PURPOSE: The purpose of this study was to examine the effects of structured nursing intervention for caregivers on maintenance of intravenous (IV) infusions in infants. METHODS: The structured nursing intervention was developed through the processes of interviews with nurses from pediatric wards. To identify the effects of the developed nursing intervention, a non-synchronized non-equivalent control group pretest-posttest design study was conducted with caregivers from the pediatric ward from a hospital in Seoul. Of 100 admitted infants, 50 caregivers were assigned to the intervention group and received the structured nursing intervention for maintenance of intravenous infusions in infants. The others were assigned control group and received routine care. Data were collected on patient factors, IV insertion factors, treatments, and IV related complications. The form developed for the structured nursing intervention was used by staff nurses. RESULTS: Compared to the control group, IV insertion frequency in experimental group infants was significantly lower and IV related complications decreased (p<.05). CONCLUSION: These results suggest that the structured nursing intervention for caregivers on maintenance of intravenous infusions may have effects on maintenance of intravenous infusions in infants, and decreasing IV related complications. This nursing intervention can be used to improve IV related problems of admitted infants.

Drug-induced thrombocytopenia and purpura have boon developed by many various agents. Rifampin and Pyrazinamide have been known as bactericidal antituberculous drugs, but, the above side effects have been a problem. Especially, hematologic side effects art fatal to patients occasionally. Rifampin-induced thrombocytopenia and purpura have been well known, also, pyrazinamide-induced thrombocytopenia have been reported. A new quilonone agent Ciprofloxacin, has been commonly used in clinics now, but it's side effects are not known well. So, we report a case of a 23-year-old female with thrombocytopenia and purpura after taking Rifampin, Pyrazinamide, and Ciprofloxacin as antituberculous agents.

PURPOSE: This study aimed to estimate the effects of a health mentoring program on fasting blood sugar, total cholesterol, triglyceride, physical activity, self care behavior and social support changes among community-dwelling vulnerable elderly individuals with diabetes. METHODS: A non-equivalent control group pre-post-test design was used. Participants were 70 community-dwelling vulnerable elderly individuals with diabetes. They were assigned to the experimental (n=30) or comparative (n=30) or control group (n=28). The experimental group participated in the health mentoring program, while the comparative group participated in health education program, the control group did not participate in any program. Data analyses involved a chi-square test, Fisher's exact test, a generalized linear model, and the Bonferroni correction, using SPSS 23.0. RESULTS: Compared to the control group, the experimental and comparative groups showed a significant decrease in fasting blood sugar, total cholesterol, and triglyceride. Compared to the comparative and control groups, the experimental group showed significant improvement in self care behavior. However, there were no statistical differences in physical activity or social support among the three groups. CONCLUSION: These findings indicate that the health mentoring program is an effective intervention for community-dwelling vulnerable elderly individuals with diabetes. This program can be used as an efficient strategy for diabetes self-management within this population.

BACKGROUND: Penicillin- and multidrug-resistant Streptococcus pneumoniae became a global problem during recent decades. Multidrug resistance poses a serious threat to clinical medicine due to restriction of selecting appropriate antimicrobial agents to treat with. Current data suggest that any single antimicrobial agent cannot be a satisfactory option to treat pneumococcal infections caused by multidrug-resistant strains, particularly in meningitis. The aim of the study was to assess in vitro efficacy of several antimicrobial combinations that are commonly used in clinical practice, and to obtain reasonable candidate regimens that can be applied to in vivo model. METHODS: We performed time-kill studies of antimicrobial combinations including penicillin, cefotaxime, vancomycin, gentamicin, imipenem and ampicillin against five multidrug-resistant strains and two penicillin-susceptible strains. Penicillin, cefotaxime and vancomycin were combined with gentamicin, respectively. Cefotaxime plus vancomycin, imipenem plus vancomycin, and cefotaxime plus ampicillin combinations were also tested. Synergy was defined as a >or =100-fold or 2-log decrease in colony count at 24 h by the combination compared with that by the most active single agent. RESULTS: Penicillin plus gentamicin, cefotaxime plus gentamicin, and vancomycin plus cefotaxime combinations were demonstrated to have in vitro synergistic activities against multidrug-resistant strains. CONCLUSION: Three combinations showed in vitro synergism against penicillin- resistant pneumococci. Experimental animal study is warranted to determine the clinical relevance of the in vitro results.

Fiberglass, as a substitute of asbestos, is used for more than 60 years as a insulator material. Health hazards including irritation of skin, mucosa and respiratory system associated with use of fiberglass is reported. Many studies on the fibrogenicity and carcinogenicity of fiberglass was conducted but evidence is not sufficient to confirm the carcinogenicity or fibrogenicity. Authors studied the health hazards among 152 inhabitants(71 men and 81 women) from 32 households living around the fiberglass factory which produced the fiberglass insulators and glasswool panels for 20 years. Questionnaire survey on household and persons, examination of underground water, pathologic examination of subcutaneous tumors and examination of fiberglass in tumor tissues were done. The results are as follows; 1. Fiberglass concentration of underground water sampled from 33 households in the study area was 13. 7-95. 9 fiber/cc with the diameter to length ratio more than 1:20. 2. Prevalence of dermatosis among study subjects was 23.0 % (35 cases). Prevalence was not associated with the distance from the factory nor duration of exposure. 3. There were 15 cases of subcutaneous tumor with prevalence of 9. 9 %. Age of subcutaneous tumor cases was all above 30 year-old except one cases, who was 5 year old child, who lived in the surveyed area since he was born. Prevalence of subcutaneous tumor was significantly high in area A(42.9 %) than area B(4.6 %, p<0.01). Prevalence of subcutaneous tumor was higher in long-term exposed inhabitants, but was not statistically significant. 4. Pathologic examination exhibits partly encapsulated fat tissue masses and cut surfaces were pale yellow with gritty sensation. The masses consist of mature fat cells showing variation in size and shape. On polarizing microscope, the peripheries of tumors include small irregular threads of doubly refractile material probably represent glass fibers in fibrocollagenous tissue. Concentration of fiberglass in tissue was 5.1-10.2 fiber/rag wet tissue in case 1, 25.8-184.9 fiber/mg wet tissue in case 2 and 40.8-126.5 fiber/mg wet tissue in case 3. Length of fiberglass was shorter than that in underground water. 5. Cases of malignant tumor among inhabitants since last 10 years were 4, 3 of whom was developed in a same household just near the factory. Diagnoses of malignancy cases were stomach cancer, stomach and esophageal cancer, oral cavity cancer, and stomach cancer with liver metastasis. On review examination of tissues of endoscopic biopsy specimen from a case of stomach cancer, there was adenocarcinoma with no evidence of fiberglass materials. Authors concluded there is evidences that fiberglass was strongly associated with the development of the health hazards including dermatosis and benign subcutaneous tumor. However, the association of fiberglass exposure with the development of malignant tumor was not clear, although strongly suggested. For the prevention of development of further health hazards, it is recommended that under ground water source should be closed and further experimental study to confirm the mechanism of the tumorigenesis and follow up survey on the inhabitants should be conducted.

Patients undergoing peritoneal dialysis are at risk for protein-energy malnutrition because of nutrient losses during dialysis. This study determined the nutritional status of patients undergoing continuous ambulatory peritoneal dialysis (CAPD). Forty-four patients receiving CAPD were divided into two groups according to dialysis period. We investigated the nutritional status of the patients by measuring anthropometric and biochemical parameters, as well as food intake, self-appetite, dietary habits, a subjective global assessment, and a total nutritional status assessment. Group I subjects (7 males, 13 females) had received dialysis for < 2 years, whereas the group II subjects (18 males, 6 females) received dialysis for > or = 2 years. Energy intake with added dextrose in the dialysate per kg of body weight was 30.3 +/- 5.8 kcal in group I and 29.0 +/- 8.1 kcal in group II. The average protein intake per kg of weight was 1.0 +/- 0.3 g in group I and 1.0 +/- 0.4 g in group II, which were less than the recommended protein intake for patients undergoing CAPD (1.2-1.5 g/kg). Mean serum albumin level was significantly lower in group II than that in group I (p < 0.05). A recent self-appetite score was significantly higher in group II than that in group I (p < 0.01). The dietary habits score was significantly lower in group II than that in group I (p < 0.05). The subjective global assessment was significantly higher in group I (85.0%) than that in group II (54.2%) under normal nutrition status (p < 0.05). The dialysis period was significantly and negatively correlated with the subjective global assessment (r = -0.502, p < 0.01) and the total nutritional status assessment (r = -0.575, p < 0.01). These results demonstrated that patients undergoing CAPD for > or = 2 years had worse nutritional status than those who had been undergoing dialysis for < 2 years. Good nutritional status can predict the long-term survival of patients undergoing peritoneal dialysis. Additionally, the exact evaluation of nutritional status before 2 years will be important to maintain long-term dialysis therapy in patients undergoing CAPD.

PURPOSE: This aim of this study was to evaluate changes in gastric volume and organ position as a result of delayed gastric emptying after a subtotal gastrectomy performed as part of the treatment of stomach cancer. MATERIALS AND METHODS: The medical records of 32 patients who underwent concurrent chemoradiotherapy after a subtotal gastrectomy from March 2005 to December 2008 were reviewed. Of these, 5 patients that had more than 50 cc of residual gastric food detected at computed tomography (CT) simulation, were retrospectively enrolled in this study. Gastric volume and organ location was measured from CT images obtained before radiotherapy, twice weekly. In addition, authors evaluated the change of radiation dose distribution to planning the target volume and normal organ in a constant radiation therapy plan regardless of gastric volume variation. RESULTS: A variation in the gastric volume was observed during the radiotherapy period (64.2~340.8 cc; mean, 188.2 cc). According to the change in gastric volume, the location of the left kidney was shifted up to 0.7 - 2.2 cm (mean, 1.2 cm) in the z-axis. Under-dose to planning target volume (V43, 79.5+/-10.4%) and over-dose to left kidney (V20, 34.1+/-12.1%; Mean dose, 23.5+/-8.3 Gy) was expected, given that gastric volume change due to delayed gastric emptying wasn't taken into account. CONCLUSION: This study has shown that a great change in gastric volume and left kidney location may occur during the radiation therapy period following a subtotal gastrectomy, as a result of delayed gastric emptying. Detection of patients who experienced delayed gastric emptying and the application of gastric volume variation to radiation therapy planning will be very important.

Carcinoid tumors originate from the enterochromaffin cells that are located in the gastrointestinal mucosa. Most of these tumors are asymptomatic and gastric carcinoid with massive gastrointestinal bleeding is found rarely. We experienced a case of gastric carcinoid admitted to our hospital due to melena and dizziness. The esophagogastroduodenoscopic examination revealed an ulcerofungating mass in the lower body which was thought to be a Borrmann type III adenocarcinoma. Microscopic examination of the biopsy specimens showed benign gastric ulcer without malignant cells. Repeated endoscopic biopsy revealed the same results. A computed tomographic scan of the abdomen showed a gastric mass with multiple perigastric enlarged lymph nodes. Histopathological examination of the operative specimen showed gastric carcinoid. We report a case of gastric carcinoid with massive bleeding mimicking advanced gastric cancer.
