2.Effect of paclitaxel on carcino-embryonic antigen carcino-embryonic antigen, carbohydrate antigen 19-9 and carbohydrate antigen 72-4 levels in patients with advanced gastric cancer

Objective To analysis the effect of paclitaxel on CEA, CA19-9 and CA72-4 levels in patients with advanced gastric cancer.Methods46 patients who were diagnosed with advanced gastric cancer in Pinghu hospital of traditional Chinese medicine were collected.All patients were randomly divided into experimental group and control group,23 cases in each group.The control group was treated with cis-platinum complexes and capecitabine, and the experimental group was treated with paclitaxel and capecitabine, after the treatment, the serum levels of CEA, CA19-9, CA72-4, vascular endothelial growth factor, endostatin and clinical curative effect were detected in all patients.ResultsAfter treatment, compared with control group,①the serum level of CEA was lower in the experimental group (P<0.05);②the serum level of CA19-9 and CA72-4 were lower in the experimental group (P<0.05);③the serum level of VEGF was lower in the experimental group (P<0.05);④the serum level of Endostatin was higher in the experimental group (P<0.05);⑤The clinical efficiency of the experimental group was higher(P<0.05).ConclusionsThe paclitaxel can significantly reduce the serumCEA, CA19-9, CA72-4 and VEGF levels, elevated serum Endostatin level in patients with advanced gastric cancer, improve the treatment effect, and have a guiding significance for clinica.
4.Recent progress in molecular genetics and therapy for X-linked juvenile retinoschisis

Juvenile retinoschisis (RS or XLRS,MIM#312700)is a rare X-linked inherited disorder,mainly affects bilateral retina,and is characterized by cartwheel-like changes of the macular region of the retina and schisis or splitting within the inner retinal layers,leading to visual deterioration.The electroretinogram is beneficial in the diagnosis of juvenile retinoschisis.The a-wave can be of normal or nearly normal amplitude in this disorder,whereas the amplitude of the b-wave is appreciably reduced,giving a decrease in the proportion of b/a.The responsible gene,XLRSl,maps to Xp22 and was identified by positional cloning.This paper makes a brief review about the latest XLRS research of pathogenesis,animal experiments,clinical therapy,and 25 references are cited.
6.Progress in the personalized medicine using pharmacometabonomics.

Pharmacometabonomics, as an emerging branch of system biology, has been increasingly used in personalized medicine and showed broad prospects. By means of metabonomics, the complicated and detailed metabolic profile of the patient is described, thus providing more detailed description of the disease phenotype. With this understanding, response of different individuals to the drugs are predicted or evaluated through inherent genetic information of the individual combined with the environmental factors. As a result, appropriate drugs and dosage are chosen, which greatly promotes the realization of the individualized therapy goals. This article describes the emerging field of pharmacometabonomics, and the research results of personalized medicine based on the pharmacometabonomics in recent years are reviewed in detail.

7.Application of the magnetic resonance angiography in elderly patients with arterial diseases of lower extremity

Objective To evaluate the magnetic resonance angiography(MRA) in senile lower extremity arterial disease and to compare the diagnostic value of two dimensional time of flight(2D TOF) with three dimensional contrast enhancement(3D CE) technique. Methods Seventy patients with a mean age of 75 years underwent MRA for suspected acute or chronic lower extremity arterial disease. MRA was performed with a 1 5 T MR scanner and Gd DTPA was used by manual injection in 3D CE MRA. Among 52 patients examined with 2D TOF MRA, 18 of them underwent an additional 3D CE MRA, 32 patients took Doppler ultrasound examination, and 4 patients received emergency operation. Results In all 70 patients, 52 showed atherosclerotic stenosis disease, 17 atherosclerotic occlusive disease (4 of them were verified by emergency operation) and one patient atherosclerosis with aneurysmal dilatation. A total of 997 vessel segments were shown on MRA images and 600 of them were abnormal including 486(81%) mild to moderate stenoses, 114(19%) severe stenoses and 451 curved arterial segments. In 32 patients who underwent Doppler ultrasound, the diagnosis and its grading based on MRA was well correlated with that from Doppler ultrasound (coincidence rate was 91% and 96% respectively). A total of 764 suprapopliteal and 125 infrapopliteal arteries were depicted by 2D TOF MRA (visibility was 99% and 40% respectively). In 18 patients who received additional 3D CE MRA, totally 306 arteries were depicted on MRA (visibility 100%). Doppler ultrasound showed 40 segments with arterial calcification and 32 atherosclerotic plaques. Conclusions MRA is quite useful in detecting lower extremity arterial disease because of its noninvasion and convenience. The results derived from MRA are well correlated with that of Doppler ultrasound and clinical outcome. 3D MRA is superior to 2D TOF MRA in image quality, accuracy and time efficiency and is best for emergency case. Pitfalls of MRA such as overestimation in the severity of the disease and the long scanning time of 2D TOF must be taken into consideration .
8.The early diagnosis and treatment of cervical vertebrae injury

Objective To study the early diagnosis and treatment of cervical vertebrae injury.Methods 86 cases with cervial vertebrae injury were retrospectively analyzed.8 cases were treated by head halter treaction,78 cases were treated by skull traction,15 cases treated in early-day operation,27 cases seclected-day operation.Results According to frankel grading.A degree was 36 cases,no betterment,one case died;B degree 18 cases,2 cases improved to C degree,3 cases improved to D degree,13 cases had no improverment;9 cases,all had one or two degree improvement.20 cases with the nerve root injury all had determinate improverment or were cured.Conclusion Quick and correct early diagnosis and treatment after cervical vertebrae injury are the key steps in decreasing the death rate and disability rate.
9.Neiguan Massaged to Prevent Ache and Nausea and Vomit after Laparoscopic Cholecystectomy—A Randomized Controlled Clinical Trial

[Objective] To confirm the effects of Neiguan massaged to prevent ache nausea and vomit after laparoscopic cholecystectomy(LC).[Methods] 400 patients were randomly divide into 2 groups;the group 1 were treated by Neiguan massage during the LC,the group 2 nothing during the operation.[Results]Group 1 suffered the same ache in the 1st hour as group 2,but less in the last 24h,and less nausea and vomit too.[Conclusions ]Neiguan massage could obviously prevent ache nausea and vomit after LC.
