2.The Effect of Living with Spouse on Cognitive Decline and Depressive Symptom in Elderly People.

OBJECTIVE: The aim of this study was to determine whether living with spouse contributes to cognitive decline and depressive symptom among elderly people. METHODS: 939 participants were recruited through Keum-Cheon center for dementia in Seoul. All subjects were assessed using the Korean version of Mini-Mental Status Examination (MMSE-K), Korean version of Consortium to Establish a Registry for Alzheimer's disease (CERAD-K) and Geriatric Depression Scale (GDS). Furthermore, epidemiological factors (age, sex, education year, and characteristics of family) assessed to find correlation with cognitive decline. RESULTS: Controlling for gender, age, and education year, living with spouse was significantly associated with cognitive decline (B=-0.486, S.E=0.203, Wald=0.331, Exp (B)=1.107, p=0.01). Living with spouse group has lower depressive symptom, compared to living without spouse group (F=14.6, p<0.01). CONCLUSION: Correctly, our results suggest that spouse absent state may accelerate cognitive decline and depression. Further, elderly people living alone should be closely monitored for both depression and cognitive impairment.

3.Characterization of mesenchymal cells beneath cornification of the fetal epithelium and epidermis at the face: an immunohistochemical study using human fetal specimens.

Fetal development of the face involves a specific type of cornification in which keratinocytes provide a mass or plug to fill a cavity. The epithelial-mesenchymal interaction was likely to be different from that in the usual skin. We examined expression of intermediate filaments and other mesenchymal markers beneath cornification in the fetal face. Using sections from 5 mid-term human fetuses at 14–16 weeks, immunohistochemistry was conducted for cytokeratins (CK), vimentin, nestin, glial fibrilary acidic protein, desmin, CD34, CD68 and proliferating cell nuclear antigen (PCNA). Fetal zygomatic skin was composed of a thin stratum corneum and a stratum basale (CK5/6+, CK14+, and CK19+) and, as the intermediate layer, 2–3 layered large keratinocytes with nucleus. The basal layer was lined by mono-layered mesenchymal cells (CD34+ and nestin+). Some of basal cells were PCNA-positive. In the keratinocyte plug at the external ear and nose, most cell nuclei expressed PCNA, CK5/6, CK14, and CK19. Vimentin-positive mesenchymal cells migrated into the plug. The PCNA-positive nucleus as well as mesenchymal cell migration was not seen in the lip margin in spite of the thick keratinocyte layer. The lingual epithelium were characterized by the CK7-positive stratum corneum as well as the thick mesenchymal papilla. CD68-positive macrophages were absent in the epidermis/epithelium. Being different from usual cornification of the skin, loss of a mesenchymal monolayer as well as superficial migration of mesenchymal cells might connect with a specific differentiation of keratinocyte to provide a plug at the fetal nose and ear.

5.A Case of Very-late-onset Schizophrenia-like Psychosis.

This paper presents the case of a 67-year-old woman who visited the Psychiatry Department complaining of persecutory ideas and auditory hallucinations after a buccal cancer operation. On neuropsychological testing, she demonstrated paranoid psychosis and bizarre thoughts. Hospital admission was recommended for supportive care and treatment with antipsychotics. She was initially treated with olanzapine, but this medication had little effect and was replaced with amisulpride, which reduced the residual symptoms. The aim of this report was to discuss the diagnostic process and treatment of very late-onset schizophrenia-like psychosis.

6.Predictive Value of Dental Maturity for a Positive Gonadotropin-Releasing Hormone Stimulation Test Result in Girls with Precocious Puberty.

Dental maturity is associated with skeletal maturity, which is advanced in girls with central precocious puberty (CPP). We investigated the performance of dental maturity as a screening method for CPP using mandibular second premolar and molar calcification stages, assessed the associated anthropometric and laboratory factors, and evaluated pubertal response predictors using the gonadotropin-releasing hormone stimulation test (GnRHST) in prepubertal and pubertal girls. A prospective case-control study was conducted in girls, aged 7.0–8.9 years, classified into pubertal (peak luteinizing hormone [LH] after GnRHST ≥ 5 IU/L), prepubertal (peak LH < 5 IU/L), and control groups. Auxological and biochemical tests, panoramic radiographs, and GnRHSTs in participants with breast development were conducted. Dental maturity was assessed using the Demirjian index (DI). We included 103 girls (pubertal, 40; prepubertal, 19; control, 44). Chronological age (CA) was not significantly different between groups. Bone age (BA) and BA advancement was higher in the pubertal and prepubertal groups. Increased DI values at the mandibular second premolar and molar were significantly associated with CA, BA, BA advancement, height standard deviation score (SDS), peak LH after GnRHST, and insulin-like growth factor-I (IGF-I) (all P < 0.05). Moreover, odds ratio (OR) of the mandibular second premolar and molar (a DI value of ≥ E) for predicting a positive response to GnRHST was 8.7 (95% confidence intervals [CI], 2.9–26.1) and 5.2 (95% CI, 2.2–12.7), respectively. Dental maturity was a strong predictor for diagnosing CPP. Determining dental maturity in girls with suspected precocious puberty might help determine the performance of GnRHSTs.

7.A Case of Melanonychia Caused by Trichophyton rubrum.

We report a case of fungal melanonychia caused by Trichophyton (T.) rubrum in a 69-year-old female patient. She had brownish to black pigmentation affecting all toe nails that clinically simulated melanonychia. Mycologic examination of toe nails revealed T. rubrum. The causes of melanonychia are numerous, but fungal origin is rare. Although T. rubrum is known to be one of the frequently isolated organisms of fungal melanonychia, there have been no reports of T. rubrum-induced melanonychia yet in Korea.

8.A Study about the Skin and General Disease Pattern of the Vietnam Veterans Exposed to Dioxin.

BACKGROUND: Vietnam veterans have suffered from a number of skin and general diseases. But in Korea, there are few reports about the relationship between dioxin and skin disease and dermatologists need more information about it. OBJECTIVE: The purpose of this study was to evaluate the skin and general disease pattern of Koreans who have a history of exposure to dioxin in Vietnam. METHODS: 332 Vietnam veterans who visited St. Mary's hospital between July, 2001 and September, 2001. We perfomed a clinical evaluation, physical examination, medical history, serum and urine chemistry, electrocardiogram, electrodiagnostic study, and a roentgenographic study. For dermatologic evaluation, pathologic examination, KOH mount, fungus culture, and phototest were performed. RESULTS: 1. Among the 332 participating soldiers in the Vietnam war, the mean age of patient was 56.9 years old (the youngest-53 years, the oldest-61 years old). 2. The prevalence of hypertension, hyperlipidemia, diabetes mellitus, liver disease, and peripheral neuropathy of subjects was 12.0%, 6.9%, 5.7%, 4.5%, and 1.2%. 3. The prevalence of xerotic eczema, seborrheic dermatitis, psoriasis vulgaris, photoallergic dermatitis, and chronic urticaria of subjects was 3.9%, 3.9%, 0.9%, 0.6%, and 0.3%. CONCLUSION: The prevalence of general diseases was not significant between Vietnam veterans and controls.

9.Smart Stress Care: Usability, Feasibility and Preliminary Efficacy of Fully Automated Stress Management Application for Employees.

OBJECTIVE: We developed a fully automated smartphone-based stress management application and explored its usability, potential feasibility, and preliminary efficacy for stress management in Korean employees. METHODS: Healthy employees working in large public enterprises were enrolled. Participants used our automated stress management application for four weeks. With the application, they monitored their stress level and life style factors. Personalized stress management techniques, including psychoeducation and cognitive behavioral technique, were also provided based on their stress level and lifestyle pattern. In 2014, additional relaxation techniques were incorporated. Participants’ mental health status and lifestyle pattern were self-assessed at baseline and at 4 weeks after using the application. RESULTS: A total of 68 subjects were recruited. The application generally received high satisfaction rating. After the intervention, perceived stress level was significantly decreased, both in 2013 and 2014 (BEPSI-K score pre. vs. post. 14.27 vs. 11.00, F=12.49, p=0.001 in 2013; 12.05 vs. 10.00, F=17.18, p < 0.001). In 2014, depression symptom severity was also significantly decreased (CES-D score pre- vs. Post-, 17.66 vs. 11.95, F=9.76, p=0.004). The effects were more significant in females and in those < 35 years. CONCLUSION: Our fully automated stress management application is acceptable and usable, showing preliminary efficacy for reducing employees’ stress levels.

10.Reliability of Diastolic Flow Velocity of the Left Pulmonary Artery for the Diagnosis of Patent Ductus Arteriosus in Preterm Infants.

PURPOSE: Diagnosis of a hemodynamically significant patent ductus arteriosus (PDA) that should be treated is difficult to determine by clinical and echocardiographic examination. The purpose of this study is to clarify the usefulness of diastolic flow velocity (DFV) of the left pulmonary artery (LPA) determined by echocardiography in the assessment of significant PDA in preterm infants. METHODS: Clinical and echocardiographic findings, including DFV in LPA, of PDA were evaluated at 24 hours, 48-72 hours of age and after indomethacin treatment in thirty-nine infants ranging from 25 to 34 gestational weeks of age. DFVs of the study group (N=13) with significant PDA were compared with those of healthy control group (N=26) without significant PDA. RESULTS: DFVs in healthy preterm infants were high in the first few days and were significantly decreased after spontaneous ductal closure. DFVs in preterm infants with significant PDA who underwent indomethacin treatment were significantly higher than that of healthy control infants. After indomethacin treatment, DFVs in this study group remained high with continuing significant PDA and markedly decreased with disappearance of significant PDA. Until ductus arteriosus closed, DFVs showed a significant correlation with the magnitudes of ductal shunt. A cutoff value for DFV of 30 cm/sec showed a sensitivity of 77% and a specificity of 92% as a predictor of significnat PDA in preterm infants. CONCLUSION: Measurement of DFV in LPA by echocardiography is a useful method for assessing the significnat PDA which may require treatment in preterm infants.
