2.A Case of Oligoarthritis Associated with Gastric Adenocarcinoma.

Malignancy may cause variable musculoskeletal symptoms which resemble connective tissue disorder. This symptoms as a fine sign of hidden neoplasm can precede the diagnosis of underlying malignancy. But only one case of stomach cancer associated with cancer arthritis was reported in the world. This fact is very interesting in Asia which has high prevalence of stomach cancer. We experienced one case of gastric adenocarcinoma with oligoarthritis involving both knee and right elbow. A 48-year-old man was diagnosed stomach cancer by gastric endoscope and biopsy. After then, he complained pain, tenderness, heating sensation, swelling of both knee and right elbow with high fever. Both knee MRI showed abundant synovial effusion and soft tissue edema. Full dose of nonsteroidal anti-inflammatory drug was not effective to control fever and arthritis. But fever and arthritis were gone after resection of tumor.

3.Comparative study of surface roughness between several finishing and polishing procedures on ormocer-based composite resin and nanohybrid composite resin.

STATEMENT OF PROBLEM: Proper finishing and polishing enhance both the esthetics and the longevity of restored teeth. Blade finishing technique would be suited for smoothing and finishing. Evaluation of this technique are necessary. PURPOSE: The purpose of this study was to evaluate the blade finishing and polishing procedures on the surface profile and roughness of ormocer-based composite resin and nanohybrid composite resin. MATERIAL AND METHODS: The material included a ormocer-based composite resin (Admira(R) & Admira(R) Flow); a nanohybrid composite resin (Grandio(R) & Grandio(R) Flow). One hundred forty specimens of each group were prepared using a mylar strip and randomly divied into blade finishing and rubber polishing groups (n=10). The average surface roughness (Ra) in micrometers was measured and the surface profile was examined by scanning electron microscopy (SEM) (Magnification X 200). The data were analyzed by Mann-Whitney Test at 0.05 significance level. CONCLUSION: The results of this study indicated that the mylar strip produced the smoothest surface on all materials and among the finishing-polishing methods was not significanct difference (P>0.05). Ormocer-based flowable composite resin performedthe lowest variability in initial surface roughness among the tested materials.

4.A Case of Unilaterally Involved Glomerulocystic Kidney Disease.

Metastatic tumors occur more frequently in the liver than in any other organ, Guided percutaneous fine-needle aspiration (FNA) of the liver is often recommended for confirmative diagnosis of the metastatic lesion, because of its simplicity, high yield, and reasonable safety. The authors studied retrospectively cytologic findings of 110 cases of metastatic tumors to the liver. The frequent primary sites were the stomach (23 cases), pancreas (19 cases), gallbladder (12 cases), and periampullary lesions (6 cases). Most of the metastases were carcinoma (106 cases). There were only 4 cases of sarcoma. The characteristic cytologic findings of FNA of meatastatic tumors were dirty background, abrupt change between hepatocytes and malignant cells, and desmoplasia. Some tumors displayed rather distinctive cytologic appearance that suggests primary sites. For example, the colonic adenocarcinoma showed tall columnar cells with a palisading arrangement, adenocarcinoma of gallbaldder showed focal squamous differentiation in some cases, and metastatic renal cell carcinoma and neuroblastoma showed also distinctive cytologic findings. Because the cytologic features of metastatic tumor are very similar to those of primary tumor, correct cytologic typing may be helpful in pursuit of an occult primary site of metastatic liver lesions, reducing extensive diagnostic investigation in poor prognostic patients.

5.Recent advances in the reconstruction of cranio-maxillofacial defects using computer-aided design/computer-aided manufacturing

With the development of computer-aided design/computer-aided manufacturing (CAD/CAM) technology, it has been possible to reconstruct the cranio-maxillofacial defect with more accurate preoperative planning, precise patient-specific implants (PSIs), and shorter operation times. The manufacturing processes include subtractive manufacturing and additive manufacturing and should be selected in consideration of the material type, available technology, post-processing, accuracy, lead time, properties, and surface quality. Materials such as titanium, polyethylene, polyetheretherketone (PEEK), hydroxyapatite (HA), poly-DL-lactic acid (PDLLA), polylactide-co-glycolide acid (PLGA), and calcium phosphate are used. Design methods for the reconstruction of cranio-maxillofacial defects include the use of a pre-operative model printed with pre-operative data, printing a cutting guide or template after virtual surgery, a model after virtual surgery printed with reconstructed data using a mirror image, and manufacturing PSIs by directly obtaining PSI data after reconstruction using a mirror image. By selecting the appropriate design method, manufacturing process, and implant material according to the case, it is possible to obtain a more accurate surgical procedure, reduced operation time, the prevention of various complications that can occur using the traditional method, and predictive results compared to the traditional method.

6.Position Value for Relative Comparison of Healthcare Status of Korea in 2017

The purpose of this study was to compare the health status of South Korea with those of Organization for Economic Cooperation and Development (OECD) countries and examine the trends. Position vAlue for Relative Comparison (PARC) was used as a gauge for comparison, and five sectors of the health care system were measured: demand, supply, accessibility, quality, and cost. The Mann-Kendall test was used as a statistical analysis method to examine trend of PARC values obtained from 2000 to recent years. According to the results, the demand, supply, accessibility, and quality sectors were higher than the OECD average, while the cost was lower than the average. However, there is a recent trend of sharp increases in health care costs. Some indicators: health employment, quality of primary care and mental health care were lower than the OECD average, and health determinants showed a worsening trend. Therefore, policy-makers need to take this into account and make efforts for sustainable health care.
7.Two Cases of Hydrothorax Associated with Pleuroperitoneal Communications Proved by CT Peritoneography and Peritoneoscintigraphy.

With the increasing use of PD as a replacement therapy in chronic renal failure, the number of complications related to PD also has increased. One of these is hydrothorax associated with pleuroperitoneal communications. Diagnosis is based upon confirming that the fluid in pleural space is dialysate. The chemical characteristics of the pleural fluid can help diagnose dialysate in pleural space. One can use dialysate mixed with dye (methylene blue, indigo). Peritoneographies with contrast and radionucleotides are safe, accurate, and reliable methods of diagnosing peritoneal defects. While these substances do not damage the peritoneum, their diffusion is low. We report 2 cases of hydrothorax associated with pleuroperitoneal communications in CAPD patients. We employed CT peritoneography and peritoneoscintigraphy to diagnose the hydrothorax associated with pleuroperitoneal leak.

8.The Association between Hand Grip Strength and Health-Related Quality of Life in Korean Adults.

We investigated the association between hand grip strength (HGS) and health-related quality of life (HRQOL). A total number of 9,211 participants from the sixth Korean National Health and Nutrition Examination Survey (2014– 2015) were included. HRQOL was measured by EuroQol-5 dimension (EQ-5D) and EQ-5D index score. Participants were classified as normal HRQOL group or impaired HRQOL group according to the EQ-5D responses and the EQ-5D index scores. HGS was measured using Takei dynamometer. Impaired HRQOL group represented lower mean HGS than that of normal HRQOL group. According to the quintile groups of HGS, group with the lowest HGS showed the lowest mean EQ-5D index score in both sexes. And the more HGS increases, the more EQ-5D index score increases also (p-value for trend <0.001). Men and women with lower grip strength were more likely to report ‘some’ or ‘extreme problem’ in EQ-5D domains (except anxiety/depression and pain/discomfort domain in men), and also more likely to belong to the lowest 20% level group in EQ-5D index score according to logistic analyses. Our findings suggest that HGS is associated HRQOL. Low HGS is associated with impaired status of HRQOL by EQ-5D and EQ-5D index score in Korean men and women. More research is needed to evaluate the specific plausibility between HGS and HRQOL.

9.The Association between Sitting Time and Health-Related Quality of Life According to Body Mass Index in Elderly Korean.

BACKGROUND: Although obesity and health-related quality of life (HRQOL) in elderly are well known to be associated with obesity and sitting time, it is unclear whether effect of sedentary lifestyle on HRQOL is affected by body mass index (BMI) or not. So we analyzed the relationship between sitting time and HRQOL according to BMI groups in elderly Korean. METHODS: Participants aged over 60 from the 6th Korea National Health and Nutrition Examination Survey (2014 and 2015) were included. Participants were classified as normal weight and overweight/obese groups. Mean sitting times were compared between groups according to the EuroQol 5 dimension (EQ-5D) And logistic regression analyses were performed. RESULTS: Men with mobility, usual activity, or pain/discomfort domain problem had longer sitting time. Women with a problem in every domain had longer sitting time. Overweight/obese women with problem showed longer sitting time. Odds ratios (ORs) of mobility, usual activity, pain or discomfort, and low EQ-5D score domain were increased regardless of BMI groups in men. But, ORs of all domains were increased only in overweight/obese group in women. CONCLUSIONS: In elderly Korean, prolonged sitting time associated with decreased HRQOL. Impaired HRQOL is associated with increased sitting time regardless of BMI in men. But only overweight/obese group showed association between prolonged sitting time and impaired HRQOL in women. These results represents that decrease in quality of life according to the increase of the sitting time differs according to the BMI in elderly Korean women.

10.Effect of ketamine and midazolam on oculocardiac reflex in pediatric strabismus surgery.

BACKGROUND: The oculocardiac reflex (OCR) can be elicited during manipulation of the orbital structures in the strabismus correction surgery. A sinus bradycardia is the most common manifestation of OCR; and cardiac dysrhythmia and asystole may also occur. Various efforts to reduce OCR have been attempted, but without coherent outcome results. METHODS: Sixty one children, undergoing elective strabismus surgery, were randomly allocated into 2 groups: Group K received ketamine 1.0 mg/kg; and Group M received midazolam 0.15 mg/kg for induction of anesthesia. Anesthesia was maintained with 1-1.3 MAC of sevoflurane with 50% N2O in O2. Heart rate and blood pressure were measured 30 seconds before extraocular muscle (EOM) traction and immediately after traction. The OCR was defined as a decrease in heart rate more than 20% of the baseline heart rate, following manipulating EOM. Postoperative nausea and vomiting (PONV) and emergence agitation (EA) were assessed in postanesthetic care unit (PACU). RESULTS: Blood pressure before tightening EOM in Group K was higher than that in Group M (P < 0.05). However Delta HR (2.7 +/- 15% vs. - 0.9 +/- 16%) and incidence of OCR (10.0% vs. 19.4%) after traction an EOM were not different between the two groups. The occurrence of PONV (6.7 vs. 9.7%) and EA (30.0% vs. 22.6%) were similar. CONCLUSIONS: Ketamine does not reduce the incidence of OCR compared with midazolam in pediatric strabismus surgery. In addition, ketamine does not increase the incidence of PONV and EA. In conclusion, it is reliable to use ketamine in pediatric strabismus surgery.
