1.Total thyroid resection on the quality of life in patients with nodular goiter

Objective To investigate the analysis of low calcium intravenous calcium supplementation agent and active vitamin D3 on total thyroidectomy on quality of life in patients with nodular goiter. Methods 40 cases of nodular goiter with total thyroidectomy were performed in our hospital from March 2014 to September 2016. It was randomly divided into control group and experimental group, 20 cases in each group. The patients in the control group were nodular thyroidectomy and the experimental group underwent total thyroidectomy. The quality of life of two groups of patients was evaluated by using the European five-dimensional health-related living quality scale (eq-5d). Results Two groups of patients, relevant questionnaire survey of the patient's ability to act, self-care ability, daily activity ability, pain or discomfort, anxiety, or depression, and the comparison of VAS score, according to the results of difference was not statistically significant. In comparison with ordinary people, the questionnaire survey results showed that the proportion of patients with moderate or above differences was higher and the difference was significant (P< 0.01). There was no statistical significance compared to the average person. Conclusion All patients with nodular goiter are treated with total thyroidism, and the effect is good, and the quality of life of patients is relatively small.
2.Application of hysteroscopy in assisted reproduction

Objective To explore the application of hysteroscopy in assisted reproduction. Methods A retrospective analysis was made on results of routine hysteroscopy in 150 cases of infertile women scheduled for assisted reproduction.Results Among the 150 cases,abnormal hysteroscopic findings were identified in 47 cases(31%),including endometrial polyps in 18 cases(12%),endometrial hyperplasia in 17 cases(11%),uterine deformity in 7 cases(5%),uterine adhesion and stenosis in 3 cases(2%),endometrial tuberculosis in 1 case(0.7%),and submucous myoma or adenomyoma in 1 case(0.7%).To think of hysteroscopic findings as the standard,the sensitivity,specificity,positive predicted rate,negative predicted rate,and accuracy were 66%(31/47),98%(101/103),94%(31/33),86%(101/117),and 88%(132/150) for transvaginal ultrasonography,respectively,and were 26%(12/47),100%(95/95),100%(12/12),73%(95/130),and 75%(107/142) for uterophotography,respectively.Conclusions Hysteroscopy is a simple and reliable way to explore the uterine cavity.It may be selected as a routine examination before assisted reproduction.
4.Effectiveness of the Spot Vision Screener on vision screening of children without cycloplegia

AlM: To evaluate the effectiveness of Spot Vision Screener on vision screening of children without cycloplegia.METHODS:A total of 87 children (174 eyes) aged from 2~9 years old were examined with Spot Vision Screener and optometrist before cycloplegia.RESULTS: Statistical analysis demonstrated that the cylinder diopter and axis, the equivalent spherical diopter in both eyes, and the spherical diaopter in left eye had no significant change ( P>0. 05 ). However, the spherical diaopter in right eye had statistical significance. CONCLUSlON: Spot Vision Screener is a suitable instrument in vision screening of children without cycloplegia.
5.Effects of different doses of dexmedetomidine administered intranasally on median effective concentration of sevoflurane inhibiting responses to laryngeal mask airway insertion in pediatric patients

Objective To evaluate the effects of different doses of dexmedetomidine administered intranasally on the median effective concentration (EC50) of sevoflurane inhibiting responses to laryngeal mask airway (LMA) insertion in the pediatric patients.Methods American Society of Anesthesiologists physical status Ⅰ or Ⅱ pediatric patients of both sexes, aged 1-3 yr, with body mass index of 20-25 kg/m2, scheduled for elective surgery under general anesthesia, were randomly divided into 3 groups: control group (group C) , dexmedetomidine 1 μg/kg group (group D1) , and dexmedetomidine 2 μg/kg group (group D2).In D1 and D2 groups, dexmedetomidine 1 and 2 μg/kg (in 1 ml of normal saline) were administered intranasally at 1 h before induction of anesthesia, respectively.Group C received the equal volume of normal saline administered intranasally.The children were separated from their parents at 45-60 min after intranasal administration and admitted to the operating room.The children inhaled 8% sevoflurane until they lost consciousness.After loss of consciousness, the end-tidal concentration of sevoflurane was determined using the modified Dixon's up-and-down method.The initial end-tidal concentration of sevoflurane was set at 2%.The concentration was increased/decreased by 0.2% in the next patient according to the response to LMA insertion.The patient's sedation status and with LMA acceptance were evaluated, and the patient's satisfaction with sedation and with LMA acceptance was recorded.Probit analysis was used to calculate the EC50 and 95% confidence interval of sevoflurane inhibiting responses to LMA insertion.Results The patient's satisfaction with sedation and with LMA acceptance was significantly higher in D1 and D2 groups than in group C, and in group D2 than in group D1 (P<0.05).The EC50 (95% confidence interval) of sevoflurane inhibiting responses to LMA insertion was 2.03% (1.91%-2.15%), 1.76% (1.65%-1.87%) and 1.63% (1.53%-1.73%) in C, D1 and D2 groups, respectively.The EC50 was significantly lower in D1 and D2 groups than in group C, and in group D2 than in group D1 (P<0.05).Conclusion Dexmedetomidine 1 and 2 μg/kg administered intranasally can decrease the EC50 of sevoflurane inhibiting responses to LMA insertion in the pediatric patients, and 2 μg/kg produces better efficacy.
6.Clinical Analysis of Pulmonary Fungal Infection in 40 Children in Intensive Care Unit

Objective To investigate the risk factors of pulmonary fungal infection in intensive care unit(ICU),and discuss the strategy of prevention and treatment.Methods Forty children with pulmonary fungal infection in ICU of Wuhan Children's Hospital from Jan.2003 to Jan.2007 were analyzed retrospectively,including primarily diseases,application of antibiotics,adrenal cortical hormone and virulence operation,therapy and turnover.Results All children were accepted the therapies of broad spectrum antibiotics and glucocorticoids for long time before definite diagnosis of pulmonary fungal infection.Seventy-five percent children were received invasive operations or therapies.Their average time of stayed in hospital was 37.8 d.The clinical symptoms and imaging examinations were untypical.Blastomyces albicans was the main pathogen.After the antifungal agents and supportive treatment used in time,35 cases(87.5%) were cured and 5 cases(12.5%) died.Conclusions The major risk factors of children pulmonary fungal infection are long-time use of broad spectrum antibiotics and glucocorticoids.The pulmonary fungal infection can decrease by rational use of broad spectrum antibiotics and glucocorticoids,decreasing the unnecessary invasive operations,strengthening the supportive therapies of micro-ecosystem,and applying the antifungal agents in time.
7.Depressing Effect on Biofilm Formation by Antimicrobial Plastics

OBJECTIVE To study the antimicrobial activity of antimicrobial plastics and its depressing effect to biofilm on the plastics′s surface. METHODS Test tube dilution method was adopted to study the antimicrobial activity and spectrum of antimicrobial plastics. Plate live bacterial recording method and scanning electron microscope(SEM) were introduced to study the formation of biofilm. RESULTS Wide inhibiting spectrum to pathogenic microbe such as Pseudomonas aeruginosa,Staphylococcus aureus,and Escherichia coli was confirmed,and the biofilm was formed after four stages,ie deposit,adhesion,propagation and biofilm formation,and propagation of microbe was found to be inhibited greatly in the surface of antimicrobial plastics,so that the formation of biofilm in the surface of antimicrobial plastics was held back,and the observation of SEM also confirmed that no biofilm was formed in the antimicrobial plastics′s surface. CONCLUSIONS Antimicrobial plastics could inhibit pathogenic microbe remarkably with wide inhibiting spectrum,and it could also depress the formation of biofilm in the surface,which indicated that it is a promising approach to prevent catheter related infections by adopting antimicrobial catheters.
9.Clinical features of acute macular neuroretinopathy

Objective To observe the clinical features of acute macular neuroretinopathy (AMN).Methods Six patients (11 eyes) with AMN were included in this study,with every 2-week follow-ups till six months.Among them,five had preceding dengue fever (83.3%),one had history of head trauma (16.7%).All patients received routine examination,fundus photography,infrared reflectance (IR) imaging,spectral-domain optical coherence tomography (SD-OCT) scanning and fluorescein fundus angiography (FFA) initially,and fundus photography,IR,SD-OCT during follow-up.Results Sudden onset of central/paracentral scotoma in one eye or both eyes was the main visual symptom.There were 1 eye with normal fundus,2 eyes with wedge-shape lesions,8 eyes with yellow-white or brown sheet lesion.IR imaging demonstrated localized areas of hypo-reflection in the macula.SD-OCT scanning through these areas revealed hyper-reflection in the photoreceptor layer and disruption of its normal reflective structures.Subsequent SD-OCT demonstrated that the hyper-reflection of the photoreceptor layer regressed gradually,followed by thinning of the outer nuclear layer.The external limiting membrane and ellipsoid zone became continuous;however,the interdigitation zone was not restored.There was no remarkable findings of the AMN lesions on FFA.The scotomas persisted in all 6 patients (11 eyes) by the last visit.Conclusions IR imaging demonstrated localized areas of hypo-reflection in the macula.SD-OCT revealed hyper-reflection in the photoreceptor layer in acute stage and the interdigitation zone was not restored in late stage.AMN has a relative poor prognosis with persistent scotomas through at least 6 months.
10.Clinical study of 3D laparoscopic surgery for adrenal pheochromocytoma / paraganglioma

Objective To investigate the efficacy and safety of 3D-laparoscopic surgery in adrenalpheochromocytomas /paragangliomas.Methods From December 2012 to July 2014,23 cases of adrenal pheochromocytomas/paragangliomas were treated by 3D-laparoscopic surgery.Among them,7 cases were male and 16 cases were female,whose mean age are 47 (32-68) years old.Their clinical symptoms including sustained hypertension in 11 cases,intermittent hypertension in 8 cases,sustained hypertension with intermittent progression in 2 cases and asymptom in 2 cases.All tumors were diagnosed by endocrine examination,image test and nuclear medicine.Mean diameter of tumors are 8 cm,ranged from 3-14cm.In order to adjust the blood pressure,patients took α-receptor blocker for 2-4 weeks preoperatively.Under the general anesthesia,19 patients received the 3D-laparoscopic surgery via retroperitoneal approach and 4 cases received the procedure via peritoneal approach.Results All operations were completed successfully.There were no major intraoperative complications.Mean operating time was (78 ± 21) min and mean estimated blood loss was (54.8 ± 36.3) ml.Mean hospital stay after operation was (3.8 ± 1.4) days.The pathological diagnosis included pheochromocytomas in 15 cases and paragangliomas in 8 cases.During the 3 to 18 months following up,no recurrence or metastasis was found in this study.Conclusions 3D-laparoscopic surgery has obvious advantage in spatial location and the sense of depth for adrenal pheochromocytomas /paragangliomas,which shortens the operation time and increase the safety obviously.
