PURPOSE: This study was attempted to grasp the factors affecting the decision-making conflicts of preoperative cancer patients. METHODS: The subjects of this study were 110 cancer patients scheduled to undergo surgeries at K university hospital. Data were collected utilizing scales for decision-making conflicts and anxiety state, and measurement tools for attitude toward treatment and, shared decision-making. RESULTS: The level of decision-making conflicts over cancer treatment of preoperative cancer patients turned out to be slightly lower than the median. There were significant correlations between shared decision-making between physicians and patients, and quality of life which was a subdomain of attitude toward treatment. In multiple regression analysis, the most powerful predictor was the shared decision-making of physicians and patients. Overall, the explanatory power of the measured variables for decisionmaking conflicts about cancer therapy was 20%. CONCLUSION: The study highlights the importance of shared decision-making and quality of life which involves attitude toward treatment. Thereby, it is deemed to be necessary to develop intervention strategies regarding decision-making conflicts about cancer treatment in consideration of these variables.

Yonsei University College of Medicine, Seoul, Korea Maffucci's syndrome is characterized by dysccondroplasia with hemangiomata. Since Angelo Maffucci (1881) first described this entity, 105 cases and some more were reported in the literature till 1976. We recently observed a patient suffering from this disorder. The patient is 21 year-old girl, who has multiple hemangiomes on her right foot, right buttock, left wrist and enchondromas on both proximal phalanx, distal phalanx of both great toes and proximal, middle phalanx of right second toe. The disease started at age of 11 and these lesions grew rapidly last 3 years. The diagnosis was based on clinical, radiological and histological findings. Because of its rarity, we report it with brief review of literature.

The blood management act was revised on December 4, 2020, to enhance transfusion management and has been established in accordance with the department of transfusion management in the medical institution. The department of transfusion management is involved in education related to transfusion, transfusion monitoring, and ward rounding. The department of transfusion management is essential for promoting the stability and safety of transfusions. This paper aims to help the department of transfusion management operation by sharing the experience of the department of transfusion management.

We report a case of acute transient encephalopathy with mental alteration, myoclonic jerks, and periodic triphasic wave electroencephalographic patterns caused by a therapeutic dose of baclofen. The clinical and electroencephalo-graphic abnormalities improved to a normal range shortly after baclofen was discontinued. We discuss the pathogenesis and review the literature about baclofen-induced encephalopathies.

Intralymphatic histiocytosis (ILH) is a rare cutaneous condition with uncertain pathogenesis. It is characterized by dilated lymphatic vessels that contain histiocytes within their lumina. Although the etiology of ILH remains unknown, it has been associated with various inflammatory and neoplastic diseases, such as rheumatoid arthritis (RA), reaction to metal joint implants and Merkel cell carcinoma, breast cancer and colon cancer. An 83-year-old female presented with an erythematous patch on the left forearm that had appeared six months previous. She had suffered from osteoarthritis (OA) and the cutaneous lesion was located in the vicinity of the affected joint. Skin biopsy from the lesion showed dilated dermal vessels and some ectatic vessels that contained many mononuclear histiocytes. Based on the clinical and histopathological findings, we diagnosed her with ILH with OA. Two sessions of intralesional triamcinolone acetonide injection (5 mg/ml) were administered to treat the skin lesion, which gradually improved over a period of a few months. We here report a rare case of ILH associated with degenerative OA.

Objective: : Gamma Knife radiosurgery (GKRS) is an effective and noninvasive treatment for high-risk arteriovenous malformations (AVMs). Since differences in GKRS outcomes by nidus type are unknown, this study evaluated GKRS feasibility and safety in patients with brain AVMs. Methods: : This single-center retrospective study included patients with AVM who underwent GKRS between 2008 and 2021. Patients were divided into compact- and diffuse-type groups according to nidus characteristics. We excluded patients who performed GKRS and did not follow-up evaluation with magnetic resonance imaging or digital subtraction angiography within 36 months from the study. We used univariate and multivariate analyses to characterize associations of nidus type with obliteration rate and GKRS-related complications. Results: : We enrolled 154 patients (mean age, 32.14±17.17 years; mean post-GKRS follow-up, 52.10±33.67 months) of whom 131 (85.1%) had compact- and 23 (14.9%) diffuse-type nidus AVMs. Of all AVMs, 89 (57.8%) were unruptured, and 65 (42.2%) had ruptured. The mean Spetzler-Martin AVM grades were 2.03±0.95 and 3.39±1.23 for the compact- and diffuse-type groups, respectively (p<0.001). During the follow-up period, AVM-related hemorrhages occurred in four individuals (2.6%), three of whom had compact nidi. Substantial radiation-induced changes and cyst formation were observed in 21 (13.6%) and one patient (0.6%), respectively. The AVM complete obliteration rate was 46.1% across both groups. Post-GKRS complication and complete obliteration rates were not significantly different between nidus types. For diffuse-type nidus AVMs, larger AVM size and volume (p<0.001), lower radiation dose (p<0.001), eloquent area location (p=0.015), and higher Spetzler-Martin grade (p<0.001) were observed. Conclusion : GKRS is a safe and feasible treatment for brain AVMs characterized by both diffuse- and compact-type nidi.

OBJECTIVES: The purpose of this study was to investigate the prevalence of sleep problems in Korean elementary school children and the differences in sleep problems related to their school grade and gender. METHODS: One public elementary school in Dae-gu, Korea was randomly selected. And, parents who agreed to participate this study were asked to complete a modified version of Tucson Children's Assessment of Sleep Apnea screening questionnaire (TuCASA). RESULTS: The most common sleep problem was 'falling asleep in a vehicle (44.2%)' and the prevalence rate of this problem differed by school grade. The prevalence rate of falling asleep while watching TV before 8 P.M. (8.0%), daytime fatigue (21.9%) and nocturnal enuresis during the past 6 months (5.3%) were significantly differed by school grade. The prevalence rate of falling asleep while doing home work, falling asleep during a lesson, snoring and bruxism were 12.6%, 1.0%, 26.7% and 13.2% respectively. Although these rates did not show any differences between grades, snoring was more in boys and daytime fatigue was more in girls.. CONCLUSION: This study demonstrates that many elementary school children may have various sleep problems. Lower grade elementary schoolers had sleep problems which related to neuro-developmental factors, whereas higher grade elementary schoolers had sleep problems related to sleep deprivation and stress. Finally, more female schooler showed signs of fatigue or sleepiness than male schoolers.
