OBJECTIVES: Breast cancer has been the most prevalent female cancer in South Korea since 2001. Early detection of this disease is the most effective strategy for reducing mortality. The objective of this study was to identify factors which could predict advanced stage at diagnosis of breast cancer. METHODS: Participants who were initially diagnosed with breast cancer and referred to the Stress Clinic of the Breast Cancer Center at Kyungpook National University Hospital were included. Through a semi-structured interview, the authors investigated psychosocial variables such as the extent of marital and family functioning and emotional-economic family burden as well as sociodemographic and health behavior-, health characteristic- and cancer-related variables. RESULTS: Data were collected from 219 participants. One hundred and twenty(54.8%) subjects were diagnosed with advanced-stage breast cancer. Variables that were significantly different between the advanced-stage and early-stage groups included : monthly breast self examination(p<0.000), annual mammographic screening(p< 0.000), mode of tumor detection(p<0.000), nature of the first symptoms(p<0.000), time to treatment after di-agnosis(p<0.000), overloaded economic and family burden(p=0.018), marital functioning(p<0.000) and family functioning(p<0.00). Logistic regression analysis indicated that irregular annual mammography screening(OR=7.431 ; 95% CI 2.407-22.944) or a lack of screening(OR=25.299 ; 95% CI 7.855-81.482) and a dysfunctional marital relationship(OR=4.772 ; 95% CI 2.244-10.145) were significantly associated with advanced stage at diagnosis of breast cancer. CONCLUSIONS: We reconfirmed screening behavior to be a risk factor for delayed diagnosis of breast cancer. Our findings also emphasized the importance of psychosocial factors such as marital functioning in early detection of breast cancer. Psychiatric consultation in the area of martial functioning could be beneficial for increasing early detection in breast cancer.

PURPOSE: This study was conducted to compare the postural habits, body image, and peer attachment of adolescents with idiopathic scoliosis and healthy adolescents, and to examine the correlation between body image and peer attachment in each group. METHODS: Eighty-three adolescents with idiopathic scoliosis and 83 healthy adolescents were recruited from 4 middle schools located in W city. Data were collected from November 22 to December 6 2010 using a structured questionnaire including items about postural habits, body image, and peer attachment. Data were analyzed using descriptive statistics, chi2-test, independent t-test and Pearson Correlation Coefficient. RESULTS: Compared to healthy adolescents, adolescents with idiopathic scoliosis were more likely to use one hand, to lean back while seated, and to put weight on one leg while standing. The score for body image was significantly lower for adolescents with idiopathic scoliosis compared to healthy adolescents, but peer attachment was not significantly different between the two groups. Body image was significantly correlated with peer attachment only for the adolescents with idiopathic scoliosis. CONCLUSION: Interventions to promote balanced postural habits in adolescents with idiopathic scoliosis are needed. Information on body image and peer attachment for these adolescents should also be considered when developing interventions.

PURPOSE: The purpose of this study was to verify prevalence rates of atopic dermatitis (AD) in middle school students in Korea and to define AD and associated factors. METHODS: This study was conducted using secondary analysis of raw data from the 'The 6th Korean National Adolescent Health Behavior Survey, 2010' conducted by the Korea Centers for Disease Control. The participants were 37,570 students. General characteristics and health behavior-related characteristics were analyzed using descriptive statistics, chi2-test and logistic regression analysis. RESULTS: Results are as follows. 1. There were 8,494 middle school students (22.6%), who had been diagnosed with AD during the last 12 months. 2. Factors associated with AD in middle school students were gender (p<.001), grade level (p=.001), father's education (p<.001), mother's education (p=.032), regular dietary habits (breakfasts, p=.025; lunches; p=.003), stress (p<.001), recognition of subjective awareness of one's health conditions (p<.001), hand washing before meals at school (p=.013), and cessation education for both smoking (p=.002) and alcohol (p=.031). CONCLUSION: Results of the study indicate that in order to alleviate AD symptoms in these students, there is a need to develop nursing interventions and education programs taking into consideration these variables and verifying the effects of interventions and programs.

PURPOSE: The study was done to influence of family function and degree of depression on impulsive behavior in adolescents. METHOD: Using convenience sampling 780 adolescents were recruited from October 29 and November 9, 2011. Data were collected using self-report questionnaires which included items on general characteristics, family function, depression, and impulsive behavior. Data were analyzed using t-test, ANOVA, Scheffe test, Pearson Correlation Coefficient, and hierarchical regression analysis. RESULTS: The major findings of this study were as follow; 1) There was a significant negative relationship between family function and impulsive behavior (r=-.236, p<.001) and a positive relationship between depression and impulsive behavior (r=.342, p<.001). 2) Factors with a significant negative association with impulsive behavior were higher school records (beta=-.112, p<.001) and family function: positive resolution of family problems (beta=-.213, p=.008), while a significant positive association was found for unsatisfactory school life (beta=.103, p=.016), family function: emotional reactivity (beta=.159, p=.028), and depression (beta=.301, p<.001). These variables explained 20.1% of variance for impulsive behavior. CONCLUSION: The results of this study identify a relationship between family function, depression, and impulsive behavior in adolescents indicating variables to be addressed to reduce impulsive behavior. These variables can be used in developing intervention programs.

PURPOSE: This study was done to identify the impact of resourcefulness and communication style on nursing performance in nurses working in hospitals. METHOD: Though a convenience sampling method 312 nurses were recruited between from July 4 and 17, 2011. Data were collected using a questionnaire, which included items on work related characteristics, resourcefulness, communication style, and nursing performance. Data were analyzed using t-test, ANOVA, Scheffe test, Pearson correlation coefficient and hierarchical regression analysis. RESULTS: The major findings of this study were as follow; 1) There were significant relationships between nursing performance and resourcefulness (p<.001), informative communication style (p<.001), affiliativeness communication style (p<.001), and dominance communication style (p<.001). 2) The nursing performance was significantly associated with career in current department, resourcefulness, informative communication style, affiliativeness communication style, and dominance communication style in capability which explained 45.6% of variance in nursing performance. CONCLUSION: The results of this study demonstrate a relationship between resourcefulness, communication style, and nursing performance in hospital nurses indicating the need to use the study results to plan programs to prompt nurses in their use of resourcefulness and communication style in nursing care.

beta-amyloid peptide (Abeta) consisting of 40 to 42 amino acid is the principle constituent of senile plaques in Alzheimer's disease. Although, the hypothesis that deposition of AP triggers a cascade of events leading to the pathology of Alzheimer's disease has been widely accepted, direct evidence for triggering accumulation of phosphorylated tau in paired helical filament is rare. In this study, we examined neurotoxicity induced by 3 kinds of beta-amyloid peptides 1 ~28, 25~,35 and 1~40 to elucidate the way of mechanism trading to neuronal cell death caused by Abeta using cultured hippocampal neurons. For this purpose, we measured lactate dehydrogenase (LDH) in the culture media after treatment with Abeta combined with anti-oxidant drug, trolox, or not. By histochemical and TUNEL method, we studied the change of immunoreaction to anti-MAP-2 (microtubule associated protein -2, the main component of neuritis) and detected apoptotic cells, respectively, in the hippocampal neurons treated with Abeta. To investigate whether tau phosphorylation involve neurotoxicity induced by Abeta, we immunostained the neurons with anti-SMI-31 to recognize phosphorylated Ser 396/404 of tau. From our data, we suggested that Abeta1-40 and Abeta25-35 induced marked neurodegenerative changes, and the mechanism responsible for cell death caused by Abeta -neurotoxicity was associated with the apoptosis. Because Abeta-neurotoxicity was not inhibited by anti-oxidant, trolox, we suggested that anti-oxidant did not protect the neuronal cells against the damage induced by Abeta in ou. expo.imental envi.onment. Finally, we suggested that AP treatment did not potentiate the immunoreactivity to anti-phosphorylated tau antibody and we speculated that Abeta-neurotoxicity led hippocampal cells to apoptosis without tau phosphorylation.

BACKGROUND: Extended-spectrum beta-lactamase (ESBL)-producing Escherichia coli and Klebsiella pneumoniae are main pathogens of bloodstream infection. We compared the epidemiology of ESBL-producing E. coli and K. pneumoniae. METHODS: From January 2003 through March 2007, we retrospectively investigated the clinical characteristics and comorbidities of patients with bloodstream infection by ESBL-producing E. coli and K. pneumoniae. RESULTS: During the study period, 79 patients (54 with E. coli and 25 with K. pneumoniae) with blood cultures positive for ESBL-producing E. coli or K. pneumoniae were studied. When comparing the source of bloodstream infections, urinary tract infections (UTIs) were more commonly caused by E. coli (24% vs. 0; P=0.007), and respiratory tract and soft tissue infection (36% vs. 15%; P=0.04, 12% vs. 0; P=0.03, respectively) were more commonly associated with K. pneumoniae. Among hospital-acquired bloodstream infection, third-generation cephalosporin was more commonly used in patients with E. coli than patients with K. pneumoniae (81% vs. 24%, P<0.001). Medical devices (central venous catheter and urinary catheter) were more commonly used in patients with K. pneumoniae. CONCLUSION: ESBL-producing E. coli bloodstream infection is more common in urinary tract infections. ESBL-producing K. pneumoniae is more common in respiratory tract infections and in skin and soft tissue infections. In hospital-acquired infections, ESBL-producing E. coli bloodstream infection is more common in biliary tract infection. ESBL-producing E. coli was more commonly associated with prior frequent antibiotics use and K. pneumoniae was more commonly associated with use of medical devices.

This study was conducted to develop a nutrition education workbook to improve picky eating in children and to evaluate the effect of nutrition education using this developed workbook. The developed workbook focused on 2nd and 3rd grade students and consisted of five units. The contents included: multi-grain, vegetables (2 units), fish, and milk. The activities, including observation, and learning the roles and names of the foods, were developed mainly to increase motivation for eating a balanced diet. This workbook was developed from April to December 2008, and was applied at 15 elementary schools containing 1,674 students from April to September 2009 in Changwon City, Korea. We evaluated changes in knowledge before and after education on nutrition, eating behavior, dietary habits, and educational activities using self-administered questionnaires. The children demonstrated significant improvements in nutritional knowledge (P<0.001), eating behavior (P<0.001), and dietary habits (P<0.001). Most of the children answered that the education program was helpful and exciting. Based on these results, we believe that the developed workbook is suitable for children picky eaters, and hope it will be used in the field of child nutrition education.

PURPOSE: The purpose of this study was to investigate the current status of dietary education carried out by dieticians or nutrition teachers at elementary, middle, and high schools in Gyeonggi province. We compared dietary education status not only among school level but also between dieticians and nutrition teachers. METHODS: The survey using self-administered questionnaires was conducted by dieticians or nutrition teachers at 91 schools (43 elementary schools, 33 middle schools, and 15 high schools) in Gyeonggi province in December, 2015. The questionnaire consisted of questions regarding general characteristics of respondents, current status of dietary education, opinion about dietary education methods, and cooperation level of persons concerned. RESULTS: The placement rate of nutrition teachers was 50.5% overall. Only 36 out of 91 schools conducted regular dietary education, and the percentage of schools conducting regular dietary education was significantly different according to school level with the lowest percentage in high schools, equivalent to 20% (p = 0.003). The average annual dietary education time was 12.4 hours, and it was very low in high schools (5.1 hours) and in schools without a nutrition teacher (1.6 hours). The levels of cooperation and support from principals, teachers, parents, and students were significantly lower in high schools than elementary and middle schools (all p < 0.001) and in schools without a nutrition teacher than schools with a nutrition teacher (all p < 0.05). CONCLUSION: The results show that the level of dietary education is not enough and needs to be improved, especially at middle and high schools and at schools without a nutrition teacher. Government support polices need to be implemented to encourage dietary educational activities.

OBJECTIVES: The purpose of this study was to investigate the clinical utility of the Korean Wechsler Intelligence Scale for Children-Fourth Edition (K-WISC-IV) and General Ability Index (GAI) in children with attention-deficit hyperactivity disorder (ADHD). METHODS: The GAI and Full Scale IQ (FSIQ) in K-WISC-IV were compared for a group of 79 children with ADHD and 234 normal children. The mean differences within each group were testified by the t-test and Cohen's d. The Wechsler's descriptive classifications for each GAI and FSIQ score were analyzed in the children with ADHD. Additionally, the critical value for the difference between the means of the FSIQ and GAI was calculated in the normal children. RESULTS: The score on the GAI was higher than that on the FSIQ in the children with ADHD, with a mean difference of 2.19 points. The implications of these findings are discussed regarding the cognitive characteristics of the children with ADHD. According to Wechsler's descriptive classification, the application of the GAI showed a lower (7.59%), equal (61.65%) or higher (60.76%) prevalence of ADHD compared to the application of FSIQ. CONCLUSION: The implications of these findings are discussed regarding the cognitive characteristics of the children with ADHD. The GAI reflecting the attention and behavioral problems in children with ADHD could be utilized as an alternative global intelligence estimate that considers their potential for development.
