2.Balloon Cell Malignant Melanoma: A case report.

Balloon cell malignant melanoma (BCMM) is a rare histologic variant of malignant melanoma, which is composed either predominantly or entirely of large clear or foamy cells. The incidence of balloon cell malignant melanoma is about 0.15% of all cutaneous malignant melanomas. Recently, we experienced a case of cutaneous balloon cell malignant melanoma in the right lower abdomen with right inguinal and both axillary lymph node metastasis in a 56-year-old man. The cutaneous lesion was 4.5 3.5 cm in size, a well-demarcated black nodular mass, involving abdominal skin and subcutaneous tissue. The tumor was histologically composed of two types of tumor cells: predominant clear cell and focal, pigmented spindle cell. The nodal lesion was composed entirely of clear cells. Both clear and spindle cells showed positive reaction for S-100 protein and HMB-45 on immunohistochemistry.

5.Enhancing medical students’ documentation skills: the impact of an assessment and feedback program

Purpose:
We not only developed a clinical practice program for the assessment and feedback vis-à-vis medical students’ medical records but also evaluated the effectiveness of this program via a self-assessment of medical students’ competence in writing medical records pre- and post-program.
Methods:
In 2022, 74 third-year medical students were divided into four groups and participated in a 2-week program. The students’ medical records were graded on a scale ranging from 1 to 3 daily, and the mean scores for 2 weeks were compared. Pre- and post-program, the students’ self-assessment survey was conducted.
Results:
The mean scores increased from 1.30 in the first week to 2.14 in the second week. The mean score of self-assessment showed significant improvements, increasing from 2.43 to 4.00 for medical record, 2.64 to 4.08 for write present illness, 2.08 to 3.89 for initial orders, 2.35 to 4.34 for signature, and 2.38 to 3.97 for consent (all p<0.001).
Conclusion
We found that providing students with real-time assessment and feedback on their medical records increased their skills and confidence in medical records writing.
6.Development of a Breastfeeding Effectiveness Scale (BES).

PURPOSE: The purpose of this study was to develop and evaluate breastfeeding effectiveness scale to measure effectiveness of breastfeeding for mothers in the early postpartum period. METHODS: A conceptual framework was constructed from properties of effective breastfeeding (Yang and Seo, 2011), and item construction was derived from literature review and analysis of the data along with interviews with breastfeeding mothers. Content validity was tested by experts. Each item was scored on a five-point Likert scale. The preliminary questionnaire was administered to 248 breastfeeding mothers. Data were analyzed using item analysis, factor analysis, Pearson correlation coefficients, and Cronbach's α. RESULTS: From the factor analysis, 20 items in seven factors were derived. The factors were identified as mother's satisfaction, suckling, assurance of milk quantity, infant's satisfaction, latching on, infant's feeding desire, and breastfeeding positioning. The seven factors explained 65.1% of total variance, Cronbach's α of the total items was .83 and the factors ranged from .44 to .75. CONCLUSION: Results of this study suggest that breastfeeding effectiveness scale is a reliable and valid instrument to measure breastfeeding effectiveness of mothers in the early postpartum period.

7.A Study of Partial Excision and Suvdermal Exicision in Surgical Treatment of Axillary Osmidrosis.

There are 3 basic methods for surgical treatment of axillary osmidrosis; 1) method that removes only subcutaneous cellular tissue without removing skin 2) method that removes skin and subcutaneous cellular tissue en bloc, and 3) method that partially removes skin and subcutaneous cellular en bloc as well as removing the subcutaneous cellular tissue of the adjacent region. We studied the results of partial removal of the skin and subcutaneous cellular tissue en bloc, as well as the removal of subcutaneous cellular tissue of the adjacent region to compare the results of the bipedicled flap with the graft conversion method. There was no difference between two methods in results and complication rates. There are 3 advantage to this procedure. First, about 70-80% of apocrine glands were centrally distributed among the axillary hairbearing region therefore, resection of the central portion of axillary hair distribution area is important for good result. Second, the preservation of the subdermal plexus with careful excision of adjacent underlying subcutaneous tissue under the aid of the magnifying surgical loupe, is important for good wound healing. Third, the central excision of the axillary hair distribution area provides good exploration for undermining and defatting of the undersurface of the adjacent area, therefore it tooks a shorter operation time.

8.Distress and Quality of Life in Cancer Patients Receiving Chemotherapy.

PURPOSE: The purpose of this study was to identify the levels of distress and quality of life in cancer patients receiving chemotherapy and to identify the relationship between distress and quality of life. METHODS: Data were collected during 2013 from 128 cancer patients receiving chemotherapy at a hospital located in Seoul, South Korea. Distress thermometer and problem list were used to measure distress, and the Functional Assessment Cancer Therapy-general questionnaire was used to measure quality of life. Descriptive statistics, t-tests, one-way ANOVA, the post-hoc Scheffe test, and Pearson's correlation coefficients were used to analyze the data. RESULTS: Among the 128 participants, 68 (53.1%) were male and the mean age was 52.55 years. Forty two (32.8%) were diagnosed with stage 4 and 73 (57.0%) were having recurrence or metastasis. The mean score for distress was 4.01 out of 10. Seventy four (57.8%) participants had a score of 4 or higher, indicating clinically significant distress. Emotional problems had the highest item mean among five domains. The mean score for quality of life was 68.51 +/- 14.22 out of 108. Distress and quality of life were negatively correlated (r= -.52, p<.001). CONCLUSION: Distress level with specific problems should be regularly assessed to relieve distress and improve the quality of life of cancer patients receiving chemotherapy.

9.Nursing Need and Satisfaction of Families with their Hospitalized Children.

This study was done to evaluate the need and satisfaction for nursing care of the families with their hospitalized children. The data were collected through the questionnaire from March 15, 2002 to April 7. Subjects were the 103 families caring for their hospitalized children at pediatric ward in two university hospitals in Daegu. The nursing need instrument was developed by Seo(1999) and modified by the researcher of this study based on the classification of nursing care area(nursing assessment, direct nursing, education and counseling, and facilities and environment). The nursing satisfaction instrument was developed by Wandelt & Ager(1974) and modified by Park(1994) based on classification of nursing care area (psychosocial care, physical care, general care, professional care, and communicative care). The results are summarized as follows: 1. The scores in the nursing need showed in the Direct nursing(3.41 +/- .42), Facilities and Environment(3.38 +/- .46), Education and Counseling (3.35 +/- .40), and Nursing Assessment(3.14 +/- .41) area in order. 2.The scores in the nursing satisfaction showed in the Psychosocial care(3.70 +/- .74), Commu- nicative care(3.60 +/- .72), General care(3.42 +/- .76), Professional care(3.38 +/- .82), and Physical care(3.32 +/- .70) area in order. 3.General characteristics of families which influence on the satisfaction showed a significant difference according to their educational(F=5.63, p=.001) and economical level(F=4.47, p=.006), and hospitalized experience(t=2.30, p=0.02). 4. There was no correlation between the nursing need and the nursing satisfaction.

10.Epithelioid Leiomyosarcoma of Retroperitoneum: A case report.

Epithelioid smooth muscle tumor is relatively rare and potentially malignant, especially in retroperitoneum. Distinct criteria for malignancy still have not been clarified in this epithelioid variant arising in retroperitoneum. We report a deceptively benign-appearing epithelioid leiomyosarcoma in a 50-year-old female. She was admitted with abdominal discomfort and dysuria. Abdominal CT showed a well-demarcated, 10 10 cm sized, solid mass in retroperitoneum. Concomitant metastatic lesions were noted in right lung field. Surgical excision of retroperitoneal mass and right lung lobectomy were performed. The retroperitoneal mass showed yellowish-tan, well-delineated and lobulated appearance. Histologically, this tumor was composed of predominantly epithelioid, round to oval cells with distinct clear cytoplasm and slightly atypical nuclei. Mitosis was rare (0~1/50 HPF). Lung lesions were morphologically similar to that of retroperitoneum.
