3.A Study on the Usefulness of Birth Registration Data in Rural Korea.

The onprovement of civil registration reguires continuous study rather than periodic efforts. More and better statistics, however, are urgently required to formulate development programs and planning, Data obtainable from the civil registration are usually marred by errors of omission which are difficult to correct. This study aimed at finding out the problems occurring when a set of crude birth registration data in a rural area is used. Data Sources of this study are : 1) For birth registration : government birth registration records obtained from myun office and other government offices. 2) for the actual number of births : birth and child records from the Kang Wha Community Health Project. The study area is Sunwon Myun and Naega Myun in Kang Wha Gun, Gyunggido. The reference period for the accumulated data is one full year : Jan. 1st 1975 - Dec. 31st 1975. Major findings are as follows : If the number of registered birth is compared with the actual number of births which occurred in the target area, the former os far greater than the latter. The general assumption usually is , that the actual number of exceeds the registered number of birth in Korea. The observation from this specific study in this specific target area, shows the opposite trend. The number of births which actually occurred during the year of 1975 in the study area namely 256. The difference comes mainly from the fact that many cases of births from other areas were registered in the target area. In other words birth is not registered where it occurred but where the permanent residence address is. Among 550 births registered in the target area 66% did not occur in the target area. Only one third of all registered births were registered within the legal period for birth registration which is 2 weeks. 34% of the registered births actually occurred in 1974, but were registered in 1975. In 55% of the cases a difference was observed between the actual data of birth and the registered data of birth. From the 256 births which occurred in the target area, only 153 births (59%) were registered at the myun office and the remaining 130 births (41%) were not resistered there in the year of study. 6% of the 550 cases listed as registered have no separate registration sheets. Nevertheless, they definitely have been registered in the birth list at the myun office. 3% of the 550 cases are not recorded in this list but have a separate registration sheet at the myun offices. In conclusion, birth registration data have many errors and problems. Their usefulness as a source for vital and other statistics should be reconsidered. A series of sound methological studies will be necessary to establish their actual usefulness. A continuous and permanent compulsory system of birth recording is needed.

4.Use of Gore-Tex in Bleb Revision after Trabeculectomy with Mitomycin.

Trabeculectomy, as a surgical intervention in the treatment of glaucoma, has become most popular. The most common cause of filtration failure is obstruction of aqueous outflow due to bleb fibrosis. Adjunctive intraoperative mitomycin-C has been reported to improve the success of filtration surgery in eyes of high risk for failure. But its use increases the rate of complications after filtering surgery: especially postoperative hypotony, wound leakage, and scleral necrosis. We have experienced two patients whose scleral flaps were necrotized after trabeculectomy with mitomycin-C in uncomplicated glaucoma. We report successful results in revision of filtering bleb by using Gore-Tex as a patch graft.

6.Partial characterization of a 29 kDa cysteine protease purified from Taenia solium metacestodes.

A 29 kDa cysteine protease of Taenia solium metacestodes was purified by Mono Q anion-exchanger and Superose 6 HR gel filtration chromatography. The enzyme was effectively inhibited by cysteine protease inhibitors, such as iodoacetic acid (IAA) and trans-epoxy-succinyl-L-leucyl-amido (4-guanidino) butane (E-64) while inhibitors acting on serine- or metallo-proteases did not affect the enzyme activity. The purified enzyme degraded human immunoglobulin G (IgG), collagen and bovine serum albumin (BSA), but human IgG was more susceptible for proteolysis by the enzyme. To define the precise biological roles of the enzyme, more detailed biochemical and functional studies would be required.

7.Ultrasonographic findings of gastric carcinoma

Stomach carcinoma is more common disease in korea than western contries. The reported ultrasonographicfindings of gastric carcinoma were thickening of gastric wall and “pseudokidney” sign. The auther analizedultrasonographic findings of 101 cases with gastric carcinoma who were performed ultrasonography and gastroscopyat Kyung Hee Universtiy Hospital from Oct. 1982 to Oct. 1985. The results were as followings; 1. Types of gastriccarcinoma were consisted with infiltrative type 68 cases, infiltrative type with ulceration 16 cases, polypoidtype with ulceration 1 cases, infiltrative and polypoid type 4 cases, linities plastica type 3 cases, lcerativetype 1 case and polypoid type 1 case. 2. Extent of the lesions were in body and antrum 45 cases, entire stomach 18cases, antrum 18 cases, body 12 cases, body and fundus 6 cases. 3. Ultrasonography was useful in demonstrating theextent of the tumor and the presence of materials elsewhere in abdoment.

The prognosis of esophageal cancer is poor and strategies for treatment depend on the tumor stage at the time of diagnosis. Surgery is the main therapeutic modality in esophageal cancer and known as the only treatment for cure. Preoperatively it is most important to assess whether the primary tumor is completely resectable or not. Previous staging modality such as CT can not clearly define the depth of invasion and lymph node metastasis of esophageal cancer which is the most important factor in assessing the possibility of curative resection. Endoscopic ultrasonography is now considered as an useful method in evaluating staging and resectability of esophageal cancer. We compared the findings of endoscopic ultrasonography with pathology result to evaluate the accuracy of this new technique in staging of esophageal cancer in 4 esophageal cancer patients who received surgery among the 23 patients assessed by endoscopic ultrasonography due to esophageal cancer, The depth of invasion, lymph node metastasis, and staging was correct in 3 among 4 patients. We consider endoscopic ultrasonography is an useful technique in staging of esophageal cancer.

9.A Clinical Study of Androgenetic Alopecia (V).

BACKGROUND: Androgenetic alopecia is characterized by local and gradual transformations of terminal scalp hair to vellus hair, which has a shorter and thinner shaft. The proportion of patients with androgenetic alopecia, among the total number of patients with alopecia, seems to be gradually increasing. OBJECTIVE: The purpose of this study was to evaluate the family history, and clinical and endocrine status of patients with androgenetic alopecia. METHOD: 796 patients with androgenetic alopecia were assessed at the Alopecia Clinic, Department of Dermatology, College of Medicine, Chung-Ang University Hospital over a 3 year period (from January 2001 to December 2003). RESULTS: The following results were obtained: 1) There were found to be 1.7 more male patients (498) than female patients (298). They were also found to be mostly in their twenties (male 253; 50.8%, female 90; 30.2%). Previously, male to female ratio was 3.46: 1, but was now found to be 1.7: 1. 2) In the 498 male patients, Norwood class II was dominant (130 patients; 26.2%). In the 298 female patients, Ludwig class I was superior (204 patients; 68.5%). 3) 321 patients (64.5%) of 498 male patients and 177 (59.4%) of 298 female patients hada family history of androgenetic alopecia. 4) The most common accompanying disorder was seborrheic dermatitis (male 287; 73.0%, female 137; 64.6%). 5) Serum testosterone levels were increased in 84 (16.9%) of 498 male patients and 44 (14.8%) of 298 female patients. CONCLUSION: Most of these results are compatible with our previous study carried out in 1995. But, female androgenetic alopecia patients are nowadays increasing in number, therefore, the ratio of male to female is markedly different.

10.mRNA Expression of Thrombospondin-1 and -2 in Severe Endometriosis Patients in Korean Women.

OBJECTIVE: We investigated the expression of TSP-1 and -2 in eutopic endometrium of advanced endometriosis and control patients. METHODS: The 33 endometriosis patients and 32 controls were enrolled. Endometrial samples were obtained from 65 premenopausal women aged 29-44 years, undergoing laparoscopic surgery or hysterectomy for non-malignant lesions. Sufficient samples were collected from 33 patients with endometriosis stage III and IV and 32 controls without endometriosis confirmed by laparoscopic surgery. The mRNA expression from eutopic endometrium for TSP-1 and -2 were analyzed by RT-QC PCR. RESULTS: The mRNAs of TSP-1 and -2 were expressed in eutopic endometrium from endometriosis and normal controls throughout the menstrual cycle. There were no significant differences in expression of TSP-1 and TSP-2 in eutopic endometrium between controls and endometriosis patients. CONCLUSION: Our results indicated that TSP-1 and -2 had no crucial role compared to other molecules in the regulation of angiogenesis. These findings also suggest that dysregulation of other angiogenic regulators would be concerned in pathophysiologic role in endometriosis development.
