1.Expression of Claudin-1 and -4 in Benign Lesions and Invasive Ductal Carcinomas of the Breast.

BACKGROUND: The claudins are a family of transmembrane proteins associated with tight junctions and they are critical for maintaining cell-to-cell adhesion in sheets of epithelial cells. However, their role in the progression of cancer remains largely unexplored. The aims of this study were to evaluate the expression patterns of claudin-1 and -4 in benign lesions and invasive ductal carcinomas (IDC) of the breast, and relationships between the expression of these markers and the clinicopathological characteristics in IDC patients. METHODS: We examined the claudin-1 and -4 protein expressions by performing immunohistochemical stainings in 54 benign lesions and 120 IDCs via the tissue microarray method. We evaluated the correlation between the expression of these markers and the clinicopathological characteristics of IDC. RESULTS: The expressions of claudin-1 (p=0.099) and -4 (p=0.000) were up-regulated in IDCs as compared with benign lesions. The claudin-1 expression correlated with the loss of estrogen receptor (p=0.036) and progesterone receptor (p=0.011). The claudin-4 expression correlated with lymph node metastasis (p=0.043), the nuclear grade (p=0.030), the histologic grade (p=0.007), and the loss of estrogen receptor (p=0.001) and progesterone receptor (p= 0.029). CONCLUSIONS: These results suggest that claudin-1 and -4 may play a significant role in the carcinogenesis of IDC of the breast and these may represent novel markers for this disease.

2.Establishing Homepage for Basic Nursing Science Education: in the Nervous System of Human Body.

In this study, we developed an educational content for learning the nervous system of human body which is one of the educational topics in basic nursing science. We also developed and operated Homepage program based on that content. This program was designed for providing learning contents consistently to the nursing students and nurses regardless of the education system or location. The strengths of the present study are as follows: 1) Learning contents of the study were systematically developed by the methodological research method, 2) The students who could access the program would be broadened because the program is internet-based, 3) The students and teachers could have effective communication via a visiting list or bulletin board. In the future, it is expected that similar internet based learning programs are developed continuously in many other areas in the basic nursing science as well as the nervous system of human body.

4.Study on the Development of Nursing Information System about Drugs.

The purpose of this study was to develop an information system about drugs. The Joint Application Design Method which is popular in developing information systems was used. This system will be useful for the health care personnel and nursing students with easy access to information by using computer. The user requirements for this system were collected and analyzed through a survey of 50 nurses in a hospital and 30 students at a school of nursing. Based on this analysis, the specific contents were designed. The characteristics of this system as follows. Firstly it is the fast searching function which uses not only names of drugs(generic name, functional name, commercial name), but also names of diseases. Secondly, it is the function which maintains an updated database by the way of adding, updating and deleting the data. Thirdly, this system is useful for the education. In teaching and learning process, Computer-Assisted Instruction(CAI) is considered as one of the outstanding method bacause the individual differences in learners are respected and thus educational effect in pharmacology is expected.

6.Emergency Department Utilization by In-hospital Healthcare Workers after COVID-19 Vaccination

Background:
This is an observational study to analyze an emergency department (ED) utilization pattern of coronavirus disease 2019 (COVID-19) vaccinated in-hospital healthcare workers (HCWs).
Methods:
We included 4,703 HCWs who were administered the first dose of the COVID-19 vaccine between March 4 and April 2, 2021, in a tertiary hospital in Korea where fast-track and post-vaccination cohort zone (PVCZ) were introduced in ED. We analyzed data of participants' age, sex, occupation, date and type of vaccination, and their clinical information using SPSS v25.0.
Results:
The sample comprised HCWs, who received either the ChAdOx1 (n = 4,458) or the BNT162B2 (n = 245) vaccines; most participants were female (73.5%), and 81.1% were under 50 years old. Further, 153 (3.3%) visited the ED and reported experiencing fever (66.9%) and myalgia (56.1%). Additionally, 91(59.5%) of them were in their 20s, and 106 (67.5%) were assigned to the PVCZ. Lastly, 107 (68.2%) of the patients received parenteral management.No patient required hospitalization.
Conclusion
In conclusion, vaccinated HCWs who visited the ED with adverse events had a high incidence of fever and a low likelihood of developing serious illnesses. As the COVID-19 vaccination program for Korean citizens continues to expand, strategies to minimize unnecessary ED overcrowding should be put into effect.

8.Review of atypical cytology of thyroid nodule according to the Bethesda system and its beneficial effect in the surgical treatment of papillary carcinoma.

PURPOSE: For the atypical cases of fine needle aspiration (FNA) cytology of thyroid nodules, ultrasonographic findings are a primary guideline for the surgical treatment. However, they have the intrinsic risk of overtreatment, as well. In this study we examined whether the Bethesda system could provide a real effect on the diagnostic rate of atypical cytology, and thereby reduce the number of cases diagnosed as atypical from FNA cytology. METHODS: We reviewed 166 cases diagnosed as atypical by FNA cytology at this institute between the years 2005 to 2010. We classified these cases on the basis of ultrasonographic and cytological findings and compared them with the histological results. RESULTS: Ultrasonographically, findings suspicious for malignancy and indeterminate were associated with 83.7% and 47.2% of malignancy rates, respectively. Cytopathologically, the malignancy rates varied according to the main cytological features and the highest malignancy rate was 77.3%. Based on the Bethesda system, 39.2% of the cases diagnosed as atypical could be grouped into the category of suspicious for malignancy and yielded a malignancy rate of 76.9%. CONCLUSION: Although ultrasonography provides an excellent guideline for the surgical treatment of atypical cases, it also showed considerable risk of overtreatment. The Bethesda system did not offer definitive effects on the rate of atypical cytology, but this system seemed to provide stricter boundaries for the atypical cytology and to aid in reducing the rates thereof. This in turn may permit that more limited cases are allotted to ultrasonographic decision making.

9.Level of Problem Drinking, Health Status and Perception of Need for Moderate Drinking by Male Blue-collar Workers in Steel Factory.

OBJECTIVE: This study was conducted to investigate the level of problem drinking by male blue-collar workers in light of their health status and their perception of drinking amount. METHOD: A steel factory located in the central region of Korea was chosen and 182 questionnaire responses from male workers were analyzed using the SPSS/WIN 11.0 in reference to each worker's physical examination record. RESULTS: According to the criteria set by WHO, 19 workers(10.4%) were found to have 'problem drinking', 81 workers(44.5%) 'alcohol use disorder'and 14 workers(7.7%) 'alcohol dependence'. Only the remaining 68 workers(37.4%) were found to have 'normal drinking'. In relation to the workers'health status by CMI, 'alcohol dependence'scored 62.92, and 'alcohol use disorder'scored 56.53, both of which were far higher than the 'problem drinking'score of 44.52, In particular, it was found that each of the diabetes group(t=2.32, p<.05) and the liverdisease group(t=2.01, p<.05) drank much more than the non-diabetes group or the non-liver-disease group. In terms of the workers'perception of drinking amount, 'alcohol use disorder'and 'alcohol dependence'scored 147.62 and 144.21 respectively, both of which were far higher than the 'problem drinking'score of 138.57. CONCLUSION: The above results suggest the need to educate workers on the necessity and practices of moderate drinking before they enter a state of alcohol use disorder or alcohol dependence. Accordingly, as a part of early management and treatment of the workers'drinking problem, it may well be necessary to activate OAPs (Occupational Alcohol Programs).

10.Successful Transplantation of 3 Cases of Oligomeganephronia.

"We retrospectively reviewed the results of 1,850 fine needle aspiration cytology (FNAC) of thyroid nodules performed from 1990 to 1991 in the Department of Pathology, Seoul National University Hospital. Among 1,528 cases and 322 cases aspirated by clinicians and a pathologist, 465 cases (30.4%) and 13 cases (4.0%) of the aspirates were inadequate, respectively. In 227 cases, correlation of the FNAC diagnosis and histologic diagnosis was done. Excluding the inadequate cases, the sensitivity for the detection of neoplasm (malignancy together with follicular adenoma) was 86.4% and the specificity was 70.7%. The overall diagnostic accuracy was 79.0%. There were 16 false-positive cases (7.0%), and 19 false-negative cases (8.4%). The predictive value of each cytologic diagnosis was 92% in papillary carcinoma, and 100% in Hashimoto's thyroiditis. The expectancy of malignancy was 52.8% in ""suspicious malignancy"" and 26.7% in ""atypical lesion""."
