2.A Cutaneous Odontogenic Sinus.

Cutaneous odontogenic sinuses are the most common cause of chronic draining sinus tracts on the face and neck. These lesions present as erythematous nodules with surrounding dimples, which intermittently drain purulent fluid. Although most patients do not have dental symptoms, careful questioning of a patient's past symptoms may help the clinician identify a dental etiology. A history of toothache in the affected area that resolves without dental intervention is useful information. Clinical and radiographic examination may reveal carious teeth with radiolucent areas. Elimination of the source of infection by endodontic treatment or tooth removal results in resolution of the sinus tract, typically within 2 weeks. Early precise diagnosis and appropriate dental treatment can circumvent unnecessary and ineffective antibiotic therapy or surgical treatment. We report a case of a cutaneous odontogenic sinus in the right cheek area.

3.Effects of Air Bubble on the Corneal Endothelium.

During intraocular surgery including cataract, air bubbles occasionally enter the anterior chamber during irrigation. The influences of these air bubbles on the cornea endothelium, however, have yet to be known. The purpose of this study is to investigate the effects of bubbles on the rabbit corneal endothelium during irrigation and aspiration mode in experimental phacoemulsification. After anesthesia, the anterior chamber was irrigated and aspirated for 1 minute with balanced salt solution. The rabbits were then randomly divided into 4 groups. In group 1, corneal endothelial irrigation and aspiration were performed without air bubbles for 1 minute. In group 2, corneal endothelial irrigation was performed in the presence of air bubbles in the anterior chamber for 1 minute. In group 3, corneal endothelial irrigation and aspiration were performed in the presence of air bubbles in the anterior chamber for 1minute. In group 4, corneal endothelial irrigation was performed with silicone oil at 50 mmHg pressure in the presence of air bubbles in the anterior chamber for 1 minute. In group 1, the cornea endothelial cells showed normal F-actin without cell damage. In group 2, a ring shaped endothelial cell destruction area adjacent to the border was found along the margin of the air bubble. In group 3, similar ring-shaped lesion found in group 2 were observed but with more extensive cell destruction. In group 4, a partial number of cell to cell separation was found without endothelial cell destruction. In conclusion, air bubbles in the anterior chamber during irrigation with balanced salt solution showed destructive changes in rabbit corneal endothelial cell.

4.Quality of Life, Upper Extremity Function and the Effect of Lymphedema Treatment in Breast Cancer Related Lymphedema Patients.

OBJECTIVE: To evaluate quality of life (QOL), upper extremity function and the effect of lymphedema treatment in patients with breast cancer related lymphedema. METHOD: The basic data comprised medical records (detailing age, sex, dominant side, location of tumor, cancer stage, operation record, cancer treatment and limb circumferences) and questionnaires (lymphedema duration, satisfaction, self-massage). Further to this, we measured upper extremity function and QOL, administered the DASH (Disabilities of Arm Shoulder and Hand outcome measure) and used the EORTC (European Organization for Research and Treatment of Cancer)-QLQ-C30 and the EORTC-QLQ-Br23. Results of these were calculated as main outcome variables. RESULTS: The questionnaire responses and arm circumferences of 59 patients with breast cancer related lymphedema were analyzed. In the DASH questionnaire, it was found that the older the lymphedema patient was, the lower their upper extremity function. On the EORTC-QLQ, patients with metastasis had significantly lower scores in physical functioning and role functioning. In terms of upper extremity circumference, there was a significant upper extremity size reduction after lymphedema treatment. CONCLUSION: There were several dissociations between some subscales of quality of life questionnaires and those of upper extremity functions. Upper extremity function was correlated with the age of breast cancer patients and QOL was influenced by M-stage. Lymphedema treatment was found to be effective in reducing edema in patients with breast cancer related lymphedema.

5.A Case of Acquired Dermal Melanocytosis Occurring on the Chest.

Dermal melanocytosis, which is histologically characterized by the presence of dermal melanocytes, is most commonly found in Asians and other darkskinned people. It is observed in various congenital conditions such as the nevus of Ota, the nevus of Ito, the Mongolian spot and the blue nevus, and typically appears at birth or in early childhood. However, several cases that have appeared in adult life have been reported as acquired dermal melanocytosis. Herein, we report a unique case of acquired dermal melanocytosis diffusely affecting the left side of the chest of a 45-year old female. Additionally, a review of Korean literature regarding reported cases of unusual acquired dermal melanocytosis will be presented.

6.A case of recurrent lead fracture and complete dislocation after permanent pacemaker implantation.

With the increased use of implantable cardiac devices, the incidence of hardware problems has also increased. Some of the hardware problems might be induced by patient factors. We experienced recurrent pacemaker lead fracture and dislocation after permanent pacemaker implantation. The patient was a bus driver who used his left arm vigorously when he turned the steering wheel. After a new lead was inserted via an axillary vein approach and the patient changed his occupation, no more lead problems have occurred. When a recurrent hardware problem with an implantable pacemaker or defibrillator occurs, patient factors related to anatomy or behavior must be considered.

7.A Case of GNAS1 Mutation in Pseudohypoparathyroidism Type Ia.

Pseudohypoparathyroidism (PHP) is a group of genetic disorders in which the kidneys fail to respond to parathyroid hormone. Genetic defects in the GNAS complex locus lead to reduced Gsalpha (alpha-subunit of the heterotrimeric stimulatory G protein) activity in PHP type Ia patients. These patients exhibit characteristics of Albright hereditary osteodystrophy (AHO) and hypocalcemia, increased parathyroid hormone, and resistance to other Gsalpha protein-coupled hormones. AHO has a wide range of manifestations such as short stature, obesity, round face, subcutaneous ossification, and bone shortening in the hands and feet. In this study, we present the case of a 47-yr-old woman who was diagnosed with PHP type Ia with AHO. She showed tetany, dizziness, irritability to light, decreased visual acuity, cognitive impairment, and motor dysfunction. Direct sequencing identified a heterozygous missense mutation in exon 6 (c.466G>A, p.Asp156Asn) in GNAS1. To our knowledge, this case is the first report in Korea of PHP type Ia caused by a heterozygous missense mutation in exon 6 (c.466G>A, p.Asp156Asn) in GNAS1.

8.Weighted Needle Pinprick Sensory Threshold (WPT) and Arthropornetric Parameters.

To assess the relationship of pinprick sensory threshold with height and weight, Ninety seven healthy young (age 19 to 28) males were tested on their anatomical snuff box of both Hands. Their WPT was measured by device holding and moving weighted needle sensory threshold apparatus. Mean of the WPT was 0.43(+/-0.24) g in left and, 0.53( 0.30) g in right hand. Difference of WPT between the right and left hand appeared statistically significant (p<0.01) . Correlation coefficient of WPT with height was 0.26461 (p<0.01) in left and, 0.31856 (p<0.01) in right hand. Weight was statistically significantly correlated with WPT in left hand (r=0.24932, p<0.01) The study shows that the left-right difference and influence of height in pinprick sense may exist in normal healthy male adults.

9.Association Between Carotid Artery Stiffness and Headache Following Cilostazol Use in Cerebral Infarction Patients.

BACKGROUND: Cilostazol leads to inhibition of platelet aggregation and to vasodilatation. It is widely used for the secondary prevention of cerebral infarction. However, headache is a well-known adverse effect of cilostazol, and these headaches may be caused by the vasodilation of the cerebral artery. The goal of our study was to assess the frequency and severity of headaches following cilostazol treatment and to evaluate factors related to the development of these headaches. METHODS: Seventy patients with cerebral infarction were included in this study. We measured the carotid intima media thickness (IMT), the distensibility of the carotid artery (CAD), the brachial ankle index (ABI), and the brachial ankle pulse wave velocity (PWV) in order to quantify the degree of atherosclerosis and arterial stiffness. Patients were then given 100 mg of cilostazol in tablet form twice daily. For three days, we evaluated headache incidence and severity using a verbal rating scale (0-10). RESULTS: Twenty three (32.9%) patients reported headache during cilostazol medication and 7 patients had severe headache. Women were more likely to develop headaches than men (p=0.03). In addition, the mean IMT was lower in subjects with cilostazol-induced headache than in the headache-free subjects (0.8+/-0.1 vs 1.01+/-0.2 mm, p=0.001), while CAD was higher in these subjects (0.3+/-0.1 vs 0.25+/-0.1, p=0.03). There was no difference in PWV and ABI. CONCLUSIONS: Lower carotid IMT, increased CAD, and female gender may be associated with the development of cilostazol-induced headache in patients with cerebral inafarction, but not the systemic arterial stiffness.

10.Two cases of spontaneous bacterial peritonitis in adults with nephrotic syndrome.

Spontaneous bacterial peritonitis is one of the important complication of childhood nephrotic syndrome which occurs not infrequently but this complication in adults with nephrotic syndrome is ,however, very rare. The fact that ascites formation is more frequently seen in childhood nephrotic syndrome and that minimal change disease, the commonest pathology found in childhood is associated with impairment in both cellular and humoral immunities may be an explanation for the discrepancy. We have experienced two cases of spontaneous bacterial peritonitis complicated in adults with nephrotic syndrome. The age of patients is 23 and 68 years respectively. The serum IgG level of younger patient is markedly decreased (375 mg/dL) and that of elderly patient is lower normal limit (765 mg/dL). Peritonitis was complicated during relapse in the younger patient. Both of the patients were receiving steroid therapy before complicated by peritonitis. Both patients recovered from peritonitis by antibiotic therapy but, elderly patient died from nosocomial pneumonia.
