3.Development of a Health Information Web Site Evaluation Categories with Items for Diabetes Mellitus.

OBJECTIVE: The purpose of this study was to develop a health information web site evaluation categories with items for diabetes mellitus in order to offer verified health information, and to propose directions for development of web sites. METHODS: A preliminary evaluation categories with items was developed using domestic and foreign web site evaluation standards for health information and through a review of the literature. The evaluation categories with items was composed of 'evaluation for web site attributes' and 'evaluation for content information on diabetes mellitus'. Through expert validity tests and a feasibility test of the preliminary tool, the health information Web site evaluation tool for diabetes mellitus was finalized. RESULTS: This evaluation categories with items included 'evaluation for web site attributes' and 'evaluation for content information on diabetes mellitus'. 'Evaluation for web site attributes' included 8 categories with 28 items. The 'evaluation for content information on diabetes mellitus' included 7 categories with 40 items. 'Evaluation component for content' and 'evaluation of category content' included 4 items on 'accuracy', 6 on 'harmfulness' and 3 on 'comprehensibility'. CONCLUSION: From this study guidelines are propose for providing health information for diabetes mellitus, so that health information providers will be able to offer consumers verified good quality health information themselves.

4.A Case of Acute Pancreatitis in Systemic Lupus Erythematosus.

Systemic lupus erythematosus (SLE) is a multisystemic disease that can involve the gastrointestinal tract, liver, and biliary system. Symptomatic pancreatic involvement, however, has rarely been reported. It may be part of the primary disease process, such as vasculitic or autoimmune etiology, or associated with drug therapy, in particular corticosteroid. We report here a lupus patient who developed severe pancreatitis within 30 hours of initiation of corticosteroid therapy; we also discuss the relation between pancreatitis and systemic lupus erythematosus.

5.Histochemical Electron Microscopic Findings of the Distribution of the Glycosaminoglycans of Cultured Sensory Retina.

PURPOSE: This study was performed to investigate the distribution of glycosaminiglycans by the activated retinal neuronal cell on the cultured retinal tissue. METHODS: The retinal tissue was obtained from the donor eyeball without the pathological findings of the retina. The dissected retinal tissue was cultured for 2 weeks in the culture media and reacted with ruthenium red dye to observe the glycosaminoglycans reaction with transmission electron microscopy. RESULTS: The histochemical reaction to ruthenium red on the cultured retina tissue was prominent on the internal limiting membrane, intercellular space between the axons and dendrites at the outer and inner plexiform layers and interphotoreceptor matrix. CONCLUSIONS: These findings suggest that GAGs may be released into the extracellular spaces between the axons and dendrites, which modulate the retinal circuits.

6.Development of the Information System for Nursing Process: An Implementation of Nursing Diagnosis System using Neural Network.

This project developed a neural network based on nursing diagnoses among 98 nursing diagnoses Korea Nurses Association recommended when related factors and clinical signs are entered. This nursing diagnosis system consists of five sub-systems: data-input system from which a nurse can obtain knowledge from experts; data-base system in which nurses can manage statistical data; inference support system which provides learning effect; inference system which increases inference ability of nurses using existing data and nursing diagnoses data; and result-retrieve system in which nurses can look at the outcomes of nursing process. This nursing diagnosis system provides a interface which enables maintenance and revision easier as well as to induce a diagnosis through the communication between the nurse and the patient. In addition, this system makes nurses determine the nursing diagnoses in a more accurate manner by linking the learning effect of the system whit intra-neural network algorithm. This nursing diagnosis system will assist nurses to provide foster and more accurate way to implement nursing diagnosis.

7.The Influence of Restricted Arm Swing on Symmetry, Movement of Trunk and Pelvis Rotation according to Using a Mobile Phone.

PURPOSE: This study was conducted to investigate the effects of variations in arm swing during gait on movement of the trunk and pelvis. During the gait task, the angle of the trunk and pelvic rotation were analyzed according to arm swing conditions. METHODS: Seventeen healthy males participated in this study. All subjects were analyzed for gait on a treadmill three times each under three different types of arm swing conditions – natural arm swing, restricted arm swing using a phone, restricted swing in both arms. 3-D motion analysis systems were used to collect and analyze the kinematic data of trunk and pelvic movements, and repeated one-way ANOVA was used to compare the trunk and pelvic kinematic data and symmetry index. The level of significance was α=0.05. RESULTS: The results showed kinematic differences in trunk and pelvic during gait based on the arm swing conditions. Specifically, there were significant differences in trunk rotation, left and right trunk rotation and symmetry index of trunk rotation during gait among the three arm swing conditions. ROM was used to calculate a symmetry index (SI) based on the average left and right trunk rotation in which a value closer to zero indicated better balance. The SI obtained for arm swing restricted with the phone was closer to –1 than the other conditions. CONCLUSION: Restricted arm swing due to use of a phone had the possibility to induce instability of postural control while walking, which could be seen to suggest a risk of falling during gait.

8.Effect of Trunk Side Shift Exercise on the Cobb's Angle of Patients with Idiopathic Scoliosis.

PURPOSE: This study examined the effects of trunk side shift exercise on the Cobb's angle. METHODS: Fifty-eight subjects (control group=30, scoliosis group=28) were enrolled in this study. The idiopathic scoliosis groups were divided randomly into two groups, a side shift exercise (SSE) group, and a trunk stabilization exercise (TSE) group. The SSE group performed side shift exercise on the developed chair training for eight weeks. The TSE group performed trunk stabilization exercise. A one-way ANOVA test was carried out to compare the results within the idiopathic scoliosis patient group before and after the exercises based on the different exercise methods. RESULTS: The Cobb's angles were compared among control group and SSE group and TSE group. As a result, there was a significant difference from SSE group and TSE group(p=0.000), but there were no significant differences between the SSE and TSE groups (p=0.085). CONCLUSION: Side shift exercises are effective in improving the Cobb's angle. Therefore, the use of the side shift exercise chair designed in this study can replace general exercise either at work or during studies. Therefore, this method is easily accessible for busy modern students who are exposed to scoliosis or spinal disorders due to a pattern of inactivity.

9.Elimination of Lancet-Related Needlestick Injuries Using a Safety-Engineered Lancet: Experience in a Hospital.

BACKGROUND: Lancet-related needlestick injuries (NSIs) occur steadily in clinical practices. Safety-engineered devices (SEDs) can systematically reduce NSIs. However, the use of SEDs is not active and no study to guide the implementation of SEDs was known in South Korea. The lancet-related NSIs may be eliminated to zero incidence using a SED lancet with effective sharp injury protection and reuse prevention features. MATERIALS AND METHODS: We implemented a SED lancet by replacing a conventional prick lancet in a tertiary hospital in a sequential approach. A spot test of the new SED was conducted for 1 month to check the acceptability in practice and a questionnaire survey was obtained from the healthcare workers (HCWs). A pilot implementation of the SED lancet in 2 wards was made for 1 year. Based on these preliminary interventions, a hospital-wide full implementation of the SED lancet was launched. The incidence of NSIs and cost expenditure before and after the intervention were compared. RESULTS: There were 29 cases of conventional prick lancet-related NSIs for 3 years before the full implementation of SED lancet. The proportion of prick lancet-related NSIs among yearly all kinds of NSIs during two years before the pilot study was average 11.7% (22/188). Pre-interventional baseline incidence of all kinds of NSIs was 7.01 per 100 HCW-years. After the full implementation of SED lancet, the lancet-related NSIs became zero in the 2nd year (P = 0.001). The average direct cost of 18,393 US dollars (USD) per year from device and post-exposure medical care before the intervention rose to 20,701 USD in the 2nd year of the intervention. The incremental cost-effectiveness ratio was 210 USD per injury avoided. CONCLUSION: The implementation of a SED lancet could eliminate the lancet-related NSIs to zero incidence. The cost increase incurred by the use of SED lancet was tolerable.
