Objective To analyze the utilization of community health services(CHS) and influencing factors in Shenzhen. Methods Surveys were conducted by means of cluster random sampling and utilization of CHS for health promotion, utilization of single services and integrated utilization were analyzed using descriptive methods. Results The rate of integrated utilization was 35.90% and the most utilized single services were patient care and drug purchase, with general physical checkup, health guidance and disease prevention coming next. Conclusion Factors such as the layout, convenience and knowledge of CHS are conducive to enhancing the rate of CHS utilization whereas factors such as residence in a special economic zone, student status, and higher educational backgrounds have a negative influence on CHS utilization.

Objective To investigate the effect of dynamic regulation of negative pressure values in vacuum sealing drainage (VSD) on healing of soft tissue wounds.Methods From January 2013 to December 2015,115 patients were treated at our institute for open soft tissue defects of Gustilo-Anderson type ⅢaⅢc.They were 69 males and 46 females,aged from 17 to 76 years (average,44.7 years).They were divided into 4 groups to be subjected to different negative pressure values.Group A (n =29) was subjected to negative pressure values from-40 to-20 kPa,group B (n =29) to values from-80 to-60 kPa,group C (n =29) to values from-40 to-20 kPa for the first 72 hours followed by values from-80 to-60 kPa,and group D (n =28) to values from-60 to-80 kPa for the first 72 hours followed by values from-40 to -20 kPa.The 4 groups were compared in terms of drainage volume,wound shrinking,cleaning time,healing time and complications.Results The drainage volumes on the 7th day in groups B and D (4.89 ± 0.66 mL/cm2 and 4.33 ± 0.96 mL/cm2) were significantly higher than those in groups A and C (2.90 ± 0.67 mL/cm2 and 2.99 ±0.56 mL/cm2);the wound shrinking areas on the 14th day in groups B and D (14.16 ± 1.77 cm2 and 13.84 ± 1.65 cm2) were significantly larger than in groups A and C (12.57 ± 1.92 cm2 and 10.95 ± 1.37 cm2) (P ＜ 0.05).The cleaning time in group A was significantly longer than in the other 3 groups (P ＜ 0.05).The healing time in groups A and C was longer than in groups B and D (P ＜ 0.05).The incidences of pain in groups B and C were significantly higher than in groups A and D (P ＜ 0.05).The incidences of infection and blocking in group A were higher than in the other 3 groups (P ＜ 0.05).Conclusion Dynamic regulation of negative pressure values from-80 to-60 kPa for the first 72 hours followed by values from-40 to-20 kPa is an optimal choice for VSD management of soft tissue defects due to adequate drainage,obvious wound shrinking,quick cleaning and healing,and limited complications.

ObjectiveTo investigate the development of apoptosis during ischemia/reperfusion (IR) injury and acute rejection, and to explore the significance of apoptosis in the Graft Mesenteric Lymph Node (GALT) in a rat heterotopic small bowel transplant (SBT) model.MethodsSBT was performed in F344/N rats with either freshly harvested or preserved (4 h, in ringer lactate solution at 4 ℃) syngeneic and allogeneic (Wistar/A-F344/N) grafts. Bowel and GALT samples were collected 2 h after reperfusion and on small bowel transplant postoperative days (POD) 1, 4, and 7. Histopathology assessment of the graft and GALT were prepared for hematoxylin-eosin (H&E) staining. Apoptosis was detected by the TUNEL and the electron microscope. ResultsThe number of apoptotic cells 2 h after reperfusion increased profoundly in association with preservation. After a significant decrease on POD 1, the apoptotic cells rose again between POD 3 and 7 only in allogeneic grafts. On the other hand, the apoptotic cells in allogeneic GLAT markedly increased from POD 1 to day 3; at that time point, neither histological findings of rejection nor increase in apoptotic crypt cells were present in the graft jejunum. ConclusionIR injury and acute rejection may both induce extensive apoptosis. The graft jejunum distinct second increase in apoptosis may be an early and specific sign of acute rejection. Apoptosis of GLAT cells was well correlated with and ahead of progression of acute rejection.

Objective To study the application of SLINKY technique in MRA of head and neck by comparison of four TOF MRA methods. Methods We obtained images on 21 patients and 5 volunteers with SLINKY on all the 26 cases, MOTSA on 18 cases, single volume on 16 cases, and 2D on 12 cases. Three experienced radiologists evaluated the images, unknowing which method the images were. The evaluations included clearness of vessel branches, smoothness of vascular wall, and vascular continuity. Signal-to-noise (SNR), contrast-to-noise (C/Ns), and scan time were measured. Results SLINKY displayed vessel branches better and had better smoothness of vascular wall and vascular continuity than the other three methods(P

Objective To observe and analyze the CT and MR imaging of the structures in the region of the jugular foramen (JF) on the base of thin-slice anatomic study. Methods Having been scanned by multislice CT and 1.5T MR scanner, two formalin-preserved adult cadavers were dissected into 1.0 mm thickness contiguous sections in transverse plane. Twenty cases without skull base and nasopharyngeal history received routine and post-contrast CT examinations with spiral and HQ mode. Twenty healthy volunteers received MR scanning, including SE T 1WI, FSE T 2WI, and 3D RF-FAST (3D Radio-Frequency Fourier Acquired Steady-State) sequences. Results JF region was divided into three levels, which included inner aperture, the jugular cavity, and the outer aperture. At the entrance of JF, there were glossopharyngeal canal and vagal canal, which wrapped the Ⅸ nerve and Ⅹ and Ⅺ nerves, respectively. CT images could display these canals in 20 cases (100%). Furthermore, the Ⅸ, Ⅹ, and Ⅺ nerves could be identified on different MR sequences. 17 cases (85%) were displayed on 3D RF-FAST, 14 cases (70%) on SE T 1WI, and 10 cases (50%) on FSE T 2WI, respectively. From the anterior to the posterior compartment within the JF cavity, the structures ranged as follows: the Ⅸ nerve, the inferior petrosal sinus, the Ⅹ and Ⅺ nerves, and the jugular bulb. These neuro-vessel structures could not be distinguished on CT, SE T 1WI, and FSE T 2WI images, except for 3D RF-FAST (16 cases, 80%). The outer aperture of JF was adjacent to the hypoglossal canal. The shape of JF outer aperture could be evaluated on CT images. On the oblique plane, which was parallel to the hypoglossal canal, the posterior cranial nerves could be detected to enter/exit the skull through the JF and hypoglossal canal separately. Conclusion The complement of CT and MR imaging would be helpful to distinguish the structures in the region of JF.

OBJECTIVE To investigate and analyze hand carrier state of medical staff in Tianjin and the antibiotic resistance of the pathogens.METHODS Bacteria or pathogens were isolated from the samples using a sterilized cotton swab on the whole hand before hand washing or hand disinfection,and drug sensitivity test was performed.RESULTS In 16.72% of all samples(50/299),there were 64 kinds of bacteria or pathogens isolated.A.baumannii,Staphylococcus aureus and A.lwoffii were the most common bacteria,40.6% resistant to more then 3 kinds of drugs.Four Meticillin-resistant S.aureus(MRSA) strains were detected out.CONCLUSIONS The high rate of hand carrying bacteria and resistance in Tianjin should pay attention to.

Objective To evaluate the inhibitory activity of Herba Taraxaci extract on Escherichia coli DH5α (E. coli DH5α) and to investigate proteomic response of E. coli. Methods Medicinal powder of Herba Taraxaci was extracted with the solvents of different polarity ( n-hexane, ethyl acetate, distilled water) , and then the obtained 8 different extracts were subjected to thin layer chromatography ( TLC) analysis. Microdilution method was performed to detect the minimum inhibitory concentration ( MIC) of different extracts and the growth curves were described. The protein expression profiles of E . coli treated with the extracts were analyzed by sodium dodecyl sulfate polyacrylamide gel electropheresis ( SDS-PAGE) and two dimensional electrophoresis (2-DE) . Results Water decoction of Herba Taraxaci could obviously suppress the growth of E. coli with a MIC of 1.95 mg/mL. The different extractions exhibited no antibacterial activity except ethyl acetate phase 3 with a MIC of 0.13 mg/mL, which was equal to 19.23 mg/mL of crude drugs. The results of TLC analysis showed that chlorogenic acid was undetectable in n-hexane extract and ethyl acetate phase 1 extract, and ethyl acetate phase 2 and 3 extracts showed obviously increased spots. The results of SDS-PAGE and 2-DE showed that water decoction of Herba Taraxaci had inhibitory effect on the expression of functional protein. The results of 2-DE showed that after treatment with ethyl acetate phase 3 at the concentration of 2 × MIC for 21 hours, the amount of protein spots were 92 less than those of the blank control group, the spots of E. coli DH5α soluble protein with expression amount down-regulated doubly were 24, and those with expression amount up-regulated doubly were 19. Ethyl acetate phase 3 extract had an effect on down-regulating the protein expression of E. coli DH5α soluble protein pH3-10, and water decoction of Herba Taraxaci had inhibitory effect on E. coli DH5αprotein expression. Conclusion Herba Taraxaci has significant antibacterial activity on E. coli DH5α, and the water-soluble fraction of chlorogenic acid and caffeic acid might be the active components. The possible antibacterial mechanism may be related with the regulation of bacterial protein expression.

BACKGROUND; Authors have proposed the hypothesis that, the mechanism change may result in the mismatch between the energy production and energy consumption during the aerobic exercise, and pyruvate can be transformed into lactic acid, which may prevent the accumulation of pyruvate in cytoplasm and in the energy production of glycolysis so as to ensure the fast energy supply in zymolysis; the mechanism of this biochemical event may be the adjustment of energizing velocity via glycomechanism zymolysis.OBJECTIVE: To observe the effect of oxygen inhalation on metabolic transition, study the mechanism of metabolic transition under the lactate threshold intensity in human body and animal, and verify the result consistency between the two.DESIGN: Randomized control observation.SETTING: Department of Physical Education, Hebei Normal University; Department of Physical Education, Langfang Teachers College.PARTICIPANTS: A total of 24 male university students majoring physical education were adopted, weight (58±4) kg,height (175±6) cm, age (21 ±2) years. They were consisted of 12 Level B national athletes and12 common students.Additionally 30 SD male rats were used.METHODS: The experiment was carried out in the Laboratory of Physical and Physiological Function, Department of Physical Education in Hebei Normal University from April to June in 2006. Twenty-four students were recruited to exercise incrementally in ergometer; in addition, thirty SD rats were assigned to swim incrementally, 15 rats in each group. First, the intensities of metabolic transition were determined, then the exercise protocol was repeated on the conditions of inhaling and not inhaling oxygen. For student group, 50 W loading was incremented every 2 minutes, while the rats were added with 1% of their weights until unacceptable. Gradually incremented loading was used to transform the aerobic mechanism to anaerobic mechanism. The vein blood oxygen partial pressure, pyruvate and lactate contents were measured before and during the exercise (lactate threshold intensity) to evidence the reliability and validity of hypothesis.MAIN OUTCOME MEASURES: The vein blood oxygen partial pressure, pyruvate and lactate contents under lactate threshold intensity and oxygen inhaling supplementary.RESULTS: All 24 testees and 30 rats were involved in the result analysis. ①During the gradually incremented exercise,the lactic acid curve obtained at the end of 2-minute loading showed the difference of metabolic transition intensity and training level in accordance with individual lactic acid threshold, which was obviously lower in the trained exercisers.②Under the lactate threshold intensity, the blood lactate was not correlated to the oxygen partial pressure whether in human body or rats and whether inhaling oxygen or not [(3.61±0.56), (5.43±0.55) mmol/L; (4.46±0.86), (7.80±0.27) kPa,r =0.31, 0.31, P ＞ 0.05]; there was significant difference between the blood lactate and pyruvate contents [(1.04±0.16),(0.91±0.37) mmol/L, P ＜ 0.001]. The human body's saturation of blood oxygen was no less than 98% during the entire protocol. ③Under the repeated exercise and lactate threshold intensity, the pyruvate average value was (0.97±0.17),(1.04±0.16) mmol/L; (0.93±0.25), (0.91 ±0.37) mmol/L, respectively. There was no significant difference between the blood pyruvate before the exercise and under the lactate threshold intensity in both human body and animals (P ＞ 0.05).CONCLUSION: There is no hypoxia at the transition from aerobic to anaerobic metabolism. Oxygen inhaling supplementary has no influence on the mechanism transition; It is not easy for the pyruvate to pass the myocyte membrane, but the lactate can. The result demonstrates that the pyruvate can transform to lactate directly, which can also prevent the accumulation of pyruvate in kytoplasm.

Objective To investigate the iodine content of food in six provinces of China,to add the results of this survey to the food iodine content database,and to provide a scientific basis for iodine supplementation in different parts of China.Methods A total of 8 categories and 39 species common food produced locally in the six provinces of Fujian,Chongqing,Shandong,Anhui,Gansu and Jilin were collected.Samples of cereals,beans and other dry samples were crushed into powder; samples of fresh fruits and vegetables were washed and dried to constant weight,and crushed into powder; poultry,meat and fish samples were washed and then their edible parts were crushed into meat paste,bake dried to constant weight,and crushed into powder.Iodine content in the above-mentioned food was determined by catalytic spectrophotometry,and the wavelength was 405 nm.Data processing and statistical analysis were carried out by using SPSS 13.0 statistical software.The results of total iodine content of the various types of food were expressed as median(P50) and interquartile range(P25 and P75).Results The iodine content of the cereal in Fujian,Chongqing,Shandong,Anhui,Gansu and Jilin were 11.9,12.0,48.0,95.1,13.0and 3.1 μg/kg,respectively; of the potato were 53.9,26.3,74.9,43.7,76.8 and 38.5 μg/kg,respectively; of the meat and the eggs were 56.0,30.4,78.6,124.6,47.7 and 34.8 μg/kg,respectively; of the aquatic products were 319.3,144.7,186.6,241.3,155.4 and 213.3 μg/kg,respectively; of the vegetables were 166.6,145.1,131.7,218.0,205.4 and 98.1 μg/kg,respectively; of the fruits were 105.5,17.8,80.9,1.7,76.7 and 10.3 μg/kg,respectively; of the kelp and laver were 36.0 × 103,1292.0 × 103,2810.0 × 103,48.0 × 103,75.0 × 103 and 120.0 × 103 μg/kg,respectively; of the Chinese pickled vegetables were 640.4,4163.5,3073.7,2635.3,1540.9 and 492.0 μg/kg,respectively.ConclusionsThe iodine content of different types of food,and same kind of food from different provinces are different.The results are a complement to the 2004 Chinese food composition database.

Objective To study the mechanism of serum thyroxin and insulin change after hemihepatectomy. Methods The hemihepatectomy was performed in rabbits, the fasting peripheral blood samples were drawn on the day prior to surgery, and 24 hour, 48 hour, the first week, one month after operation respectively for monitoring the serum thyroxin and insulin.Results The level of TT 3 was (1.93?0.47) nmol/L, TT 4 (53.56?8.4) nmol/L preoperatively. The levels of TT 3 and TT 4 significantly decreased at 24 hour postoperatively, and gradually rose at 48 hour and up to normal levels in the first month postoperatively. The level of insulin rapidly rose immediately after operation, and began to decrease in the first week, and returned to the preoperative level in one month after operation. Conclusions The syndrome of normal thyroid disease and the syndrome of non thyroid disease can occur after hemihepatectomy; and hyperinsulinemia also can occur after hemihepatectomy.
