Pseudohypoaldosteronism type 1 is a genetic renal tubular disease of salt wasting, presenting in young infants. Tubular unresponsiveness to elevated endogenous and exogenous aldosterone is the suggested pathogenetic mechanism. Oral sodium chloride supplementation relieve the clinical symptoms and electrolyte distrubances. We experienced 2 cases of PHA type 1 in 38-day and 45-day old male infants who were presented with failure to thrive, vomiting and/or dehydration. Laboratory data showed hyponatremia, hyperkalemia, hypochloremia and metabolic acidosis. Renal and adrenal functions were normal. Plasma renin activity and plasma aldosterone concentration were markedly elevated. Under the diagnosis of pseudohypoaldosteronism type 1, oral supplementation of NaCl and/or kayexalate improved the clinical states of the patients.

PURPOSE: This study is to assess the quality of life(QOL) of hematic cancer survivors after hematopoietic stem cell transplantation(HSCT) and received chemotherapy(RC) to prepare basic information for nursing interventions in order to improve the patients' QOL. METHOD: The data were collected by self-reporting questionnaire from January to March, 2003 intended for outpatients at the Cancer center of D university hospital in Busan. All 44 of them were diagnosed as hematic cancer and had spent 100 days after getting HSCT and complete remission(CR) throughout RC. The collected data were analyzed with descriptive statistics, t-test. ANOVA using SPSS/WIN 10.0 program. RESULTS: The total mean score of the QOL was moderate. In case of survivors in HSCT, the total mean score of the QOL was 5.81+/-.08, and that of survivors in RC was 5.94+/-.13. The facts above has not been considered statistically as the result of analysis of differences in each domain of the QOL depending on the general characteristics of the objects of this study. CONCLUSION: The total mean score of the QOL was at moderate levels, indicating that the survivors after HSCT and RC were perceiving their QOL as moderate. In the nursing business aspect, the most important thing is to understand the QOL which the 2 groups of the survivors perceive, and the plans of nursing intervention that can be helpful to more qualitative life should be studied constantly.

Purpose: The purpose of this study was to investigate the relationship between social network characteristics, flow in class, communication skills, and problem-solving skills of nursing students in simulation. Methods: For this study a descriptive survey design was used. Participants were 100 nursing students who attended in 2 university and completed the self-report questionnaire. Data were collected from October 14 to December 6, 2019 and were analyzed with SPSS 26.0, AMOS 21.0 and Netminer 4 evaluation version. Results: It was confirmed that the task advice network centrality of nursing students fully mediates their communication skills and affects their problemsolving skills and that friendship network centrality completely mediates flow in class and communication skills, and affects problem-solving skills. Task advice network centrality, friendship network centrality, flow in class, and communication skills were found to explain problem solving skills by 51.8%. Conclusion In order to increase the problem-solving skills of nursing students in simulation nursing education, a strategy to improve their flow in class and communication skills is essential and it suggests the need to make it part of the curriculum. In addition, in order to increase the communication skills of nursing students, it is necessary to be careful when making a team to minimize the team members isolated within the team so that smooth interaction can occur.

BACKGROUND: Beta-adrenergic receptor Kinase 1(betaARK1) is a serine/threonine kinase attached, which inhibits the coupling of beta-adrenergic receptor with G-protein. Myocardial betaARK1 level is usually elevated in heart failure and hypertrophy, but it is not known whether the circulating betaARK1 level is related with the degree of cardiac hypertrophy. This study was performed to evaluate the association of the betaARK1 level in circulating mononuclear leukocytes(MNL) in untreated hypertension with left ventricular mass in hypertensive patients. Method: Nineteen non-treated hypertensive patients were included for this study. High blood pressure was confirmed when systolic BP is over 150 mmHg or diastoli BP is over 95 mmHg. Echocardiography was performed to evaluate the degree of hypertrophy by measuring the left ventricular mass index(LVMI) and relative wall thickness(RWT), and test the LV function by measuring the ejection fraction(EF) according to ASE guideline. At the same time, blood was collected from each patient and MNL were isolated by gradient centrifuge with Ficoll-400. Total RNA was purified from MNL and semi-quantitative RT-PCR was performed. After reverse transcription, PCR was done with primers for human betaARK1 and GAPDH as external control. betaARK1 levels were expressed by ratio to GAPDH level and estimated the relations with clinical and Echocardiographic parameters. Result: We studied confirmed 19 hypertensive patients(10 men and 9 women, mean age of 50.6 years). Echocardiographically measured indices(mean+/-SD) were as follows; LVMI(137.3+/-30.6g/m2), PWT(0.53+/-0.09) and EF(54.6+/-8.5%). Ratio of betaARK1 levels to GAPDH was from 0.10 to 0.96 (0.62+/-0.25). betaARK1 levels were correlated with LVMI(correlation coefficient: r=.502, p=.029) and RWT(r=.627, p=.004). But Systolic BP(r=0.009, p=.93), diastolic BP(r=.07, p=.85) or EF(r=.045, p=.84) were not related to level of betaARK1. CONCLUSIONS: The betaARK1 level of circulating MNL was correlated well with the degree of the cardiac hypertrophy estimated by LVMI and RWT. This data suggests that activation of sympatho-adrenal system would exert a major role in developing cardiac hypertrophy and we can expect the decreased responsiveness to catecholamine in the heart of hypertensive patients. betaARK1 in circulating MNL might be used as a predictor or marker for LV hypertrophy in hypertensive patients.

Herpes simplex esophagitis can occur in those with normal immune function, but is more often seen in those who are immunocompromised. In one series, 5 percent of post-kidney transplant recipients had herpes esophagitis. We experienced a case of herpes simplex esophagitis, following renal transplantation in a 9 year old male. He complained of epigastric pain, nausea and blood-tinged vomiting. Endoscopic examination showed volcano ulcer, mucosal friability and multiple confluent ulcers covered by whitish exudates on elevated margin in the middle and lower esophagus. Microscopic findings revealed multinucleated giant cells, margination of chromatin, intense nonspecific inflammation and strong positive for herpes simplex virus immunohistochemical staining. Esophageal lesions and symptoms improved after acyclovir therapy.

RESULTS: The risk group was 70.3% and normal group was 29.7% of the subjects. Self-esteem, peer relationship, and academic achievement were important factors influencing children's mental health state. CONCLUSION: Results of this study suggest interventions to improve self-esteem, peer relationship, and academic achievement for promoting children's mental health state.

PURPOSE: This study was conducted to analyze and clarify the concept of 'motivation for vocational rehabilitation in persons with mental disabilities'. METHODS: A hybrid model was used to analyze the concept of motivation for vocational rehabilitation in persons with mental disabilities. The model included a comprehensive literature review and field study. Field study was conducted through in-depth interviews with 5 persons with mental disabilities who had employment experience and participant observation in rehabilitation facilities. RESULTS: The concept of motivation for vocational rehabilitation in persons with mental disabilities was found to be a complex phenomenon having meaning in two dimensions: Goal-oriented and cognitive dimensions. Five attributes and fourteen indicators were defined. Motivation for vocational rehabilitation in these persons was defined as a recognition of the need to get a job, and the willingness and confidence to get and keep a job in order to achieve normal life, happiness, economic stability, and their value. CONCLUSION: The findings from this study will contribute to the development of assessment tools and nursing interventions to reinforce the motivation for vocational rehabilitation in persons with mental disabilities.

PURPOSE: The purpose of this study was to identify the factors that affect nursing students' performance on standard precaution of infection control. METHODS: For the analysis of the study results, mistakes and percentages, descriptive statistics, Pearson's coefficient, and multiple regression analysis were performed. RESULTS: Among the participants, 76.5% received hospital infection education, and 72.5% experienced an exposure to blood or body fluid, and the most of the exposures were occurred during the first semester of the third year. The knowledge score was 20.08±2.09, the awareness score was 4.63±.41, and the performance score was 4.34±.55. In the relationship between the standard precautions knowledge, awareness, and performance, the awareness and the performance were in positive correlations, while the factors that affected the performance level of the standard precautions were awareness and the school year (third year), which had 37.0% of the total exposures. CONCLUSION: In order to increase the performance level of the standard precautions among the nursing department students, after identifying the differences between school years and demands of the students, it is necessary to provide continuous and regular education of standard precautions.

This study was carried out to investigate the relationship between diabetic control and related factors of the practice of diet therapy which affects mostly diabetic patients' dietary compliance. A questionnaire survey was conducted on 128 diabetic patients who had visited Internal medicine endocrinology clinic at University Medical Center of Daegu area. The questionnaires including clinical characteristics, meal regularity and food intake which shows dietary compliance, intrinsic barriers to the practice of diet therapy and knowledge of diet therapy were asked and analyzed. The factor which affects HbA1c was intrinsic barriers and HbA1c became higher as the level of intrinsic barriers was increasing. The education on diet therapy had no influence on the intrinsic barriers, but the level of knowledge on diet therapy was shown higher in the educated patients. The above results suggest that the practice of diet therapy should be leaded to develop behavioral aspects through resonable motivation on dietary compliance along with removing intrinsic barriers rather than simply providing information.
