3.The Present and Future of Artificial Intelligence-Based Medical Image in Diabetes Mellitus: Focus on Analytical Methods and Limitations of Clinical Use

Artificial intelligence (AI)-based diagnostic technology using medical images can be used to increase examination accessibility and support clinical decision-making for screening and diagnosis. To determine a machine learning algorithm for diabetes complications, a literature review of studies using medical image-based AI technology was conducted using the National Library of Medicine PubMed, and the Excerpta Medica databases. Lists of studies using diabetes diagnostic images and AI as keywords were combined. In total, 227 appropriate studies were selected. Diabetic retinopathy studies using the AI model were the most frequent (85.0%, 193/227 cases), followed by diabetic foot (7.9%, 18/227 cases) and diabetic neuropathy (2.7%, 6/227 cases). The studies used open datasets (42.3%, 96/227 cases) or directly constructed data from fundoscopy or optical coherence tomography (57.7%, 131/227 cases). Major limitations in AI-based detection of diabetes complications using medical images were the lack of datasets (36.1%, 82/227 cases) and severity misclassification (26.4%, 60/227 cases). Although it remains difficult to use and fully trust AI-based imaging analysis technology clinically, it reduces clinicians’ time and labor, and the expectations from its decision-support roles are high. Various data collection and synthesis data technology developments according to the disease severity are required to solve data imbalance.
4.Clinical Effects of Ureaplasma urealyticum Colonization in Maternal Vaginal Fluid at Second Trimester on Neonatal Outcomes of Premature Infant ≤35 Weeks' Gestational Age.

PURPOSE: The purpose of this study was to evaluate the relationship between maternal genital Ureaplasma urealyticum colonization in the second trimester and the neonatal outcomes. METHODS: We studied 577 premature infants born at ≤35 weeks' gestational age (GA) at Hallym University, Kangnam Sacred Heart Hospital from January 2008 to December 2014. Maternal vaginal specimens were collected and polymerase chain reaction and/or culture tests were done in the second trimester of pregnancy. The subjects were divided into 2 groups: a Uu colonization-positive group (UU) and a Uu-negative (control) group. Subgroup analyses were performed in extremely premature (23–28 weeks' GA) and premature (29–35 weeks' GA) infants. Various clinical outcomes were compared. RESULTS: Of 577 preterm infants, 311 were delivered with maternal Uu infection (UU, 53.9%). Mean GA (30.0±3.6 weeks vs. 31.0±3.7 weeks, P=0.010) and birth weight (1,685.6±680.4 g vs. 1,932.7±733.1 g, P=0.003) were lower in the UU group than in controls. However, there were no significant differences in GA and birth weight in extremely premature group. Premature labor (P=0.002) and histological chorioamnionitis (P=0.032) was significantly more common in the UU group, especially in mothers who delivered in third trimester (29–35 weeks' GA). In particular, the incidence of moderate-to-severe BPD was statistically higher in UU extremely premature group (60.8% vs. 32.2%) (P=0.001). CONCLUSION: Maternal colonization with Uu was associated with an increase in premature delivery and lower birth weights. Moderate-to-severe BPD was more common in premature infants born at ≤28 weeks' GA with maternal Uu colonization.

5.Small bowel herniation through trocar site after operative laparoscopy.

6.Bilateral Inguinal Hernias Detected by Peritoneal Scintigraphy during the Evaluation of Scrotal Swelling in a Patient on Continuous Ambulatory Peritoneal Dialysis.

A 47-year-old man with end-stage renal disease due to diabetic nephropathy underwent a peritoneal scintigraphy to evaluate the cause of recently developed scrotal swelling. Two liters of dialysate mixed with 111 MBq of Tc-99m sulfur colloid were administered into the peritoneal cavity via the dialysis catheter. Various anterior images of the abdomen and pelvis were obtained at 15 min, 2 hr and 4 hr after the tracer instillation. At 15 min, anterior images of the abdomen and pelvis demonstrated linear tracts of activity through both inguinal canals, which were more prominent in the right side (A). Images at 2 hr revealed a passage of the radioactive fluid into the right hemiscrotum. At the same time, there was a considerable accumulation of activity in the right inguinal canal (B). In the delayed image, there was a progressive accumulation of activity in the inguinal canals and a prominent passage of the tracer into the scrotum (C). Both abdominal and inguinal hernias are commonly associated with continuous ambulatory peritoneal dialysis (CAPD). Overall incidence of CAPD-induced hernia ranges from 2.7% to 25%.1) Inguinal hernias were frequently manifested as scrotal swelling. Leakages of dialysate fluid into the scrotum has been noted in CAPD patients with scrotal swelling, with or without clinical findings of inguinal hernia.1,2) In the present case, the right side had leakage from a clinical inguinal hernia and the left side, leakage from a subclinial inguinal hernia. A subclinical inguinal hernia was easily demonstrable with peritoneal scintigraphy. Peritoneal scintigraphy is extremely helpful in the evaluation of scrotal swelling in a patient on CAPD.

7.Comparison of in vitro Antimicrobial Activities of Tc-99m Infecton and Ciprofloxacin.

PURPOSE: There was little evidence that Tc-99m labeled ciprofloxacin (Infecton) located inside of bacteria. Antimicrobial activity of Infecton could be an indirect evidence of its location. We compared in vitro antimicrobial activities of Infecton and ciprofloxacin. MATERIALS AND METHODS: Minimum inhibitory concentration (MIC) and minimum bactericidal concentration (MBC) of Infecton and ciprofloxacin against three standard strains of bacteria, Staphylococcus aureus ATCC 29213, Escherichia coli ATCC 25922 and Pseudomonas aeruginosa ATCC 27853 were measured using modified broth macrodilution techniques and pour plate methods, respectively. Data were expressed as mean+/-SE (range). RESULTS: MICs of Infecton and ciprofloxacin were 1.12+/-0.20 (0.8~1.6) microgram/ml and 0.36+/-0.04 (0.2~0.4) microgram/ml for S. aureus, 0.03+/-0.005 (0.025~0.05) microgram/ml and 0.011+/-0.001 (0.006~0.012) microgram/ml for E. coli, and 0.96+/-0.16 (0.8~1.6) microgram/ml and 0.56+/-0.098 (0.4~0.8) microgram/ml for P. aeruginosa, respectively. MBCs of Infecton and ciprofloxacin were 2.56+/-0.39 (1.6~3.2) microgram/ml and 0.88+/-0.2 (0.4~1.6) microgram/ml for S. aureus, 0.04+/-0.06 (0.025~0.05) microgram/ml and 0.02+/-0.01 (0.025~0.05) microgram/ml for E. coli, and 2.24+/-0.39 (1.6~3.2) microgram/ml and 1.44+/-0.16 (0.8~1.6) microgram/ml for P. aeruginosa, respectively. CONCLUSION: Although both MICs and MBCs of Infecton were higher than those of ciprofloxacin, all three standard bacterial strains were sensitive to Infecton. It could be an indirect evidence that Tc-99m Infecton be a specific imaging agent for bacterial infection.

9.The realities of internet shopping malls for agricultural products.

Purchasing professionals utilize a e-commerce to sell or buy better products with good price. The marketers of agricultural products recognized that e-commerce has made significant inroads, and the Internet is fast becoming an essential tool for purchasing. The purpose of this research was to assess the realities of internet shopping malls of agricultural products including the actual condition of the sites, payment methods, order methods, membership and shipping methods. The results show that operating internet shopping mall of agricultural products are operated poorly in terms of buyer-seller relationship, payment and distribution. The results also provide some practical implication for marketers regarding the establishment of database, improvement of distribution and internet shopping mall operation, and importance of education on e-business for agricultural markets.

10.Establishment of Embryonic Stem Cell Line from Isolated Blastomeres from Mouse Preimplantation Embryos.

OBJECTIVE: The aim of this study was to investigate whether embryonic stem (ES) cells can be established from isolated blastomeres of mouse embryos. METHODS: Blastomeres were separated from mouse (C57Bl/6J) 2- or 4-cell embryos. Isolated blastomeres or whole 4-cell embryos were co-cultured with mitosis-arrested STO feeder cells in DMEM supplemented with recombinant murine leukemia inhibitory factor and ES-qualified fetal bovine serum. After the tentative ES cell lines were maintained from isolated blastomeres or whole embryos, some of them were frozen and the others were sub-cultured continually. Characteristics of tentative ES cell lines as were evaluated for specific gene expressions with immunocytochemistry and RT-PCR. RESULTS: One ES cell line (3.0%) was established from isolated blastomere of 2-cell embryo and one cell line (4.0%) from isolated two blastomeres of 4-cell embryo. And five cell lines (16.7%) were established from whole 4-cell embryos. Both cell lines from isolated blastomere and whole embryo expressed mouse ES cells specific markers such as SSEA-1, Oct-4 and alkaline phosphatase. Marker genes of three germ layers were expressed from embryoid bodies of both cell lines. CONCLUSION: This study suggests that mouse ES cells could be established from isolated blastomeres, although the efficiency is lower than whole embryos. This animal model could be applied to establishment of autologous human ES cells from biopsied blastomeres of preimplantation embryos in human IVF-ET program.
