3.Depression and Anxiety in Korean Patients with Age-Related Macular Degeneration.

PURPOSE: To examine the degree of depression and anxiety in Korean patients with age-related macular degeneration (AMD) and to evaluate the influence of mood disturbance on patient's treatment satisfaction and compliance. METHODS: Forty-four patients with AMD and thirty-three controls comprised the study populations. Patients and controls rated themselves on the Korean Beck Depression Inventory (BDI) and Beck Anxiety Inventory (BAI). The effect of mood disturbance on the patient's satisfaction and compliance was evaluated. RESULTS: The depression and anxiety scores were significantly higher in patients with AMD. The mood disturbance was negatively correlated with the patient's satisfaction and compliance. 14 patients (31.8%) were lost to follow-up 1 year later. The depression and anxiety scores of these patients were higher than those of remaining thirty patients. CONCLUSIONS: Depression and anxiety of patients with AMD affect on patient's satisfaction and compliance. So recognition and management of mood disturbance may help improving visual function and maintaining quality of life in patients.

5.High-dose Sulbactam Treatment for Ventilator-Associated Pneumonia Caused by Carbapenem-Resistant Acinetobacter Baumannii.

BACKGROUND: Several antibiotics can be used to treat ventilator-associated pneumonia caused by carbapenem-resistant A. baumannii (CRAB-VAP) including high-dose sulbactam. However, the effectiveness of high-dose sulbactam therapy is not well known. We report our experience with high-dose sulbactam for treatment of CRAB-VAP. METHODS: Medical records of patients with CRAB-VAP who were given high-dose sulbactam between May 2013 and June 2015 were reviewed. RESULTS: Fifty-eight patients with CRAB-VAP were treated with high-dose sulbactam. The mean age was 72.0 ± 15.2 years, and the acute physiology and chronic health evaluation II (APACHE II) score was 15.1 ± 5.10 at the time of CRAB-VAP diagnosis. Early clinical improvement was observed in 65.5% of patients, and 30-day mortality was 29.3%. Early clinical failure (odds ratio [OR]: 8.720, confidence interval [CI]: 1.346-56.484; p = 0.023) and APACHE II score ≥ 14 at CRAB-VAP diagnosis (OR: 10.934, CI: 1.047-114.148; p = 0.046) were associated with 30-day mortality. CONCLUSIONS: High-dose sulbactam therapy may be effective for the treatment of CRAB-VAP. However, early clinical failure was observed in 35% of patients and was associated with poor outcome.

6.Effects of Improved Methods of Endotracheal suction and Hand Washing on Incidence of Nosocomial Pneumonia in the Neurosurgical Intensive Care Unit.

BACKGROUND: This study aimed at identifying the effects of improved methods of endotracheal suction and hand washing on incidence of nosocomial pneumonia (NP) in the neurosurgical ICU. METHOD: Repeated training of nurses about hand washing and adherence of the single use of suction catheter and sterile normal saline of endotracheal suction was provided. Then hand washing compliance of nurses before and after training was investigated. and the incidence of NP in all patients hospitalized for more than 72 hours in the neurosurgical ICU was investigated from Sep 1, 2000 to Jan 31, 2001 (2nd surveillance). The NP rate of that period was compared with the incidence of the 1st surveillance period, from Sep 1, 1999 to Jan 31, 2000. The data were analyzed with chi-square-test. Fisher's exact test and logistic regression analysis by SAS 8.0. RESULT: The proportion of hand washing performance increased from 15.6% to 46.8%. The rates of NP per patient during 1st surveillance and 2nd surveillance were 10.3%, 3.9%(p=0.01) patient-days rates of NP were 10.0 cases/1,000 patient days, 4.1 cases/1,000 patient days(p=0.04), and ventilator-days rates of NP were 67.6 cases/1.000 ventilator days, 16.2 cases/1,000 ventilator days(p=0.00) respectively. Out of 29 isolates, 18 were methicillin-resistant Staphylococcus aureus, which was the most frequently isolated microorganism. The risk factors associated with incidence of NP were age, length of stay, Glasgow coma scale, diabetes mellitus, duration of intubation, insertion of endotracheal tube, duration of tracheostomy, presence of tracheostomy, duration of ventilator, use of ventilator, and insertion of nasogastric tube. CONCLUSION: These results showed that single use of suction catheters and sterile normal saline and increased compliance of hand washing were important factors to decrease the incidence of NP.

7.Comparison of midsagittal reference plane in PA cephalogram and 3D CT.

OBJECTIVE: The aim of this study is to find the most helpful midsagittal reference plane for diagnosis in PA cephalometry compared with 3D CT. METHODS: The subjects consisted of 25 adults who showed no facial asymmetry by gross inspection. 3D CT and posteroanterior cephalogram of the subjects were taken. To find the most helpful midsagittal reference plane in PA cephalometry, we considered five kinds of midsagittal planes from which the distances to five landmarks were measured and compared the result with that of 3D CT. The midsagittal plane for 3D CT was determined by the landmarks Nasion, Sella and Basion. RESULTS: PA measurements using the midsagittal reference plane on a perpendicular plane lying through the midpoint of the right and left latero-orbitales was closest to those of 3D CT. CONCLUSIONS: It was considered that latero-orbitale perpendicular could be used as the helpful midsagittal reference plane to assess facial asymmetry in PA cephalometry.

9.Level of Problem Drinking, Health Status and Perception of Need for Moderate Drinking by Male Blue-collar Workers in Steel Factory.

OBJECTIVE: This study was conducted to investigate the level of problem drinking by male blue-collar workers in light of their health status and their perception of drinking amount. METHOD: A steel factory located in the central region of Korea was chosen and 182 questionnaire responses from male workers were analyzed using the SPSS/WIN 11.0 in reference to each worker's physical examination record. RESULTS: According to the criteria set by WHO, 19 workers(10.4%) were found to have 'problem drinking', 81 workers(44.5%) 'alcohol use disorder'and 14 workers(7.7%) 'alcohol dependence'. Only the remaining 68 workers(37.4%) were found to have 'normal drinking'. In relation to the workers'health status by CMI, 'alcohol dependence'scored 62.92, and 'alcohol use disorder'scored 56.53, both of which were far higher than the 'problem drinking'score of 44.52, In particular, it was found that each of the diabetes group(t=2.32, p<.05) and the liverdisease group(t=2.01, p<.05) drank much more than the non-diabetes group or the non-liver-disease group. In terms of the workers'perception of drinking amount, 'alcohol use disorder'and 'alcohol dependence'scored 147.62 and 144.21 respectively, both of which were far higher than the 'problem drinking'score of 138.57. CONCLUSION: The above results suggest the need to educate workers on the necessity and practices of moderate drinking before they enter a state of alcohol use disorder or alcohol dependence. Accordingly, as a part of early management and treatment of the workers'drinking problem, it may well be necessary to activate OAPs (Occupational Alcohol Programs).

10.Epidural Anesthesia for Cesarean Section in a Parturient with Eisenmenger's Syndrome.

Eisenmenger's syndrome is defined as pulmonary hypertension with right-to-left or bidirectional shunting of blood through an intracardiac or aorto-pulmonary commumication. It can occur with complex congenital cardiac malformations, such as septal defect and patent ductus arteriosus. Parturients with Eisenmenger's syndrome are at high risk for peripartum morbidity and mortality. We experienced a case of parturient for cesarean section with Eisenmenger's syndrome and performed epidural anesthesia with fractionated doses of 2% lidocaine and fentanyl. The sensory block reached to T10 level and blood pressure was maintained with intravenous phenylephrine. After baby out, sudden hypotension with severe bradycardia developed and arterial oxygen saturation dropped. Immediate intubation and resuscitation was done, but cardiac rhythm disturbance, hypoxemia, and acidosis did not corrected. Cardiac standstill developed and stopped resuscitation. The neonate's Apgar score was 7 and he was transferred to pediatric ICU.
