1.A case of ruptured omental pregnancy.

Abdominal pregnancy is a rare form of ectopic pregnancy, which has been classified as early and advanced abdominal pregnancies. In case of early abdominal pregnancy, we may miss the diagnosis because the symptoms and clinical characteristics are non-specific. However, recently, the early diagnosis is made because of the development of quantitative beta-hCG measurement, transvaginal ultrasonography and the development of laparoscopy. Omental pregnancy is a very rare form of abdominal pregnancy, which is hard to detect early. When ruptured, it accompanies with massive hemorrhage. So, early diagnosis and treatment is essential to prevent high morbidity, mortality and serious complications. Definite diagnosis is made by cytopathologic examination of tissue specimen after surgery. We have experienced a case of ruptured early omental pregnancy and reviewed it briefly.

2.CT Findings of Tumor and Mature Cystic Teratoma.

PURPOSE: A tumor and mature ovarian cystic teratoma rarely coexist, but since it may affect treatment andprognosis, appropriate pre-surgical diagnosis is important. The purpose of this study was to evaluate thedifference in CT findings between a tumor coexisting with a mature ovarian cystic teratoma and a simple matureovarian cystic teratoma. MATERIALS AND METHODS: CT scans of seven cases of tumor coexisting with mature ovariancystic teratoma were reviewed. In each case, size, margin, nature, septation, solid portion, ascites, invasion,and metastasis were analysed. RESULTS: Coexistent tumors were histopathologically diagnosed as squamous cellcarcinoma in three patients, carcinoid in three, and fibrothecoma in one. In contrast with a simple matureteratoma, a tumor coexisting with a mature cystic teratoma developed in older patients and had a more solidportion, which showed contrast enhancement but did not show calcification or a fat component. Where a tumor andmature cystic teratoma coexisted, ascites and the invasion of adjacent structures were also common. CONCLUSION: If, in an older patient, CT scanning reveals an ovarian cystic tumor with a large solid portion but nocalcification or fat, coexistent tumor should be suspected.

3.A case of primary ovarian pregnancy.

Ovarian pregnancy is comparatively rare form of ectopic pregnancy. Although earlier diagnosis is now possible due to the availability of quantitative beta-hCG measurement and the development of transvaginal ultrasonograghy, it is mostly difficult to diagnosis before surgery, and frequently misdiagnosed as a ruptured corpus luteum accompanied with massive hemoperitoneum. Definite diagnosis is made only by cytopathologic examination of tissue specimen. The treatment of ovarian pregnancy has been operative management including oophorectomy, salpingo-oophorectomy and ovarian wedge resection. But recently conservative management using laparoscopic technique has become the preferred treatment. We have experienced a case of primary ovarian pregnancy and reviewed it briefly.

4.A Comparative Study on the Treatment of Scaphoid Nonunion with Matti-Russe Technique and Fisk-Fernandez Technique.

PURPOSE: Established scaphoid nonunion may lead to carpal instability and osteoarthritis. The purpose of this study is to compare the clinical and radiological results of the osteosynthesis between Matti-Russe group and Fisk-Fernandez group. MATERIALS AND METHODS: Our retrospective study included 21 nonunion cases in 20 patients treated by Matti-Russe or Fisk-Fernandez technique from September 2001 to August 2006. The mean duration of follow up was 31 months. Based on the questionnaire which included clinical information and the radiologic evaluation, we compared the objective and subjective results between two surgical groups. RESULTS: There were no significant differences between two groups in respect of the postoperative satisfaction and the pain. Both the range of motion and the grip power decreased compared to that of the unaffected side, but there were no significant differences between two groups. Also, there were no significant differences in the union rate and the rate of successful correction of DISI deformity. In all cases in which DISI deformity was detected preoperatively, the deformity was corrected postoperatively. There were 7 cases of osteoarthritis, but among them only one case appeared newly after the surgery. CONCLUSION: There were no significant differences between two surgical groups in clinical and radiological results. Both techniques were effective in achieving bony union and functional recovery.

5.The effect of high concentration of magnesium with ropivacaine, gentamicin, rocuronium, and their combination on neuromuscular blockade.

BACKGROUND: Magnesium, ropivacaine, gentamicin, and rocuronium block neuromuscular (NM) transmission by different mechanisms. Therefore, concurrent administration of these agents may induce prolonged muscle paralysis via synergistic interaction. This study investigated the efficacy and safety of NM block caused by the administration of high concentrations of magnesium in combination with ropivacaine, gentamicin, and rocuronium. METHODS: Eighty-three left phrenic nerve-hemidiaphragms from male SD rats (150-250 g) were hung in Krebs solution. Three consecutive single twitch tension (ST, 0.1 Hz) and one tetanic tension (TT, 50 Hz for 1.9 s) were obtained before drug application and at each new drug concentration. The concentration of MgCl2 and MgSO4 in Krebs solution was increased until an 80 to 90% reduction in ST was reached. To test the effects of combinations of NM agents, a Krebs solution was premixed with MgCl2 alone, MgCl2 and ropivacaine, or MgCl2, ropivacaine, and gentamicin. The concentration of ropivacaine, gentamicin, or rocuronium was then progressively increased until an 80 to 90% reduction in ST was reached. The effective concentrations were estimated with a probit model. RESULTS: The potency of MgCl2 was greater than that of MgSO4, and pretreatment with MgCl2 increased the potency of gentamicin and rocuronium. Unexpectedly, MgCl2 did not potentiate ropivacaine, and the potency of gentamicin and rocuronium failed to show an increase when premixed with 0.5 microM ropivacaine. CONCLUSIONS: The concomitant administration of high concentrations of magnesium and ropivacaine together with clinically relevant concentrations of gentamicin or rocuronium potentiated NM blockade but not with clinically relevant concentrations of ropivacaine.

6.Early Results of Hemicondylar Hamate Replacement Arthroplasty in Proximal Interphalangeal Fracture-Dislocation Injury.

PURPOSE: We assessed early clinical and radiological results of hemicondylar hamate replacement arthroplasty in fracture-dislocation injury of the proximal interphalangeal joint. MATERIALS AND METHODS: From March 2007 to February 2008, four patients who had suffered from comminuted unstable fracture-dislocation of the proximal interphalangeal joint were treated by hemicondylar hamate replacement arthroplasty. All patients were male, and average age at operation was 21.5(range, 14~27) years. All cases showed subluxation of the proximal interphalangeal joint, and mean articular surface involvement was 50.8(range, 42.1~62.5)%. We clinically measured ROM, grip strength, pain of donor & recipient site, and radiologically assessed whether or not union, malalignment, or step-off occurred. RESULTS: Average follow-up period was 9.5(range, 6~14) months. Mean flexion contracture was 12.5 (range, 10~15)degrees in affected side and 0degrees in unaffected side. Mean further flexion was 92.5 (range, 85~100)degrees in affected side and 95.0 (range, 90~100)degrees in unaffected side. Mean VAS pain scores was 1.75 (range, 1~3) in recipient site and 1.25 (range, 0~2) in donor site. Mean grip strength was 11 (range, 8~15) lb in affected side and 14 (range, 10~17) lb in unaffected side. Union without malalignment was achieved in all cases. CONCLUSIONS: Early results of hemicondylar hamate replacement arthroplasty in the treatment of fracture-dislocation of the proximal interphalangeal joint showed satisfactory functional and radiological outcome.

7.A case of jejunal primitive neuroectodermal tumor in an adult female.

Primitive neuroectodermal tumor (PNET) is frequent in children and adolescents, but rare in adults. Most of the extraosseous Ewing's sarcoma or PNET occur in the soft tissues of the extremities, the paravertebral region, and the pelvic cavity. PNET in the gastrointestinal tract is uncommon. We report herein a case of PNET arising from the jejunum in a 38-year-old woman. She presented with anorexia, nausea, vomiting, and weight loss. Abdominal computed tomography revealed a mass in the small bowel with obstruction. She had a small bowel segmental resection, from which PNET was diagnosed. Multi-agent chemotherapy comprised of vincristine, adriamycin, cyclophophamide, ifosfamide, and etoposide (VAC/IE) was administered for 1 year. The treatment was well-tolerated. She remains alive and continues to be disease free 30 months postoperatively.

8.Mucinous and Nonmucinous Type of Bronchioloal veolar Carcinoma: Difference in CT Findings.

PURPOSE: To search for CT findings which helpfully differentiate mucinous from nonmucinousbronchi-oloalveolar carcinoma and to assess the difference in stages between the two types of tumors. Twenty-two patients with pathologically proven bronchioloalveolar carcinoma (BAC) were included inthis study. On the basis of CT findings, tumors were classified as either solitary or multiple and as eithermass/nodule, consolidation, or mixed type. CT stages of the tumors were determined by two radiologists andconclusions were reached by consensus. RESULTS: Twelve patients had nonmucinous BACs and ten had mucinous BACs.Among the ten cases of mucinous BAC, six were solitary and four were multiple. These were mass/nodule (n=3),consolidation (n=5), and mixed pattern (n=2). In contrast, among the twelve cases of nonmucinous BAC, six weresolitary and six were multiple. All were mass/nodule, except for one mixed type. Among the mucinous BACs, threewere operable and seven (above stage IIIa) were inoperable. Among the nonmucinous BACs, four were operable andeight were inoperable. CONCLUSION: Consolidation was more common in mucinous BAC and mass/nodule was more commonin non-mucinous BAC (p<0.05). There was no difference in tumor stages between mucinous and nonmucinous types ofBAC (p>0.05).

9.Changes in Tongue Position, Airway Width, Gonial Angle, Lower Facial Height after Bilateral Sagittal Split Ramus Osteotomy in Mandibular Prognathic Patients.

INTRODUCTION: In patients with mandibular prognathism, Bilateral Sagittal Split Ramus Osteotomy(BSSRO) combined with orthodontic treatment reduces oral volume and influences tongue and other surrounding tissues. Purpose of this study was to analyze post-operative tongue position and airway dimension, as well as mandibular changes in vertical, horizontal, and angular dimensions. MATERIALS AND METHODS: Height of dorsum of tongue, width of airway, gonial angle and lower facial height of mandibular prognathic patients who visited Kangdong Sacred Heart Hospital from Jan. 2001 to Dec. 2006 were anaylzed via pre-operative and post-operative cephalograms. T-test was used to compare pre-operative and post-operative measurements. Also, correlations among pre-operative measurements of the patients were analyzed. RESULTS AND CONCLUSION: A significant correlation was shown between ANS-Xi-PM area and location of dorsum of tongue in pre-operative patients. A significant superior movement of tongue and decrease of airway width was observed in post-operative patients. Also the upper gonial angle decreased significantly.

10.Comparison of Hypoxanthine-Guanine Phosphoribosyltransferase Activity between Normal and Gout Patients.

OBJECTIVE: It has been known that the enzyme deficiency associated with uric acid production is the minor cause of gout. The purpose of this study is to evaluate the role of hypoxanthine- guanine phosphoribosyltransferase (HGPRT) activity deficiency in the development of gout. METHODS: Assay of HGPRT activity was performed on 38 gout patients and 107 controls compromising 82 men and 25 women. In the patients with gout, age of onset, sex, family history of gout, duration of disease, serum uric acid level, and uric acid concentration of 24-hour urine were analyzed. RESULTS: In gout patients and control male group, the HGPRT activity was lowest in the age of forties. But, in female, there was no decreasing tendency with aging. In 4 gout patients, HGPRT activities were severely deficient and their ages of onset were less than 30 years old, and all have a family history of gout. CONCLUSION: In men, the lowest HGPRT activity in the age of forties could be one of the factors that make the onset of gout is highest in the fifth decade of age. The deficiency of HGPRT enzyme and family history of gout are the important factors in the pathogenesis of early onset gout.
