1.Characterization of CTL Clones Specific for Single Antigen, H60 Minor Histocompatibility Antigen.

BACKGROUND: Disparities of Minor H antigens can induce graft rejection after MHC-matched transplantation. H60 has been characterized as a dominant antigen expressed on hematopoietic cells and considered to be an ideal model antigen for study on graft-versus-leukemia effect. METHODS: Splenocytes from C57BL/6 mice immunized with H60 congenic splenocytes were used for establishment of H60-specific CTL clones. Then the clones were characterized for proliferation capacity and cytotoxicity after stimulation with H60. Clone #14, #15, and #23 were tested for the TCR binding avidity to H60-peptide/H-2Kb and analyzed for TCR sequences. RESULTS: H60-specific CTL clones showed different levels of proliferation capacity and cytotoxic activity to H60-stimulation. Clones #14, #15, and #23 showed high proliferation activity, high cytotoxicity, and low activities on both aspects, respectively, and have TCRs with different binding avidities to H60-peptide/H-2Kb with t 1/2 values of 4.87, 6.92, and 13.03 minutes, respectively. The TCR usages were Valpha12D-3-01+Jalpha11-01 and Vbeta12-1-01+Dbeta1-01+J2-7-01 for clone #14, Valpha13D-1-02+Jalpha34-02 and Vbeta13-1-02+Dbeta2-01+Jbeta2-7-01 for clone #15, and Valpha16D+Jalpha45-01 and Vbeta12-1-01+Dbeta1-01+Jbeta2-5-01 for clone #23. CONCLUSION: The results will be useful for modeling GVL and generation TCR transgenic mouse.

2.Subdominant H60 antigen-specific CD8 T-cell response precedes dominant H4 antigen-specific response during the initial phase of allogenic skin graft rejection.

In allogeneic transplantation, including the B6 anti-BALB.B settings, H60 and H4 are two representative dominant minor histocompatibility antigens that induce strong CD8 T-cell responses. With different distribution patterns, H60 expression is restricted to hematopoietic cells, whereas H4 is ubiquitously expressed. H60-specific CD8 T-cell response has been known to be dominant in most cases of B6 anti-BALB.B allo-responses, except in the case of skin transplantation. To understand the mechanism underlying the subdominance of H60 during allogeneic skin transplantation, we investigated the dynamics of the H60-specific CD8 T cells in B6 mice transplanted with allogeneic BALB.B tail skin. Unexpectedly, longitudinal bioluminescence imaging and flow cytometric analyses revealed that H60-specific CD8 T cells were not always subdominant to H4-specific cells but instead showed a brief dominance before the H4 response became predominant. H60-specific CD8 T cells could expand in the draining lymph node and migrate to the BALB.B allografts, indicating their active participation in the anti-BALB.B allo-response. Enhancing the frequencies of H60-reactive CD8 T cells prior to skin transplantation reversed the immune hierarchy between H60 and H4. Additionally, H60 became predominant when antigen presentation was limited to the direct pathway. However, when antigen presentation was restricted to the indirect pathway, the expansion of H60-specific CD8 T cells was limited, whereas H4-specific CD8 T cells expanded significantly, suggesting that the temporary immunodominance and eventual subdominance of H60 could be due to their reliance on the direct antigen presentation pathway. These results enhance our understanding of the immunodominance phenomenon following allogeneic tissue transplantation.

4.Analysis of the association between necrotizing enterocolitis and transfusion of red blood cell in very low birth weight preterm infants.

PURPOSE: To investigate the association between necrotizing enterocolitis (NEC) and red blood cell transfusions in very low birth weight (VLBW) preterm infants. METHODS: We studied were 180 VLBW preterm infants who were admitted to the neonatal intensive care unit of CHA Gangnam Hospital from January of 2006 to December of 2009. The subjects were divided into 2 groups: an NEC group (greater than stage II on the modified Bell's criteria) and a control group (less than stage II on the modified Bell's critieria). We defined red blood cell transfusion before NEC diagnosis as the frequency of transfusion until NEC diagnosis (mean day at NEC diagnosis, day 18) in the NEC group and the frequency of transfusion until 18 days after birth in the control group. RESULTS: Of the 180 subjects, 18 (10%) belonged to the NEC group, and 14 (78%) of these 18 patients had a history of transfusion before NEC diagnosis. The NEC group received 3.1+/-2.9 transfusions, and the control group received 1.0+/-1.1 transfusions before the NEC diagnosis (P=0.005). In a multivariate logistic regression corrected for gestational age, Apgar score at 1 minute, the presence of respiratory distress syndrome, patent ductus arteriosus, premature rupture of membrane, disseminated intravascular coagulopathy and death were confounding factors. The risk of NEC increased 1.63 times (95% confidence interval, 1.145 to 2.305; P=0.007) with transfusion before the NEC diagnosis. CONCLUSION: The risk for NEC increased significantly with increased transfusion frequency before the NEC diagnosis.

Purpose:
This study examined the effects of training using the slime on the hand function and problem behavior in adults with intellectual disabilities.
Methods:
Five adults with intellectual disabilities who were judged as Level 1 or Level 2 were selected. The training consisted of one session (50 min)/day, one day/week for 10 weeks (preparatory activities: 5 min, slime activities: 40 min, finishing activities: 5 min) The functions of the hand (Purdue Pegboard Test [PPT], grip strength test [GST]) and problem behavior (Korean Behavior Problems Inventory [K-BPI]) were assessed three times (first assessment; pre, second assessment; post-5 weeks, third assessment; post-10 weeks). Wilcoxon signed-rank tests were used to analyze changes in the subjects’ performance before and after the intervention.
Results:
In the PPT, the use of the non-dominant hand was increased significantly (p<0.05). No significant differences were observed in the remaining results, but the mean value increased. In addition, although the problem behavior was not statistically significant, an increase in the problem behavior score was noted.
Conclusion
The training using the slime for adults with intellectual disabilities is effective in hand function improvement and reducing problem behavior.

7.Reliability study of 6-axis model surgery simulator for orthognathic surgery.

The purpose of this study was to evaluate the reliability of 6-axis model surgery simulator (6AMSS) for orthognathic surgery. A rectangular parallelepiped plastic block was assembled to model-mounting plate of 6AMSS. Left-right (X), anterior-posterior (Y), up-down (Z) translation and pitching (empty set X), rolling (empty set Y) and yawing (empty set Z) rotation was planned and performed using 6AMSS. The actual translation and rotation were measured with dial gauge and precisional protractor, respectively. Comparison between the planned and actual movements of plastic block for each variable were made using paired t- test. Statistical analysis for X, Y, Z, empty set X, empty set Y and empty set Z movement have shown no significant differences between planned and actual movement (P > 0.05). This indicate that model surgery performed with the aid of the 6AMSS is accurate in 3D translation and rotation. The 6AMSS is practically useful for accurate fabrication of surgical splint for orthognathic surgery.

8.Association between Insufficient Physical Activity and Hearing Loss in Korean Adults

Background:
Insufficient physical activity is a major risk factor for cardiovascular disease, and some studies report relationship between physical activityand hearing. We aimed to analyze association between hearing loss and physical activity level in Korean adults.
Methods:
We used data from the 6th Korea National Health and Nutrition Examination Survey. Insufficient physical activity was defined as a combinedphysical activity of less than 150 minutes per week.. Hearing loss was identified when the audible threshold decreased more than 40 dB. Weperformed multiple logistic regression analysis of major covariates and stratified the participants by age (≥60 versus <60).
Results:
We analyzed 3,237 participants for whom no values were missing. In the final multivariate logistic analysis, the odds ratio of hearing loss was1.227 (95% confidence interval [CI], 1.008–1.494) in the all frequency group and 1.361 (95% CI, 1.073–1.727) in the low frequency group. The resultfor the high frequency group was not statistically significant. In the group aged ≥60 years, the odds ratio of hearing loss in the all, low, and highfrequency groups were 1.277 (95% CI, 1.011–1.613), 1.405 (95% CI, 1.074–1.839), and 1.298 (95% CI, 1.013–1.662), respectively.
Conclusion
In this study, insufficient physical activity was associated with hearing loss in Korean adults. This result was more significant in the groupaged ≥60 years. Further studies should aim to validate these results and determine the causal relationship between physical inactivity and hearingloss.
9.A Case of Synchronous Occurrence of Gastric Schwannoma and Gastric Adenoma.

Gastric schwannomas are rare benign tumors among the mesenchymal tumor of the gastrointestinal tract, arising from the nerve tissue which has a schwann cell sheath. These neoplasms are grossly similar to gastrointestinal stromal tumor (GIST), however, the clinical course of GIST is more aggressive compared to that of schwannomas. Synchronous occurrence of gastric epithelial and subepithelial tumors is rare. Besides, most subepithelial lesions are GIST. We present the case of a 57-year-old male with synchronous gastric schwannoma and gastric adenoma in the stomach. We found a gastric subepithelial lesion incidentally, the size of which was increasing and tubular adenoma was discovered in the covering mucosa during endoscopy follow-up. The synchronous lesion was removed by wedge resection, which was diagnosed as schwannoma with adenoma.

10.A case of catamenial hemoptysis.

Endometriosis is relatively common gynecologic diseases, but pulmonary endometriosis is exceedingly rare among various types of external endometriosis. Patients with pleural pulmonary endometriosis present with chest pain, dyspnea, pneumothorax or pleural effusion in relation to menstruation. Patients with parenchymal pulmonary endometriosis present with hemoptysis at the time of menstruation, with or without lung lesions on their chest X-ray. The diagnosis of pulmonary endometriosis is usually made on the basis of the clinical history and the exclusion of other causes of recurrent hemoptysis including tuberculosis, bronchial carcinoid, pulmonary infarction, chronic bronchitis, congenital abnormalities and carcinoma. It can be treated by progesterone, GnRH agonist, Danazol, surgical treatment etc. Hereby we experienced 23 year-old multiparous woman with catamenial hemoptysis. The site of disease/was localized with bronchoscopy and chest CT scanning, and we treated her with Danazol. The literature associated with pulmonary endometriosis is briefly reviewed.
