3.Occupational Skin Disease in Hairdressers: Results of Patch Test.

BACKGROUND: Hairdressers are individuals at high risk of occupational skin disease, especially occupational allergic contact dermatitis (OACD), because of continuous exposure to water, detergents, hair dyes and bleaches, permanent wave solutions and metal equipment. OBJECTIVE: We evaluated the frequency of OACD among hairdressers and the sources of sensitization through patch test and questionnaires. METHODS: We analyzed the occupational characteristics (age, gender, duration of work), site of dermatitis, suspicious sensitizers, and use of preventive measures in a group of 96 hairdressers who answered a questionnaire. Among them, 61 hairdressers who were suspected of having occupational allergic contact dermatitis were patch-tested with the Korean standard series and hairdressing series. RESULTS: Among 96 hairdressers, 74 hairdressers (78%) responded with their experiences of work-related skin problems. The results of patch test revealed an occupationally-relevant sensitization in 82% of the 61 hairdressers. Mean frequencies of sensitization ranked as follows: nickel sulfate (37.7%), cobalt chloride (34.4%), paraphenylenediamine base (21.3%), thimerosal (11.5%), potassium dichromate (9.8%), mercury ammonium chloride (8.2%), Balsam Peru (6.6%), glyceryl monothioglycolate (4.9%), and fragrance mix (4.9%). We also classified these allergens as hairdressing agents, and hair dye showed the highest frequency of positive reactions (30%). CONCLUSION: From this study, we were able to unravel the important sensitizers related to the hairdressing profession. To improve the safety of their job and to the prevent occupational allergic contact dermatitis, hairdressers should be aware of these common sensitizers, and try to improve protective measures and their applications.

Ipsilateral fractures of the femur neck and shaft show diagnostic difficulties and complexity of the treatment. This femoral double fracture, when not initially diagnosed, carries a much high risk of complications, such as aseptic necrosis of the femoral head. Early recognition is of importance. The definitive treatment of a fractured femur should be begun after the ipsilateral hip and the knee joint have been examined clinically and radiologically. At the Department of Orthopedic Surgery, Dae Han General Hospital, from July 1985 to March 1990, four cases of the concomitant ipsilateral femur neck and shaft fracture had been treated and followed-up for 2.4 years in average (ranging from 1.1 to 4.7 years).

6.A Case of Cutaneous Metastatic Adenosquamous Carcinoma of the Pancreas.

Pancreatic adenosquamous carcinoma is a rare variant of ductal adenocarcinoma and accounts for only 1~4% of exocrine pancreatic malignancies. Although pancreatic carcinoma shows an aggressive biologic behavior, cutaneous metastasis from pancreas carcinoma is rare. We report a case of a pancreatic adenosquamous carcinoma which clinically manifested as two firm nodules on the right upper extremity. It was this first clinical manifestation that made it possible to diagnose pancreatic adenosquamous carcinoma.

7.A Case of Thymic Carcinoma with Direct Invasion into the Skin.

Thymic carcinoma is a rare malignant neoplasm of the thymic epithelium, distinguished from invasive thymoma by the presence of malignant cytology and a greater incidence of local invasion and embolic metastases. The symptoms are anterior chest pain, dyspnea, coughing, and superior vena cava syndrome usually caused by compression of mediastinal organs. Histologic grade is the most important prognostic factor. Although it is common to observe intrathoracic invasion or metastasis, extrathoracic invasion is extremely rare, and only a few cases have been reported to date. Herein, we report a case of a patient with a thymic carcinoma which showed extrathoracic invasion, presenting as a solitary, pinkish nodule on the chest wall.

9.Combined trial of fish oil and exercise training prevents impairment in insulin action on glucose transport of skeletal muscle induced by high-fat diet in rats.

The purpose of the present study was to determine the preventive effects of combined interventional trial of fish oil treatment and exercise training on insulin resistance of skeletal muscle in high-fat fed rats. Male Wistar rats were randomly divided into chow diet (CD), high-fat diet (HF), high-fat diet with fish oil (FO), high-fat diet with exercise training (EX), and FO+EX groups. The rats in control group were fed chow diet containing, as percents of calories, 58.9% carbohydrate, 12.4% fat, and 28.7% protein. High-fat diet provided 32% energy as lard, 18% as corn oil, 27% as carbohydrate and 23% as casein. The fish oil diet had the same composition as the high fat diet except that 100 g menhaden oil was substituted for corn oil. Insulin sensitivity was assessed by in vitro glucose transport in the soleus muscle after diet treatment and treadmill running for 4 weeks. While the FO or EX only partially prevented insulin resistance on glucose transport and visceral obesity induced by high-fat diet, these interventions completely corrected hyperinsulinemia and hyperglycemia from the high-fat diet. The rats in the FO+EX showed normalized insulin action on glucose transport, plasma chemicals and visceral fat mass. Insulin-mediated glucose transport was negatively associated with total visceral fat mass (r=-0.734; p<0.000), plasma triglyceride (r=-0.403; p<0.05) and lepin (r=-0.583; p<0.001) concentrations with significance. Multiple stepwise regression analysis showed that only total visceral fat mass was independently associated with insulin-mediated glucose transport (r=-0.668; p<0.000). In conclusion, combined interventional trial of FO+EX recovered insulin resistance on glucose transport of skeletal muscle induced by high-fat diet. Visceral fat mass might be more important factor than plasma TG and leptin to induce insulin resistance on glucose transport of skeletal muscle in high-fat fed rats.

10.Anterior Ceervical Interbody Fusion: Comparison Study between Simple Fusion, Locked and Non-locked Plating Systems.

The use of metallic plate and screws in the anterior cervical fusion has become generally acceptable and popular. Mainly, there are two different plating systems available. However, there has been few attempts to compare their clinical results with and without a plating system or between two different systems in a single series of clinical study. The authors reviewed 117 patients who underwent anterior cervical fusion during the period of January 1992 to September 1996. Simple fusion without a plating system was applied in 55 cases(group 1), a bicortical non-locked plate screw system in 25(group 2), and a monocortical locked plate screw system in 37(group 3). The average follow-up period was 13. 2 months. In group 1, twenty-two patients(40%) with fracture-dislocation or corpectomy required a rigid brace such as halo brace and Minerva cast for 3 months postope ratively and seven patients(13%) experienced graft complications, mainly graft extrusion. In group 2 and 3, the patients required only soft b races for 4 to 8 weeks and no patients experienced serious graft complications like ones in group 1 but, instead two patients with screw breakages, two back-outs and one non-union were observed. In comparison of the clinical complications such as hoarsness, there were no significant differences between the groups. In conclusion, the plating system in anterior cervical fusion appears to be safe despite the facts that mo re technical demands are required during surgery. It also provides better postoperative stability in the spine and permits earlier ambulation without a rigid brace. A monocortical locked plate screw system appears to have less hardware failures and better su rgical results than a bicortical non-locked plate screw system.
