The visual acuity changes following strabismus surgery are generally short-lived and tend to disappear with time. However, some cases of persistent decreased vision is considered to be caused by changes of corneal curvature following strabismus surgery, resulting in increased corneal astigmatism. To investigate the amount and frequency of corneal astigmatiy changes following horizontal strabismus surgery, the authors periodically examined pre-and postoperative corneal curvature using keratometer in 56 eyes of 34 patients who had undergone horizontal strabismus surgery from March, 1987 to February, 1989 at Gyeongsang National University Hospital and Masan Koryo General Hospital. An average of 0.41D in horizontal curvature and an average of 0.05D in vertical curvature was decreased at 8 weeks after operation. The changes of +0.5D or more were present in 53.6% in horizontal curvature and 42.9% in vertical curvature. In comparison of one muscle operation with two muscle operation in one eye, both horizontal and vertical curvature showed no significant changes statistically(p>0.05).

PURPOSE: We evaluated the structure, innervation of the extraocular pulley smooth muscle in the rabbit. METHODS: A rabbit was transcardially perfused with physiological saline followed by 4% formalin. The rabbit orbit was dissected en bloc, paraffin processed, and serially sectioned for Hematoxylin-Eosin and Masson's trichrome stain. The distribution of the extraocular pulley smooth muscle was confirmed by immunohistochemistry with human smooth muscle(SM) alpha-actin. RESULTS: We found that the pulley structure consisted of focal condensations of eleastin and collagenous connective tissue in relationship to the rectus muscle in the equatorial Tenon's fascia. The pulley smooth muscle in relation to medial rectus muscle was immunostained with TH and nNOS. CONCLUSIONS: The data support the presence of the extraocular pulley smooth muscle in the rabbit orbit having both parasympathetic and sympathetic innervation.

Trabeculectomy, as a surgical intervention in the treatment of glaucoma, has become most popular. The most common cause of filtration failure is obstruction of aqueous outflow due to bleb fibrosis. Adjunctive intraoperative mitomycin-C has been reported to improve the success of filtration surgery in eyes of high risk for failure. But its use increases the rate of complications after filtering surgery: especially postoperative hypotony, wound leakage, and scleral necrosis. We have experienced two patients whose scleral flaps were necrotized after trabeculectomy with mitomycin-C in uncomplicated glaucoma. We report successful results in revision of filtering bleb by using Gore-Tex as a patch graft.

PURPOSE: To estimate and standardize horizontal ocular movement of normal Korean people and, to adapt correlations among three Methods for clinical approach. METHODS: This study investigated the amounts of horizontal ocular movement in normal Korean people whose age was ranged from 20 to 79 (50 persons in each age decades) by the lateral version light reflex Method, the limbus test of motility and the imaginary vertical line Method, and compared relationships among the amounts measured with above three Methods by Spearman correlation coefficient. The study subjects were three hundred people who visited Dept. of Ophthalmology at Gyeongsang National University Hospital, whose best corrected visual acuity was no less than 20/50 Snellen visual acuity chart, who had no history of ophthalmic operation and diseases, whose age was ranged from 20 years to 79 years (50 persons in each age decades). RESULTS: In imaginary vertical line Method, the excessive ocular movements were more frequent in younger people than in elder people. With age the proportion of the excessive ocular movement was gradually decreased and that of the normal ocular movement was increased until 6th decades. With other two Methods, the amounts of horizontal ocular movement was higher in younger ages than in elder ages and they decreased with aging. There were significantly moderate correlation (rs = - 0.54 to - 0.64) between age and ocular movement, and high correlation (rs = 0.79 to 0.86) among ocular amounts measured with imaginary vertical line Method, the lateral version light reflex test, and the limbus test of motility of Kestenbaum. CONCLUSIONS: This study suggests that any Method of three is reliable clinically.

The authors clinically analysed 317 industrial ocular injuries among 16,466 patients who visited department of ophthalmology of Masan Koryo General hospital during the 5 years from January 1, 1982 to December 31, 1986. The results were as follows: 1. The ambulation rate of industrial ocular injuries was 1.9% of all eye patients and 18.4% of all industrial injuries. 2. The incidence was higher in male(95.6%) and in the age of 3 rd decade(5.2%). 3. The incidence of affected side was 54.0% on the right, 37.5% on the left, and 8.5% on both sides. 4. There was no significant differences in seasonal distribution. 5. The most common cause of industrial ocular injuries was iron piece(32.8%) as a flying particle, followed by wire and nail(15.1%) and chemicals(8.2%). 6. Corneal foreign body(15.8%) was the most common disease of all industrial ocular injuries, followed by corneal laceration(12.7%) and thermal burn, conjunctiva and cornea(8.9%). 7. 29.3% of all industrial ocular injuries were hospitalized. Corneal laceration(21.0%) was the most common disease among in-patients and surgical procedures in in-patients included corneal suture(33.0%), lens extraction(17.5%), corneosclecral suture(13.6%). The average duration of hospitalization was 16.8 days. 8. The average duration of treatment required for all industrial ocular injuries was about 46.3 days. 9. The final visual acuity was improved to 0.9 or more in 45.3% and 34 eyes(9.9%) were below 0.1. 10. The most common complication of industrial ocular injuries after treatment was traumatic cataract(18.4%), followed by corneal opacity(16.8%) and irregular astigmatism(14.4%).

PURPOSE: To investigate the changes of the dopaminergic system in rat striatum after visual deprivation during a critical period of postnatal development. METHODS: The changes of dopamine D1 and D2 receptor (Rc) mRNA were investigated by using Northern blot analysis, in situ hybridization in the rat striatum. The right eyelid of visually deprivated rat was sutured at 10 postnatal days. After visual deprivation for 4 weeks, rats were sacrificed and striatum tissues were removed for analysis. RESULTS: By Northern blot analysis and in situ hybridization, decreased expression of D1 and D2 Rc mRNA was noted in the ipsilateral striatum to the deprivated eye. CONCLUSIONS: These results have shown that visual deprivation during a critical period of postnatal development influences the dopaminergic system in the striatum.

PURPOSE: The authers studied the effect of modified anterior transposition of the inferior oblique muscle for hypertropia in superior oblique muscle palsy combined inferior oblique muscle overaction. METHODS: We retrospectively analysed 19 cases of superor oblique palsy, which were treated by modified inferior oblique muscle anterior transposition from January 1999 to march 2001. Mean follow-up was 14.6 months. The medial portion of inferior oblique muscle was transpositioned to 1mm posterior position of the temporal insertion of inferior rectus muscle and lateral portion of inferior oblique muscle was moved 5mm on imaginary line, which is the line of between temporal insertion of inferior rectus and inferior insertion of lareral rectus muscle, to the direction of inferior insertion of lateral rectus muscle from temporal insertion of inferior rectus muscle, then backward 6mm from that point perpendicularly. RESULTS: The success rates in modified anterior transposition were 94% in under 20delta of hypertropia and 89.5% in over +3 of IOOA. CONCLUSIONS: Modified anterior transposition is an effective procedure of +3 to +4 inferior oblique muscle overaction and hypertropia in superior oblique muscle palsy as a primary surgery.

We compared the result of 6 patients[group A]treated with high dose steroid only with the result of 14 patients[group B]treated with optic canal decompression after at least 24 hours of high dose steroid therapy without improvement. 4 of 6 patients[66%]in group A and 7 of 14 patients[50%]in group B improved in visual acuity.But 1 of 6 patients[17%] in group A and 6 of 14 patients[43%]in group B had marked improvement in visual function[above 0.02].Of 5 patients with optic canal fracture in orbit CT in group B, all had improved visual function and 4 had marked improvement in visual function. If CT demonstrates bony fragments impinging on intracanalicular optic nerve, or if vision deteriorates or fails to improve during the first 24 hours of high dose steroid and initial visual acuity is no light perception, optic canal decompression is considered effective and valid treatment modality in TON.

To clarify neuronal connection of the medial rectus muscle at brain stem, Bartha strain of the psecudorabies virus(PRV-Ba) and cholera toxin-horeserdish peroxidase(CT-HRP) was injected into medial rectus muscle of the rat. About 84 hours after PRV or CT-HRP injection, the brain was removed and processed immunohistochemical stain for PRV-Ba and neurohistochemical stain for CT-HRP using tetramethyl benzidine. The CT-HRP positive reaction was only present in ipsilateral oculomotor nucleus, contralateral abducence nucleus and bilaterally in rostral interstitial nucleus of medial longitudinal fasciculus, olivary pretectal nucleus, medial and superior vestibular nucleus, nucleus prepositus hypoglossi, prerubral field, nucleus Darkschewitsch, lateral substantia nigra, terminal nucleus of accessory nucleus, dorsal raphenucleus, locus ceruleus and pontime reticular formation. This study confirmed that both oculomotor nucleus and abducence nucleus were connected with each other through the medial longitudinal fasciculus, and neuronal connection of the medial rectus muscle in the rat brain stem.

PURPOSE: To evaluate the pattern VEP in adult amblyopic patients seen in consultation for ophthalmic evaluation as a past of physical examinations for conscription. METHODS: We retrospectively analyzed, 67 men, 20-year-old or older, who had pattern VEP done for the diagnosis of amblyopia from January 2004 to May 2009. P100 latency and P100 amplitude were analyzed. RESULTS: Thirteen patients were non-amblyopic, and 54 patients had amblyopia. Binocular amblyopia and monocular amblyopia were found in 23 and 31 patients, respectively. In the binocular amblyopic patients, four patients were hyperopic, seven patients were myopic, and 12 patients were astigmatic amblyopia. In the monocular amblyopic patients, 15 patients were anisometropic, 12 patients were strabismic, and four patients had organic amblyopia. The value of P100 latency and P100 amplitude were statistically significantly different between non-amblyopic and amblyopic eyes, with check size of 32x32. However, the types of amblyopia among the patients were not different at a statistically significant level. Visual acuity and P100 amplitude were significantly positively correlated. CONCLUSIONS: VEP might be a useful tool for diagnosis of adult amblyopia, especially using a 32x32 check size. This tool may impart the ability to decide relationship between amblyopia and visual acuity by analyzing P100 latency and amplitude values.
