1.The Types and Perceptions of Touch in Nurse-Elderly Patient Interactions.

This study examined the types of touch received by elderly patients from nurses, and explored the elderly patient' and nurses' perceptions of touch occurring during nursing activities. Non-participant observation and semi-structured interviews were the methods employed for data collection. The study subjects were composed of 24 nurses of the three medical wards, and their 83 elderly patients who were hospitalized at S Hospital in Seoul from February 15th to March 15th in 1996. The Observation Schedule, the Perception of Touch Instrument, and the Patient classification were study instruments. The most common types of touch occurring during the nurse-elderly patient interactions were : task touch(82.9%), caring touch(4.9%), and concurrent type of touch (12.2%). The mean score of the elderly patients' perception of touch was higher at 30.45(range 5-35)compared to the mean score of the nurses' perception of touch which was 23.01(range 5-35). Elderly patients received the nurses' touch much more positively than the nurses. But the two scores were not significantly correlated(r=.06, 29). The elderly patient's touch perception score was higher(p<.05) in the group that was touched upon approach than the group was touched later. Elderly patients preferred caring touch to task touch, but it was not significant. Nurses' touch perception scores were higher(p<.05) for female than male. Elderly patients felt most comfortable when the nurses administered the touch on their painful site.

4.Clinical observation for low birth weight infants.

Clinical observations were made on 338 low birth weight infants, who were delivered at Pusan Wallace Memorial Baptist Hospital, during the 3 year periods from 1989 to 1991. The results were as follows; The incidence of LBWI was 5.6% and sex ratio was 1:1.09. Among them AGA was 62.7%. 2) Of all infants, 47.3% were in the birth weight range of 2251-2500 gm and 25.1% were in the gestational age group of 37-38 week. 3) LBWI were most prevalent among mothers under 20 years old and over 41 years old. 4) The Incidence of LBWI was higher in multiparity than primiparity and in multiple pregnancy than single pregnancy. 5) Vaginal delivery was 63.9% and c-section delivery was 36.1%. 6) The order or frequency of the etiology of LBWI was multiple pregnancy, premature rupture of membrane and toxemia. 7) The mortality rate of LBWI was 12.7% and mortality rate by weight-gestational age was lower in AGA group than SGA and LGA group. 8) A Higher mortality rate was noted in the group of lower birth weight infant, and in the group of more shortened gestational age. 9) 48.8% of death occured within ist day of birth. The common causes of death were respiratory distress syndrome, asphyxia, and apnea.

5.Prevalence and Clinical Features of Ketotic Hypoglycemia in Childhood.

PURPOSE:Idiopathic ketotic hypoglycemia (IKH) is an important cause of hypoglycemia in children. The purpose of this study was to determine the prevalence of ketotic hypoglycemia and to describe the clinical manifestation. METHODS:We conducted a retrospective chart review of children presenting hypoglycemia to the emergency department (ED) of Ewha Womans University Hospital between January 1995 and June 2004. Information recorded for subject included age, gender, weight, time of presentation, presenting symptoms, and laboratory data. RESULTS:Eighty two children were identified for hypoglycemia during the study period. IKH was the ultimate diagnosis for 66 patients (80.5%). Other diagnoses included hyperinsulinism (2.4%), drug adverse event (1.2%), sepsis (1.2%), and other disorder. The mean age for presentation of IKH was 37.9+/-18.6 months. Thirty eight boys and twenty eight girls were identified. The median time of presentation was 9:48 a.m. Of the 66 subjects, body weight of 46.9% patients was below 25th percentile for age. The average blood glucose was 41.4+/-14.2 mg/dL. Symptoms at presentation included lethargy (24.2%), mental change (16.7%) and vomiting (16.7%). 49 patients (74.2%) were described as having a concomitant illness. CONCLUSION: IKH was responsible for 80.4% of cases of hypoglycemia in pediatric ED and it had usually concomitant intercurrent illness which resulting in poor oral intake. Awareness of IKH, and its presenting characteristics, will be helpful in the ED when taking care of hypoglycemic children.

9.Factors Influencing the Hospital Presentation Time of Stroke Patients.

PURPOSE: This cross-sectional survey research was undertaken to identify the factors influencing time from onset to hospital arrival of stroke patients and to provide basic information for the development of intervention programs for stroke patients. METHODS: The data were collected using a convenient sampling method from three hospitals in Inchon. The subjects were 78 patients who were diagnosed as stroke by doctor and they voluntarily participated in the study. RESULTS: On the average, subjects arrived at the hospitals by 16.72 hours after the onset of stroke events with the range from 0.17 hours to 72 hours. Thirty-four(43.6%) subjects arrived within 3 hours which can maximize treatment effects. There was significant difference in hospital presentation time according to the level of knowledge(chi-square=18.629, p=.0003). A negative correlation was found between the hospital presentation time and self-efficacy (r= -.320, p=.004). Stepwise multiple regression analysis revealed that the most powerful predictor was self-efficacy. Self-efficacy, the level of knowledge and physical symptoms were significant factors and accounted for 21.7% of the variance of hospital presentation time in stroke patients. CONCLUSION: According to the results, self-efficacy is a useful concept for reducing the hospital presentation time from onset of attack in stroke patients. Therefore, nurses should consider educational programs which include not only a knowledge of stroke and recurrence prevention but also the concept of self-efficacy.

10.The Isolation and Characterization of Muscle Derived Stem Cells from Gastrocnemius Muscle of Rats Using the Modified Preplate Method.

PURPOSE: This study attempted to characterize the muscle derived stem cells isolated from the primary cultured skeletal muscle of the rat gastrocnemius muscle; in addition, we modified the preplate method and then compared this to the original preplate method. MATERIALS AND METHODS: The hind limbs (gastrocnemius muscles) were removed from a 3-6 week olds SD-rat and the bone was dissected away. The muscle mass was finely minced and chopped using razor blades. In an original preplate method, the cells were dissociated using a triple enzyme mixture (collagenase XI, dipase and trypsin) for 1 hour at 37degreesC. The muscle cell extract was preplated on culture flasks as described by Dr. Qu (Qu et al., 1998). The pp1-pp4 cells were referred to as the early plate (EP) cells, and the pp5-pp6 cells were referred to as the late plate (LP) cells. When we modified the preplate method, the pp1-pp2 cells were called the early plate (EP) cells and the pp3-pp4 cells were called to late plate (LP) cells. The phenotypical characteristics of EP and LP cells were compared by immunostaining and FACS. RESULTS: In the original preplate methods, the early plate (EP) cells were mixed with myogenic cells (mostly fibroblasts, <15% desmin + cells) and the LP cells were highly purified muscle derived cells that contain pure myogenic cells (>90% desmin + cells). Yet in the modified preplate method, the muscle derived stem cells were determined to be CD34 (+ or -), CD45- and desmin- cells by immunohistochemical staining and FACS. CONCLUSIONS: In original methods, the LP cells exhibited stem cell properties (CD34+, less than 30%), and they were not from a hematogeous origin (CD45-), but rather, they were from a myogenic origin (desmin+). Yet in the modified preplate method, we purified the LP cells much earlier than the original method. The LP cells displayed CD34+(more than 50%), and CD45-; thus, we isolated more primitive (desmin-) cells.
