As combined procedure with pars plana vitrectomy, pars plana lensectomy lowers the incidences of corneal edema and constriction of pupil, and elimi-nates the risk of corneal incision leakage.When intraocular lens is inserted, conventional procedure requires large anterior capsulectomy to prevent after-cataract.To maintain barrier between anterior and posterior seg-ments, anterior capsule was preserved and lens epithelial and cortical cells were polished off thoroughly with enhancement of visualization by intraocu-lar illuminator.Intraocular lenses were inserted into ciliary sulcus after vitrectomy.After a minimum of 1 year, postoperative vision was better in 17 of the 24 eyes (71%).the same in six eyes (25%)and worse in one eye (4%). Postoperative complications included after-cataract in 4 eyes (17%), decen-tration of intraocular lens in 2 eyes (8%), retinal detachment in 1 eye (4%) and neovascular glaucoma in 1 eye.Only two eyes required Nd:YAG laser capsulotomies. If intraocular illumination is utilized properly, lens epithelial and cortical cells can be removed thoroughly after pars plana lensectomy combined with vitrectomy and anterior capsule can be maintained without significant increase of after-cataract.

Perfluorocarbon liquids have been used to facilitate surgery in complicated retinal detachments or giant retinal tears which were difficult to manage. The results of perfluorophenanthrene usage in 26 eyes that had been taken vitreoretinal surgery were analyzed.The causes of retinal detachments were giant retinal tears in five eyes[19%], pseudophakia or aphakia in three[12%]and trauma in three.As tamponades, C3F8 gas was used in 11 eyes and silicone oil in 15 eyes. Overall, anatomical success rate was 73%and functional success rate was 58%.All of the giant retinal tears were successfully reattached and four eyes[80%]were functionally successful.The rates of anatomical success were 75%in eyes with grade C proliferative vitreoretinopathy, and 64%in grade D.Regarding postoperative tamponade materials, there was no significant difference between C3F8 gas and silicone oil.Releasing retinectomy was perfomed in four eyes and there was no functional success. In conclusion, perfluorocarbon liquid is a useful instrument that improves the result of operation of complicated retinal detachment if used selectively.

Fixation failure is a major complication of intertrochanteric fracture. The causes of fixation failure were assessed in a series of 80 patients with intertrochanteric fractures, which had been internally fixed with either a sliding hip screw or a Gamma interlocking nail. The overall rate of fixation failure was 16.3%, in which the cutting-out of the implant from the femoral head was the only cause of the instance. The cutting-out rate was influenced by the accuracy of fracture reduction, the position of implant placement within the femoral head, and the bone mineral density. Anatomical reduction and the central placement of implant within the femoral head showed the most excellent results compared to other treatment modalities. Age, gender, fracture type and a kind of fixation implant had no significant effect. Our results showed that the intertrochanteric fractures should be reduced as anatomically as possible and it is essential that the central placement of the implant within the femoral head be obtained.

PURPOSE: We evaluated the relationship between the delayed visual acuity (VA) improvement and retinal features, including in the photoreceptor layer, after a successful macular hole surgery using optical coherence tomography (OCT) images during long-term post-operative observation. The goal was to find the mechanism in delayed VA improvement. METHODS: Postoperative OCT scans were analyzed in nine eyes of eight patients who underwent idiopathic macular hole surgery between June 2003 to June 2005. Horizontal and vertical OCT scans taken of the operated eyes immediately after absorption of intraocular gas and after VA improvement were exported to Adobe Photoshop 7.0. We measured changes in parameters of the central foveal thickness (central 1 mm area), photoreceptor layer thickness, relative reflectivity of photoreceptor to retinal pigment epithelium, and the defective area of photoreceptor layer. RESULTS: The median visual acuity was determined to be 0.2 (range: 0.15~0.3) at the VA unimproved period and 0.5 (range: 0.3~1.0) at VA improved period. The mean central foveal thickness decreased from 221.3 micrometer (range: 155~265 micrometer) to 191.2 micrometer (range: 150~231 micrometer), as VA improved (p=0.007). The mean photoreceptor layer thickness increased from 15.2 micrometer (range: 4.2~27.6 micrometer) to 22.6 micrometer (range: 4.2~35.8 micrometer) as VA improved (p=0.032), and mean relative reflectivity of photoreceptor layer increased from 0.43 (range: 0.08~0.67) to 0.48 (range: 0.10~0.70), as VA improved (p=0.415). The defective area of photoreceptor layer decreased from 70% (range: 90~45%) to 27% (range: 8~5), as VA improved (p=0.001). CONCLUSIONS: A decrease of the central foveal thickness and of photoreceptor layer defective signal, and an increase of photoreceptor layer thickness were observed on OCT during delayed vision improvement after a successful macular hole surgery. Improvement of macular edema and photoreceptor reorganization are suggested as important parts of the mechanism toward vision recovery.

Because primary lymphoma of the appendix is a very rare disorder and commonly presented as acute appendicitis,it is seldom diagnosed by preoperative imaging study. We encountered a patient with pathologically proved primaryappendiceal lymphoma associated with acute and chronic appendicitis. Ultrasonogram revealed a non-compressiblesausage-shaped hypoechoic mass with a linear hyperechoic center caused by mucosa-lumen interface in right lowerquadrant. Post-contrast CT examination showed a markedly enlarged target-like appendix with obliteration of thelumen; the outer layer showed higher attenuation than the central portion. There were also multiple strands in theperiappendiceal fat and thickening of adjacent lateroconal fascia and colonic wall, and this suggested acuteappendicitis associated with appendiceal lymphoma.

BACKGROUND: An accurate ABO blood type is essential for transfusion therapy. Genetic structures of ABO blood group and subgroup have been investigated and so far about 100 ABO alleles have been reported worldwide. This study was performed to investigate the molecular characteristics of B subgroups in the Korean population. METHODS: A total of 19 samples of B subgroups were collected from patients (n=11) and from blood donors (n=8) of Korean Red Cross blood centers; these samples had been typed serologically for the ABO blood group. Allele-specific polymerase chain reaction (PCR), direct sequencing of exon 6 and 7, and allele separation were performed for ABO gene analysis. RESULTS: The ABO PCR-RFLP genotyping results of 18 samples among the provisional 19 B subgroups were identical regardless of their phenotypes. Two new B alleles showing 255C>T base change and 547G>A base change were observed in B3 and A1B3 subgroups. CONCLUSIONS: Serologically unidentified B subgroups were unequivocally identified through molecular analyses of the ABO gene. And new ABO alleles observed only in the Korean B subgroups were recognized.

PURPOSE: To report a case of bilateral acute myopia and angle-closure with ciliochoroidal detachment in Vogt-Koyanagi-Harada (VKH) syndrome. CASE SUMMARY: A 43-year-old Korean woman diagnosed with VKH syndrome underwent intravenous methylprednisolone steroid pulse treatment. After oral medication was given for 2 days, the anterior chambers became shallow in both eyes. Intraocular pressure (IOP) increased to 25 mm Hg in the right eye and 23 mm Hg in the left eye. Subretinal fluid increased and visual acuity decreased with myopic shift in both eyes. IOP did not decrease despite maximum tolerated medical therapy. Ultrasound biomicroscopy (UBM) revealed that ciliochoroidal effusion caused forward displacement of the lens-iris diaphragm, which resulted in anterior chamber shallowing and angle closure in both eyes. The patient was treated with cycloplegic eyedrops and intravenous steroid pulse therapy. After intravenous steroid pulse treatment was given for 2 days, the anterior chambers became deep in both eyes. IOP reduced to 13 mm Hg and 14 mm Hg in the right and left eye respectively. Visual acuity increased with normalization of myopic shift. UBM revealed that the ciliochoroidal effusion had resolved in both eyes.

STUDY DESIGN: This is a retrospective study analyzing the clinical results of various surgical methods for the treatment of delayed posttraumatic kyphosis. OBJECTIVES: The purposes of this study are to define the risk factors of delayed postraumatic kyphosis and to establish a rational therapeutic guideline for the treatment of established kyphosis. METHODS: Sixteen cases of surgically treated delayed posttraumatic kyphosis were analyzed. Their initial injuries were burst type in 10, flexion-distraction in 5 and uncertain in l. Initial treatments were conservative in 10 and surgical in 6. Treatments for established kyphosis were posterior fusion in 2, anterior fusion in 5, combined anterior-posterior fusion in 9. Anterior decompression was carried out in 5 treated with anterior fusion and in 4 treated with combined anterior-posterior fusion. RESULTS: Mean follow-up was 2.7 years. The mean preoperative kyphosis of 28.6。 was reduced to 18.5。 (35.3% correction) with most pronounced correction in the group with combined anterior and posterior stabilization(44.8% correction). Early loss of correction averaged 2.7。 (18.5%) with least loss in the combined anterior-posterior group. Neurological improvement was related to the duration of the symptoms(P < 0.05). Back pain was improved in all patients. CONCLUSIONS: Thoracolumbar fractures with initial three column injuries or those rendered unstable by laminectomy are highly susceptible to development of delayed posttraumatic kyphosis; Combined anterior and posterior fusion offered the most satisfactory result in correction and stabilization of the deformity; Neurologic improvement was more pronounced with shorter duration of the symptoms.
