3.The Clinical Significance of SCC, CEA and TPA as Tumor Markers in Cervical Cancer.

The clinical role of tumor markers to detect the extent of tumor invasion and recurrence in cervical cancer has been debated. To evaluate the chncal significance of SCC, CEA and TPA as tumor markers in cervical, we studied 744 patients weith cervieal cancer from June 1990 to Mey 1994. The cut-off val ues of SCC, CEA and TPA were 1.5 ng/ml, 4.5 ng/mi and 110 U/I respectively. Followings were the results. 1. The serum concentration and positive rates of SCC before therapy(567 cases) were 3,0+/-7.0ng/ml(40.4%) for stage I,8.7+/-13.9 ng/ml(71.6%) for stage II, 10.8+/-14.7 ng/ml(85.7%) for stage III, 23.9+/-24.3 ng/ml(94.7%) for stage IV, and 13.4+/-19.1 ng/ml(75.0%) for recurrent cancer. It was increased with advancing clinical stage(p<0.01). 2. The seum levels and positive rate of CEA before therapy(627cases) were 3.4+/-4.3 ng/ml (18%) for stage I, 7.1+/-12.3 ng/ml(37.2%) for stage II, 8.4+/-9.6 ng/ml(57.9%) for stage III, 15.4+/-22.2 ng/ml(52.6%) for stage IV, and 10.3+/-16.2 ng/ml(46.4%) for recurrent cancer. It was increased with advancing clinical stage from stage Ito stage III(p<0.01). 3. The serum concentration and positiceive rate of TFA before therapy(301cases) were 51.7+/-53.8 U/l(9.5%) for stage I, 105.3+/-108.8 U/l(32.3%) for stage II, 186.3%+/-159.8 U/l(50%) for stage III, 191.3+/-l06.2 U/I(63.6%) for stage IV, and 135.4+/-117.0 U/l(46.4%) for recurrent cancer. It was increased with advencing clinical stage(p<0.01). 4. In 64 patients{24.2%) with lymph node invasion of 265 patients treated by operation, the mean serum levels of SCC, CEA and TPA were higher than lymph node negative group(p<0.05). 5. The serum levels of SCCand CKA after therepy were 82.8% in sensitivity. 94.3% in specificity, 67.9% in positive predictive value, 97.4% in negative predictive value.

4.Nasal Hump Correction Using Modified Autospreader Graft: Report of Two Cases.

It is common to find a nose with a hump, lower nasal tip and a radix among Koreans. A common procedure as an aesthetical improvement for such feature, involves rasping of the hump, while lifting the tip and the bridge at the same time. As for the hump, en bloc resection is a well-known, traditional surgical method, but it may cause an open roof deformity, which leads to a high possibility of infection if dorsal augmentation using silicone was performed through it as it connects the nasal cavity and dorsum. To overcome this complication, the hump on the bony portion and cartilaginous has been improved by a modified autospreader graft. Also, the graft can also be used for tip-plasty in dorsal augmentation using silicone, septal extension graft, strut and onlay graft. As a result of constant monitoring for a year, the reformation of the hump and infection was not found.

5.Significance of Postoperative Peritoneal Drainage and Peritoneal Dialys is in Infants with congenital Heart Disease Who Underwent Open Heart Srugery.

PURPOSE: We reviewed 5 years worth of experience with peritoneal drainage and dialysis in infants who underwent open heart surgery. The aim of this study was to investigate the effect of peritoneal drainage and peritoneal dialysis on fluid balance and several parameters of intensive care. METHODS: Six(10%) of 60 consecutive infants who underwent open heart surgery required peritoneal dialysis during peritoneal drainage. Simple peritoneal drainage was performed in the remaining 54 infants. The silicone rubber peritoneal dialysis catheter was inserted into the center of abdominal cavity just after operation, and subsequent peritoneal drainage was maintained during intensive care. RESULTS: Mean age of the study group was 0.48+/-0.21 years(M: F=32: 28). Early postoperative mortality amang the infants with congenital heart disease was 1.6%. Total amount of output was 7.0+/-2.28mL/kg/hr, urine output 5.14+/-2.9lmL/kg/hr, pleural fluid 0.80+/-0.5lmL/kg/hr, and peritoneal fluid 1.20+/-0.90mL/kg/hr. The ratio of output to intake(O/I) was 1.06. None of the complications required early termination of peritoneal drainage or peritoneal dialysis. Hemodynamics and pulmonary function were maintained steadily during postoperative intensive care. CONCLUSION: The early institution of peritoneal drainage and peritoneal dialysis in infants with congenital heart disease after cardiac operations not only removes fluid, thus easing fluid restriction, but may also improve cardiopulmonary function.

6.Recognition status of early at-risk alcohol intake.

BACKGROUND: Alcohol has much effect on our lives as a taste food that people in almost all of the world generally have had. The effects of alcohol cause social problems as well as physical and psychological problems. Recently, interest about drinkirg increasingly rises as the rate of drinking accident increases in Korea. For this, our study examined the amount of alcohol consumption and early at-risk alcohol intake. METHODS: The prepared questionnaire was presented to subjects who were employees in Inchon . The anonymous, self-administered questionnaire was composed of questionnaire concerning alcohol consumption, early at-risk alcohol intake and doctor's role in drinking behaviour. RESULTS: Among 305 subjects, there were 189 (62%) men, 116 (38%) women whose mean age was 37. 9years. 87(28.5%)subjects were classified as high alcohol group and 218(71.5%) low alcohol group. High alcohol group was defined as early at-risk alcohol intake for men who were consuming 135g of alcohol per day, 630 per week and for women 90g per day, 270g per week. Low alcohol group was defined as early at-risk alcohol intake for men whose consumption was 90g per day, 450 per week and for women 45g per day, 270g per week . Mean alcohol consumption of drinker was 158.9g per week. 3 days per week people should not drink in order to avoid risk Although 54% of respondents thought doctors should ask about their drinkirg habits, only 16.4% of respondents reported ever being asked about alcohol intake on the subject. CONCLUSION: Early at-risk drinking subject in the study was much more than those suggested by NIH(National Institutes of Health). We conclude that primary care physicians should take more interest in screening, treating and preventing alcohol related problems and advise and educate patients to cut down or quit drinking.

7.A Study on Bioelectric Steady State Potentials and Intraosseous Induced Poentials

Great strides have been made not only in the research of the endogenous electrical signals,but also in the clinical application on nonunions using exogenous electrical signals. We measured human skin steady state potentials on the anteromedial surface of the twenty-eight left proximal tibiae and investigated intraosseous induced potentials created by a capacitively coupled signal applied on the proximal tibia of ten rabbits' right hind limbs. Following results were observed: 1. Human skin epiphysio-metaphyseal and epiphysio-diaphyseal potential differences in resting state were 116.1±11.7µV, and 120.5±29.4µV, respectively(P> 0.05), while those in full weight bearing were 310.3±169.4µV, and 404.2±154.1µV, respectively (P<0.05). 2. Skin steady state potentials were affected by muscle contraction and load bearing. 3. Thraugh a sequence of applied voltage of 2, 5, 10, 20, and 30 volts peak to peak, linear increase in the intraosseous induced potentials were produced. 4. Intraosseous induced potentials increased, although their increasing rates being decelerated, with increasing frequencies of 2, 20, 60, 120, and 200 KHz. 5. Higher intraosseous induced potentials were produced,as conductive jelly was applied between the capacitor plates and skin.

8.Study on Basic Culture Method of Tumor Chondrocytes

In vivo and in vitro culture of rat tumor chondrocytes is a good culture model for physiologic, pathologic and biochemical studies of tumor chondrocytes. Human tumor chondrocyte culture is another method, but it is difficult to maintain a cell line and in vivo culture. So called, Swarm Rat Chondrosarcoma, which wss developed spontaneously in a Spra-gue-Dawley rat and maintained by Dr. R. Swarm, can be cultured in vivo and in vitro at the same time. It is easy to maintain Swarm Rat Chondrosarcoma and is said that there are nearly no notable changes in cellular characteristics during consecutive cultures. In this study, in order to establish the basic culture method of rat tumor chondrocytes, the inoculated tumor mass were studied with swarm rat chondrosarcoma cell line which had been preserved at Orthopedic Research Laboratory of the Msssachusetts General Hospital. In vivo culture, injection of digested 1×10(6) cells, 1×10(7) cells and direct inoculation of tumor of mass were done at each 10 Sprague-Dawley rats group and examination was done at postinoculation 2, 3, 4, 5, 6, 7 and 8 weeks. In vitro culture, 1×10(6)/ml, 2×10(6)/ml and 4×10(6)/ml concentration cell suspensions were plated at 96-well-plate and observed at 2, 3, 4 and S days with inverted microscope. The results of this study are as follows. 1. In vivo culture, the best result was observed at direct inoculation of tumor fragments among 1×10(6) cells, 1×10(7) cells and direct tumor fragments inoculation. 2. The ideal time of obtaining tumor mass growing in rsts is between 4 to 6 weeks after inoculation. 3. In vitro culture, the proper cell density in 96-well-plate was 1 x 10(6)/ml among 1×10(6)/ml, 2×10(6)/ml snd 4 × 10(6)/ml.

10.The Effect of Atropine on Myopic Progression in Children.

PURPOSE: To examine the effects of atropine therapy on myopic progression in myopic children. METHODS: Fifty myopic patients, aged from 7 to 14 years and less than -5 diopters, visited our clinic for correction of refractive error from August 2001, and were followed up for at least 1 year. Of these 50 patients, the atropine therapy group was 23 and the control group was 27. Right eye cycloplegic refraction and axial length changes of the two groups were compared and analyzed. RESULTS: There was a mean increase of myopia of 0.25+/-0.43 D/yr in the atropine therapy group and of 0.83+/-0.37 D/yr in the control group (P=0.000). There was a mean increase of axial length of 0.12+/-0.19 mm/yr in the atropine therapy group and 0.39+/-0.21 mm/yr in the control group (P=0.000). In refraction and axial length changes according to age, there was no statistically significant difference within the group, aged from 7 to 9 years. However, there was a statistically significant difference within the group, aged from 10 to 14 years. In refraction and axial length changes according to myopic grade, there were a statistically significant differences between the groups. CONCLUSIONS: Atropine therapy is effective in slowing the myopic progression in myopic children with refractive error less than -5 diopters.
