1.CT Diagnosis of Pulmonary Echinococcosis

Objective To analyse the CT features of pulmonary echinococcosis and to assess CT value in diagnosing this disease.Methods CT fingings of pulmonary echinococcosis in 35 cases proved pathologically were analyzed retrospectively.Results Pulmonary granulosus echinococcosis was 33 cases including simple hydatid cyst (11 lesions),datugter cyst hydatid (3 lesions),with calcification (1 lesion) ,rupture (11 lesions),merging infection (4 lesions) and rupture into thorax (3 lesions).Pulmonary alveolar echinococcosis was 2 lesions,CT findings apperared as irregular,peripherally scattered more nodules with small vacuoles or calcification and in combination with hepatic alveolar echinococcosis.Conclusion Pulmonary echinococcosis had characteristic features on CT,CT examination can provide more imaging informations for the treatment of pulmonary echinococcosis
2.Effect of ligustrazine hydrochloride on coagulation reaction and inflammation reaction in single valve replacement patients with rheumatic heart disease undergoing cardiopulmonary bypass.

OBJECTIVETo observe the protection effect of Ligustrazine Hydrochloride (LH) on coagulation reaction and inflammation reaction in single valve replacement patients with rheumatic heart disease undergoing cardiopulmonary bypass (CPB).
METHODSTotally 40 patients undergoing single valve replacement were recruited in the study and randomly assigned to the two groups, the treatment group and the control group, 20 in each group. In treatment group LH (3 mg/kg) was intravenously infused from the jugular vein. LH (3 mg/kg) was also added in the CPB priming. In the control group LH was replaced by equal amount of normal saline. Endothelial micro-particles (EMP) count was detected before CPB, 30 min after CPB, 1 h and 24 h after CPB finished. The coagulation reaction time (R), coagulation time (K), clotting formation velocity (alpha angle), maximum amplitude (MA), coagulation index (CI), platelet (PLT), hypersensitive C reactive protein (hs-CRP), IL-6, and IL-10 were detected before CPB, 1 h and 24 h after CPB finished.
RESULTSThere was no statistical difference in aorta arresting time, period of CPB, post-operative drainage volume, plasma transfusion volume, post-operative respirator assistant time, and hospitalization time between the two groups (P >0.05). Compared with pre-CPB in the same group, the count of EMP was much higher at 30 min after CPB and 1 h after CPB finished (P < 0.01). R and K, hs-CRP, IL-6, and IL-10 increased at 1 h and 24 h after CPB finished (P <0.01,P < 0.05). The alpha angle,.MA, CI, and PLT decreased 1 h after CPB finished (P <0.01). The a angle increased, while CI and PLT decreased 24 h after CPB finished (P <0.05). Compared with the control group in the same period, the count of EMP was lower in the treatment group 30 min after CPB and 1 h after CPB finished (P <0. 05, P <0. 01). R and K values obviously decreased in treatment group 1 hour after CPB finished (P <0. 05), while a angle, MA, CI, and PLT increased (P <0. 05, P <0. 01). hs-CRP and IL-6 decreased in the treatment group 1 h and 24 h after CPB finished (P <0.05), while IL-10 increased (P <0.05). The count of PLT increased 24 h after CPB finished in the treatment group (P <0. 05).
CONCLUSIONLH had certain protection effect on the vascular endothelium undergoing CPB, and lower excessive activation of coagulation reaction and inflammation reaction in patients undergoing CPB.

Antimicrobial resistance is a growing problem in the eradication of Helicobacter pylori. A combination therapy with proton pump inhibitor, clarithromycin, and amoxicillin is recommended as the first-line treatment regimen in Korea. However, the eradication rate with a standard triple therapy has been unsatisfactory in the last decade, and one of the main reasons for treatment failure is the increasing prevalence of strains resistant to antimicrobials. Therefore, comprehensive and detailed information on antimicrobial resistance is mandatory to optimize the strategy of eradication treatment. The antimicrobial resistance of H. pylori is reported to vary according to study population, geographical region, and test methods. In this review, the prevalence of antimicrobial resistance of H. pylori isolates in Korea is summarized on the basis of recent studies.

5.Common linear B-cell epitopes in human hepatitis B virus core protein and woodchuck hepatitis virus core protein.

OBJECTIVETo search for and verify some common B cell epitopes in the core proteins of woodchuck hepatitis virus and human hepatitis B virus.
METHODSMonoclonal antibodies against both core proteins of woodchuck hepatitis virus (WHV) and human hepatitis B virus (HBV) were prepared by inoculating Balb/c mice with denatured recombination WHV and HBV core proteins. ELISA and immunoblotting assays for WHcAg and HBcAg were carried out by using these antibodies. Immunohistochemistry was carried out with liver tissue sections of both WHV-infected woodchucks and chronic HBV-infected patients. The epitopes were mapped with the mouse mAbs (6D1 and 1H4) by using a panel of 24 16mer overlapping peptides covering the entire WHcAg. The amino acid sequences of WHcAg and HBcAg were compared.
RESULTSCross-reactions were observed between mAbs (6D1 and 1H4) and WHcAg and between Mabs and HBcAg/HBcAg in ELISA and immunoblotting assay. Liver tissue sections of both WHV-infected woodchucks and chronic HBV-infected patients could be stained specifically by mAbs. The epitopes were mapped at aa1-8 (6D1) and aa125-140 (1H4) of the core proteins of both WHV and HBV by using ELISA assay. WHcAg and HBcAg share similar amino acids sequences at aa1-8 and aa125-140 respectively.
CONCLUSIONThe core proteins of woodchuck hepatitis virus and human hepatitis B virus share common linear B cell epitopes which span aa1-8 and aa125-140 respectively.

6.Matrix Metalloproteinase-2, -9 Immunohistochemical staining in Cervical carcinoma: The Relationship with Clinicopathologic Prognostic Factors.

OBJECTIVE: Matrix metalloproteinase (MMP) has been reported to have a key role in the sequence of events that lead to local tumor invasion and metastasis. The present study was conducted to evaluate the role of MMP-2 and MMP-9 in cervical neoplasia. METHODS: Ten cases of cervical intraepithelial neoplasia, and 40 cases of cervical carcinoma were analysed by immunohistochemical staining. Paraffin-embedded archival tissue sections were examined for the expression of MMP-2 and MMP-9. RESULTS: of overexpression were analyzed for the correlation with clinical characteristics and prognostic factors. RESULTS: The overexpression of MMP-2 and MMP-9 was significantly higher in invasive cervical carcinoma than in CIN cases. There was no statistical significance in the correlation between MMP-2 or MMP-9 overexpression and the clinical prognostic factors. However, higher degree of MMP expression was observed in tumors of larger size, poorly differenciated and with lymph node metastasis compared to tumors of smaller size, well differentiated and without lymph node metastasis. CONCLUSIONS: From these findings, it can be postulated that MMPs may possibly play a role in the late stage of tumorigenesis, implying prognostic value. However, to draw a definite conclusion on their prognostic value, larger number of cases should be studied further.

7.Intra-Individual, Inter-Vendor Comparison of Diffusion-Weighted MR Imaging of Upper Abdominal Organs at 3.0 Tesla with an Emphasis on the Value of Normalization with the Spleen.

OBJECTIVE: To compare the apparent diffusion coefficient (ADC) values of upper abdominal organs with 2 different 3.0 tesla MR systems and to investigate the usefulness of normalization using the spleen. MATERIALS AND METHODS: Forty-one patients were enrolled in this prospective study, of which, 35 patients (M:F, 27:8; mean age ± standard deviation, 62.3 ± 12.3 years) were finally analyzed. In addition to the routine liver MR protocol, single-shot spin-echo echo-planar diffusion-weighted imaging using b values of 0, 50, 400, and 800 s/mm2 in 2 different MR systems was performed. ADC values of the liver, spleen, pancreas, kidney and liver lesion (if present) were measured and analyzed. ADC values of the spleen were used for normalization. The Pearson correlation, Spearman correlation, paired sample t test, Wilcoxon signed rank test and Bland-Altman method were used for statistical analysis. RESULTS: For all anatomical regions and liver lesions, both non-normalized and normalized ADC values from 2 different MR systems showed significant correlations (r = 0.5196-0.8488). Non-normalized ADC values of both MR systems differed significantly in all anatomical regions and liver lesions (p < 0.001). However, the normalized ADC of all anatomical regions and liver lesions did not differ significantly (p = 0.065-0.661), with significantly lower coefficient of variance than that of non-normalized ADC (p < 0.009). CONCLUSION: Normalization of the abdominal ADC values using the spleen as a reference organ reduces differences between different MR systems, and could facilitate consistent use of ADC as an imaging biomarker for multi-center or longitudinal studies.

8.Postsynaptic calcium pathway contributes to synaptic plasticity between retinal cones and luminosity-type horizontal cells.

It was previously found that the efficacy of synaptic transmission between retinal cone systems and luminosity-type horizontal cells (LHCs) was activity-dependent. Repetitive activation of red-cone pathway increased the LHCos hyperpolarizing response to red light, and the response enhancement was reversible. In this study, intracellular recording and pharmacological method were applied to investigate the mechanism(s) underlying red-flickering-induced response enhancement. Lowering intracellular Ca(2+) in the LHC by intracellular injection of Ca(2+) chelator EGTA prevented the development of red-flickering-induced response enhancement, which implicates the importance of postsynaptic calcium signal. The response enhancement could also be eliminated by a potent antagonist of Ca(2+)-permeable AMPA receptor (CP-AMPAR), which suggests the possibility that Ca(2+) influx via glutamate-gated calcium channels is related to the changes of [Ca(2+)](i). Furthermore, the administration of ryanodine or caffeine also attenuated the phenomenon, which gives evidence that the local calcium signal caused by intracellular calcium-induced calcium release (CICR) may be involved. Taken together, our data implicate that postsynaptic CICR and CP-AMPAR are related to the activity-dependent response enhancement.

9.Oral paroxetine for premature ejaculation: a randomized controlled study.

OBJECTIVETo validate the therapeutic efficacy of paroxetine in the treatment of premature ejaculation (PE).
METHODSEighty PE patients up to the inclusion criteria were equally randomized to an experimental and a control group. We observed all the patients for 4 weeks and recorded the baseline data on intravaginal ejaculatory latency time (IELT) and sexual satisfaction scores, followed by oral medication of paroxetine at 20 mg/d for the patients in the experimental group and placebo for the controls. Thirty days after the treatment, we again recorded IELT and sexual satisfaction scores of the patients.
RESULTSAfter the treatment, the experimental group showed significantly prolonged IELT ([5.75 +/- 1.24] min) and increased sexual satisfaction score (6.4 +/- 1.2) as compared with the baseline data ([0.89 +/- 0.21] min and [2.7 +/- 0.9]) (P < 0.01). The control group exhibited no significant differences before and after the medication either in the mean IELT or in sexual satisfaction scores ([1.06 +/- 0.28] min vs [0.97 +/- 0.18] min and 3.6 +/- 1.3 vs 3.1 +/- 1.1, P > 0.05).
CONCLUSIONOral medication of paroxetine at 20 mg/d for 30 days could improve IELT and sexual satisfaction in PE patients.

10.A study on effects of various chronic diseases on quality of life

Objective To learn the current situation of Quality of Life (QoL)among patients with various Chronic Diseases (CDs)and the effects of CDs on QoL.Methods Data was from the fifth health service survey in Ningbo City with self -reported CDs cases and QoL was measured by EQ -5D scale.Semi -parameter Additive Models and Semi -parameter Generalized Additive Models (GAMs)were used to analyze influencing factor of QoL.Results Different CDs had different effects on EQ -5D index,with the most severe impairments for stroke which was followed by tumor,COPD,diabetes and hypertension successively.Stroke,tumor and diabetes could improve the probability of self -reporting "problems"in all five dimensions (including mobility,self -care,usual activities,pain/discomfort and anxiety/depression dimension)and COPD could only increase the probability of self -reporting "problems"in dimension of usual activities.Hypertension could affect the other four dimensions except for the dimension of anxiety/depression.The relationship between Body Mass Index (BMI)and EQ -5D index was appeared inversed U -shaped with the maximum EQ -5D index located around the BMI of 20.90.Conclusion Different CDs had different effects on QoL and reaching optimal BMI was beneficial to improve the QoL.
