1.Epigenetic regulation and gastric cancer

The initiation and development of gastric cancer is due to the effects from both external and internal factors.Recent researches show that external environment factors regulate and control the key genes expression during the process of cell's growth and development through epigenetic modification such as DNA methylation or demethylation,histone modification and small RNA,thus playing an important role in gastric cancer's occurrence and development.
2.Arthroscopic one-stage reconstruction of anterior and posterior cruciate ligaments with LARS artificial ligaments

Objective To evaluate the efficacy of arthroscopic one-stage reconstruction of anterior and posterior cruciate ligaments with LARS artificial ligaments. Methods From October 2009 to December 2011, 8 patients with combined injury to anterior and posterior cruciate ligaments received arthroscopic one-stage reconstruction with LARS artificial ligaments. They were 7 men and one woman, aged from 26 to 52 years ( average, 35. 3 years ) . The time from injury to operation ranged from 17 to 21 days, averaging 19 days. Pre-operative and intraoperative inspections showed complete tear of both anterior and posterior cruciate ligaments. Inter national Knee Documentation Committee ( IKDC ) scoring was used at 6 months postoperation to grade the knee movement of the patients; Lysholm scoring was used at final follow-ups to evaluate the knee function. Results The operating time for this series ranged from 85 to 102 minutes ( average, 93 minutes ) . All the patients were followed up for 6 to 24 months ( average, 12 months ) . Wound infection at upper tibia occurred in one patient, which responded to debridement and screwing 4 weeks after operation. All the patients got rid of instability of the knee joint, and gained normal knee extension and fine functional recovery. At final follow-ups, the range of motion of the knee ranged from 100° to 125° ( average, 108. 4°) . Follow-ups revealed no breakage or loosening of the ligaments. By the IKDC scoring, of the 8 patients who had been graded all as D before opera-tion, one was improved into A and 7 into B. The Lysholm scores at final follow-ups averaged 86. 1+4. 8, sig-nificantly higher than the postoperative value ( 18. 7+3. 1 ) ( P < 0. 05 ) . Conclusion Arthroscopic one-stage reconstruction of anterior and posterior cruciate ligaments with LARS artificial ligaments can restore the stability of knee joint and obtain satisfactory short-term results.
5.Technique of 3D virtual surgery based on medical images

Objective To research the key technique in the virtual surgery, and to build a 3D virtual surgical system for medical imaging. Methods In VC++ platform, the volume data were segmented by using a 3D segmentation algorithm. The 3D reconstruction and virtual cutting were achieved with the help of VTK, and the performance of the varied cutting methods was analyzed. Results The interactive 3D reconstruction, virtual scalpel cutting and virtual surgical planning of the medical CT/MR images were realized. Conclusion This system offers great help for doctor to simulate the operative process, to observe the 3D models of human organs and to carry out the computer-aided surgery.
7.Efficacy of prostaglandin E1 as adjuvant therapy for mild to moderate acute pancreatitis

Objective ToobservetheclinicaleffectandefficacyofprostaglandinE1(PGE1),thedrugformicrocirculationimprovement, inthetreatmentofacutepancreatitis(AP).Methods Atotalof80patientswithmild-to-moderateAPwhowerehospitalizedandtreated in Second Department of Hepatobiliary and Pancreatic Surgery of the First Hospital of Jilin University from May 2014 to January 2015 were enrolled and randomized into two groups.Forty-four patients in control group received the conventional comprehensive therapy for AP,and 36 patients in experiment group received PGE1 in addition to the conventional therapy.The time to disappearance of abdominal symptoms and the time for serum and urine levels of amylase and serum levels of lipase,C-reactive protein (CRP),and procalcitonin (PCT)to re-turn to normal were compared between the two groups.The independent-samples t-test was applied for comparison of continuous data be-tween the two groups,and the chi-square test or Fisher′s exact test was applied for comparison of categorical data between the two groups. Results ThetwogroupshadsignificantdifferencesinthetimeforserumlevelsofamylaseandCRPandpercentageofneutrophilstoreturn to normal and hospital costs (P=0.041,0.030,0.012,and 0.026,respectively).PGE1 quickly relieved abdominal pain and distention, reducedtheserumlevelofamylase,shortenedthelengthofhospitalstay,andreducedhospitalcosts.Conclusion PGE1hasgoodclinical effect and safety in the treatment of AP,and can be applied as an adjuvant drug in the comprehensive therapy for AP.
8.Conversion from cyclosporine to tacrolimus in patients with chronic allograft nephropathy

Objective To investigate and compare the effect of cyclosporine A (CsA) vs. tacrolimus (TAC)-based immunosuppressive regimen on chronic allograft nephropathy. Methods Ninety-six patients who received a cadeveric kidney transplantation in our unit during Jan. 1995 to Jan. 2004 more than 12 months prior to study enrollment and who were being treated with CsA-based immunosuppressive treatment were included. All patients received allograft biopsy and were diagnosed as CAN. Patients were differentiated according to following regimen. Patients were either converted to tacrolimus (TAC group, n=58) or remained on their initial CsA-based immunosuppression (CsA group, n=39). The clinical data at study entry and after 3, 6 and 12 months including serum total cholesterol (TC), triglyceride (TG), low density lipoprotein (LDL), high density lipoprotein (HDL), blood urea nitrogen (BUN), creatinine (SCr), albumin were recorded during a follow-up of over 12 months. Results Though TC, TG and LDL levels remained unchanged in CsA group, while statistically reduced in TAC group respectively ( 6.60? 1.34 mmol/L vs. 5.20? 0.75 mmol/L, 3.00? 1.40 mmol/L vs. 1.90? 0.86 mmol/L, and 3.70? 0.93 mmol/L vs. 3.00? 0.72 mmol/L, P
9.CLINICAL STUDY OF TEMPORAL BONE FRACTURE AND ITS COMPLICATIONS

To explore the characteristics and treatment of temporal bone fractures and its complications, clinical data of 48 cases of temporal bone fracture admitted to our hospital from January 1989 to November 1999 were retrospectively analyzed. The results showed that in these 48 patients temporal bone fracture caused by traffic accidents accounted for 66 67%. Forty three cases survived (89 58%) and 5 died (10 41%). Middle inner ear or craniocerebral injury held 77 08% and hearing loss or tinitus held 48%; The incidences of CSF otorrhea and facioplegia were 36 7% and 3%, respectively for the longitudinal fracture, while for the transversal fracture they were 25% and 37 5%, respectively. Our conclusions are: (1) Traffic injury is the most dangerous factor of temporal bone fracture; (2) The dangerous complication is injury to cranium and brain, or multiple organs, resulting in death; (3) Middle inner ear injury is the commonest complication in temporal bone fracture; (4) CSF otorrhea is common in the longitudinal fracture and facioplegia is common in the transversal fracture.
