1.Phone Call Following-up in Patient Education of Home Rehabilitation in Orthopedic Patiens

Objective To explore the effect of phone call following-up on patient education in home rehabilitation of orthopaedic patients.Methods The special instruction about family rehabilitation was offered by the nurses with phone call following-up for orthopaedic patients according to individual plan at different periods.Results By the phone call following-up, the realization to the family rehabilitation, brace using and compliance of patients improved.Conclusion The phone call following-up has urging, consulting and directing effects on patient education in family rehabilitation of orthopedic patients, and can make a better relationship between the patients and medical staff.
2.Analysis of prognostic factors of 324 patients with multiple traumas

Objective To explore the effective prognostic factors of patients with multiple traumas. Method During January 2003 to March 2007, totally 324 patients with multiple traumas were treated in the emergency department of Renmin Hospital, Wuhan University. The relationship between prognosis and age, visiting time after injury, injury severity score (ISS) ,main wounded part and shock degree was analyzed. The data were tested using ehi-square test and logistic regression method. Results Seventy-eight patients died and 246 patients survived, the total mortality was 24.1%. Statistical differences existed in mortalities of patients with different visiting time (visiting hospital within 1 h,16.7%; visiting hospital beyond 1h, 37.4%),ISS (16≤ISS≤24, 5.7%; 25≤ISS≤40, 19.2%; IS8>40, 41.3%) and shock degree (non-shock, 11.8%; mild shock, 13.6%; moderate shock,27.7%; severe shock,46.9% ) ( P<0.01). There were no significant difference in mortalities among patients with different age (<18, 23.8% ; 18~38, 21.7% ; 38~58, 24.2% ;>58, 32.5% ) and with various main wounded part (head, 26.0%; neck, 25.3%; chest, 27.3%; abdomen, 24.1%; limbs, spine and pelvis,16.3%), P>0.05. Logistic regression study showed that mortality increased with postponed visiting time after injury, augment of ISS and aggravation of shock degree, with the exception that there was no differenoe in mortality of patients with between non-shock and mild shock. Conclusions Visiting time after injury, ISS and shock degree were valuable progrostic predictors for multiple traumas. It's vital to follow principle of damage controlled surgery and emphasis on the timeliness and integrity of medical rescue and control shock actively to reduce mortality of patients with multiple injuries.
3.Effects of r-HuEPO on RBC immunity in CRF patients

Objective To study the effects of recombinant human erythropoietin(r-HuEPO) on RBC immunity in chronic renal failure(CRF) patients.Methods Forty-five CRF patients were divided into two groups : 20 patients without r-HuEPO(control group) and 25 patients with r-HuEPO more than one month(experimental group).The RBC-C_(3b)RR and RBC-ICRR in two groups were counted under microscope and the correlations between HB or CRE and RBC-C_(3b)RR were calculated.Results The concentrationd of Hb,RBC-C_(3b)RR in experimental group were much higher than those in control group(P
4.CT and MRI diagnosis of facial nerve neuroma

Objective To investigate the CT and MRI findings of facial neuroma to promote diagnostic accuracy. Methods Nine facial neuromas consisted of 8 schwannoma and 1 neurofibroma. High resolution CT were performed on 7 cases, routinal CT scan on 2 patients. Six cases were examined by MRI, in which 5 cases were used constrast media. Results Five tumors on right side and 4 on left were detected. The tumor affected the internal auditory canal segment of facial nerve in 1 case, labyrinth segment in 4, anterior genu in 6, tympanum segment in 7, mastoid segment in 6,horizontal segment in 5 and parotid segment in 3. CT demonstrated enlargement of the facial nerve canal, benign bone remodeling in the location of geniculate ganglion and stripe soft issue along the course of facial nerve. MRI found that involved facial nerve became thickened markedly as a stripe or soft mass by several segments, that were isointense to brain on T 1WI and isointense or hyperintense on T 2WI and enhanced obviously. Conclusion The segmental soft tissue mass along the facial nerve course is the characteristic of facial neuroma. Gadolinium enhanced MRI is an effective method for detecting the lesion.
5.Endoscopic Surgery in Breast Diseases

Objective To evaluate the safety and efficacy of endoscopic techniques for diagnosis and treatment of breast diseases. Methods Related literatures of recent years were reviewed. Results A minimally invasive endoscopic technique can be performed through small incisions. This can contribute greatly to reducing postoperative pain, shortening recovery time, and achieving a good cosmetic outcome. Under endoscopy, meticulous dissection and hemostasis can be achieved. Endoscope assisted subcutaneous mastectomy, immediate mammary reconstruction, sentinel lymph node biopsy and axillary lymph node dissection, for breast cancer can be performed safely. Endoscopic surgery can also be applied for the diagnosis and treatment of benign breast tumor and transaxillary removal of glandular tissue in gynecomastia. In addition, fiberoptic ductoscopy can be used to diagnose patients with nipple discharge. Endoscopic surgery for patients with breast diseases can offer an excellent cosmetic outcome and maintain normal physiologic functions without a noticeable scar. It helps to give the patients confidence and improve the quality of life. Conclusion Breast surgery is a good candidate for endoscopic techniques.
6.Status analysis on visual disability population in Yuzhong district of Chongqing

To investigate the status of visual disability population in Yuzhong district of Chongqing, in order to provide a counter measure for improving their quality of life.METHODS:The data of visual disability population was analyzed, which was registered in federation of the disabled of Yuzhong district of Chongqing from January 21, 2009 to July 17, 2013, collected the information of their gender and age constituent ratio, visual disability level differences, and the differences of their disability pathogeny.RESULTS: There was no significant difference between the total numbers of male and female visual disability, and before the age of 60, the number of male was more than female, but after 60, it was opposite. The grade four of visual disability accounted for the largest proportion, followed by a level of grade one, two, and three. The main causes of visual disability in proper order were:ametropia ( high myopia) , retina and pigment membrane disease, ocular trauma, optic nerve disease, glaucoma, corneal disease, congenital anomaly or eccyliosis, cataract, amblyopia, agnogenic, methysis, trachoma, and the others ( hyperpyrexia) .CONCLUSlON: The diagnosis and treatment of juvenile in Yuzhong district must be strengthened, also include retina and pigment membrane disease, to reduce the visual disability caused by these diseases.
7.Study of retinal structure changes after YAG laser ablation in physiological vitreous floaters

AIM: To observe the structure changes of vitreous and retina after YAG laser ablation in patients with physiological vitreous floaters.METHODS: The selected 40 patients of physiological vitreous floaters,before YAG laser ablation,were checked for best corrected visual acuity,non-contact intraocular pressure,took their anterior segment photos,measured their foveola thickness(FT) and retinal nerve fiber layer(RNFL) by OCT.The YAG laser ablation was completed by one experienced surgeon.After the YAG laser ablation,all patients were checked for non-contact intraocular pressure and gave pranoprofen eye drops tid for 3d.At 2d,1wk,1 and 3mo after the surgery,they were reviewed for best corrected visual acuity,non-contact intraocular pressure.At 3mo later anterior segment photos were taken.At 1wk,1 and 3mo after the surgery,FT and RNFL were measured again.RESULTS: The postoperative 2d,1wk,1 and 3mo,best corrected visual acuity,non-contact intraocular pressure of the patients had no significant difference with preoperative(P>0.05).The preoperative anterior segment photos showed obvious single or sheet opacity of vitreous,the postoperative 3mo photos showed that vitreous opacities decreased or disappeared,no other abnormal changes were found.The preoperative OCT data showed that FT was 214.60±9.35μm,the postoperative 1wk,1 and 3mo FT were 213.75±9.07μm,213.40±8.83μm,213.85±9.22μm.The preoperative RNFL were upper 130.26±14.23μm,lower 133.15±14.46μm,nasal 82.48±13.50μm,temporal 75.40±11.89μm;The postoperative 1wk RNFL were upper 130.02±14.02μm,lower 132.99±14.05μm,nasal 82.35±13.07μm,temporal 75.42±11.66μm.The postoperative 1mo RNFL were upper 130.28±14.43μm,lower 133.08±13.99μm,nasal 82.31±13.72μm,temporal 75.45±12.03μm.The postoperative 3mo RNFL were upper 130.43±14.30μm,lower 133.22±14.20μm,nasal 82.27±13.11μm,temporal 75.46±11.91μm.The differences of preoperative and postoperative 1wk,1 and 3mo FT and RNFL had no statistical significance (P>0.05).CONCLUSION: YAG laser ablation has no adverse effects to vitreous and retinal structure in patients with physiological vitreous floaters,it is effective and safe.
8.The enigmatic ERH protein: its role in cell cycle, RNA splicing and cancer.

Enhancer of rudimentary homolog (ERH) is a small, highly conserved protein among eukaryotes. Since its discovery nearly 20 years ago, its molecular function has remained enigmatic. It has been implicated to play a role in transcriptional regulation and in cell cycle. We recently showed that ERH binds to the Sm complex and is required for the mRNA splicing of the mitotic motor protein CENP-E. Furthermore, cancer cells driven by mutations in the KRAS oncogene are particularly sensitive to RNAi-mediated suppression of ERH function, and ERH expression is inversely correlated with survival in colorectal cancer patients whose tumors harbor KRAS mutation. These recent findings indicate that ERH plays an important role in cell cycle through its mRNA splicing activity and is critically required for genomic stability and cancer cell survival.

9.Reference jitter values in voluntarily activated extensor digitorum communis muscle recorded with concentric needle electrode

Objective To validate the technique and establish the reference jitter values in voluntarily activated extensor digitorum communis using a concentric needle electrode (CNE) in healthy Chinese adults.Methods From January to August 2013,forty-two Chinese subjects from healthy examination center of our hospital were prospectively studied,including 20 males and 22 females.Routine electromyogram was tested and jitter was recorded with CNE in all subjects.The jitter values of action potentials pairs of muscle fibers were expressed as the mean consecutive difference (MCD) after 20 analyzed potential pairs.Results The mean MCD of 42 subjects was (23.0 ± 3.1) μs ranging from 17 to 32 μs.The mean jitter value of all 840 potential pairs was (22.8 ± 7.5) μs ranging from 8 to 54 μs.Upper 95% confidence limits for mean MCD and individual MCD were 29.2 μs and 37.8 μs,respectively.The mean value of MCD of 20 males and 22 females were (23.2 ± 2.8) μs and (22.8 ± 3.4) μs,respectively.There was no statistic difference between genders in MCD (t =0.44,P =0.66).There was no correlation between age and MCD (r =0.11,P =0.48).The mean value of mean interpotential interval was (802 ± 139) μs ranging from 541 to 1 160 μs.Conclusion The present study confirms the suitability of jitter analysis with CNE,which can serve as an objective and valuable method for testing the function of neuromuscular conjunctions.

Remote ischemic postconditioning (RIP) refers to a transient and non-lethal ischemic adaptation in non-vital organs after the vital organs having experienced long-term lethal ischemic injury. Relative to the remote ischemic preconditioning (RIPC),RIP has more value in clinical application. However,RIP is still in the cognitive stage of research. This article reviews the advances in research on RIP of each system.
