1.ENDOSCOPIC FOREHEAD LIFT COMBINED WITH ALLODERM(R) IMPLANTATION TO CORRECT FOREHEAD DEEP WRINKLES.

Forehead deep wrinkles make man's face ugly. Especially, in case that a young people has short forehead, one or two forehead deep wrinkles is likely go give a aggressive impression to people. Traditional forehead lift basically not only perfrom the selective excision of frontalis muscle which cause forehead wrinkles, but extend the width of forehead by practical lift through scalp excision. Recently, various filling materials such as collagen have been developing, and laser resurfacing and endoscopic plastic surgery has gradually been generalized, so the frequency of the traditional forehead lift which needs the extensive operation through coronal incision is gradually diminished. But collagen injection is repeatedly taken every regular times and it has a weak point that the width of forehead cannot be regulated. Laser resurfacing can't solve the problem of deep wrinkles. The purpose of endoscopic brow lift which has been generalized currently is mainly brow lift, and so this method is not good for deep wrinkles because it can't modualte frontalis muscle which cause forehead transverse wrinkles. Recently it has been reported that Alloderm composed of dermal collagen from cadavar skin is universally used in covering as well as filling up soft tissue defect and its usability is also successful. Authors recently correct three young persons who have forehead deep wrinkles by combination of endoscopic forehead lift and subcutaneous Alloderm implantation under the wrinkles for 2 years. At first, we extended the width of forehead and smoothed the depth of wrinkles by endoscopic brow lift. After having endoscopic operation, we inserted Alloderm in subcutaneous pocket and performed suspension suture outside. The average increase of forehead width is 5 mm and all patients had corrected transverse wrinkles remarkably. During the 9 months, wrinkles didn't recur and implanted Alloderm also didn't absorbed. As a better method, authors reported that combining endoscopic brow lift with Alloderm implantation for the correction of short forehead and deep wrinkles could be used in place of traditional forehead lift.

3.The effect of prostaglandin e1on the muscle flap in the rectus abdominis muscle of the rat.

Flap survival is critical to the success in reconstructive surgery, there have been many investigations to increase the blood supply to the flaps such as surgical delay and pharmacologic delay. Prostaglandin(PG) is released from various tissues including blood vessel in response to physical stimulus. Among the Prostaglandins, PGE1 has been proven to be a vasodilatation property and many authors have demonstrated its effect to increase blood supply after random cutaneous flap surgery. Clinically, however, muscle flap or musculocutaneous flap is more significantly used in reconstructive surgery and hemodynamic effects of PGE1 of this type of flap are still not documented. The authors designed the random muscle flap to study the hemodynamic effects of PGE1 of the muscle flap. Superior based rectus muscle flap was elevated from rats and the superior epigastric artery, its major vascular pedicle, was ligated to create the random-type muscle flap. Twenty two rats were divided into two experimental groups and each group had 11 rats; Group I: No drugs Group II: PGE1 injection group for 7 postoperative days intraperitoneally The average muscle flap survival rate of group I was 46+/-3.0 precent and it had a higher survival rate than the control group(23+/-4.3%). The muscle flap survival rates showed significant differences between the two groups (p< 0.005) This study shows that the administration of the PGE1, in clinical usage of the rare random muscle flap with a pedicle injury or musculocutaneous flap with the risk of distal cutaneous flap necrosis, such as TRAM flap, which might be much safer and popular.

4.A Case of Cobb Syndrome.

Cobb syndrome is a rare neurocutaneous angiomatosis characterized by a vascular skin nevus associated with a spinal cord angioma of the same metamere. A 14-year-old girl had an asymptomatic large cutaneous hemangioma distributed from the TI dermatome downward to the L3 dermatome since birth and complained of a gait disturbance and urination difficulty for 1 year. A biopsy specimen in the skin lesion revealed the findings of capillary hemangioma. From C7 downward to L4 posterior epidural hemangioma composed of arteriovenous and cavernous components was diagnosed by radiological examination and surgical exploration. Because of very extensive cord hemangioma, only partial removal of the tumor at T11, T12 and L1 level was performed and postoperatively she was transferred to a special facility for rehabilitative therapy.

8.pH-Dependent Expression, Stability, and Activity of Malassezia restricta MrLip5 Lipase

Background:
The lipophilic yeasts Malassezia spp. are normally resident on the surface of the human body, and often associated with various skin diseases. Of the 18 known Malassezia spp., Malassezia restricta is the most predominantly identified Malassezia sp. found on the human skin. Malassezia possesses a large number of genes encoding lipases to degrade human sebum triglycerides into fatty acids, which are required not only for their growth, but also trigger skin diseases. Previously, we have shown that MrLIP5 (MRET_0930), one of the 12 lipase genes in the genome of M. restricta, and is the most frequently expressed lipase gene in the scalp of patients with dandruff.
Objective:
In this study, we aimed to analyze the activity, stability, and expression of MrLip5, with particular focus on pH.
Methods:
We heterologously expressed MrLip5 in Escherichia coli, and purified and analyzed its activity and expression under different pH conditions.
Results:
We found that MrLip5 was most active and stable and highly expressed under alkaline conditions, which is similar to that of the diseased skin surface.
Conclusion
Our results suggest that the activity and expression of MrLip5 are pH-dependent, and that this lipase may play an essential role at the M. restricta-host interface during disease progression.
9.The Effect of Roller Acupuncture Stimulation on Baesu Spots of Joktaeyang Bangkwang Kyeong in the Reaction to Physiologic Pain in Neonates.

PURPOSE: This study was done to verify the effect of roller acupuncture stimulation on baesu spots of joktaeyang bangkwang kyeong in the reaction to physiologic pain(heart rate, percutaneous oxygen saturation, respiration rate) in neonates. METHOD: This study was a nonequivalent control group non-synchronized design. The participants were 40 normal neonates who were born at a hospital in Busan. These neonates were undergoing heel puncture for blood type tests. The neonates were divided into 2 groups: 20 in the experimental group who were stimulated with roller acupuncture before the heel puncture and 20 neonates in the control group who were not stimulated. The heart rate and percutaneous oxygen saturation were measured using a cardiopulmonary monitor and the respiration rate was measured directly. The data were analyzed with SPSS 10.0 program using chi2-test, t-test and Repeated Measure ANOVA. RESULTS: There was a significant difference (F=3.287, p=.043) for heart rate on the interaction between time and group. There was a significant difference (F=5.122, p=.008) for percutaneous oxygen saturation on the interaction between time and group. CONCLUSION: On the basis of results, it was verified that the roller acupuncture stimulation on baesu spots of joktaeyang bangkwang kyeong had effect of relieving pain in the neonates.

10.A Case of a Congenital Lacrimal Outflow Dysgenesis with Supernumerary Lacrimal Puncta.

PURPOSE: To report a case of congenital lacrimal outflow dysgenesis with supernumerary lacrimal puncta. CASE SUMMARY: A 45-years-old woman presented with chronic bilateral epiphora with no specific medical history. Slit-lamp examination revealed bilateral upper and lower double puncta. The accessory puncta were situated along the lid margin, medial to the normal one and had a typical slit configuration. Additionally, there was a lower canalicular system block that appeared unresponsive to simple probing. Bilateral endoscopic conjunctivodacryocystorhinostomy with Jones tube was performed. Chronic bilateral epiphora was relieved after bilateral endoscopic conjunctivodacryocystorhinostomy with Jones tube. CONCLUSIONS: A case of congenital lacrimal outflow dysgenesis with supernumerary lacrimal puncta was observed, which has not been previously reported in Korea. Surgical repair of the congenital lacrimal outflow dysgenesis with supernumerary lacrimal puncta should be considered to achieve a functional recovery of the lacrimal drainage system.
