Malignant melanoma is a tumor arising within the melanocyte system of the skin, eyes and juxtacutaneous mucous membrane. Tbrough the lymphatic channel developed early metastasis and regional lymphade- nopzthy developed. Malignant melanoma may arise on apparently normal skin, in a junction nevua, rareh in a compound nevus and extremeIy rarely in an intradermaI nevus and 20% of total incidence of malignant melanoma are preceded by a nevocellular nevus. A 41 year-old male patient who had multiple, pea to walnut sized, black colored. nodules developing on entire body visited our Dermto. dept. About one year ago, solitary black colored nodule arose from preexisting pigmented nevus which had been from 20 yrs ago on Rt. ant. axillary area and then nodule remcved by wide exeision and regional lymphadenectomy at a Hosp. 2 months after surgical operation, multiple pea to walnut sized, black colored nodules has developed on entire body. There was no lymphadenopathy on entire body. In H-E & Fontana-Masson stain, numerous bizzarre large cells with hyperchromatic variable sized melanin pigments are seen in the dermis and subcutaneous tissues Mitotic figutes are present. Diagnosis was confirmed by the clinical and histological feature. The patient was treated with BCG waccination(4*10^6-8*10^6 live organisms weekly) by altemately intralesional inj, on the lesion and subcuraneous inj. on both shoulder during 6wks and has been in follow-up.

Tuberculosis of the skin may present clinically as plaques, ulcers, verrucous, lesions, nodules, tumors, vegetatives reactions, and cicartricial infiltration. The reaction of the host to the presence of Mycobacterium tuberculosis also varies with the extent and the degree of invasion, the virulence of the bacilli, and nonspecific factors such as age and generaI state of health. The incidence of cutaneous tuberculosis has tendency to decline in the world because of thc availability of effective antiTB drugs, elimination of infected milk herbs, and an elevation of living standards, but in Korea, cutaneous tuberculosis remained to be a significant medical problem. We experienced 21 year-old male who had a case of 3 clinical types including Lupus vulgaris, Scrofuloderma, Papulonecrotic tuberculid. Diagnosis of 3 clinical types vere established by clinical history and signs, histo pathology, tuberculin test and effective of treatment vith INH, streptomycin and literature were reviewed.

The solitary type of Koratoacanthoma is a common, rapidly growing, benign tumor with a natural history of spontaneous involution which was first described by Hutchinson in 1889. It was well known that this Keratoacanthoma is close resemblence to squamous cell carcinoma clinicallt & histopathology. The etiology of Kertoacanthoma is ucertain but it have been associated with many exogenous factors, including sun exposure, occupational exposure to heat and trauma, and tar and with autoimmune etiology and viral infection. We experienced 2 cases of soitary type of Keratoacantoma with typical clinical and histopathologic findings which presented on the pubic area and forehead but had no predisposing factors of Keratoacanthoma. The one was 66-year-old woman who had been a history of proritic, rapidly growing tumor on Mons Pubis since about 6 months ago. The another one was 40 year-old male who had been a history of asymromatic, growing tomor on forehead since about 3 months ago. In the H-E stained sections, there were horn filled invagination, dyskeratotic cell, horn pearl in the epidermis and an inflammatory infiltration in dermis. Diagnosis was cinfirmedby the clinical and histopatholigical pictures. The former was treated by electrocautery after simple excision and the latter by curretage and electrodesiccation.

PURPOSE: The purpose of this study was to identify the factors related to depression of elderly patients in geriatric hospitals. METHODS: The subjects were 195 elderly patients who met the inclusion criteria of scores more than 18 on the K-MMSE score and no reported mental disease. The data were collected from February 20 to March 20, 2009. The research instruments utilized in this study were depression (GDSSF-K), activities of daily living and self esteem (RSES), social support, life satisfaction. Data were analyzed Pearson correlation and Multiple Stepwise Regression using SPSS 15.0. RESULTS: Depression score were negatively correlation with ADL, social support, life satisfaction and self-esteem. Among the factors studied related to depression, life satisfaction had highest explanatory power of 36.5% and it was followed by physical health status and activity of daily living. These explained 43.7% of the depression. CONCLUSION: The mean GDSSF-K 8.94, which indicates the higher than middle levels of depression. The findings suggest that it is important to develop educational programs to increase life satisfaction, physical health status and activity of daily living. Nursing interventions, including volunteer activities, health promotion program, and sports program could be useful in enhancing these factors.

This study was aimed at evaluating the nutritional status of Korean lactating women, who were attending peripheral community clinics in low-income areas, by anthropometric measurement, estimating dietary intakes and analyzing the contents of breast milk components, serum lipid content and fatty acid composition. Regarding anghropometric measurements, maternal circumferences decreased significantly at the trunk but not at the limbs. The skinfold thickness on all regions had the tendency to decrease during lactation. It was found that intakes of energy, protein, lipid and carbohydrates were desirable while calcium, zinc, copper and vitamin A were low and sodium was excessive. The energy % of fat was 22.41% of the total energy which is higher than the RDA for adults but it is proper for lactating women, but energy % from each fatty acid was usually low and an intake ratio of omega6/omega3 fatty acids was desirable. Accordingly, it is thought that dietary fat intake was balanced in quality. Concentrations of the nutrients in matured human milk showed the same range as that reported form different regions with the exception of calcium and magnesium which were relatively low. Serum lipid contents were decreased with the time postpartum and the average serum lipid content of lactating women were similar to the normal range of adults. Serum polyunsaturated fatty acids. especially omega6 fatty acids have gradually increased with the time postpartum, but the composition were not greatly influenced by dietary intakes.

OBJECTIVES: This study was aimed to examine the prevalence of neuroleptic-induced irregular menstruation and its related factors as well as the possible relationship of irregular menstruation with psychopathology and extrapyramidal symptoms in schizophrenic patients. METHODS: Thirty five schizophrenic inpatients at Naju National Mental Hospital who satisfied the diagnostic criteria of DSM-IV were selected. They had drug-free periods for more than 1 month before admission and had been taking neuroleptics for more than 3 months after admission. From all patients, reliable information regarding the sociodemographic characteristics, the menstrual histories before and after admission and clinical characteristics, including previous and current medications were investigated. The following were also evaluated: Serum prolactin level(by radioimmunoassay), psychopathology(by positive and negative syndrome scale: PANSS), and extrapyramidal symptoms(by extrapyramidal symptom rating scale: ESRS). RESULTS: Twelve patients(34%) showed regular menstruation and 23 patients(66%) were suffering from irregular menstruation. Of those with irregular menstruation, 13 patients(37%) had oligomenorrhea and 10 patients(29%) had amenorrhea. There were no differences in sociodemographic characteristics and menstrual histories before admission between the regular and irregular menstruation groups. But, the significant differences between the two were found in the following variables: The irregular menstruation group had shorter durations of illness, higher daily doses of current neuroleptics, higher serum prolactin levels, higher scores on the general psychopathology scale of PANSS, and higher scores on the parkinsonian symptom scale, hypokinetic symptom factor and clinical global impression of ESRS. CONCLUSION: The high prevalence of the irregular menstruation in schizophrenic patients reflects a state of hyperprolactinemia due to the interference of dopamine activity by high doses of neuroleptics.

This paper aims to examine the factors that affect the amount of official development assistance (ODA) a developing country receives for healthcare by the Korean government. We empirically tested to what extent the amount of Korea's ODA in health care services, infrastructure, HIV/AIDS, and tuberculosis are affected by the relevant demand factors in the recipient countries. To do so, we carried out country-level multivariate regression analyses by setting the amount of ODA for four health care sectors as dependent variables and the relevant demand factors and economic factors as independent variables. A panel dataset was constructed by combining ODA data from the Organisation for Economic Cooperation and Development and World Development Indicators. The analyses showed that the ODA for health care in Korea is partly meeting the recipient's health care needs. In particular, the recipients with a smaller number of physicians are likely to receive more ODA for medical services. Meanwhile, the amount of international trade with Korea is likely to affect the amount of ODA for medical services. However, disease factors, such as prevalence of HIV/AIDS and tuberculosis, did not positively affect the amount of ODA for those diseases. These results indicate that Korea's ODA system for health care needs to be improved to meet the demand of the recipients in order to achieve the humanitarian objectives set by the international community. We hope that the medical community and the government of Korea can cooperate in setting the global policy agenda for health care ODA based on concrete evidence-based healthcare policy research.

Nutrition support for preterm infants is important to achievement of a postnatal growth rate optimizing normal fetal intrauterine growth rates and for improvement of patient outcome. However, sufficient nutrient delivery is not easy because of underlying illness. Although enteral nutrition (EN) is preferred over parenteral nutrition (PN), immature gastrointestinal features and inability to suck and swallow until 33 weeks' gestation disrupt. In patients with inability to tolerate adequate EN for more than 2 or 3 days, to optimize nutrition support and minimize complications, PN should begin within 24 hours after birth. The purpose of this study is to review the PN in preterm infants for the most appropriate nutritional support to meet each patient's requirement.
