1.A Case of Bart's Syndrome.

Bart's syndrome was initially described as a genodermatosis characterized by congenital localized absence of the skin with blistering and nail deformities 1-3. However, it is considered as any type of epidermolysis bullosa(EB) with localized congenital absence of the skin on the extremities. A 33-day-old fbmale baby was presented with congenital absence of the skin over the left shin and dorsa of both feet which were covered with the thin, translucent, and brown-red glistening membranes. Blistering of the right calf and left great toe nail deformity were also noted. She was diagnosed as a recessive dystrophic EB by the histopathological, ultra- structural and immunomapping studies.

2.Treatment for Pseudocyst of the Auricle with Intralesional Injection of Minocycline: A Report of One Case.

Pseudocyst of the auricle is a disease of degenerative cartilage associated with asymptomatic swelling of the external ear caused by an intracartilaginous accumulation of serous fluid. Histological examination shows an intracartilaginous cavity without an epithelial lining. This disease is characterized by frequent relapses despite various therapeutic approaches. In this report we describe one patient with pseudocyst of the auricle who was successfully treated with intralesional injection of 1 mg/mL of minocycline hydrochloride.

3.Two Cases of Herpetic Syringitis.

The histology of herpes virus infection of the skin is characterized as an intraepidermal vesicle formed as the result of acantholysis and ballooning degeneration of kerationocytes. Dermal changes is limited to polymorphous inflammatory cells infiltrate, leukocytoclastic vasculitis, and herpetic folliculitis. Involvement of eccrine epithelium is very rare finding. We report two cases of herpes zoster with prominent viral infection of the eccrine sweat glands.

4.A Case of Adult T-cell leukemia / lymphoma.

Adult T-cell leukemia/lymphoma (ATLL) is an often aggressive and fatal malignancy of mature CD4+ T lymphocytes, characterized by leukemia, lymphadenopathy, hypercalcemia and lytic bone lesions. Diagnosis of ATLL is based cm abnormal T lymphocytes in the peripheral blood and histologically and cytologically proven lymphoid malignancy with T cell surface antigens. Anti-HTLV-I serum antibodies and demonstration of clonality to HTLV-I proviral DNA are included. We report a case of ATLL in a 64-year-old man who had a painful nodule on the right cheek and multiple scattered papules and nodules on the trunk for 2 months. He had multiple cervical and axillary lymph node enlargements and hepatosplenomegaly. On laboratory finding peripheral leukocytes count was 33,400/mm3 with 63% atypical lymphocytes. Clover-shaped lyrnphocytes with multilobulated nuclei were found in peripheral blood and bone marrow. Histologic findings showed dense bandlike infiltrations of atypical lymphocytes in the dermis with angiocentricity. The surface phenotype of these abnormal cells were CD2+, CD3+, CD4+, CD30+ and HLA-DR+ and HTLV-I specific antibodies were found.

5.A satisfaction survey of toxicological laboratory: Survey of regional and local emergency medical centers

Purpose:
The purpose of this study is to find out the current status of toxicology laboratory operated by six locations nationwide and to investigate the satisfaction of emergency medical professionals who working at local and regional emergency medical centers.
Methods:
This survey was conducted prospective. It was conducted on 665 emergency medical professionals working at regional and regional emergency medical centers across the South Korea. Among them, the analysis was conducted with data that 510 emergency medical professionals who respond to this survey. The questionnaire was conducted on an online basis for a month. To ensure statistical significance, consider a dropout rate of 10% based on a minimum response recovery rate of 70%. 506 people were selected for the survey.
Results:
According to a survey on the status of addiction analysis room usage, the average monthly usage of addiction test rooms among respondents were 406 cases.71.0 cases (17.4%) of toxicology laboratory in Seoul and 71 cases (17.4%) in Gwangju. 32 cases (7.8%), 118 cases (29.0%) requested by toxicology laboratory in Busan, and the toxicology laboratory in Daegu. Eighty two cases (20.1%), Daejeon area 25 cases (6.1%), Wonju area toxicology laboratory was 78 (19.6%). According to a survey on the satisfaction of the addiction analysis room,Seoul (4.9±2.71) and Gwangju (4.8±2.52) showed high satisfaction.
Conclusion
Due to the limited operation time of the four addiction analysis rooms currently in operation, the satisfaction level of addiction analysis by emergency medical professionals in the area is low due to the delay until the result is notified.
6.The Effect of Different Oxygen Flow Rates on Arterial Oxygenation and End-Tidal CO2 Measurements via a Nasal Cannula in Spinal Anesthesia.

BACKGROUND: The monitoring of end-tidal carbon dioxide tension (PETCO2) and oxygen supply may be required in spontaneously breathing patients during spinal anesthesia, particulary in cases involving high spinal block, underlying pulmonary disease, and use of a sedative drug. We investigated changes in PETCO2 and arterial oxygen tension versus oxygen flow rate via a nasal cannula, and the correlation between arterial carbon dioxide tension (PaCO2) and PETCO2 in spontaneously breathing patients during spinal anesthesia. METHODS: Thirty adult patients participated in this study. We performed spinal anesthesia with an optimal dose of heavy marcaine. After determining the sensory blockade level, PETCO2 was sampled from hub of a 14-gauge central catheter piercing one of the two nasal oxygen prongs, and oxygen flow rates (2, 3, 4 or 5 L/min) were measured by on-line capnography. The oxygen flow rates were varied every 5 minutes, and PETCO2 values and arterial samples for PaCO2 and arterial oxygen tension (PaO2) analysis were obtained at the end of each 5-minute period. RESULTS: No significant difference in PETCO2 was observed at the different oxygen flow rates. The results show that PETCO2 correlates closely with PaCO2 irrespective of oxygen flow rate. The PaO2 values were; 155.7 +/- 26.3, 192.7 +/- 36.6, 217.0 +/- 40.6 and 241.4 +/- 51.3 mmHg at nasal oxygen flow rates of 2, 3, 4 and 5 L/min, respectively. CONCLUSIONS: The measurement of PETCO2 via this nasal cannula was useful for continuous, noninvasive monitoring during spinal anesthesia irrespective of oxygen flow rate.

7.A Case of Rowell's Syndrome.

Rowell's syndrome is described as erythema multiforme(EM)-like lesions in the presence of lupus erythematosus(LE) associated with immunoserological characteristics of speckled antinuclear antibodies and positive tests for anti-Ro/La antibodies and rheumatoid factor. We report a case of Rowell's syndrome in a 28-year-old woman with systemic lupus erythematosus for 13 years, who presented with targetoid eruptions on the right arm and with positive immunoserologic findings as above.

8.A Case of Solar Urticaria.

Solar urticaria can be defined as an idiopathic photodermatosis characterized by the occurrence of itchy erythematous or urticarial lesions that are provoked only by sunlight and disappear nearly within 24 hours after avoidance of sun exposure. A 25-year-old male patient was presented with a 7-year history of recurrent wheals and itching, which developed after sunlight exposure. Cutaneous lesions and itching completely subside within 2 hours after avoidance of sunlight. Phototesting for UVB, UVA and visible light were all negative. But provocative test with natural sunlight showed positive result. Histopathologically, edema of papillary dermis and mild perivascular inflammatory infiltrations were seen.

9.Factors associated with Renal Scarring in Children with a First Episode of Febrile Urinary Tract Infection.

PURPOSE: Development of renal scarring is associated with delayed diagnosis and treatment of urinary tract infection(UTI). This study was performed to clarify how soon treatment should be started to inhibit renal scarring after onset of UTI and the factors associated with renal scarring in children with a first episode of febrile UTI. METHODS: We retrospectively reviewed 163 patients with a first episode of febrile UTI under the age of 2 years from April 2000 to April 2004. All patients had a DMSA renal scan and voiding cystourethrogram done in the diagnostic period, 6 months after which a follow-up renal scan was done. After patients were divided into 2 groups according to the duration of fever prior to start of treatment, the duration of fever after start of treatment, and total duration of fever, initial and follow-up DMSA scan findings were analyzed among the different groups. We compared the factors associated with renal scars between the groups with and without renal scars. RESULTS: The initial DMSA renal scan identified abnormal finding in 23% of the patients who were treated < or =24 hr from the onset of disease and in 43% of those with fever more than 24 hr. Renal scars developed in 33% of patients who were treated < or =24 hr and 38% of those with fever >24 hr prior to treatment. Renal scars developed in 34% of patients with remission of fever < or =48 hr after treatment and in 50% of those with fever >48 hr after treatment. The risk for renal scars was significantly higher in children who had total duration of fever >72 hr(67%) than in those with shorter duration(19%). In children with renal scars, VUR was most highly associated with an increased risk of renal scar formation. CONCLUSION: Although children with a first episode of febrile UTI are treated within 24hr after onset of the fever, renal damage cannot be prevented completely and it is mainly associated with VUR.

10.Therapeutic Effect of Combination Treatment of Antihistamine and Montelukast in Allergic Rhinitis.

BACKGROUND: Montelukast is a type 1 cysteinyl-leukotrienes receptor antagonist that has been widely used in allergic disease. However, the effect of combination of leukotriene receptor antagonist and antihistamine is controversial. The aim of this study was to compare the effect of combination treatment of montelukast and antihistamine, fexofenadine, over antihistamine alone in patients with allergic rhinitis (AR). SUBJECTS AND METHODS: Retrospective chart review of 60 patients with AR was undertaken. Patients were classified into combination group (montelukast and fexofenadine, n=28) and antihistamine only group (fexofenadine, n=32) according to treatment modalities. Questionnaire survey was performed and allergic symptoms (VAS scale, 5pointscale), and SNOT (sinonasal outcome test)-20 score were obtained before and after the treatment. RESULTS: Mean follow-up duration was 6.7+/-4.6weeks. There was no significant difference in demographic data between two groups. Allergic symptoms and SNOT-20 score(nasal, QOL domain) were improved significantly in both groups after the treatment (all p < 0.001). Combination treatment group showed better improvement in nasal obstruction than antihistamine treatment group(p = 0.03). However, there were no significant differences in other allergic symptoms (rhinorrhea and sneezing) and SNOT-20 between two groups (all p > 0.05, respectively). CONCLUSION: A combination treatment of montelukast and fexofenadine showed more efficacies in nasal obstruction than single fexofenadine treatment in patients with AR. Therefore, montelukast could be used effectively with antihistamine in patients with AR complaining nasal congestion.
