1.A retrospective study of efficacy and safety of itraconazole for treatment of invasive fungal infection in hematologic diseases

Objective To investigate the effieacy and safety of intravenous itraconazole for the treatment of invasive fungal infection(IFI)in patients with hematological disease or undergoing allogeneic hematopoietic stem cell transplantation(HSCT).MethodsSix hundred and sixty-six patients with above mentioned conditions and diagnosed as IFI from January.2007 to July,2007 were enrolled.Intravenous itraconazole was administered at a dose of200 nag every 12 hours for 2 days and followed by 200 mg every 24 hours.Patients were then switched to oral itraconazole according to the clinical situation Responses were determined on the basis of clinical and microbiological criteria.Results The probability of defervescence Was 69.8%and the total response rates among related to itraconazole were the proven.probable and possible IFI patients were 73.7%.68.1%and 68.2%(P=0.380).Adverse effects were found in 58 patients (8.7%).which were mainly mild to medium reversible dysfunction of liver and gastrointestinal tract, Conclusion Itraconazole is an effective and safe antifungal agent for patients with hematological disease or undergoing HSCT and is suitable for empirical antifungal therapy.
2.Progress on ventilator-induced lung injury and lung protective ventilation strategies in pediatric patients

Ventilator-induced lung injury is a serious complication of mechanical ventilation. At present a lot of researches have explored lung protective ventilation strategies to prevent and treat ventilator associated lung injury in adult. But researches on children are relatively rare. This article reviewed the pathophysiological mechanisms of pediatric ventilator-induced lung inju-ry and the clinical application of lung protective ventilation strategies in children.

Purpose]Research on otolaryngology disease properties and the classical prescription method, explore the mechanism and the application of the treatment of otolaryngology disease. [Methods]Selection of swel ing ear, ear vertigo, slowly closing pus ent common diseases such as ear, combined with the Treatise on Febrile Disease and Synopsis provisions, analysis of the disease by syndrome differentiation thinking, expound the classical prescription principles and the range for the treatment of the disease. [Results]Treating most otolaryngology disease can use the classical prescription, the curative effect of treatment of otolaryngology disease has obvious advantages. [Conclusion]It has been verified by classical prescription and clinical use, efficacy, and the system of subtlety, ingredients to streamline, familiar with the classical prescription treatment of otolaryngology disease, wil improve the level of physician treatment based on syndrome differentiation and patients to reduce the economic burden, has broad prospects.
4.The analysis of effect of preoperative chemotherapy on bulky cervical cancer by internal illiac arterial infusion

Objective To investigate the therapeutic effect of arterial interventional chemotherapy on bulky cervical cancer. Methods One hundred and twenty-six patients with bulky cervical cancer were randomly divided into two groups. Arterial interventional chemotherapy group(n =74, C gronp), and radiotherapy group(n =52 R group). Patients in C group underwent internal iliac arterial infusion chemotherapy by using Seldinger technique. The chemotherapy regimens of cervical squamous cell carcinoma were prescribed including cisplatin and BLM, and cervical adenocarcinoma were prescribed including cisplatin and ADM and VCR. Patients in R group were only given radiotherapy Ir192 high-dose rate intracavitary radiotherapy was performed with A point dose at 24 Gy. Both groups of patients were followed up after two weeks. Results The tumor regression rate of C group was 93.24 %, significantly higher than 71.15 % in R group(P 0.05). The 3-year recurrence rate between the two groups had obvious difference(P 0.05). Conclusion Neoadjuvant chemotherapy can effectively reduce tumor volume, and postoperative 3-year recurrence rate, increases surgery rate on bulky cervical cancer. But the effect on long-term survival rate needs to be evaluated further through long-term follow-up.
5.A comparative assessment of torque capabilities between lingual and labial brackets using three-dimension finite element

Objective:To analyse the difference of torque performance between lingual and labial brackets as well as the influence of the archwires of different materials with different dimension on the torque capability of lingual brackets.Methods:The 3D finite mo-del of lingual and labial brackets from the right upper canine to the left upper canine was established,all brackets were with 0.56 mm slot size.The center of the bracket slot of the left upper central incisor was used as the central point and the two brackets were rotated clockwise and counterclockwise with a rotation angle of ±20°along the horizontal axis of bracket slot.The torque load on the left up-per central incisors with different materials and dimensions of the archwires was simulated.Results:The torque force of both lingual and labial brackets increased with the increase of rotation angle and the elastic modulus of archwire.The lingual archwire generated torque force was nearly 3 -5 times more than the labial system under the same torque angle.The torque value generated with 0.48 mm ×0.64 mm archwires was greater than 0.43 mm ×0.64 mm.The lingual bracket generated torque force decreased gradually from nickel-titanium(NiTi),Beta-titanium(TMA)to stainless steel(SS)under the same dimension.Conclusion:Torque moments of lin-gual and labial brackets both increases with the increase of torque angle in each operating condition.The lingual orthodontic brackets produce more torque force than labial brackets under the same torque angle.Torque performance of lingual bracket was affected by the material and dimension of the archwires.

Pathogeneses and molecular mechanisms for different types of constipation are waiting to be further clarified. New biomarkers and therapeutic targets for clinical diagnosis of constipation are so urgent. As for current problems in diagnosis and treatment of constipation, it is necessary to use the concept of translational medicine to break existing imprisonment of thinking, and find out new thinking ways of research methods, diagnosis and treatment approaches, thereby improving diagnosis and treatment levels.

7.Clinic analysis of the malignant probability and probable risk factor of solitary thyroid nodule in 902 cases

Purpose:To study the malignant probability of a solitary thyroid nodule,probable risk factor,the status of B-mode ultrasonography,computed tomography,fine needle aspiration and frozen section in the operation and other adjuvant examination in the diagnosis.Methods:In the retrospective analysis of 902 cases of solitary thyroid nodule treated by surger- y and diagnosed by pathology in the Cancer Hospital of Fudan University from March 1998 to May 2001,we analyzed the value of B-mode ultrasonography,computed tomography,fine needle aspiration and frozen section in the operation and com- pared them with the final pathological conclusion.Results:This disease occurred mostly in 20~50 years old women,while the malignant nodules occurred more frequently in men(P50 years) (P50 years) is a risk factor.B-mode ultrasonography can be used as routine preoperative examination.CT is valuable in the cases with metastasis to neck lymph nodules.With FNA the pathological diagnosis of the nodule suspected to be malignant can be made preoperatively.
9.Comparison on Dendritic Cells Function of Yang-Huang and Yin-Huang Syndrome in HBV Related Hepatic Failure Patients

This study was aimed to compare the function of dendritic cells (DCs) in patients of Y in-Huang syn-drome and Y ang-Huang syndrome from the peripheral blood of HBV related hepatic failure. Jaundice patients of HBV related hepatic failure were divided into two groups, which were the Y ang-Huang syndrome group and Y in-Huang syndrome group. And a healthy control group was also established. The DCs were separated and cultured in vitro from PBMCs in the peripheral blood. Surface molecules such as HLA-DR, CD80, CD86, CD83 and CD1αwere detected by the flow cytometry. And the level of IFN- α and IL-4 in the supernatant of DCs were also de-tected. The expression difference of inflammatory cytokines and immune cells between Y in-Huang and Y ang-Huang syndrome in patients of HBV related hepatic failure were compared. The results showed that compared with healthy people, the expression rate of DCs phenotype such as HLA-DR, CD1α, CD83, CD80, CD86 in patients of HBV related hepatic failure was significantly decreased (P < 0.01); the excreted factor IFN-α had a significant rising (P < 0.01). Compared to Y ang-Huang syndrome group, the expression rate of CD83 and CD86 in the Y in-Huang syndrome group of HBV related hepatic failure patients was significantly reduced (P < 0.01). And the ex-creted factor of IL-4 had a significant rising in Y in-Huang syndrome group (P < 0.01). Compared to the Y in-Huang syndrome group, the excreted factor IFN-α in the Yang-Huang syndrome group had a significant rising (P< 0.01). It was concluded that DCs function of patients with HBV related hepatic failure in both Y in-Huang syn-drome and Y ang-Huang syndrome group showed low immune function. And the immune function of the Y in-Huang syndrome group was lower. There was excessive releasing of inflammatory factors in the Y ang-Huang syn-drome group. And there was enhanced anti-inflammatory cytokine expression in the Y in-Huang group.
