher operative mortality are found in the older patients, the recovery period after operation isn't obviously prolonged. The operative outcomes are satisfactory.
